Myeloproliferative neoplasms in the young: unique disease patterns and therapeutic strategies.

PURPOSE OF REVIEW: 10-20% of myeloproliferative neoplasms (MPNs) are diagnosed in adolescent and young adult (AYA) patients. This review aims to summarize current literature on AYA MPN, to highlight unique clinico-pathological patterns, specific treatment paradigms and areas for further research. RECENT FINDINGS: Epidemiological data highlights the female predominance in AYA MPN, reflective of the higher burden of essential thrombocythemia (ET) within this population. Fatigue is common and can be severe but is frequently overlooked within the healthcare setting. Compared to older MPN, cytopen

Proteomics as a theranostic compass in BCR::ABL1-negative myeloproliferative neoplasms: Integrating biomarker discovery with therapeutic stratification.

Classic BCR::ABL1-negative myeloproliferative neoplasms (MPNs)-polycythaemia vera, essential thrombocythaemia, and primary myelofibrosis-are clonal haematopoietic stem cell disorders with marked heterogeneity in clinical phenotype, disease trajectory, and therapeutic response. Genomic stratification by driver and cooperating mutations only partially accounts for this variability, leaving gaps in predicting thrombotic risk, fibrotic progression, leukaemic transformation, and treatment benefit. Proteomics bridges this gap by providing function-proximal readouts of protein abundance, post-transla

Dysregulation of HIF-1α-regulated and thromboinflammatory genes may contribute to thrombotic risk in patients with BCR::ABL1-negative myeloproliferative neoplasms.

BACKGROUND: Hypoxia-inducible factor-1α (HIF-1α) regulates multiple pathways involved in metabolism, inflammation, and coagulation and may contribute to thrombotic complications in patients with BCR::ABL1-negative myeloproliferative neoplasms (MPN). OBJECTIVES: This study aimed to investigate the expression of selected HIF-1α-regulated and thromboinflammatory genes (SLC2A1, F3, SELP, VEGFA, SERPINE1, PROS1, NLRP3, and THBS1) and their association with thrombotic events, JAK2 mutation status, and cytoreductive therapy. METHODS: Data of 120 patients with MPN (40 with polycythemia vera, 40 with

Abnormal Eosinophils With Large, Distinctly Basophilic Granules (Harlequin Cells) on Peripheral Blood Smear: A Clue for Diagnosing Chronic Myeloid Leukemia.

BACKGROUND: Chronic myeloid leukemia (CML) often presents with hematologic findings that overlap with reactive leukocytosis and other myeloproliferative neoplasms (MPNs), creating diagnostic uncertainty that may delay targeted therapy or prompt unnecessary molecular testing. Harlequin cells-abnormal eosinophils containing basophilic granules-are well described in acute myeloid leukemia (AML) with CBFB::MYH11 fusion, but their diagnostic relevance in CML has not been systematically assessed. METHODS: We retrospectively reviewed 177 peripheral blood smears: 53 CML; 30 non-CML MPN and related di

Analysis of CALR-mutated essential thrombocythemia as a distinct disease entity compared with JAK2 V617F-mutated and triple-negative patients.

BACKGROUND/AIM: Calreticulin (CALR) mutations in essential thrombocythemia (ET) are associated with younger age, higher platelet counts, and lower thrombosis rates. The present study analyzes the demographic, laboratory, and clinical features of the CALR mutation and its prognostic impact. MATERIALS AND METHODS: The clinical impact of CALR mutations was assessed in 391 ET patients. RESULTS: CALR-mutation patients were more commonly male than JAK2 V617F-positive and triple-negative patients. Age at diagnosis was similar across all groups, although patients with type 2 CALR mutations were youn

BCL-XL drives fibrotic and leukemic progression in myeloproliferative neoplasms.

BACKGROUND: Myeloproliferative neoplasms (MPNs) are frequently accompanied by bone marrow fibrosis and leukemic transformation, yet the cellular and molecular mechanisms that sustain apoptosis resistance and fibrotic progression remain unclear. METHODS: Bone marrow mesenchymal stromal cells (BM-MSCs) from patients with polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF) were analyzed for fibrotic phenotype, apoptosis signaling, and pathway activation using cell counting kit-8, immunofluorescence, flow cytometry, western blotting, hydroxyproline and transmis

Risk of Transformation to Acute Myeloid Leukaemia and Myelodysplastic Syndromes in Patients With Myeloproliferative Neoplasms Over Attained Age and Time Since Diagnosis: A Nationwide Cohort Study.

BACKGROUND: Individuals with polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF) face transformation risks to acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS). While older age is linked to increased risk, it remains unclear whether risk increases with age or with disease duration. OBJECTIVES: To examine how age and disease duration affect AML and MDS transformation in MPN. METHODS: We used Swedish nationwide register data to model transformation rates as continuous functions of age and disease duration for each subtype and outcome. Cumulati

[Sequential development of BCR::ABL1-positive chronic myeloid leukemia during treatment of JAK2 V617F-positive essential thrombocythemia].

