Birth-Related Subdural Haemorrhage in Asymptomatic Moderate-to-Late Preterm Neonates: Magnetic Resonance Imaging Features and Temporal Evolution.

INTRODUCTION: Birth-related subdural haemorrhages (SDH) are common in asymptomatic neonates. While most resolve without long-term effects, their natural history in moderate-to-late preterm (MLP) neonates remains uncertain. Rebleeding into birth-related SDH is also sometimes suggested as an alternative explanation for SDH findings in suspected abusive head trauma. We aimed to assess the prevalence, distribution, risk factors and temporal evolution of birth-related SDH in asymptomatic MLP neonates, and to compare these to those reported in term neonates. METHOD: We reviewed a cohort of asymptom

Secondary exophytic glioblastoma of the cerebellopontine angle: a case report and review of the literature.

PURPOSE: Glioblastoma (GB), a World Health Organization (WHO) grade IV astrocytoma, is the most aggressive primary brain tumor in adults, with a median survival of 12-15 months despite multimodal therapy involving maximal safe resection, radiotherapy, and temozolomide. While GB predominantly arises in the supratentorial region, brainstem glioblastoma (BS-GB) represents an exceptionally rare and clinically devastating subset, accounting for only 1-2% of all GBs. METHODS AND RESULTS: We report a case of a 54-year-old male patient who presented with dizziness and diplopia for half a year. Examin

Location-Specific Hematoma Volume Predicts Early Neurological Deterioration in Supratentorial ICH.

OBJECTIVE: Early neurological deterioration (END) adversely affects outcomes in patients with intracerebral hemorrhage (ICH). This study aimed to determine the location-specific hematoma volumes for END in supratentorial ICH patients. METHODS: We retrospectively analyzed supratentorial ICH patients presenting from two prospective cohorts. END was defined as a ≥ 4-point increase in the National Institutes of Health Stroke Scale (NIHSS) score or a ≥ 2-point decrease in the Glasgow Coma Scale score within 24 h of admission. The training cohort was used to determine location-specific hematoma vol

Increased risk of neurodevelopmental impairment associated with reduced brain volume at term-equivalent age in preterm infants with germinal matrix hemorrhage.

PURPOSE: To assess the association of low-grade germinal matrix–intraventricular hemorrhage (GM-IVH) with reduced supratentorial and white matter volumes at term-equivalent age, and to evaluate whether these volumes are related to neurodevelopmental impairment (NDI). METHODS: This study included preterm infants (gestational age at birth < 37 weeks) who underwent cranial ultrasound within three days of birth and three-dimensional T1-weighted brain magnetic resonance imaging at 34–46 weeks’ postmenstrual age from December 2020 to August 2024. Participants were categorized into the control, GMH (

Fusiform dilatation of the ascending aorta and outpouching of the left A2 anterior cerebral artery segment in COL4A1 -related disorder.

BACKGROUND: COL4A1 encodes the alpha-1 chain of type IV collagen, which plays a crucial role in vascular basement membranes. The clinical manifestations of COL4A1-related disorders are yet to be fully defined. There is increasing evidence that COL4A1 may play crucial roles across the vascular system. METHODS: We studied a 71-year-old man with a novel splice acceptor variant, COL4A1 [NM_001845.6] c.1466-1G > C. Clinical, radiological (including comprehensive imaging of the brain as well as chest and abdomen), and genetic assessments were performed. RESULTS: His medical history included fusifo

Concurrent phenylketonuria and immune-mediated central nervous system involvement: a case report highlighting rare co-occurrence.

BACKGROUND: White-matter abnormalities are common in phenylketonuria (PKU) and are typically attributed to metabolic hypomyelination or intramyelinic edema. Immune-mediated CNS involvement co-occurring with PKU has rarely been described in adults. CASE: An 18-year-old male university freshman diagnosed with classical PKU presented with a 3-week history characterized by progressive bilateral lower-limb weakness and gait instability. Cervical spinal magnetic resonance imaging (MRI) revealed intramedullary lesions, indicating possible immune-mediated CNS involvement overlapping PKU. Cerebrospinal

SLIPPERS Reconsidered: Clinical, Radiological, and Pathological Overlap with PACNS-A Case Report.

