Thrombocytosis: Rapid Evidence Review.

Thrombocytosis is defined by guidelines as a platelet count of 450 × 103/μL or greater. It most often results from secondary causes and should resolve with treatment of the underlying condition. Primary thrombocytosis in adults is most commonly due to myeloproliferative neoplasms, including essential thrombocythemia. Thrombocytosis may be identified incidentally in asymptomatic patients or when performing blood tests to investigate symptoms or signs that could indicate microvascular conditions (eg, headaches, paresthesia, erythromelalgia), splenomegaly, thrombosis, and hemorrhage, which are pr

Impact of Atrial Fibrillation on Thrombotic Complications in Myeloproliferative Neoplasms Real-World Analysis.

BACKGROUND: Philadelphia chromosome-negative myeloproliferative neoplasms (MPN) are clonal hematopoietic disorders characterized by an increased risk of thrombotic complications due to a persistent pro-thrombotic and inflammatory state. Atrial fibrillation (AF) is a common cardiac arrhythmia and an established risk factor for thromboembolism; however, its impact on thrombotic complications in patients with MPN remains incompletely characterized. MATERIAL AND METHODS: This multicenter, real-world retrospective study analyzed 2,683 MPN patients diagnosed between 1976 and 2024 to investigate the

Pre-transplant JAK-inhibition improves post-transplant outcome in myelofibrosis.

Allogeneic hematopoietic stem cell transplantation (alloHSCT) remains the only curative approach in myelofibrosis (MF). Janus-Associated Kinase inhibitors (JAKi) have changed the treatment paradigm of MF patients, becoming a therapeutic mainstay in those with splenomegaly and/or constitutional symptoms. The impact of JAKi prior to allogeneic transplant is still under investigation. The aim of the current study was to analyze the outcome and prognostic factors, especially JAKi impact on transplantation outcome in MF patients. The study comprised 96 patients (64 - MF, 21 - post-polycythemia vera

Clinical Associations of Osteoporosis in BCR::ABL1 Negative Myeloproliferative Neoplasms.

BACKGROUND/AIM: Osteoporosis can be seen in some hematologic diseases, such as BCR::ABL1-negative myeloproliferative neoplasms. The incidence and prevalence of osteoporosis and osteopenia in BCR::ABL1-negative MPN are not very well known. MATERIALS AND METHODS: Patients diagnosed with one of the classical BCR::ABL1-negative myeloproliferative neoplasms, such as essential thrombocythemia, polycythemia vera, and myelofibrosis, were included in the study. Due to the retrospective design, a matched control group was not available, and comparisons with the general population were made for contextu

Incidence, Prevalence, and Survival Outcomes of Patients with Myeloproliferative Neoplasms in the United States: a SEER database analysis, years 2000-2021.

Myeloproliferative Neoplasms (MPNs) are rare hematologic neoplasms. The epidemiology and outcomes of these rare neoplasms warrant an update considering new diagnostic tools and advances in treatment options. NCI's Surveillance, Epidemiology and End Result (SEER)-17 registries were interrogated to analyze adult patients diagnosed with MPN [ICD-O-3 codes: polycythemia vera (PV) 9950/3, essential thrombocythemia (ET) 9962/3, primary myelofibrosis (PMF) 9961/3, and chronic myeloid leukemia (CML) 9875/3] during the years 2000 to 2021. Data of 63,242 (ET, n=24 172; PV, n= 23 456; PMF, n=6 131; CML,

Acute STEMI in a young adult without conventional risk factors: a case report of JAK2-positive essential thrombocythemia.

BACKGROUND: The prevalence of acute coronary syndrome (ACS) in young adults (<40 years) is rising, with a substantial proportion attributable to non-atherosclerotic causes. One cause is essential thrombocythemia (ET), a myeloproliferative neoplasm (MPN) characterized by increased risk of thrombosis and bleeding. Recognizing ET as an aetiology of ACS is clinically important, as it alters acute and long-term management. This case highlights myocardial infarction as the presenting event of JAK2-positive ET in a young patient without traditional risk factors. CASE SUMMARY: A 31-year-old man with

Myeloproliferative Neoplasms, Clonal Thrombosis, and Vascular Disease: Can Cardiology Learn from Hematology?

Philadelphia-negative myeloproliferative neoplasms (MPNs), encompassing polycythemia vera, essential thrombocythemia, and primary myelofibrosis, represent a group of chronic clonal hematopoietic disorders characterized by excessive proliferation of myeloid lineages and with a propensity for arterial and venous thrombosis. These disorders provide a unique human model of inflammation-driven, clone-mediated vascular disease with mechanistic insights highly relevant to cardiovascular medicine. The interplay of elevated hematocrit, leukocytosis, activated platelets, neutrophil extracellular traps,

Impact of echocardiographic hemodynamics on outcomes among patients with polycythemia vera or essential thrombocythemia.

