Understanding the Translabyrinthine Approach for Neurosurgery Residents: Operative and Educational Video.

Vestibular schwannomas are benign tumors of the vestibular division of the eighth cranial nerve, with an incidence of 1 to 2 per 100,000 annually. Large tumors (>3 cm) may cause disabling symptoms such as progressive hearing loss, vestibulopathy, or trigeminal nerve dysfunction, often necessitating microsurgical resection. This video demonstrates the translabyrinthine resection of a 3.2-cm left-sided vestibular schwannoma in a 67-year-old woman with worsening sensorineural hearing loss and new-onset lip numbness ( Video 1 ). The procedure was performed at a tertiary center with continuous intr

Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Microsurgical anatomy of the anterior inferior cerebellar artery- systematic review with meta-analysis.

PURPOSE: The anterior inferior cerebellar artery (AICA) is a major vessel of cerebellopontine angle (CPA) and one of main cerebellar arteries. Many studies described a variety of AICA variants, however it has never been systematically reviewed and pooled in order to provide the neurosurgeons with the standardized knowledge in regard to most common types, including their clinical associations. This study aimed to determine AICA's microsurgical anatomy, including its morphometry, branching, origin, relationships with cranial nerves, as well as their respective neurosurgical considerations, such

Neurapraxia in Time and Space.

Since its introduction into medical terminology by Lord Henry Cohen in 1941, the term "neurapraxia" has become established in the neurological and neurosurgical lexicon, although it is often misused or misspelled. It denotes a transient dysfunction of a peripheral nerve, as distinct from axonotmesis and neurotmesis, which describe more significant structural injuries to a peripheral nerve. To expand our understanding of neurapraxia, it is necessary to examine the phenomenon in both temporal and spatial contexts. This study examines neurapraxia in: (A) Time-(1) the origins of the term neuraprax

Nuances in the management of cerebellopontine angle ependymomas: illustrative case.

BACKGROUND: Ependymomas are one of the most common pediatric brain tumors. Cerebellopontine angle ependymomas (CPAEs) are a subset of ependymomas that pose unique surgical challenges due to their location and relationship to surrounding neurovasculature, and they are poorly documented in the current literature. This report discusses the unique tenets of these lesions and their implications for successful management. An illustrative case is presented. OBSERVATIONS: A 15-month-old male was found to have a 2021 classification WHO grade 2 ependymoma extending from the fourth ventricle into the ce

Collet Sicard syndrome: A rare manifestation of jugular nerve schwannoma.

Collet-Sicard syndrome (CSS), also known as condylar jugular syndrome, is a neurological entity characterized by concurrent lower cranial nerve palsies (cranial nerves IX-XII) due to lesions involving both, the jugular foramen as well as the hypoglossal canal. CSS can be due to a variety of causes, including neoplastic as well as non-neoplastic processes, among which jugular foramen schwannomas are a rare but significant cause. We present the case of a 56-year-old male who presented with hoarseness of voice, slowly progressive dysphagia, and difficulty in walking. Examination revealed characte

Orthodontics and Myofunctional Therapy: A critical review of the best complementary treatments of orofacial dysfunctions (Part 1).

AIM: This critical review article illustrates the anatomo-physiological complexity of the orofacial district that makes orthodontic and myofunctional therapies naturally complementary. METHODS: The evolution of the face and mouth's structures and functions intimately connects not just bones and muscles but several functions: breathing, sucking, swallowing, chewing, biting, protective mechanisms such as spitting or coughing, and parafunctions such as clenching or nocturnal bruxism. These functions (among others) are mediated by an extremely complex neural system that involves, affects, or is a

Analysis of the Most Medial Osteotomy in En Bloc Subtotal Temporal Bone Resection: Anatomical Cadaveric Study and Surgical Considerations.

BACKGROUND: En bloc subtotal temporal bone resection (STBR) is a surgical technique used for the removal of temporal bone malignancies. However, few reports detailing resection of the most medial bone in this procedure have been published, although it is the most technically challenging aspect of en bloc STBR. In this study, we focused on the anatomical details of resection of the most medial bone in en bloc STBR, and we discuss the surgical nuances involved. METHODS: Dissections were performed on 10 formalin-fixed cadaveric specimens. Following the subtemporal-infratemporal fossa and retroma

Hypertrophic Pachymeningitis Possibly Secondary to Otitis Media.

This study describes a 46-year-old female patient who initially presented with hearing impairment and subsequently developed a headache as the predominant symptom, accompanied by the involvement of multiple cranial nerves, including the optic, vestibulocochlear, and trigeminal nerves. Cerebrospinal fluid pressure progressively increased, and imaging demonstrated progression from unilateral to bilateral involvement of the tentorium cerebelli. Clinically, the patient's hypertrophic pachymeningitis was considered associated with otitis media. After receiving high-dose corticosteroid pulse therapy

Virtual Anatomic Atlas for Posterior Fossa Approaches and Creating Patient-Specific Surgical 3D Models.

BACKGROUND AND OBJECTIVES: The intricate anatomy of the posterior fossa comprises layered muscles, fasciae, and both extracranial and intracranial neurovascular structures. Surgical intervention in this region requires a precise and comprehensive understanding of these structures, which can be difficult to achieve with only 2-dimensional materials. Traditional cadaver dissections provide basic 3-dimensional anatomic insight but are increasingly constrained by cost, ethical concerns, and difficulties in cadaver sourcing. Advances in 3-dimensional (3D) modeling, particularly through photogrammet

Visualization of the Trochlear Nerve Using Deep Learning-enhanced 3D T2-weighted MR Imaging at 3T.

PURPOSE: Cranial nerve imaging with 3T MRI commonly uses 3D fast imaging employing steady-state acquisition (3D-FIESTA); however, this sequence has limitations in achieving higher spatial resolution and provides poor tissue contrast between cranial nerves and adjacent vascular structures. We evaluated 3D T2-weighted imaging (T2-CUBE) with deep learning-based reconstruction (DLR) for cranial nerve visualization, focusing on the trochlear nerve, the smallest cranial nerve with the longest intracranial course, and compared it with T2-CUBE without DLR and 3D-FIESTA. METHODS: Ten healthy male volu

Hyperbaric oxygen therapy for post-partum bell's palsy associate with anti-phospholipid syndrome: a case report, literature review, and mechanistic insights.

BACKGROUND: Bell's palsy, the most common cause of acute facial paralysis, can occur more frequently during pregnancy and postpartum due to physiological changes such as fluid retention, hormonal shifts, and immune modulation. Antiphospholipid syndrome (APS), an autoimmune disorder characterized by thrombosis and pregnancy related complications, further increases the risk of microvascular ischemia affecting cranial nerves. CASE PRESENTATION: We report the case of a 37-year-old woman with longstanding APS who developed postpartum Bell's palsy unresponsive to corticosteroid therapy. Despite rec

Technical Description of the Mini Far-Lateral Approach: A Refined, Stability-Preserving Corridor to the Petroclival Region.

BACKGROUND AND OBJECTIVES: The far-lateral approach remains essential for accessing ventral and ventrolateral lesions of the craniovertebral junction. However, traditional exposures may involve extensive condylar drilling and muscle dissection, which can compromise craniocervical junction stability. This study aimed to define and quantitatively evaluate a stability-preserving mini far-lateral corridor that provides adequate surgical exposure while minimizing bony and soft-tissue disruption. METHODS: A systematic literature review was conducted following Preferred Reporting Items for Systemati
