Tuberculosis remains a major infectious disease, and the increasing incidence of multidrug-resistant Mycobacterium tuberculosis continues to limit current treatment options. This review summarizes recent medicinal chemistry efforts directed toward six important M. tuberculosis targets: InhA, DprE1, MmpL3, QcrB, DNA gyrase, and dihydrofolate reductase (DHFR). Emphasis is placed on the structural diversity and biological activity of potent molecules. SAR analysis relates changes in scaffold, substituent pattern, linker geometry, lipophilicity, and polarity to antimycobacterial potency and activi

BACKGROUND: Young children with exposure to or infection with Mycobacterium tuberculosis are at high risk of developing tuberculosis disease, and postexposure tuberculosis preventive treatment decreases this risk. Evidence is limited regarding the dosing and safety of the 12-dose once-weekly rifapentine and isoniazid regimen (3HP) for tuberculosis preventive treatment in children younger than 2 years and in children living with HIV.
OBJECTIVE: This protocol describes Tuberculosis Trials Consortium (TBTC) Study 35, which aims to establish practical age- and weight-banded rifapentine doses for

BACKGROUND: Leprosy, a curable neglected tropical disease, continues to affect vulnerable populations in low-resource settings despite global control efforts. In Ghana, although leprosy has been eliminated as a public health problem, new cases continue to be detected, and persons cured of leprosy (PCLs) continue to face long-term stigma, disability, and barriers to healthcare in segregated communities. This qualitative descriptive study aimed to understand the knowledge of leprosy, sources of healthcare, and challenges shaping health-seeking behaviours among PCLs in Godokpe, Ho Municipality, V

OBJECTIVE: In response to growing numbers of people with drug-resistant tuberculosis (DR-TB), South Africa was, in 2011, among the first countries to launch a national policy on the decentralisation of DR-TB management, preceding normative guidance from the World Health Organization. The objective was to understand the process of formulation of the South African policy on DR-TB service decentralisation.
DESIGN: Qualitative policy analysis. We reviewed policy-related documents to create a timeline, then conducted in-depth interviews with key individuals who had influence over or insights into

Chronic granulomatous infections are frequently diagnosed as tuberculosis, often supported by histopathological findings. Rapidly growing mycobacteria (RGM), a type of non-tuberculous mycobacteria, can also produce granulomatous reactions that resemble tuberculosis. RGMs are resistant to standard antituberculous treatment, and microbiological evidence is crucial for determining optimal therapy. This report presents a case of septic arthritis of the right ankle featuring granulomatous reactions in the abscess wall and bone changes in the lower end of the tibia, initially diagnosed clinically as

INTRODUCTION: Tuberculosis (TB) remains a major occupational risk for healthcare workers (HCWs), who face higher exposure and potential challenges in achieving optimal treatment outcomes. Despite extensive national TB control strategies in Malaysia, evidence on treatment success and its determinants among HCWs is limited. This study aimed to determine the proportion of TB treatment success and identify associated factors among HCWs with active TB in Selangor from 2014 to 2023 using the National Tuberculosis Registry (NTBR) system.

Sea turtles are susceptible to opportunistic infections, particularly when debilitated, injured, or exposed to environmental stressors. Although nontuberculous mycobacteria are increasingly recognized as emerging pathogens in marine wildlife, documented cases in sea turtles remain limited, and coinfections involving mycobacteria and uncommon environmental fungi are rarely reported. Here, we describe a fatal multisystemic disease involving Mycobacterium fortuitum infection, Epicoccum sp.-associated coelomitis, and severe coccidial enteritis in an adult olive ridley sea turtle (Lepidochelys oliv

INTRODUCTION: Diagnosing paediatric tuberculosis (TB) remains challenging. Currently available diagnostic tests have limited accuracy, and most sampling methods are invasive. In 2021, the World Health Organization (WHO) endorsed the use of molecular WHO-recommended rapid diagnostic tests (mWRDs) with stool as an initial diagnostic tool for paediatric TB. In this study, we evaluated the diagnostic test accuracy (DTA) of stool samples processed with the Stool Processing Kit (SPK), one of three centrifugation-free methods available at the time.

We describe the first reported Buruli ulcer case likely acquired from a ringtail possum scratch, which represents a newly recognised mechanism for Buruli ulcer acquisition. We aim to alert health practitioners of this possibility in order to educate patients and facilitate early Buruli ulcer diagnosis, which reduces morbidity and allows less complex treatment. We also aim to influence public health messaging around the risk of Buruli ulcer acquisition from handling sick or injured possums.

Mycobacterium persicum (M. persicum) is a nontuberculous mycobacterium that predominantly affects the lungs, causing pulmonary lesions, and is readily misdiagnosed as tuberculosis. Here we report a 48-year-old man with AIDS and M. persicum pulmonary disease who was initially misdiagnosed with pulmonary tuberculosis and received standard anti-TB therapy, yet showed no radiological or clinical improvement. Sputum specimens were subsequently subjected to targeted next-generation sequencing (tNGS), which identified M. persicum and established the definitive diagnosis. After switching to an anti-M.

