BACKGROUND: Genetic alterations play a pivotal role in multiple myeloma (MM) development and therapeutic resistance. Traditionally, the genetic profiling of MM requires invasive bone marrow (BM) procedures; however, these procedures are associated with patient discomfort and cannot fully capture the spatial and temporal heterogeneity of the disease. Therefore, we investigated the clinical implications of liquid biopsy using targeted deep sequencing.
METHODS: We analyzed the genetic profiles of circulating tumor DNA (ctDNA) by targeted deep sequencing from 102 patients, including those with mo

INTRODUCTION: CD20+ T-cells were described firstly in peripheral blood and later in bone marrow in patients with hematological tumors, and certain immune-mediated diseases. During our hematological diagnostic work, this peculiar subgroup of lymphocytes has been consistently observed associated with untreated monoclonal gammopathy of undetermined significance (MGUS) and myeloma (MM). Despite the expanding literature data, the exact function of CD20+ T cells remains unclear.
METHODS: We investigated the incidence of CD20+ T-cells in MGUS (n=27), and MM using a larger cohort (n=125) and compared

This focused review concentrates on eight topics of high importance for Gaucher disease (GD) clinicians and researchers: 1) The consideration of GD as distinct types rather than a spectrum. A review of the literature clearly supports the view that there are distinct types of GD. Type 1 is characterized by the absence of primary neuronopathic involvement, while types 2 and 3 are characterized by progressive primary neuronopathic disease. 2) Neurologic and neuronopathic manifestations. A growing body of evidence indicates that the peripheral nervous system may be involved in GD type 1 and that t

UNLABELLED: Multiple myeloma is preceded by monoclonal gammopathy of undetermined significance (MGUS). Only a minority of patients with MGUS will develop multiple myeloma, but precise prediction of progression is impossible using routine clinical biomarkers. Changes in the levels of blood immune markers can help predict disease progression. Data remain inconsistent for some markers of interest such as monocyte chemotactic protein-3 (MCP-3), macrophage inflammatory protein-1 alpha (MIP-1α), fibroblast growth factor-2 (FGF-2), vascular endothelial growth factor (VEGF), fractalkine, and transform

Multiple myeloma (MM) is a plasma cell (PC) malignancy that is preceded by monoclonal gammopathy of undetermined significance (MGUS) and/or smoldering multiple myeloma (SMM). MGUS and SMM PCs exhibit the same primary oncogenic abnormalities as MM but lack the end-organ damage that defines proliferative disease, suggesting that clonal PCs in these precursor conditions could exhibit senescence or senescence-like growth arrest. Herein we identified monoclonal gammopathy patient-derived PCs that exhibit senescence features and found that senescent PCs were significantly increased in MGUS patients

The risk of progression of monoclonal gammopathy of undetermined significance (MGUS) to multiple myeloma (MM) increases with advancing age, suggesting that progression may be influenced by age-related changes within the bone marrow (BM) microenvironment. We hypothesise that senescent mesenchymal stromal cells (MSCs), which accumulate in the BM with age, may contribute to MGUS progression to MM. Here, we show that, like BM MSCs from aged non-cancer controls, BM MSCs from both MM and MGUS patients exhibit a senescent phenotype characterised by enlarged, flattened morphology, increased β-galactos

The free light chain (FLC) ratio is a critical part of risk stratification for monoclonal gammopathy of undetermined significance (MGUS). Recently, revised FLC reference intervals developed using the iStopMM cohort, accounting for age and renal function, have reduced the rate of abnormal findings. Here, we examine the implications of the revision in an independent Danish MGUS cohort. Of 6993 MGUS individuals, 2641 had an abnormal FLC ratio by the original intervals, of whom 844 (32%) were reclassified as normal using the revised intervals. Reclassified individuals had no significantly increase

Diffuse normolipidemic plane xanthoma is a rare acquired dermatosis of the non-Langerhans histiocytosis subtype, often associated with underlying hematologic disorders, with half of the reported cases linked to lymphoproliferative disorders. This case report describes a 52-year-old woman presenting with asymptomatic, yellowish plaques symmetrically distributed on the eyelids, cervical region, axillae, upper back, and upper thighs, which had evolved over a two-year period (images of the eyelids and upper back are not provided). Skin biopsy was consistent with the diagnosis of plane xanthoma. La

