Preoperative imaging strategies to optimize surgical treatment in primary hyperparathyroidism (pHPT).

PURPOSE: Accurate preoperative localization of parathyroid adenomas is essential to enable targeted and minimally invasive surgery in patients with primary hyperparathyroidism (pHPT). A variety of imaging techniques are available for this purpose. In routine clinical practice, cervical ultrasound (US) combined with scintigraphy is most commonly used as the initial diagnostic approach. However, the optimal strategy or combination of imaging modalities for reliable localization prior to surgery and its relevance for postoperative outcomes remains a matter of ongoing discussion. The aim of this s

Giant Parathyroid Adenoma Masked by Severe Vitamin D Deficiency with Hypocalcemic Presentation: A Case Report.

INTRODUCTION: Giant parathyroid adenomas, defined as lesions weighing at least 3.5 grams, are rare and typically present with hypercalcemia due to excessive parathyroid hormone secretion. Severe vitamin D deficiency can alter this pattern by lowering serum calcium, which may obscure the diagnosis of primary hyperparathyroidism, especially in younger patients. This case is notable because a large parathyroid adenoma presented with hypocalcemia rather than hypercalcemia, creating an atypical biochemical profile that initially mimicked secondary hyperparathyroidism. CASE PRESENTATION: A 30-year-

Clinical and Biochemical Correlates of Parathyroid Gland Burden in Patients Undergoing Parathyroidectomy for Secondary Hyperparathyroidism: A Retrospective Observational Study.

Background/Objectives: Secondary hyperparathyroidism (SHPT) is a common and clinically significant complication of advanced chronic kidney disease and may require surgical intervention when medical therapy fails. This study aimed to evaluate the association between parathyroid gland burden, defined by gland number and size parameters, and biochemical disease severity in patients undergoing parathyroidectomy for SHPT, and to assess the concordance between preoperative imaging findings and intraoperative observations. Although parathyroid gland enlargement is a hallmark of SHPT, the clinical rel

Endoscopic detection of near-infrared autofluorescence in the parathyroid glands: a case report of two patients.

INTRODUCTION AND IMPORTANCE: Preservation of the parathyroid glands during thyroid surgery is crucial to prevent postoperative hypocalcemia. Parathyroid glands exhibit intrinsic autofluorescence with an emission peak around 820 nm, and near-infrared imaging has been applied for their intraoperative identification. However, its feasibility in endoscopic thyroid surgery remains unclear, particularly when using a small-diameter endoscope. Here, we report two cases in which parathyroid autofluorescence was successfully visualized during endoscopic surgery using a 5-mm endoscope. CASE PRESENTATION

Acidic extracellular pH-induced PTH secretion in male mouse parathyroids is OGR1-dependent.

The role of extracellular acidity in regulating PTH secretion in cultured mouse parathyroid glands (PTGs) has not been studied to date, largely because of the technical difficulty of isolating mouse PTGs. We hypothesized that acidic extracellular pH directly stimulates PTH secretion through activation of a proton-sensing receptor, specifically ovarian cancer G protein-coupled receptor 1 (OGR1, also known as GPR68). To test this, we developed a method to reliably identify and isolate PTGs from male mice by administering 5-aminolevulinic acid, which induced selective fluorescence in these glands

Pre-Parathyroidectomy PTH as an Integrated Biomarker of Glandular Remodeling and Skeletal Turnover in Secondary Hyperparathyroidism.

Secondary hyperparathyroidism (SHPT) is a major component of chronic kidney disease-mineral and bone disorder (CKD-MBD), reflecting progressive disturbances in mineral metabolism, endocrine signaling, skeletal remodeling, and parathyroid-gland biology. Traditionally, preoperative parathyroid hormone (PTH) has been used primarily as a biochemical threshold for surgical referral. However, persistent PTH elevation in advanced CKD-related SHPT may reflect more than isolated endocrine activity; available evidence suggests it integrates parathyroid-gland remodeling, receptor resistance, skeletal tur

The role of ESRRA and JUNB in oxyphil cells: insights from spatial transcriptomics and single-cell transcriptomics in uremic secondary hyperparathyroidism.

Oxyphil cells are a primary cellular component of the parathyroid gland. Due to the limitations of techniques for separating oxyphil cells from chief cells, the characteristics and functions of oxyphil cells in uremic secondary hyperparathyroidism (SHPT) remain largely unclear. Therefore, we integrated spatial transcriptomics and single-cell transcriptomics to investigate the characteristics of oxyphil cells and the association between oxyphil cells and calcitriol resistance in SHPT. 6 uremic SHPT samples and 3 normal parathyroid samples were used for single-cell transcriptomics, while the mos

Choline positron emission tomography/computed tomography imaging in parathyroid neoplasms.

Parathyroid glands are the smallest organs in our body, that regulate calcium metabolism, by producing parathyroid hormone (PTH). They are four, at least, as they might be either supernumerary or ectopic, especially in patients with chronic renal failure. Hyperparathyroidism (HPT) is a common endocrine disease, characterized by hormone imbalance, which leads to increased secretion of PTH. Primary hyperparathyroidism (PHPT) is caused by hyperfunctioning parathyroid glands, mainly by a solitary parathyroid adenoma and less frequently by parathyroid hyperplasia or double parathyroid adenomas. The

A Protocolled Implementation of Upfront Ultrasound, A Stepwise Pre-Operative Imaging Regimen in Primary Hyperparathyroidism.

