INTRODUCTION: Leprosy, caused by Mycobacterium leprae, is primarily characterized by cutaneous and peripheral nerve involvement. Nonetheless, musculoskeletal manifestations are also frequently observed and are commonly misdiagnosed as rheumatic diseases.
CASE PRESENTATION: We report a case of lepromatous leprosy presenting with chronic arthritis and HLA-B27 positivity, which was misdiagnosed as peripheral spondyloarthritis in the early stage due to the absence of typical clinical manifestations.
CONCLUSION: Lepromatous leprosy may mimic peripheral spondyloarthritis, especially with HLA-B27 p

Leprosy, caused by Mycobacterium leprae, affects the skin and peripheral nerves. Pure neuritic leprosy (PNL) is a rare subtype, characterized by nerve involvement without skin lesions. We report a rare case of PNL progressing to lepromatous leprosy (LL) with erythema nodosum leprosum (ENL), with only one such case previously reported in the literature and reviewed herein. A 35-year-old male diagnosed with PNL on sural nerve biopsy showing acid-fast bacilli (3+) completed multidrug therapy for 12 months. Nine months later, he developed painful erythematous nodules and sensory-motor deficits. Sl

Leprosy, caused by Mycobacterium leprae, is a chronic granulomatous disease primarily affecting the skin and peripheral nerves. Erythema nodosum leprosum (ENL), or type 2 lepra reaction, is an immunologic complication of lepromatous leprosy, often presenting with erythematous nodules, fever, and systemic inflammation. Early recognition is crucial to prevent chronic complications. We report a 44-year-old man who developed facial erythematous nodules and fever following a sore throat, initially suggestive of a drug eruption or viral infection. Laboratory tests revealed neutrophilia, elevated inf

INTRODUCTION: Although the prevalence of leprosy has significantly decreased in India, it has been still not eradicated, therefore necessitating ongoing efforts to raise awareness for its control.
OBJECTIVES: To determine the diagnostic utility of slit skin smear (SSS) and Fite-Faraco stain (FFS) techniques on histopathology of skin and nerve biopsies for the diagnosis of leprosy in a tertiary care center.
MATERIALS AND METHODS: The present cross-sectional study was carried out on patients clinically suspected to have leprosy consulting in a tertiary care center in Gujarat, India, from Janua

Thromboangiitis obliterans (TAO), also known as Buerger's disease, is a vasculitis associated with a history of smoking, presenting as limb ischemia. Conversely, leprosy is a chronic infectious disease caused by Mycobacterium leprae (M. leprae). While leprosy typically presents with neurological signs, this report describes a rare case of atypical borderline lepromatous (BL) leprosy in a type 1 reaction with an initial presentation mimicking TAO in an elderly woman. This atypical presentation, combined with skin changes related to aging, masked the diagnosis of BL leprosy.

BACKGROUND: Leprosy, caused by Mycobacterium leprae, presents on a spectrum ranging from tuberculoid to lepromatous disease. Borderline lepromatous leprosy represents an unstable immunological state that predisposes patients to immune-mediated reactions, including erythema nodosum leprosum (ENL), a severe inflammatory complication.
CASE PRESENTATION: We report a case of a 62-year-old female with borderline lepromatous leprosy who presented with recurrent facial cellulitis and later developed disseminated ENL. She was initially diagnosed following a series of facial infections and confirmatory

BACKGROUND: Manifestations of leprosy are largely driven by host immune response, with the lepra reactions complicating the indolent course of the disease. Research indicates that the biological processes linked to vitamin deficiencies and helminth infections increase susceptibility to other infections.
AIMS AND OBJECTIVES: To estimate serum vitamin D levels and helminthic coinfections in patients with type 2 lepra reactions and their correlation with the severity of type 2 lepra reactions.
PATIENTS AND METHODS: A prospective cross-sectional study involving 75 leprosy patients of all ages wi

BACKGROUND: Leprosy, a neglected tropical disease (NTD), is closely associated with stigma and discrimination, particularly in low- and middle-income countries, which significantly worsen mental health challenges. In Ethiopia's Amhara region, leprosy accounts for 26.1% of cases, often leading to stigma, isolation, and increased depression and anxiety risk. This study aimed to assess depression and anxiety symptoms among leprosy patients in hospitals across the Amhara region, providing a basis for targeted mental health interventions and policies.