Background and Clinical Significance: SLIPPERS syndrome (Supratentorial Lymphocytic Inflammation with Parenchymal Perivascular Enhancement Responsive to Steroids) was first described in 2015 as a variant of CLIPPERS restricted to supratentorial regions. Only a few cases have been reported so far, and its distinction from primary angiitis of the central nervous system (PACNS) remains challenging, as both may present with overlapping clinical, radiological, and histopathological features. We report two patients initially diagnosed with SLIPPERS but finally fulfilling the diagnostic criteria for

The Cerebellar Connectome Disruptions in Ischemic Stroke.

BACKGROUND: Supratentorial focal lesions following ischemic stroke can lead to crossed cerebellar diaschisis (CCD). However, it remains unclear how CCD affects the functional connectivity between the cerebellum and the rest of the brain in ischemic stroke patients. METHODS: This case-control study involved resting-state fMRI data from 65 patients with basal ganglia ischemic stroke (Stroke) and 72 healthy controls (HC). Cerebral, cerebellar, and cerebrocerebellar inter-module functional connectivity in both 7-module and 17-module conditions were calculated and compared between the Stroke and H

Supratentorial Brain Lesions in Unconscious Patients Map to a Common Human Brain Circuit.

OBJECTIVE: Impairment of consciousness is frequent in patients with supratentorial brain injury, but underlying mechanisms are poorly understood. Large-scale network dysfunction has been suggested as a unifying concept for the diverse focal infra- and supratentorial brain lesions associated with unconsciousness. Functional imaging studies have revealed that lesions in different locations that cause the same symptom can be linked to common networks using lesion network mapping (LNM). METHODS: To identify a commonly injured network for impaired consciousness, LNM was used in a prospective, bi-c

High systolic blood pressure variability is associated with impaired cerebral autoregulation and poor functional recovery after intracerebral hemorrhage.

Elevated systolic blood pressure variability (SBPV) has been linked to poor outcomes in intracerebral hemorrhage (ICH), but inconsistent evidence underscores the need for high-resolution studies. This study aimed to clarify the prognostic significance of SBPV in relation to secondary brain injury and functional outcomes in ICH. In 100 adults with supratentorial ICH, SBPV over the first 72 h—quantified using standard deviation (SD), coefficient of variation (CV), and average real variability (ARV)—along with cerebral autoregulation and hematoma dynamics, was analyzed. Higher SBPV was strongly a

Indirect structural disconnection-symptom mapping of haptic graviceptic neglect after stroke.

BACKGROUND: Graviceptive neglect (GN), characterized by disturbed verticality perception through visual, postural, or haptic modalities following brain lesions, is a common and incapacitating condition that has been recognized for decades. Yet, the neuroanatomical correlates of haptic GN (H-GN) have been minimally explored OBJECTIVES: We aimed to investigate the brain structures associated with H-GN post-stroke and present the first analysis of associated brain tracts disconnections. METHODS: Twelve participants with a first unihemispheric supratentorial or cerebellar ischemic stroke were inv

Crossed cerebellar diaschisis on CT perfusion in large vessel occlusion stroke: early predictors and clinical relevance in the hyperacute phase.

BACKGROUND AND PURPOSE: Crossed cerebellar diaschisis (CCD) is a pathophysiological phenomenon in ischemic stroke (IS) that remains poorly investigated, particularly with regard to its clinical impact and the factors associated with its occurrence during the hyper-acute phase. The aim of this study was to evaluate the prevalence of CCD detected by Computed Tomography Perfusion (CTP) in patients with large vessel occlusion (LVO) ischemic stroke and to identify potential predictors of CCD occurrence and its impact on clinical outcomes. MATERIALS: Clinical and radiological data were collected an

Outcome of CyberKnife stereotactic radiosurgery for the brain metastases in patients with Li Fraumeni syndrome.

