[Parapharyngeal salivary gland oncocytoma showing intense radiopharmaceutical accumulation identified during the evaluation of primary hyperparathyroidism by scintigraphy accompanied with SPECT/CT and its transoral endoscope-assisted removal].

Technetium-99m-MIBI (methoxy-isobutyl-isonitrile) is a radiopharmaceutical agent used in the evaluation of myocardial perfusion and parathyroid pathologies. MIBI scintigraphy is a commonly applied diagnostic method in the assessment of parathyroid adenomas for lesion localization and to exclude the presence of ectopic adenomas. However, tracer uptake may also occur in thyroid adenomas, thyroid carcinomas, and other lesions with high mitochondrial density. In our study, we present the case of a 79-year-old female patient. The patient had been under endocrinological care since 1997 due to osteop

A Simple Technique for Probe-Based NIRAF-Guided Parathyroid Identification in TOETVA.

We describe a technique for probe-based near-infrared autofluorescence (NIRAF)-guided parathyroid identification during TOETVA. The probe is mounted on a suction cannula and introduced via the central port, allowing real-time confirmation of parathyroid tissue without device modification. This approach enables integration of NIRAF into remote-access thyroid surgery.

Parathyroid histology in normocalcemic and hypercalcemic primary hyperparathyroidism with hypercalciuric renal stones.

CONTEXT: Hypercalcemic (HPHPT) and normocalcemic primary hyperparathyroidism (NHPT) are distinct conditions with different biological and histological characteristics. Understanding their histological patterns could improve disease characterization. OBJECTIVE: This study aimed to compare the histological features of NHPT and HPHPT parathyroid glands, but also rims of normal tissue, focusing on the expression of calcium-sensing receptor (CaSR), 1-α hydroxylase (CYP27B1), and vitamin D receptor (VDR). METHODS: A retrospective observational study was conducted on histological and immunohistoche

Unanticipated remission of primary hyperparathyroidism following cinacalcet.

Primary hyperparathyroidism (PHPT) is the most common etiology of hypercalcemia in an ambulatory setting and often arises from adenomatous growth of one (occasionally more than one) parathyroid gland. Patients with severe and symptomatic hypercalcemia, renal insufficiency, nephrolithiasis, osteoporosis, and younger than 50 are candidates for surgical removal of parathyroid adenoma(s). Medical treatment with a calcimimetic agent is offered to improve severe hypercalcemia when surgery is refused or considered unsafe. Here, we report a 68-year-old man with severe hypercalcemia due to PHPT and cor

Spontaneous hemorrhage from a functionally well-controlled, non-enlarged but hyperplastic parathyroid gland due to secondary hyperparathyroidism: a case report.

Chronic kidney disease causes secondary hyperparathyroidism (SHPT) with hyperplastic enlarged parathyroid glands. Deep cervical spontaneous hemorrhage has been occasionally reported to be caused by rupture of an enlarged parathyroid gland under functionally uncontrolled SHPT. We encountered an unusual case of life-threatening spontaneous hemorrhage from a parathyroid gland in a patient undergoing hemodialysis. A 40-year-old man with end-stage renal disease received renal replacement therapy for 12 years. He had SHPT, which was functionally well controlled using multiple medications. The parath

Hyperparathyroidism: historical milestones and modern therapeutic strategies.

The Viennese Medical School was at the forefront of the clinical understanding of the relationship between parathyroid and bone metabolism. A century ago, F. Mandl (1925) and E. Gold (1927) described two patients clinically (osteitis fibrosa cystica) and biochemically (hypercalcemia, hypercalciuria) with a disease which Gold had already termed (primary) hyperparathyroidism (PHPT) in 1928. The two patients were successfully treated by removing one enlarged parathyroid gland each. After the clinical and biochemical diagnosis was confirmed, surgery was recommended. In 1933, Mandl summarized his r

Autofluorescence enhances intraoperative detection of parathyroid glands: a single-center comparative study.

INTRODUCTION: Permanent hypoparathyroidism after total thyroidectomy remains a major complication, occurring in close to 10% of cases and increasing risks of cardiac, renal, and metabolic morbidity. Near-infrared autofluorescence (NIRAF) imaging has emerged as a promising adjunct to improve parathyroid gland (PG) identification and preservation. This study evaluates the efficacy of NIRAF in enhancing intraoperative PG detection compared to conventional visual inspection. METHODS: This single-center retrospective cohort study included 349 patients undergoing total thyroidectomy. Outcomes were

Neglected parathyroids: Fluoride's trigger for skeletal fluorosis and endocrine domino effect.