Variegate porphyria (VP) is a rare disorder presenting with cutaneous, neurological, and systemic symptoms. We report a 53-year-old man initially hospitalized for a depressive episode, during which blistering lesions and facial flushing were noted. He was misdiagnosed and treated for cellulitis and contact dermatitis without improvement. Repeated misdiagnoses led to extensive autoimmune, infectious, and hematologic workup, revealing coexisting monoclonal gammopathy of undetermined significance (MGUS). Over the following months, he developed abdominal pain, mood swings, cognitive impairment, ne

BACKGROUND: Peripheral neuropathy can be associated with certain monoclonal gammopathies; therefore, monoclonal protein testing is routinely performed in the evaluation of this symptom. However, limited data exist regarding the utility of monoclonal protein testing for this indication. Data suggest those receiving a new monoclonal gammopathy of undetermined significance (MGUS) diagnosis have similar rates of diagnosis-related anxiety when compared to those receiving a myeloma diagnosis (Maatouk et al., Blood Cancer J 9(2), 2019; Adami et al., Eur J Clin Invest 49(3), 2019; Rögnvaldsson et al.,

Incidence of multiple myeloma is about 1.8/100 000. About 3%-5% of adults >50 years of age are diagnosed with monoclonal gammopathy of undetermined significance (MGUS). The prevalence is higher if diagnosis is based on an abnormal ratio of serum free light chains. 300-500 people/10 000 get labelled with a diagnosis of this pre-malignant disorder by using faulty criterion of abnormal light chain ratio. Diagnoses based on an abnormal light chain ratio are incorrect in 96% of instances. However, a large number of people are subjected to psychological trauma and financial toxicity by subjecting th

IgG4-related disease (IgG4-RD), an immune-mediated fibroinflammatory disorder, has few reports in combination with monoclonal gammopathy of undetermined significance (MGUS). Herein, we present a case of a 69-year-old woman with manifestations of left orbital occupation and visual acuity decline. Ancillary tests indicated persistent positivity of IgG4 antibody, and IgG4-RD-related ophthalmopathy was diagnosed based on the criteria. Concurrently, serum protein electrophoresis revealed an M protein level of 12.23 g/L. Immunofixation electrophoresis suggested a positive IgG λ-type M protein, and M

INTRODUCTION: Alterations in the bone marrow (BM) tumor microenvironment (TME) are closely associated with the progression of premalignant clonal plasma cells in monoclonal gammopathy of undetermined significance (MGUS) to malignant clonal plasma cells in multiple myeloma (MM), though the extent to which these alterations are causative remains unclear. Whether lipidomic changes are part of the BM TME alterations associated with this disease progression remains poorly understood.
OBJECTIVES: To investigate and compare the levels of individual lipid species across different lipid classes in the

BackgroundMultiple myeloma (MM) is virtually always preceded by monoclonal gammopathy of undetermined significance (MGUS). Elevated serum markers are used to classify MGUS patients into clinical risk categories. Previous research has indicated that the absence of a normal plasma cell signature in MGUS is linked to early progression.ObjectiveTo confirm that the presence of a "polyclonal plasma cell (PolyPC) signature" serves as a robust negative predictor of MGUS progression.Methods374 MGUS patients were enrolled, including 334 patients with stable disease and 40 patients who progressed to MM w

Hematologic precursor conditions include monoclonal gammopathy of undetermined significance (MGUS), monoclonal B-cell lymphocytosis (MBL), and clonal hematopoiesis (CH). These conditions are characterized by a clonal expansion of either plasma cells, lymphoid cells, or myeloid cells without meeting criteria for active hematologic malignancy and are considered premalignant conditions. Diagnosis of these precursor conditions often occur incidentally based on abnormalities on routine laboratory assessments, such as an elevated globulin level leading to diagnosis of MGUS or smoldering multiple mye

Monoclonal gammopathy of undetermined significance (MGUS) is the term used for a minimal clonal expansion of plasma cells that is most typically detected by the presence of a small serum monoclonal protein. For most-but not all-patients, the presence of a small monoclonal protein will have no clinical significance at the time of recognition or even over time. It is imperative, therefore, that a careful-but sensible-evaluation be performed both at the time of recognition and subsequently because these monoclonal may represent significant pathology including diseases such as multiple myeloma, lo
