Syringomyelia in the Course of Resection of Foramen Magnum Pathology: A Case Report with an Intriguing Therapeutic Solution and Systematic Review.

INTRODUCTION: Syringomyelia is a rare disease of the spinal cord, and postoperative syringomyelia is an extremely rare complication. There are very few scientific reports on this subject in the literature, which is why we decided to review them and report our case of a patient after resection of a meningioma of the foramen magnum; we diagnosed syringomyelia in the course of postoperative complications. METHODS: PubMed MEDLINE, Scopus, and Web of Science were searched. The PRISMA checklist was followed for a systematic structure. We paid special attention to publications describing syringomyel

Ruptured spinal dural arteriovenous fistula with subdural hematoma: A case report.

BACKGROUND: Spinal dural arteriovenous fistulas (SDAVFs) are the most common spinal vascular malformations and can lead to significant neurological deficits if left untreated. Although typically associated with chronic myelopathy, rupture of SDAVFs is a rare yet severe complication that can result in acute spinal cord injury secondary to intradural hematoma expansion. CASE DESCRIPTION: A 53-year-old male presented with fluctuating neck and back pain, unsteady gait, right leg numbness, and urinary retention, eventually attributed to a ruptured thoracic SDAVF resulting in a T3-T4 subdural hemat

Improvement of Cobb angle and apical vertebral translation at brace application predicts brace treatment outcomes in adolescent idiopathic scoliosis.

PURPOSE: Various risk factors for curve progression during brace treatment in adolescents with idiopathic scoliosis (AIS) have been identified. However, curve-type-specific risk factors remain unclear. We aimed to identify risk factors for curve progression by curve type. METHODS: At our hospital, we analyzed 211 patients with AIS (200 females, 11 males). X-rays were obtained before and immediately after brace application. Patients were categorized into those requiring surgery during follow-up (Surgery group) and those avoiding surgery (Avoidance group). Univariate and multivariate analyses i

Craniocervical traumatic ligamentous injuries - AO spine type B injuries - Anatomy, biomechanics and clinical perspectives.

The craniocervical junction (CCJ) relies on strong ligamentous structures to maintain stability, which can be compromised by traumatic injuries. Defining stability is critical for selecting operative or non-operative management. This review summarizes the anatomy, imaging, and biomechanical relevance of the main CCJ ligaments, with emphasis on type B injuries in the AO Spine Upper Cervical Classification. Current evidence suggests that neurologically intact patients with normal CT scans (occipitoatlantal joints) and mild MRI abnormalities may be treated conservatively, with fluoroscopic tracti

Spinal extradural arachnoid cyst causing paraparesis in a child.

Spinal extradural arachnoid cyst is a rare condition, commonly located in the thoracic spine. It can be asymptomatic or present with symptoms of myelopathy due to spinal cord or nerve root compression. MRI is the initial imaging modality to diagnose this condition. CT myelography is the imaging modality of choice for detecting the actual site of communication between the subarachnoid space and arachnoid cysts; hence, it plays a pivotal role in surgical planning. We describe the imaging features of a symptomatic large spinal extradural arachnoid cyst and cord changes in a 12-year-old boy, who p

Lokomat-Assisted Robotic Rehabilitation in Spinal Cord Injury: A Biomechanical and Machine Learning Evaluation of Functional Symmetry and Predictive Factors.

BACKGROUND: Lokomat-assisted robotic rehabilitation is increasingly used for gait restoration in patients with spinal cord injury (SCI). However, the objective evaluation of treatment effectiveness through biomechanical parameters and machine learning approaches remains underexplored. METHODS: This study analyzed data from 29 SCI patients undergoing Lokomat-based rehabilitation. A dataset of 46 variables including range of motion (L-ROM), joint stiffness (L-STIFF), and muscular force (L-FORCE) was examined using statistical methods (paired t-test, ANOVA, and ordinary least squares regression)

In Vivo Assessment of Peripheral and Spinal Neuronal Activity in the PSNL Model: Insights into Neuropathic Pain Mechanisms.

Neuropathic pain represents a critical challenge in medical research and clinical practice. Enhanced peripheral nerve activity and spinal dorsal horn neuronal firing are thought to contribute to the nociceptive hypersensitivities that are observed in chronic pain conditions, including those modeled by partial sciatic nerve ligation (PSNL). However, the detailed in vivo neuronal response dynamics and underlying mechanisms in the PSNL model remain to be fully clarified. To better understand these mechanisms, we evaluated dorsal root ganglion (DRG) and spinal dorsal horn neuronal activity in the

Effects of γ-Aminobutyric Acid and Glutamate Signalings from Primary Sensory Neurons to Gate the Spinal Transmission of Mechanical Pain in Mice.

