FDX1 is associated with CALR abundance and JAK-STAT-related inflammatory signaling in myeloproliferative neoplasms.

INTRODUCTION: Myeloproliferative neoplasms (MPNs) are driven by constitutive cytokine-receptor signaling, yet the metabolic programs associated with driver genotype and inflammatory remodeling remain incompletely defined. METHODS: Here we integrated a 60-case clinical MPN cohort, the GSE174060 Affymetrix Human Transcriptome Array 2.0 microarray dataset, immune deconvolution, exploratory feature prioritization, the GSE228995 mouse single-cell RNA sequencing (scRNA-seq) dataset, human-sample validation, and functional perturbation assays to investigate whether cuproptosis-related metabolism is

Outcome heterogeneity in CALR-mutated myeloproliferative neoplasms.

Calreticulin gene (CALR) mutations are seen in 25%-30% of myeloproliferative neoplasms (MPNs). We report outcomes and high-risk genomic signatures in 149 patients with CALR-mutated MPN. CALR-mutated essential thrombocythaemia (n = 106) progressed to myelofibrosis (n = 40) at a median of 20.4 years, and 20-year overall survival (OS) was 81%. In 83 patients with myelofibrosis, the median leukaemia-free survival (LFS) was 12.9 years, and OS was 13.9 years. Median survival after leukaemia transformation was 6.8 months. High molecular risk (HMR) mutations included ASXL1, EZH2, SRSF2, U2AF1 (Q157),

Targeted literature review to understand the patient experience of essential thrombocythemia and evaluate existing clinical outcome assessment measures.

BACKGROUND: The use of Clinical Outcome Assessments (COA) instruments in blood cancers such as myeloproliferative neoplasms (MPN) is widely documented, however little is known about Essential Thrombocythemia (ET). A targeted literature review was conducted to identify the concepts of interest to the ET population and to develop a conceptual framework. Using this framework, identification and review of COA instruments will help determine if there are existing instruments fit for purpose for use in ET studies. METHODOLOGY: Targeted reviews in May and October 2023 synthesized the literature on E

Efficacy and safety of ropeginterferon in myeloproliferative neoplasms: a systematic review and meta-analysis.

BACKGROUND: Ropeginterferon alfa-2b (ropegIFN-α2b or ropeginterferon) has emerged as a therapeutic option for patients with myeloproliferative neoplasms, particularly polycythemia vera (PV), with potential disease-modifying effects. This systematic review aimed to evaluate the efficacy and safety of ropeginterferon across clinical and real-world studies. METHODS: A systematic search of PubMed, Embase, and Web of Science was conducted to identify studies reporting clinical outcomes of ropeginterferon in patients with MPNs. An additional PubMed search was performed to capture recently published

A historical review of the biological, semantic and clinical aspects of aspirin resistance.

Aspirin is one of the most widely used antithrombotic agents worldwide. Nevertheless, the concept of aspirin resistance has generated sustained controversy for more than 2 decades. Initially introduced in the mid-1990s, the term was inconsistently applied to describe either recurrent ischemic events, despite aspirin therapy, or inadequate platelet inhibition, as measured using various laboratory assays. Reported prevalence estimates consequently varied widely, reflecting profound methodological and semantic heterogeneity rather than a uniform biological concept. This historical review retraced

CO-Rebreathing in Essential Thrombocythemia and Polycythemia Vera. A Revival of an Old Methodology to Improve Diagnosis and Phlebotomy Decisions.

The diagnostic landscape for the Philadelphia-negative myeloproliferative neoplasms (MPNs), particularly in distinguishing Essential Thrombocythemia (ET) from Polycythemia Vera (PV), is compromised. The gold standard for diagnosing PV-measurement of red cell mass (RCM) by radioisotope dilution-has been largely abandoned, leading to widespread misclassification. Thus, an estimated 30%-50% (varying by diagnostic center and testing practice) of JAK2V617F-positive ET patients may, in fact, have "hidden" PV, depriving them of the cornerstone PV therapy: phlebotomy. This diagnostic inaccuracy expose

Impact of Metformin on the Incidence of Thrombosis in Older Adults with Polycythemia Vera or Essential Thrombocythemia and Diabetes.

