Venous Anatomy and Embryology of the Tentorium.

The venous system within the tentorium cerebelli has diverse communications with the cerebrum, cerebellum, and brainstem, and a thorough knowledge of the anatomy of this venous system is necessary for the safe and successful conduct of neurosurgical procedures and neuroendovascular treatments. This review outlines the embryologic development of the tentorium cerebelli and its adjacent venous structures, and the fundamental structure and classifications of the tentorial venous sinuses based on the embryologic background. The term tentorial sinus refers to any venous channel running within the t

The effect of minimally invasive intracerebral haematoma evacuation on early perihaematomal oedema formation.

INTRODUCTION: In patients with intracerebral haemorrhage (ICH), perihaematomal oedema (PHO) is considered a marker of secondary injury and is associated with worse functional outcomes. Minimally invasive surgery (MIS) has been suggested to reduce PHO when performed within 72 h of symptom onset. However, the effect of early surgery on PHO formation remains unclear. We aimed to determine the effect of MIS within 8 h of ICH onset on PHO formation. PATIENTS AND METHODS: We included patients with spontaneous, supratentorial ICH ≥10 mL undergoing MIS within 8 h from the DIST-pilot study and compare

Introduction of Universal WON-Point for Posterior Cranial Fossa Puncture in Monitoring Infratentorial Intracranial Pressure.

BACKGROUND AND OBJECTIVES: Intracranial pressure (ICP) monitoring is a gold standard in brain injury management. While supratentorial ICP monitoring is widespread, monitoring in the posterior cranial fossa is rare and lacks a standardized location. To define and validate a novel anatomic reference point-the WON point-for minimally invasive and standardized infratentorial ICP (iICP) probe placement. METHODS: The WON point was defined as the outer third toward the ear along a line connecting the center of the external auditory canal on the side of ICP probe placement to the mastoid tip on the c

Cerebrum functional network alterations following posterior fossa surgery in Children.

BACKGROUND: Pediatric posterior fossa lesions are frequent, and surgery in this area often results in cerebellar injury. Despite improved surgical methods, the impact on brain functional networks and their role in motor and cognitive performance remain unclear. Existing research mainly involves adults or children who received additional therapies, obscuring the effects of isolated cerebellar damage. Evidence from pediatric functional neuroimaging after posterior fossa surgery is limited. METHOD: In this cross-sectional study, we examined 12 postoperative children and 13 healthy peers using re

A case report of persistent cerebellar dysfunction following acute lithium toxicity.

BACKGROUND: Acute lithium toxicity can cause cerebellar impairment, but long-term follow-up data are limited. We describe a case of acute lithium intoxication followed by progressive cerebellar atrophy and permanent cerebellar dysfunction. CASE PRESENTATION: Five years ago, a previously healthy 38-year-old man developed a clinical syndrome consistent with neuroleptic malignant syndrome (NMS)-like syndrome in the setting of acute lithium and quetiapine toxicity. On admission, the serum lithium concentration was 4.07 mmol/L and the cerebrospinal fluid lithium concentration was 2.45 mmol/L. Conti

Neurodevelopmental Outcomes of Isolated Cerebellar Pathologies in Extremely Preterm Infants.

BACKGROUND AND OBJECTIVES: Cerebellar pathologies, especially cerebellar hemorrhage (CBH), in extremely preterm infants has been increasingly detected through cerebral magnetic resonance imaging (cMRI). We evaluated prevalence, associated risk factors, and neurodevelopmental outcomes of isolated CBH in a contemporary cohort of high-risk infants without major supratentorial brain injuries. METHODS: This observational cohort study included infants born at less than 28 weeks' gestation during 2017-2022 who underwent cMRI at term-equivalent age. Cerebellar pathologies, including low-grade (≤3 mm)

High-Grade Astrocytoma With Piloid Features: An Aggressive Clinicogenomic Entity Distinct From Pilocytic Astrocytoma.

BACKGROUND: High-grade astrocytoma with piloid features (HGAP) has recently emerged as an aggressive glioma entity with distinct molecular alterations, yet its clinicogenomic distinction from pilocytic astrocytoma (PA) remains to be fully elucidated. This study aims to clarify the clinical, pathological, and genomic differences between pediatric PA, adult PA, and HGAP, and to provide evidence supporting the recognition of HGAP as a new, aggressive entity. METHODS: We retrospectively analyzed 100 genetically and histopathologically confirmed PA cases (87 pediatric, 13 adult) and 25 HGAP cases

Dominant clones leverage developmental epigenomic states to drive ependymoma.

ZFTA-RELA is the most recurrent genetic alteration seen in paediatric supratentorial ependymoma (EPN) and is sufficient to initiate tumours in mice1. Despite its oncogenic potential, ZFTA-RELA (ZR) is observed nearly exclusively in childhood EPN, with tumours located distinctly in the supratentorial brain of the central nervous system1. We proposed that specific chromatin modules accessible during brain development would render distinct cell lineage programs at direct risk of transformation by ZR. To test this hypothesis, we performed combined single-nucleus assay for transposase-accessible ch

Three-Dimensional Photorealistic and Virtual Reality Study of the Supratentorial Ventricular System in Relation to Superficial Cranial and Cerebral Landmarks.

BACKGROUND AND OBJECTIVES: The complex spatial organization and deeply embedded position of the cerebral ventricular system present substantial difficulties for anatomic comprehension, particularly among neurosurgical trainees and residents. These challenges are most pronounced in understanding the supratentorial ventricular system's orientation and its projection to superficial cortical and cranial structures. To investigate how advanced three-dimensional (3D) anatomic reconstructions, using photogrammetry and virtual reality (VR) technologies, may offer an educational tool-providing interact

Fitting SLIPPERS with imaging, histopathology and treatment response.

Supratentorial Lymphocytic Inflammation with Parenchymal Perivascular Enhancement Responsive to Steroids is a clinical entity identified within the past decade. Highlighted in only a few case reports since then, our patient presentation adds to the scarce literature on this immune-mediated process. A woman in her early 60s developed worsening right-sided facial pain and diffuse headache for 2 weeks, subsequently demonstrating left upper extremity weakness. She also underwent recent endoscopic sinus surgery ahead of symptom onset. Her MRI brain scans revealed supratentorial areas of bilateral n

Massive brain swelling after cranioplasty in the setting of severe sunken flap syndrome: illustrative case.

BACKGROUND: Massive brain swelling after cranioplasty (MBSC) is a rare but frequently fatal complication characterized by cerebral edema following cranial vault reconstruction. Evidence suggests that MBSC occurs in patients with chronic intracranial hypotension, often in the setting of severe sinking skin flap (SSF) syndrome after decompressive craniectomy (DC). OBSERVATIONS: The authors report a case of MBSC in a 42-year-old man who underwent custom cranioplasty 5 months after DC. Preoperative imaging demonstrated SSF syndrome with a 15-mm midline shift. Within 1 hour after surgery, the pati
