PURPOSE: Azacitidine (Vidaza®, AZA) is a mainstay for treating acute myeloid leukemia (AML) in patients unfit for standard induction and other myelodysplastic syndromes (MDS). However, only half of the patients usually respond to this drug and almost all patients will eventually relapse. Predictive markers for response to AZA are yet to be identified. AZA is metabolized in the liver by a single enzyme, cytidine deaminase (CDA). CDA is a ubiquitous enzyme coded by a highly polymorphic gene, with subsequent great variability in resulting activities in the liver. The quantitative determination of

Lenalidomide is an immunomodulating drug structurally similar to thalidomide. It is indicated for patients with relapsing or refractory multiple myeloma in combination with dexamethasone, and for patients with myelodysplastic syndromes associated with a deletion 5q cytogenetic abnormality. It is also used to treat other myelodysplastic syndromes such as myelofibrosis and lymphoma. We report a case of organizing pneumonia leading to acute respiratory distress syndrome (ARDS) after long-term administration of lenalidomide, along with a review of the literature.

INTRODUCTION: Intracranial or spinal compressive lesions due to extramedullary hematopoiesis have been reported in the medical literature. Most of the reported cases are extradural lesions or, on rare occasions, foci within another neoplasm such as hemangioblastoma, meningioma or pilocytic astrocytoma. Often these cases occur in patients with an underlying hematological disorder such as acute myelogenic leukemia, myelofibrosis, or other myelodysplastic syndromes. Such lesions have also been reported in thalassemia major.
CASE PRESENTATION: We report the case of a 43-year-old Iranian woman in

PURPOSE OF REVIEW: The aim of this article is to discuss the relevant pathobiologic effects of lenalidomide and the most recent clinical evidence to support its use in patients with myelodysplastic syndrome.
RECENT FINDINGS: Lenalidomide is an immunomodulatory agent with biological activity in several hematologic malignancies, including myelodysplastic syndrome. The precise mechanism yielding benefit in patients with myelodysplastic syndrome and 5q- syndrome is not clear, but various molecular and pathogenic targets have been identified. Enhancement of cellular immunity through T-cell and NK-

Aplastic anemia (AA) is associated with non-trival mortality. Although diagnostic boarders with other myelodysplastic syndromes are unclear, AA is thought, at some point in the chain of events that destroy marrow hematopoiesis, to involve overproduction of tumor necrosis factor-alpha (TNF) and that TNF is the proximal death programming signal the stem cells receive. Thus agents that lower TNF may be of benefit in AA. In preliminary studies we have found that the commonly used antidepressant bupropion lowers TNF. Thus, bupropion is a potential initial small molecule TNF inhibitor to consider fo

Clonal chromosomal abnormalities occur in about one-third of patients with chronic myelomonocytic leukemia (CMMoL) and are usually those found in other myelodysplastic syndromes. Abnormalities of chromosome 3, which have been associated with abnormal thrombopoiesis, are relatively uncommon. A patient with CMMoL with inv(3)(q21q26) and normal thrombopoiesis is presented, and the cytogenetics of CMMoL are reviewed. The possible significance of chromosome 3 in thrombopoiesis is discussed.

A 35-year-old male presented with chronic myelomonocytic leukemia (CMMoL) after 6.5 years of alkylating agent therapy for IgG-kappa type multiple myeloma. The total dose of melphalan was 0.648 g. CMMoL was stable with weekly injection of alpha-interferon for one year. Thereafter, monocytosis and thrombocytopenia aggravated, and the patient died of disseminating intravascular coagulation. Prolonged drug therapy can induce CMMoL, as well as other myelodysplastic syndromes.

We studied the expression of IL 6 gene in 49 patients, including 21 chronic myelomonocytic leukemias (CML), 9 other myelodysplastic syndromes (MDS), 18 acute myeloid leukemias (11 M2 or M3 and 7 M4 or M5) and 1 case of acute biphenotypic lymphoid monocytic leukemia. IL 6 was found expressed only in the latter patient and in one patient with M5 acute myeloid leukemia. DNA analysis by Southern blot revealed no rearrangement in these 2 patients and in 10 of the other patients who showed no IL 6 expression. These results do not support an autocrine role for IL 6 in CMML or in other MDS. In acute m

Although immune mechanisms are known to be partially responsible for the thrombocytopenia of patients infected with HIV-1, an understanding of the mechanism underlying this disorder is incomplete. A casual observation that bone marrow biopsies of HIV-infected individuals seem to exhibit an unusually large number of denuded megakaryocyte nuclei (DN-MK) prompted a study comparing MK of 20 HIV-seropositive individuals with those of 10 patients with HIV-negative idiopathic thrombocytopenic purpura and 10 hematologically normal subjects. In normal marrows the number of DN-MK average 2.1 +/- 0.5 SE