BACKGROUND: Leprosy, also known as Hansen's disease, is a chronic infectious disease caused by Mycobacterium leprae. It primarily affects the skin, peripheral nerves, mucous membranes, and the eyes. If left untreated, it can lead to severe nerve damage, deformities, and permanent disabilities. According to a report by the World Health Organization in 2023, there were approximately 180,000 new cases globally. From 2014 to 2023, the total number of new cases worldwide decreased by 14.6%. Although certain achievements have been made in the prevention and control of leprosy, there are still deep-r

Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae (or M. lepromatosis) that targets macrophages and Schwann cells. The resulting neuropathy and skin lesions can lead to lifelong deformities and disabilities. Despite being poorly transmissible and readily treatable, leprosy remains a neglected disease. Management is hampered by delayed diagnosis, multidrug resistance, recalcitrant leprosy reactions, and entrenched social stigma. We report a case of imported multidrug-resistant borderline lepromatous leprosy that progressed with recurrent, steroid-dependent erythema nod

Tuberculosis (TB) and leprosy are major mycobacterial infections in Indonesia, but coinfection is rare. We describe a 57-year-old male with pulmonary TB and borderline lepromatous leprosy complicated by reversal reaction. He also had type 2 diabetes mellitus and oral candidiasis. TB was confirmed by GeneXpert MTB/RIF, rifampicin-sensitive sputum culture, and chest radiography, showing right suprahilar infiltrates. Leprosy was diagnosed by slit skin smear positive for Mycobacterium leprae. During the 6th month of multidrug therapy (MDT), he developed reversal reaction with fever, painful erythe

BACKGROUND: Early detection of nerve impairment and its progression is critical in all leprosy cases, regardless of spectrum and treatment. This study aims to identify subclinical neuropathy at presentation in newly diagnosed leprosy patients and to monitor its progression after initiating multidrug therapy (MDT) using a combination of clinical nerve palpation, small fibre neuropathy tests (e.g. vibration, temperature, touch, pain, sweat), monofilament testing (MFT), voluntary muscle testing (VMT) and nerve conduction studies (NCSs).
METHODS: We studied 38 nerves among 19 patients and followe

Leprosy shows a broad spectrum of clinical and histopathological features depending on host immunity and disease stage. The characteristic "leprosy pattern" involves superficial and deep dermal infiltrates in perivascular, peri-appendageal, and perineural locations. While tuberculoid leprosy and cutaneous sarcoidosis may occasionally have overlapping histopathology, borderline lepromatous (BL) leprosy demonstrating sarcoidal granulomas is uncommon. We report a 50-year-old female presenting with multiple infiltrated erythematous and hypoesthetic plaques, and asymmetrically thickened peripheral

BACKGROUND: Leprosy is a chronic infectious disease marked by complex immune interactions, yet the roles of specific B-lymphocyte subsets in its pathology are poorly understood.
OBJECTIVES: To investigate the presence and distribution of B-cell subsets, including B1 cells, Marginal Zone (MZ) B-cells, Regulatory B-cells (Bregs), and Effector-1 B-cells (Be1), across different clinical forms of leprosy and reactional states.
METHODS: Immunohistochemical and morphometric analyses were performed on skin lesions from patients with various clinical presentations of leprosy.
RESULTS: CD20+ B-cells

Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae (M. leprae) and/or M. lepromatosis that has a wide spectrum of clinical presentations and multi-organ involvement, primarily on the skin and peripheral nerves. Recent studies have addressed the possibility of nail involvement in leprosy, but to this day limited studies have provided histological evidence confirming the presence of acid-fast bacilli (AFB) in the nail apparatus. A case of lepromatous leprosy-related dystrophy in 20 nails in a 37-year-old male was reported. Physical examination revealed, on all 2

BACKGROUND: Erythema nodosum leprosum (ENL) is an immunologically mediated phenomenon complicating the course of leprosy. Reverse Koebner phenomenon is the term used to describe the sparing of previously injured or diseased skin by new skin lesions of the disease.
METHODS: A middle-aged woman with a known case of lepromatous leprosy for the past year presented with an eruption of reddish painful nodules over her body. The lesions were found to characteristically spare the sites of previous scars.
RESULTS: This sparing phenomenon of previous scar sites has been termed reverse Koebner phenomen