INTRODUCTION: Pulmonary hypertension (PH) and high-output heart failure (HOHF) are associated with myeloproliferative neoplasms (MPNs), including essential thrombocythemia (ET) and polycythemia vera (PV). Transthoracic echocardiography (TTE) is a non-invasive modality that can be used for the screening of PH and HOHF. However, the impact of echocardiographic HOHF on TTE on outcomes of patients with ET or PV is unclear. METHODS: This was a multicenter, retrospective cohort study of patients with ET or PV with ≥ 1 TTE. Estimated pulmonary artery systolic pressure (PASP) and cardiac index were c

JAK2 V617F as a diagnostic and prognostic biomarker in myeloproliferative neoplasms.

Molecular analysis of BCR-ABL1-negative myeloproliferative neoplasms has shown a shared codon 617 substitution of JAK2 that leads to a constitutively active tyrosine kinase and a pathogenic lesion that unites all three polycythemia vera, essential thrombocythemia, and primary myelofibrosis. This mutation interferes with the autoinhibitory regulation and results in persistent downstream JAK-STAT signaling, and thereafter, myeloid, megakaryocytic, and inappropriate erythroid proliferation. JAK2 V617F has emerged as a major molecular biomarker in the diagnostic workup and classification of myelop

Recurrent multilevel postoperative spinal epidural hematoma in a patient with essential thrombocythemia: failure of repeated decompression and hematological correction. Illustrative case.

BACKGROUND: Postoperative spinal epidural hematoma (SEH) is an uncommon but serious complication after lumbar endoscopic surgery, with an incidence of 0.1%-0.2%. Although percutaneous endoscopic lumbar discectomy (PELD) minimizes tissue disruption, SEH may still occur, particularly in patients with hematological disorders. Essential thrombocythemia (ET) presents a hemostatic paradox of simultaneous bleeding tendency and hypercoagulability, increasing postoperative hematoma risk. Massive multilevel SEH after PELD is rare. OBSERVATIONS: A patient developed severe left leg pain immediately after

Distinct clinical, molecular, and treatment response profiles in primary and secondary myelofibrosis: a single-center retrospective study.

BACKGROUND: Myelofibrosis (MF) includes primary myelofibrosis (PMF) and secondary myelofibrosis (SMF) evolving from essential thrombocythemia (ET) or polycythemia vera (PV). The clinical and molecular heterogeneity of MF remains incompletely characterized in real-world cohorts. METHODS: We retrospectively analyzed 56 patients with MF treated at a single center, including 33 with PMF and 23 with SMF (ET-MF or PV-MF). Clinical characteristics, laboratory parameters, bone marrow pathology, cytogenetic and molecular findings, treatment strategies, and follow-up outcomes were evaluated. RESULTS:

Cardiovascular complications in patients with myeloproliferative neoplasms: What hematologists need to know.

Patients with myeloproliferative neoplasms (MPNs), including essential thrombocythemia, polycythemia vera, and myelofibrosis, are at increased risk of cardiovascular disease and complications. The most recognized cardiovascular complications among patients with MPN are arterial and venous thrombotic events. However, mounting evidence suggest that nonthrombotic cardiovascular complications, including heart failure and pulmonary hypertension, are also common and are associated with adverse outcomes and overall prognosis. Therefore, it is important that hematologists become aware of such complica

Optimized post-GWAS analysis identifies therapeutic targets for essential thrombocythemia and polycythemia vera.

BACKGROUND: Limited insight into the biology of essential thrombocythemia (ET) and polycythemia vera (PV) has constrained therapeutic development. We therefore aimed to prioritize genetically supported molecular determinants underlying disease susceptibility and hematologic phenotypes, and to distinguish shared and subtype-specific regulation. METHODS: We implemented an optimized post-GWAS framework combining omics-wide Bayesian colocalization with Mendelian randomization (MR) to prioritize genetically supported molecular determinants of ET and PV. Colocalization-guided MR analyses were used

Ropeginterferon alfa-2b-njft treatment in essential thrombocythemia across different driver mutations: results from a North American, single-arm, multicentre study (EXCEED-ET).

BACKGROUND: Ropeginterferon alfa-2b-njft (ropeg), a mono-PEGylated interferon-α, showed efficacy and safety in patients with hydroxyurea-intolerant/resistant essential thrombocythemia (ET) in the phase 3 SURPASS-ET largely conducted in Asia. Here we report the results from the EXCEED-ET study, which evaluated ropeg in both treatment-naïve and hydroxyurea pre-treated ET in North America. METHODS: The single-arm, multicentre, phase 2b trial EXCEED-ET was conducted at 28 centers in the United States and Canada. Patients with ET enrolled were required to have a platelet count >450 × 109/L at scre