We report a rare case of JAK2 V617F-positive essential thrombocythemia (ET) followed by BCR::ABL1-positive chronic myeloid leukemia (CML) in a 47-year-old woman. Dasatinib induced deep molecular remission; however, persistent thrombocytosis required hydroxyurea. The persistence of the JAK2 V617F mutation despite BCR::ABL1 suppression suggests distinct clonal populations. This case underscores the importance of vigilance for secondary myeloproliferative neoplasms (MPNs) and clonal evolution when a new hematologic phenotype emerges in a patient with an established MPN. The coexistence of JAK2 V6

Dysregulation of the immune microenvironment in essential thrombocythemia: the interplay of core genes and inflammation-related signaling pathways.

BACKGROUND: Essential thrombocythemia (ET) is a myeloproliferative disorder characterized as excessive platelet production. Early and accurate diagnosis is critical to manage the disease and prevent progression to serious myeloid neoplasms like myelofibrosis or acute myeloid leukemia, though definitive diagnosis remains challenging. MATERIALS AND METHODS: We retrieved seven publicly available microarray datasets from GEO database. Data preprocessing included normalization and batch correction using limma and SVA packages. Differentially expressed genes (DEGs) were identified via robust rank a

Lipid-lowering drugs and essential hemorrhagic thrombocythemia's risk: A drug-target Mendelian randomization study.

Lipid-lowering drugs are currently widely used by clinicians in clinical practice; numerous related studies have confirmed the pivotal role of the lipid pathway in regulating platelet function. Lipid-lowering drugs, represented by statins, have been found to have an improving effect on platelet-related diseases. However, there is a lack of large-scale population studies at present, and the causal explanation of traditional observational design studies is easily limited by confounding variables. The impact of lipid-lowering drugs on the risk of essential hemorrhagic thrombocythemia is not clear

Calreticulin mutations in essential thrombocythemia and primary myelofibrosis.

Mutations in calreticulin have been identified as driver mutations in BCR-ABL1-negative myeloproliferative neoplasms (MPNs) and are especially important for diagnostics and prognosis in primary myelofibrosis and essential thrombocythemia. The vast majority of pathogenic variants are insertions or deletions in exon 9 that result in a mutant C-terminal sequence that activates the signaling of the thrombopoietin receptor, which in turn promotes megakaryocytic proliferation. This review examines the potential clinical utility of calreticulin mutations as laboratory biomarkers in essential thromboc

[Gastric intramural hematoma during treatment with aspirin and anagrelide in a patient with essential thrombocythemia].

A 62-year-old man was diagnosed with JAK2 mutation positive essential thrombocythemia (ET) after developing cerebral infarction. He was treated with anagrelide and aspirin, with anagrelide increased to 5.5 mg/day, but platelet count remained around 700,000/µl. In July 2024, he presented to the emergency department with abdominal pain and vomiting. CT angiography showed a large gastric intramural hematoma as well as pseudoaneurysm from the right gastroepiploic artery. Following transcatheter arterial embolization, his symptoms improved, the hematoma decreased in size, and his condition stabiliz

[Arterial embolism in a patient's feet].

A 69-year-old woman presented with progressive gangrene affecting the toes of both feet. Laboratory investigations showed marked thrombocytosis (800 x 109/l). A positive JAK2V617F-mutation confirmed essential thrombocytosis as the underlying cause of the gangrene. With thrombocyte-lowering therapy, platelet counts normalized and the gangrene showed no further progression.

Type-I interferon-activated NK cells control polycythemia vera in vivo.

Polycythemia vera (PV) is a clonal hematopoietic stem cell disorder resulting in overproduction of erythrocytes. Although interferon alfa (IFN-α) has shown therapeutic efficacy in PV and other myeloproliferative neoplasms (MPN), its precise mechanism of action remains poorly understood. In this study, we identify natural killer (NK) cells as primary immune effectors responsive to IFN-α treatment in PV essential for disease control in vivo. Using a transgenic mouse model of PV, we demonstrate that IFN-α induces the expansion of CD27+ NK cells in the bone marrow. In patients with PV or essential

Coagulation Disorders in Philadelphia Chromosome-negative Myeloproliferative Neoplasms-A Guide for Specialists in Coagulation Disorders.

ABSTRACT: Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs) are acquired clonal disorders of hematopoiesis that typically cause thromboembolic and hemorrhagic complications. These include an increased risk of both arterial and venous thromboembolic events as well as a heightened bleeding tendency. Venous thromboembolic events frequently manifest at atypical sites and characteristically present as splanchnic vein thrombosis. A common cause of the elevated bleeding risk is acquired von Willebrand syndrome. Clinical indicators of an underlying MPN include thromboembolic events

Platelet Proteome Links Metabolism to Reactivity in Essential Thrombocythemia.

Essential thrombocythemia (ET) is a myeloproliferative neoplasm in which JAK2, CALR, and MPL mutations are associated with distinct clinical phenotypes. Here, we analyzed the platelet proteome of ET patients by MS, combined with functional assays, to investigate platelet activation. Compared with healthy controls, ET platelets showed altered abundance of mitochondrial and metabolic proteins, including reduced levels of tricarboxylic acid cycle enzymes and a relative enrichment of glycolytic signatures. Acetylsalicylic acid decreased the abundance of metabolic proteins in CALR Type1 platelets,