BACKGROUND: The majority of dorsal root ganglion neurons are excitatory glutamatergic neurons. Recent studies suggest that a subset of dorsal root ganglion neurons are able to synthesize γ-aminobutyric acid (GABA) and are immunoreactive to vesicular GABA transporter (Vgat) that packages GABA into the synaptic vesicles critical for inhibitory neurotransmission. The current study aimed to investigate the spinal circuits innervated by these Vgat + primary sensory neurons and interrogate the role of Vgat + primary sensory neurons in nociceptive modification. METHODS: The authors used viral tracin

Is Anterior Longitudinal Ligament Rupture During Posterior Corrective Surgery for Adult Spinal Deformity a Phenomenon Unique to When Combined with Lateral Lumbar Interbody Fusion? -Finite Element Analysis with Comparison to When Combined with Posterior Lumbar Interbody Fusion-.

Background: The occurrence of ALL rupture during posterior correction of adult spinal deformity (ASD) was rare before the introduction of lateral lumbar interbody fusion (LLIF) but has become more frequent recently. It remains unclear whether this phenomenon is unique to LLIF-combined procedures or primarily related to enhanced corrective ability. Methods: The research method used in this study is finite element analysis (FEA). Using preoperative computed tomography images, LLIF cage (L group) or posterior lumbar interbody fusion (PLIF) cage (P group) were placed in the disc space with identic

Missed T8-T9 Translational Fracture with Intact Neurology: A Case Report.

Translational fracture of the thoracic spine, though rare due to its stability, is often associated with spinal cord injury and neurological deficits. Few cases present with intact neurology. We report a case of a 28-year-old woman who, after a road traffic accident, had persistent back pain with intact neurology. The initial chest radiograph showed a subtle deformity that was missed, likely because a lateral view was not done, and the patient was discharged. Persistent back pain led to CT imaging, revealing a T8-T9 translational fracture, left 4th and 5th posterior rib fractures near the cost

Predicting Nonidiopathic Scoliosis from Plain Radiographs: A Deep-Learning Approach.

BACKGROUND: Scoliosis has various etiologies, ranging from idiopathic, congenital, to those with spinal cord abnormalities. Advanced imaging is not always feasible and limited by cost, low-yield, and anesthesia risks in children. Given artificial intelligence's potential, we hypothesize that a deep learning (DL)-based image classifier will demonstrate superior performance in radiographic classification of scoliosis etiology compared witht experienced spine surgeons based on spine radiographs. METHODS: One thousand thirty-six pediatric patients from single institution with scoliosis diagnosis

Genetically engineered senescence-resistant human mesenchymal progenitor cells promote spinal cord injury repair.

Spinal cord injury (SCI) is a devastating condition affecting the central nervous system, often leading to persistent neurological dysfunction. While mesenchymal progenitor cells (MPCs) hold considerable promise for treating various disorders, their application in SCI repair remains hampered by challenges such as poor efficacy and safety concerns. In this study, we developed genetically engineered human MPCs with enhanced resistance to senescence and stress-termed senescence- and stress-resistant cells (SRCs)-and systematically evaluated their therapeutic potential and mechanisms in SCI repair

Therapeutic effectiveness of conditioned medium derived from adipose tissue mesenchymal stem cells and dehydroepiandrosterone in a rat model of spinal cord injury.

BACKGROUND AND OBJECTIVE: Spinal cord injury (SCI) is a severe neurological disorder that leads to significant complications, including loss of bladder/bowel control and increased infection risk. The current standard treatment involves methylprednisolone administration and surgical decompression, but finding an effective therapy with minimal side effects remains a major challenge. This study aimed to investigate the effects of an optimized conditioned medium derived from rat adipose-derived mesenchymal stem cells (AD-MSCs) and dihydroepiandrosterone (DHEA) on behavioral indices, oxidative stre

Navigation-guided en bloc resection of an atypical osteoblastoma leading to a kyphoscoliosis deformity in the upper dorsal spine of a 9-year-old boy: a case report.

BACKGROUND: Osteoblastomas are rare, benign but aggressive bone tumors that can cause significant spinal instability and neurological symptoms, particularly when located near critical structures. While smaller lesions may be treated with radiofrequency ablation (RFA), larger tumors often require surgical resection to prevent recurrence and preserve spinal function. However, ensuring complete removal can be challenging, especially in atypical locations. Advanced intraoperative navigation, such as the O-arm 3D system, can aid in precise tumor localization and safe excision. CASE DESCRIPTION: We

Comparison of Intraspinal Abnormalities Prevalence in Congenital Scoliosis: Is Multiple Hemivertebra Associated with Higher Risk than Single Hemivertebra?

BACKGROUND: The increasing utilization of magnetic resonance imaging has facilitated the detection of intraspinal abnormalities in congenital scoliosis (CS) caused by hemivertebra. However, the risk of intraspinal abnormalities across different hemivertebra patterns remains unclear. The aim of this study was to compare the prevalence of intraspinal abnormalities between single hemivertebra and multiple hemivertebra and identify key associated risk factors. METHODS: A total of 1,048 patients with CS caused by hemivertebra who received surgical correction were included. The radiographic and cli