Individuals with myeloproliferative neoplasms (MPNs) have an increased thrombotic risk, particularly in the presence of cardiovascular comorbidities such as diabetes (DM). Metformin (MET) has been associated with a reduced incidence of MPNs; however, data on its association with thrombotic outcomes in polycythemia vera (PV) and essential thrombocythemia (ET) are limited. We conducted a retrospective study using the Surveillance, Epidemiology, and End Results-Medicare-linked database to evaluate the association between MET use and thrombotic risk in patients with MPNs and DM. We included patien

Pediatric Essential Thrombocythemia With a Novel CALR Mutation: A Case Report.

UNLABELLED: Pediatric essential thrombocythemia (ET) is extremely rare and frequently lacks identifiable driver mutations. We report a case of a 5-year-old male with ET harboring a novel CALR exon 9 frameshift mutation complicated by acquired von Willebrand syndrome. The patient presented with purpura, extreme thrombocytosis (1900 × 109 /L), and reduced von Willebrand factor activity. Antiplatelet therapy was avoided because of bleeding risk, and anagrelide was initiated. Treatment was associated with reduction in platelet count and improvement in von Willebrand factor activity. This case expa

Obstetric and thrombotic outcomes among pregnant patients with myeloproliferative neoplasms: a propensity score matched real-world analysis.

Pregnancies complicated by BCR::ABL1-negative myeloproliferative neoplasms (MPNs) are associated with increased maternal and thrombotic risk; however, large real-world comparative data remain limited. We performed a multicenter retrospective propensity score-matched cohort study using the TriNetX database to evaluate maternal, obstetric, and thrombotic outcomes among pregnant patients with MPN compared with matched non-MPN controls. Baseline demographics, comorbidities, and antithrombotic medication use were balanced after matching (standardized mean difference < 0.1). Among 4,467,127 pregnanc

Impact of statin use on cardiovascular and hematologic outcomes among patients with myeloproliferative neoplasms.

Myeloproliferative neoplasms (MPNs), including polycythemia vera (PV), essential thrombocythemia (ET), and myelofibrosis (MF), are chronic myeloid neoplasms associated with increased risk of cardiovascular disease (CVD). Statins are a common group of cholesterol-lowering medications recommended for the primary and secondary prevention of CVD, including arterial thrombotic events. Emerging evidence suggests that statins may reduce the risk of developing MPNs and their use may be associated with improved survival. However, the effect of statins on cardiovascular and hematologic outcomes among pa

[Diagnostic work-up of erythrocytosis].

Finding increased red cell indices - elevated RBC count, hemoglobin, or hematocrit (erythrocytosis) - is common in medicine. The key question is whether this reflects a myeloproliferative neoplasm (polycythemia vera, primary myelofibrosis, or essential thrombocythemia) driven by mutations such as JAK2, because it can cause thrombotic or hemorrhagic complications or progress to myeloid leukemia. We propose a diagnostic pathway for erythrocytosis that outlines the principal causes to exclude by targeted history and physical examination and incorporates the TRAKJAK score to predict JAK2 mutation

Understanding Triple Negative Myeloproliferative Neoplasms and Identifying Molecular Drivers.

PURPOSE OF REVIEW: Triple-negative (TN) myeloproliferative neoplasms (MPNs), defined by the absence of canonical driver JAK2, CALR and MPL mutations, represent a heterogeneous and still poorly understood subgroup of chronic myeloid neoplasms. This review summarizes current knowledge on characteristics and molecular landscape of TN MPNs. RECENT FINDINGS: Although TN MPNs share clinical and morphological features with classic forms of primary myelofibrosis and essential thrombocythemia, their clinical behavior varies widely, from indolent in TN essential thrombocythemia to aggressive in TN myelo

Novel strategies targeting mutant calreticulin in essential thrombocythemia and myelofibrosis.

The discovery of calreticulin (CALR) mutations in patients with myeloproliferative neoplasms (MPNs) has paved the way for the elucidation of a unique disease mechanism that is particularly well suited to targeting by biologics. All MPN-associated pathogenic CALR mutations are characterized by a frameshift, resulting in translation of the same neoantigen peptide. This neoantigen directly activates the thrombopoietin receptor, leading to uncontrolled neoplastic cell proliferation. Current therapeutic approaches for MPNs are focused primarily on blood count control. Furthermore, current approache

Exposure-Response Analyses of Ropeginterferon Alfa-2b in Essential Thrombocythemia.