BACKGROUND: Fluoride is a necessary micromineral for bone and dental health but excessive exposure can lead to dental and skeletal fluorosis. This review investigates the effects of fluoride on the parathyroid gland focusing on its structure and functions. METHODS: Research papers were collected using various methods of data collection like PubMed, Scopus, and Google Scholar, and then reviewed thoroughly. RESULTS: Fluoride impacts parathyroid gland function through dose and time dependent modulation of PTH secretion. Various in-vitro and in-vivo studies have shown that fluoride inhibits the

Comparison of adenoma and gland hyperplasia on bone mineral density in primary hyperparathyroidism.

BACKGROUND: Parathyroid gland adenomas or hyperplasia can cause primary hyperparathyroidism where increased levels of parathyroid hormone (PTH) lead to increased bone resorption. Parathyroidectomy can attenuate or reverse this bone loss. This study compares the difference in gland histology on Bone Mineral Density (BMD) following parathyroidectomy. METHODS: A retrospective cohort study of patients with primary hyperparathyroidism comparing BMD before and after parathyroidectomy between patients with single-gland adenomas and hyperplasia on histological examination. BMD was measured using Dual

Autoinfarction of the Parathyroid Gland: An Unusual Presentation of Primary Hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) is commonly caused by a parathyroid adenoma and typically presents with hypercalcemia and elevated parathyroid hormone (PTH) levels. Spontaneous infarction of parathyroid adenoma is a rare phenomenon that can lead to spontaneous resolution of hypercalcemia. CASE PRESENTATION: We report the case of a 48-year-old male with a history of PHPT diagnosed 7 years prior and was lost to follow up, who presented with left-sided facial and finger paresthesia. Initial labs revealed low ionized calcium with a normal PTH level. Imaging identified a heterogeneo

Newly identified parathormone-secreting cells in the thyroid tissue.

BACKGROUND: After total parathyroidectomy, circulating parathyroid hormone (PTH) levels typically do not fall to zero, and the biological source of this residual secretion remains unclear. METHODS: Patients were divided into three groups. Group 1 (n = 46) comprised total thyroidectomy tissues; Group 2 (n = 46) included non-thyroid/non-parathyroid tissues (negative controls); and Group 3 (n = 7) consisted of healthy parathyroid gland tissues (positive controls). All tissues were kept in physiological saline immediately after resection for 30 min, and then 2 mL samples were taken to measure PTH

Evaluation of mitoxantrone hydrochloride injection versus carbon nanoparticle suspension in robotic thyroidectomy for papillary thyroid carcinoma.

This study aimed to evaluate whether mitoxantrone hydrochloride injection (MHI) provides lymph node tracing and parathyroid preservation outcomes comparable to those of carbon nanoparticle suspension injection (CNP) during robotic thyroidectomy for papillary thyroid carcinoma (PTC). We conducted a retrospective review of 507 patients with PTC who underwent robotic thyroidectomy from September 2024 to September 2025. Patients were categorized into CNP and MHI groups based on the tracer utilized. After 1:1 propensity score matching, 62 matched pairs were selected for the final analysis. The prim

Regulation of Proximal Tubule Phosphate Transport: New Partners, New Pathways.

Disorders of phosphate homeostasis are common but difficult to treat. The major organ systems responsible for phosphate homeostasis are the intestine, bone, kidney, and parathyroid gland. Overall, kidney handling of phosphate is the major regulator of both serum phosphate levels and overall phosphate homeostasis. However, our ability to manipulate kidney phosphate handling has been very limited. Npt2a has been more thoroughly investigated than Npt2c, but recent studies identifying the role of Npt2c in human disease have uncovered key differences in the role of Npt2c in mice and humans. Recent

Regarding Non-Classical Presentations of Rare Hereditary Hypoparathyroidism: A Case Series of CASR, GNA11, and GATA3 Mutations in Parathyroidology.

This correspondence critically evaluates the diagnostic and therapeutic paradigms presented by Ekaterina Bibik et al. in their analysis of rare hereditary hypoparathyroidism involving CASR, GNA11, and GATA3 mutations. While the case series provides valuable genomic data, the reported decade-long diagnostic delays-particularly the misidentification of mineral disorders as primary epilepsy-unmask a profound systemic failure in clinical vigilance regarding neuroimaging findings like Fahr syndrome. We emphasize that current "conventional" management, characterized by aggressive calcium and vitamin

Surgery for kidney-related hyperparathyroidism: A review.

Secondary hyperparathyroidism often occurs as a result of a disruption in calcium and phosphorus homeostasis caused by chronic kidney disease (CKD). While some individuals experience a remission of parathyroid gland disease subsequent to kidney transplantation due to the restoration of calcium homeostasis, a subset continues to exhibit elevated parathyroid hormone due to autonomous secretion, a condition otherwise known as tertiary hyperparathyroidism. This review aims to investigate the intricacies of diagnosis, presentation, medical treatment, state-of-the-art surgical options, and strategie