BACKGROUND: BCG vaccination and household exposure to tuberculosis (TB) are central to childhood TB control, yet their delivery and effectiveness vary considerably across communities. Evidence from community-based studies in high-transmission, resource-limited settings remains sparse, particularly among marginalised populations such as tea-garden worker communities. This study evaluates their protective effect against childhood TB in rural Bangladesh to address this gap.
METHODS: We conducted a community-based case-control study in Moulavibazar District, enrolling children aged 2-14 years fro

Tuberculosis (TB), caused by Mycobacterium tuberculosis, remains a major global health challenge, particularly in low- and middle-income countries. The Bacillus Calmette-Guérin (BCG) vaccine, developed from attenuated Mycobacterium bovis (M. bovis) and introduced in 1921, is the only licensed vaccine for TB prevention. Although BCG provides strong protection against morbid forms of pediatric TB, its efficacy against adult pulmonary TB is variable. Emerging evidence indicates that BCG exerts broader immunological effects beyond TB prevention, including non-specific immune protection mediated th

Mycobacterium avium subsp.

Mycobacterium ulcerans causes Buruli ulcer (BU), a skin neglected tropical disease, largely through the expression of a potent cytotoxin known as mycolactone. As mycolactone is central for BU pathogenesis, it follows that targeting its production with drugs could be one means of controlling the infection. However, this has not yet been attempted due to the difficulty of applying typical mycolactone-detecting assays, such as mass spectrometry, to routine drug screening to identify suitable drugs for this purpose. In this repurposing study, we screened the Pandemic Response Box, an open-source c

BACKGROUND: Leprosy is a chronic infectious disease affecting the skin and peripheral nerves, potentially causing deformities and disabilities. Transmission occurs mainly via aerosols and nasal secretions, with persistent endemicity in countries such as Brazil, India and Indonesia. Systematic monitoring of contacts is a key strategy for early detection and disease control.
OBJECTIVE: To investigate risk factors associated with time to disease onset among leprosy contacts and describe the temporal dynamics of disease development in a historical cohort.
METHODS: A retrospective cohort of 600 c

BACKGROUND: Despite substantial progress in reducing incidence, considerable challenges to global leprosy elimination persist. This study sought to elucidate longitudinal trends in the global leprosy-associated disease burden through multiple stratifications.
METHODS: Using data from the Global Burden of Disease Study, we analyzed the age-standardized incidence rate (ASIR) and Disability-Adjusted Life Years (DALYs) rate (ASDR) per 100,000 population for leprosy, stratified by geography (seven super-regions in the GBD regional classification), age (focusing on the pediatric population), income

The genus Mycobacterium comprises nontuberculous mycobacteria (NTM), which are opportunistic environmental pathogens primarily associated with chronic lung disease. Among current therapeutic options, tigecycline has emerged as a promising alternative for treating NTM due to its broad spectrum of activity, including efficacy against certain mycobacterial species. In this review, we compiled 28 studies investigating the mechanisms and prevalence of tigecycline resistance in NTM. The evidence suggests that tigecycline is less impacted by the monooxygenase MabTetX, the principal tetracycline resis

BACKGROUND: Non-tuberculous mycobacteria (NTMs) are environmental opportunistic pathogens capable of producing tuberculosis-like lesions in bovines, potentially complicating the diagnosis and surveillance of bovine tuberculosis. Molecular data on resistance-associated genes in bovine NTMs from India remain limited. This study aimed to isolate and characterize NTMs from bovine clinical samples and to investigate selected resistance-associated genes at molecular level.
METHODS AND RESULTS: A total of 193 samples comprising faeces (n = 101), nasal swabs (n = 21), trans-tracheal washes (n = 13),

Chronic granulomatous disease (CGD) is the most common inherited phagocytic Disorder with an increased susceptibility to recurrent infections and inflammatory manifestations due to a defect in any of the 5 NADPH oxidase subunits. Although CGD may manifest at different ages based on the defects of NADPH oxidase proteins, it is usually diagnosed in childhood with an overall median age of 2.7 to 3 years at the onset. Infections and inflammatory manifestations are known to be two major clinical presentations of CGD, with the infections occurring much earlier in life than the inflammatory manifesta

BACKGROUND: Tuberculosis (TB) remains a major global public health challenge, with Indonesia accounting for a substantial proportion of global incident disease. Although previous Indonesian studies have demonstrated the prevalence and consequences of TB-related stigma, less is known about how people receiving TB treatment experience the interconnected processes of internalised, family and public/social stigma, and how they understand the factors driving stigmatization in everyday life.
AIM: This study explored stigma and stigmatization experienced by people with TB receiving treatment in two