OBJECTIVE: To evaluate the sensitivity of a stepwise preoperative imaging regimen for patients with primary hyperparathyroidism. STUDY DESIGN: Prospective cohort study. SETTING: A high-volume tertiary referral center for parathyroidectomy. METHODS: Patients with primary hyperparathyroidism referred for parathyroidectomy underwent surgeon-performed ultrasound as the primary preoperative imaging examination. The diagnostic confidence of the examiner on a scale from 0 to 3 determined whether additional imaging was acquired. If ultrasound identified an enlarged parathyroid gland with certainty

Recurrent or persistent primary hyperparathyroidism, parathyromatosis.

Persistent primary hyperparathyroidism is defined as the persistence or recurrence of hypercalcemia within 6 months of parathyroid surgery. Recurrent primary hyperparathyroidism is defined as the recurrence of primary hyperparathyroidism more than 6 months after an initially curative parathyroidectomy. In these situations, it is essential to rule out differential diagnoses, and in particular secondary hyperparathyroidism and familial hypocalciuric hypercalcemia. Failure to remove the pathological parathyroid gland or glands during initial surgery for primary hyperparathyroidism is the most com

Case Series: Postoperative recurrent secondary hyperparathyroidism caused by ectopic submandibular parathyroid: 3 cases report and literature review.

Postoperative recurrence for secondary hyperparathyroidism (SHPT) remains a clinical challenge. Ectopic parathyroid glands are a major cause of recurrence, and those located in the submandibular region are rare. This report retrospectively analyzes three patients on maintenance dialysis who developed recurrent elevation of parathyroid hormone (PTH) levels 3 to 37 months after parathyroidectomy (PTX) for SHPT. Neck ultrasound revealed no definite lesions; however, 99mTc-methoxyisobutylisonitrile (99mTc-MIBI) single-photon emission computed tomography/computed tomography (SPECT/CT) fusion imagin

[Primary hyperparathyroidism in pregnancy resulting in hypercalcemic crisis and acute pancreatitis: A case report].

Primary hyperparathyroidism (PHPT) is caused by excessive production of parathyroid hormone by the parathyroid gland. PHPT discovered for the first time during pregnancy is extremely rare and has serious adverse effects on both the mother and the fetus. This article reports a case of pregnancy complicated by PHPT, hypercalcemic crisis, and acute pancreatitis. The patient was a 30-year-old woman at 32 weeks of gestation who presented with a four-day history of progressive abdominal pain and vomiting. Laboratory evaluation on admission revealed a markedly elevated serum total calcium of 3.56 mmo

Establishing diagnostic thresholds for adult ADHD screening in taxi drivers: validation of the CAARS-S via Item Response Theory and Machine Learning.

Among women around the globe, breast cancer is both the most common cancer and the leading cause of cancer-related death. Women in economically disadvantaged countries have a lower incidence of breast cancer, but poorer survival rates for the disease relative to women in affluent countries. Evidence suggests that breast cancer mortality can be reduced if resources are applied to the problem in a systematic way. The purpose of the Global Summit Consensus Conference was to begin a process to develop guidelines for improving breast health care in countries with limited resources-those with either

Exposed yet unmapped: evidence of differential flood exposure in deprived urban areas using citizen science.

BACKGROUND: Due to their immature immune system, neonates are at high risk of infection. This vulnerability when combined with limited resources and health education in developing countries can lead to sepsis, resulting in high global neonatal mortality rates. Many of these deaths are preventable. The BabyGel pilot trial tested the feasibility of conducting the main randomised trial, with the provision of alcohol handgel to postpartum mothers for prevention of neonatal infective morbidity in the rural community. This secondary analysis sought to evaluate the methods of detecting infections in

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Impact of intraoperative parathyroid identification and accidental removal on post-thyroidectomy hypocalcemia: a single-center 15-year experience.

INTRODUCTION: Post-thyroidectomy hypocalcemia remains a common and clinically significant complication. Despite advances in surgical technique and perioperative care, rates of transient and persistent hypocalcemia remain substantial, with reported transient rates up to 50% and persistent rates up to 4%. This study evaluates the impact of intraoperative parathyroid gland identification and accidental removal on postoperative hypocalcemia in a Swiss hospital. METHODS: We conducted a retrospective analysis of 615 adult patients undergoing thyroid surgery at the University Hospital of Basel betwe

Factors influencing identification accuracy rate of parathyroid glands using near-infrared autofluorescence during endoscopic thyroidectomy.

BACKGROUND: Near-infrared autofluorescence (NIRAF) enhances parathyroid gland (PG) identification during thyroidectomy. However, its identification rate is frequently affected by false-positive and false-negative results. METHODS: This prospective cohort study enrolled 50 patients scheduled for endoscopic thyroidectomy, employing a 1:1 case-matched design. The patients were assigned to an experimental group (NIRAF group; n = 25) and a control group (Non-NIRAF group; n = 25). RESULTS: In the NIRAF group, levels of parathyroid hormone (PTH) on the first postoperative day (P = 0.002) and at the

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Parathyroid Carcinoma Revisited: Clinical and Pathological Insights From Two Decades of Experience.

BACKGROUND: Parathyroid carcinoma is a rare endocrine malignancy (< 1% of primary hyperparathyroidism and 0.005% of all cancers) that often mimics a benign adenoma, leading to diagnosis only after surgery, unless invasion or metastasis is present. OBJECTIVE: To identify distinguishing clinical, biochemical and pathological features of parathyroid carcinoma in comparison with primary parathyroid adenoma and atypical parathyroid tumor and to determine its perioperative predictors. METHODS: A retrospective analysis compared 16 histopathologically confirmed parathyroid carcinoma cases with 378 p
