BACKGROUND: Multiple myeloma (MM) is preceded by monoclonal gammopathy of undetermined significance (MGUS), an asymptomatic premalignant condition. There are no recommendations for MGUS screening due to a lack of evidence regarding screening benefits. MGUS screening, however, could lead to early detection and timely treatment for MM. The decision to screen should balance potential benefits against harms, including overdiagnosis, which we defined as screening-detected MGUS that would not progress to MM during an individual's lifetime.
METHODS: We constructed MM natural history micro-simulation

Monoclonal gammopathy of undetermined significance (MGUS), smouldering multiple myeloma (SMM) and asymptomatic Waldenström's macroglobulinaemia (aWM) are precursor conditions with variable risk of progression to symptomatic malignancy. We aimed to characterize the asymptomatic monoclonal gammopathies in younger patients among 1237 consecutive patients of which 12.1% were <50 years, while 2% were <40 years. Among patients <50 years, most had MGUS and a female predominance was seen. Of patients <40 years, 71% were females, 50% had SMM and 50% MGUS. Patients <50 progressed with either SLiM criter

Alterations in the redox status and the DNA damage response (DDR) parameters are early, mechanistically interconnected drivers of carcinogenesis. Herein, we investigated whether such alterations, arising during the progression of Multiple Myeloma (MM), are systemically reflected in peripheral blood mononuclear cells (PBMCs). Redox status, expressed as the GSH/GSSG ratio, and DDR-related parameters, including baseline DNA damage, efficiency of key DNA repair pathways, namely nucleotide excision repair (NER) and double-strand break repair (DSB/R), as well as apoptotic sensitivity, were evaluated

BACKGROUND AND AIMS: Chronic idiopathic axonal polyneuropathy (CIAP) accounts for approximately 20%-30% of adult-onset axonal polyneuropathies. Pathogenic RFC1 repeat expansions have emerged as a frequent cause of idiopathic sensory neuropathy, but their recognition in routine clinical practice may be challenging, particularly in the presence of potentially confounding comorbidities. We aimed to determine the prevalence of pathogenic RFC1 repeat expansions in a well-defined CIAP cohort, characterize the associated clinical and electrophysiological phenotype, and evaluate whether coexisting wel

Monoclonal gammopathy of undetermined significance (MGUS) is an asymptomatic premalignant precursor to multiple myeloma (MM). While development of end-organ damage in MM largely reflects increasing clonal burden, emerging evidence indicates that qualitative properties of the monoclonal immunoglobulins in MGUS can be directly pathogenic, leading to serious organ injury independent of disease burden. This has led to the recognition of a broader spectrum of disorders collectively termed monoclonal gammopathy of clinical significance (MGCS). MGCS can be further subclassified based on the organ sys

Monoclonal gammopathy of undetermined significance (MGUS) is a common asymptomatic plasma cell disorder and the major precursor state of multiple myeloma (MM). Although progression is partly shaped by intrinsic genetic and epigenetic changes within the plasma cell clone, increasing evidence indicates that the bone marrow microenvironment plays a decisive role in determining whether the clone remains clinically silent or evolves toward symptomatic disease. This review examines how the marrow niche changes across the MGUS-SMM-MM continuum. In early MGUS, the microenvironment may already be permi

We describe a patient who has both fibrous dysplasia (FD) and monoclonal gammopathy of undetermined significance (MGUS). The simultaneous occurrence of these two pathological conditions is rare, and it is necessary to distinguish whether the bones are being affected by FD or multiple myeloma. The patient's problems began with a non-healing tooth extraction, raising suspicion of osteomyelitis. Later, he was also examined for fever of unknown origin, as he had elevated inflammatory markers, low-grade fever, and headaches. According to skeletal scintigraphy, the patient had a craniofacial skeleto

We report a complex case of neutrophilic dermatosis (ND) in a 61-year-old woman with a 19-year history of recurrent, pruritic, and painful vesiculopustular eruptions. Her clinical course was marked by evolving diagnoses, from pustular vasculitis to IgA pemphigus and pyoderma gangrenosum (PG). Concurrent investigations revealed an IgA-κ type monoclonal gammopathy of undetermined significance (MGUS). This case highlights the rare association between neutrophilic dermatoses (ND) and haematological disorders, and illustrates the therapeutic challenges posed by comorbidities such as osteoporosis an

Disseminated mucormycosis is a rare but highly fatal invasive fungal infection, typically occurring in immunocompromised hosts. We report a case of culture- and PCR-confirmed disseminated mucormycosis in a previously healthy 25-year-old male who developed sarcoidosis and IgG-kappa monoclonal gammopathy of undetermined significance after contracting COVID-19. The patient presented with progressive respiratory symptoms and later developed multi-organ involvement, including the lungs, brain, kidneys, eyes, heart, lymph nodes, prostate, and vocal cords.