BACKGROUND: Li-Fraumeni syndrome (LFS) is a hereditary cancer predisposition syndrome associated with germline TP53 mutations and an increased risk of radiation-induced malignancies, posing significant challenges in the management of brain metastases (BM). Data regarding the safety and efficacy of stereotactic radiosurgery (SRS) for BM in this population remain limited. METHODS: We performed a retrospective analysis of patients with LFS who underwent SRS for BM at our institution. Patient-level, lesion-level, and treatment-related data were collected. Outcomes assessed included local tumor co

Case Report: The molecular profile of granular cell astrocytoma predicts aggressive clinical behavior, independent of morphology.

BACKGROUND: Granular cell astrocytoma (GCA) is a rare, morphologically distinct variant of IDH-wildtype glioblastoma that can appear deceptively low-grade yet behave aggressively. Its molecular features remain poorly defined, and no methylation-based classification has previously been reported. METHODS: Two GCAs diagnosed in our clinical neuropathology department were described with the integration of clinical, intraoperative, histopathological, and molecular data, including DNA methylation profiling, a targeted next-generation sequencing panel, and, in one case, whole genome sequencing (WGS)

Infratentorial cerebral microbleeds and brain age gap in stroke patients: a cross-sectional neuroimaging study.

Stroke survivors often face long-term cognitive and motor deficits. Brain age gap (BAG), the difference between chronological age and age estimated based on MRI data, has emerged as a biomarker for neurodegeneration. While prior work links BAG to stroke outcomes, the relationship between BAG and cerebral microbleeds (CMBs), particularly infratentorial CMBs common in hypertensive arteriopathy, remains unclear. The sensorimotor network (SMN) is highly susceptible to both direct and remote injury after stroke and is structurally and functionally interconnected with infratentorial regions via path

Glioneuronal Tumour With Neurocytic Differentiation (GNTN): a Molecularly Defined Tumour Type Formerly Referred to as Extraventricular Neurocytoma.

Tumours historically classified as extraventricular neurocytoma (EVN), initially in comparison with central neurocytoma, are defined by location and morphology; however, emerging molecular data demonstrate that this term is outdated. We report 14 previously unpublished tumours with a robust EVN DNA methylation profile, independent of original histopathologic diagnosis or anatomic location. Integrated molecular analyses identified FGFR1 alterations, particularly FGFR1::TACC1 fusions, as the predominant molecular driver of this tumour class. A smaller number of tumours harboured FGFR1 hotspot mu

Two-step awake craniotomy for diffuse supratentorial gliomas.

BACKGROUND: Maximal safe awake resection is the standard for diffuse gliomas, as it optimizes extent of resection while preserving functional integrity. Progressive loss of accuracy during neurocognitive testing may preclude completion of a maximal function-based resection. We assessed the prevalence, feasibility, safety, and efficacy of a 2-step awake craniotomy approach and identified predictors for requiring a second awake procedure to achieve maximal function-based resection. METHODS: We conducted a retrospective single-center cohort study of 449 consecutive supratentorial diffuse glioma

Prediction of type 1 complex regional pain syndrome with the presence of diaschisis in hemiplegic supratentorial stroke patients.

BACKGROUND: Hemiplegic shoulder pain (HSP) and complex regional pain syndrome type 1 (CRPS) are common post-stroke complications. Crossed cerebellar diaschisis (CCD) has been associated with poor motor outcomes, but its relationship with CRPS is unclear. OBJECTIVES: To investigate whether CCD on brain perfusion SPECT is associated with CRPS type 1 in subacute unilateral supratentorial stroke. METHODS: We retrospectively analyzed 148 patients who underwent brain SPECT. CCD was defined as a cerebellar asymmetry index (AI) ≥10%. CRPS was diagnosed using the Budapest criteria and confirmed by th