Ropeginterferon alfa-2b is a new-generation interferon therapy for the treatment of myeloproliferative neoplasms (MPNs), including essential thrombocythemia (ET). This investigation was designed to characterize its population pharmacokinetics-pharmacodynamics (PopPK-PD) and delineate exposure-response (E-R) relationships in ET. A population PK model was constructed utilizing aggregated data from five clinical trials, encompassing a study population of Asian and Caucasian patients with ET and healthy volunteers. A sequential modeling strategy was employed to evaluate the PK-PD with respect to k

Polycythemia Vera and Essential Thrombocythemia: A Nationwide Population-Based Study on Treatment Patterns, Vascular Complications and Survival.

BACKGROUND: Polycythemia vera (PV) and essential thrombocythemia (ET) are associated with a well-recognized increased risk of thrombotic events, bleeding, and all-cause mortality, but the frequency of these outcomes during treatment has rarely been assessed in large cohorts. METHODS: In this nationwide, population-based study, we analyzed 2604 PV and 3141 ET patients using multiple Swedish health care registers, covering 43 612 patient-years. Rates of arterial and venous events, major bleeding, and all-cause mortality (ACM) were evaluated across therapies. RESULTS: Unexpectedly, 42.3% of low

Quantification of plasma mutant calreticulin for diagnosis and therapeutic response assessment of peginterferon-α in myeloproliferative neoplasms: A proof-of-concept study.

BACKGROUND: Calreticulin (CALR) exon 9 mutations are drivers in essential thrombocythemia (ET) and primary myelofibrosis (PMF). These mutations lead to extracellular secretion of mutant CALR, but a quantitative plasma assay to leverage this for management is lacking. OBJECTIVE: To develop and validate a quantitative method based on homemade monoclonal antibody (McAb) targeting mutant CALR for diagnosing ET and PMF, and evaluating therapeutic response in PEG-IFN-α-treated ET patients. METHODS: We generated a hybridoma clone (Cal26B11) producing anti-mutant CALR McAb and confirmed its reactivi

Myeloid-Specific Heparanase Aggravates Insulitis in Type 1 Diabetes via Heparan Sulfate Fragment-Dependent Amplification of Macrophage Polarization.

UNLABELLED: The intraislet heparan sulfate (HS) barrier is essential for β-cell survival, yet its role in type 1 diabetes (T1D) remains incompletely understood. We investigated the expression pattern, pathogenic function, and therapeutic potential of the heparanase (HPSE)-HS axis in T1D by integrating single-cell RNA sequencing data sets from human and mouse islets and peripheral immune cells, together with genetic mouse models and pharmacological intervention. HPSE was selectively enriched in myeloid cells, particularly CD14++ classical monocytes from patients with T1D and NOD mice and was as

Neutrophil to lymphocyte ratio at diagnosis predicts venous thrombosis in prefibrotic primary myelofibrosis: results from a multicenter cooperative study.

Primary myelofibrosis (PMF) is a myeloproliferative neoplasm (MPN) characterized by splenomegaly, symptoms, cytopenias, and chronic inflammation. PMF has two stages: pre-fibrotic (prePMF) and overt PMF. PrePMF and essential thrombocythemia share a similar high thrombotic risk, but few studies have examined thrombosis risk factors in prePMF. The neutrophil-to-lymphocyte ratio (NLR), reflecting the imbalance between systemic inflammation and immunity, has emerged as a prognostic biomarker in various diseases. We investigated the predictive value of NLR for thrombotic risk in a multicenter cohort

CD34-positive circulating cells quantification during follow-up in myeloproliferative neoplasms.

Polycythemia vera (PV) and Essential thrombcythemia (ET) are myeloproliferative neoplasms (MPNs) that can progress to secondary myelofibrosis, a complication associated with increased mortality. Circulating CD34-positive cells at diagnosis can be used to distinguish primary myelofibrosis from other MPNs with a threshold of 10/µL. In this study, we evaluate the interest of serial CD34-positive cell quantifications during the follow-up of MPNs. We retrospectively include 180 patients with MPN (90 ET, 55 PV and 35 myelofibrosis) with at least two measurements of circulating CD34-positive cells. C