BackgroundImaging has a key diagnostic role in monoclonal plasma cell disorders. It can be used not only to detect osteolytic skeletal lesions but also for the detection of bone marrow involvement, extramedullary manifestations, impending complications, treatment response and the relapse assessment. Whole-body computed tomography (CT) has replaced the conventional skeletal survey, while whole-body magnetic resonance imaging (MRI) and 18F-fluorodeoxyglucose positron emission tomography-CT (FDG PET/CT) supplement the morphological skeletal assessment by depicting the bone marrow and functional-m

PURPOSE: To characterize the complication profiles and implant survivorship of multiple myeloma (MM) and monoclonal gammopathy of undetermined significance (MGUS) patients undergoing primary hip and knee total joint arthroplasty (TJA). METHODS: A retrospective analysis was conducted of MM and MGUS patients who underwent TJA at our tertiary academic center from 2005-2023. Included patients had a history of MM or MGUS prior to undergoing primary TJA, with a minimum of year post-operative follow-up. The primary outcomes were perioperative or post-operative complications and need for revision surg

Hyperviscosity syndrome (HVS) is an oncologic emergency most commonly associated with Waldenstrom macroglobulinemia and immunoglobulin (Ig)M paraproteinemia. Clinically, significant HVS in smoldering multiple myeloma (SMM), particularly IgG-associated disease without overt CRAB (hypercalcemia, renal insufficiency, anemia, and bone lesions) criteria, is exceedingly rare. We report a unique case of IgG kappa smoldering myeloma complicated by symptomatic HVS, manifesting with retinal hemorrhage, epistaxis, and gross hematuria, despite the absence of overt end-organ myeloma-defining events.

L-Periaxin is a PDZ-domain protein localized to the plasma membrane of myelinating Schwann cells and plays a key role in the stabilization of mature myelin in peripheral nerves. Mutations in L-periaxin have recently been described in some patients with demyelinating peripheral neuropathy, suggesting that disruption of L-periaxin function may result in nerve injury. In this study, we report the presence of autoantibodies to L-periaxin in sera from two of 12 patients with diabetes mellitus (type 2)-associated neuropathy and three of 17 patients with IgG monoclonal gammopathy of undetermined sign

BACKGROUND: Retinal ischemia is a retinal disorder related to retinal vascular occlusion, glaucoma, diabetic retinopathy and age-related macular degeneration. The study aimed to evaluate the protective effects and underlying mechanisms of Chi-Ju-Di-Huang-Wan (CJDHW) against retinal ischemia in rats.
METHODS: High intraocular pressure (HIOP)-induced retinal ischemia was established in Wistar rats by raising their intraocular pressure to 120 mmHg for 60 min with in an eye whose anterior chamber was cannulated with a 30-guage needle adapted to a normal saline bottle through an intravenous line.

BACKGROUND: Monoclonal gammopathy of undetermined significance (MGUS) and multiple myeloma (MM) are reported to occur more frequently in Black populations, particularly African Americans. However, despite Sub-Saharan Africa (SSA) being home to the largest Black population globally, epidemiological data on MGUS and MM in the region remain scarce.
METHODS: We conducted a systematic review and meta-analysis of published studies from SSA to estimate the pooled prevalence of MGUS and MM, regional variations, survival outcomes and key clinical complications, including HIV-associated MM, renal failu

Immunoglobulins (Ig) are essential for protection against invading pneumococci but are not routinely analyzed in patients with invasive pneumococcal disease (IPD). In this prospective study, we assessed the prevalence of monoclonal Ig (M protein) and analyzed Ig concentrations in sera from 156 adult IPD patients (median age 70 years). Patients were sampled during acute infection (n = 153) and in the convalescence phase 2-4 months after acute infection (n = 76). Sixty-four sex- and age-matched individuals without IPD served as controls. During infection, M protein was detected in 22% (31/141) o

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr
