Retrospective RNA Sequencing Reclassifies a Historical t(5;12) Eosinophilic Neoplasm From Presumed PDGFRB Rearrangement to ETV6::ACSL6.

The molecular interpretation of eosinophilic neoplasms harboring t(5;12) may be challenging because cytogenetic breakpoint assignments can suggest a PDGFRB rearrangement without identifying the underlying fusion. We retrospectively investigated a woman with a chronic eosinophilic myeloid neoplasm initially investigated in 2002, in whom a subsequent t(5;12)(q31 ~ q33;p13) raised suspicion of PDGFRB involvement. Contemporary molecular investigations failed to identify the rearrangement, and an empiric trial of imatinib produced no meaningful hematologic response. Seventeen years later, blast tra

IL-5 CAR-T cell therapy induces effective remission in hypereosinophilic disorders.

BACKGROUND: Clonal and pathogenic eosinophil expansion in hypereosinophilic disorders (e.g., refractory hypereosinophilic syndrome (HES) and chronic eosinophilic leukemia (CEL)) remains an unmet therapeutic challenge, with current strategies often failing to induce durable remission. While monoclonal antibodies targeting the IL-5/IL-5Rα pathway have shown efficacy in treating eosinophil-driven diseases, a subset of patients experience incomplete responses or relapse, highlighting the need for more durable and comprehensive therapeutic strategies. Chimeric antigen receptor T (CAR-T) cell therap

Hypereosinophilic endocarditis presenting with intracardiac mass and severe mitral regurgitation: a case report of FIP1L1-PDGFRA positive myeloid neoplasm.

BACKGROUND: Loeffler's endocarditis is a serious manifestation of hypereosinophilia, and it is associated with endomyocardial fibrosis, thrombus formation, valvular dysfunction, and, rarely, intracardiac mass lesions. CASE SUMMARY: A 28-year-old male patient had progressive dyspnoea, facial oedema, and fatigue for 2 years. A 2D trans-thoracic echocardiogram revealed a 24 × 15 mm mobile echogenic mass attached to the mitral valve, associated with severe mitral regurgitation. Blood counts showed hypereosinophilia. Cardiac MRI showed subendocardial fibrosis. Bone marrow examination showed increa

Chronic Eosinophilic Leukemia Not Otherwise Specified: A Case Series.

Chronic eosinophilic leukemia (CEL) is a rare myeloproliferative neoplasm characterized by sustained elevation of eosinophil counts greater than > 1.5 × 109/L in blood or bone marrow. Approximately 25-30% of patients with persistent hypereosinophilia have somatic mutations associated with myeloid neoplasms, and next generation sequencing has led to the use of newer treatments for CEL, including tyrosine kinase inhibitors (TKIs) such as imatinib. Before the advent of imatinib, the disease had a poor prognosis with a 5-year mortality close to 50%. However, in patients with CEL without the charac

Medication Use and Guideline Adherence in Inflammatory Bowel Disease Management: A Retrospective Study from a Tertiary Care Hospital in Pakistan (2019-2024).

BACKGROUND: Inflammatory bowel disease (IBD) remains understudied in Pakistan. This retrospective observational study assessed the management of IBD patients at Shifa International Hospital, Islamabad, from January 2019 to June 2024. METHODS: Adherence to UK NICE 2019 guidelines (NG129 for CD, NG130 for UC) was evaluated. Data from 167 adult IBD patients (from initial 238 identified; 141 ulcerative colitis, 26 Crohn's disease) were extracted from electronic medical records after excluding inactive cases (n = 60), paediatric patients (n = 7), pregnant women (n = 3), and unconfirmed diagnoses (

Persistent Hemorrhagic Lip Crusts as the Presenting Sign of Pediatric FIP1L1-PDGFRA-Positive Hypereosinophilic Syndrome: A Case Report and Review of the Literature.

FIP1-like-1-platelet-derived growth factor receptor-α (FIP1L1-PDGFRA)-positive hypereosinophilic syndrome (HES) is a rare myeloproliferative neoplasm characterized by persistent eosinophilia and occurs predominantly in middle-aged adults. We report a 15-year-old boy with FIP1L1-PDGFRA-associated chronic eosinophilic leukemia who initially presented with persistent hemorrhagic crusts on the lower lip, an uncommon mucocutaneous manifestation resulting from thrombotic aggregates of eosinophil granule proteins. This case is followed by a systematic literature review of pediatric FIP1L1-PDGFRA-posi

Hypereosinophilia of persistent/unknown cause: a bone marrow-based study integrating WHO-HAEM5 criteria and myeloid next-generation sequencing.

OBJECTIVES: Hypereosinophilia (HE), defined as an absolute eosinophil count (AEC) ≥1.5 × 109/L, poses diagnostic challenges when distinguishing clonal myeloid neoplasms from idiopathic HE/hypereosinophilic syndrome, particularly under WHO-HAEM5 criteria. We characterized patients with HE of persistent/unknown cause (HEPU) undergoing bone marrow (BM) evaluation to refine diagnostic classification. METHODS: We identified 307 BM biopsy specimens (2015-2023) mentioning "eosinophilia." Patients with defined reactive or clonal etiologies were classified as group 2 (known etiology, n = 269). The rem

Suspected chronic eosinophilic leukemia in a cat treated with toceranib.

CASE SUMMARY: A 16-year-old, spayed female domestic shorthair cat was referred to the Schwarzman Animal Medical Center for evaluation of chronic, progressive eosinophilia accompanied by vomiting and weight loss despite normal appetite. Blood smear evaluation revealed immature eosinophils with dysplastic features and occasional giant forms. Abdominal ultrasound identified small-intestinal thickening, a hepatopathy and splenomegaly. Screening for feline leukemia virus, feline immunodeficiency virus and heartworm infection were negative. Chronic eosinophilic leukemia was suspected, although hyper

[Incidental finding of negative T waves: a case of endomyocardial fibrosis].

We present the case of a 58-year-old man with negative T waves in the inferior and lateral leads observed on a routine ECG in the context of persistent peripheral eosinophilia, 6 months after an intestinal parasitic infection. The diagnostic workup excluded coronary artery disease and led to a diagnosis of endomyocardial fibrosis with apical thrombosis. Endomyocardial fibrosis, also known as Loeffler endocarditis, represents the cardiac involvement of the hypereosinophilic syndrome, resulting in a cardiomyopathy with a restrictive phenotype. After initiating specific therapy, further investiga

Engraftment across major ABO blood group and rhesus E antigen incompatibility in the presence of persistent anti-E antibodies and late serologic findings coinciding with disease relapse.

ABO incompatibility is frequent in haematopoietic stem cell transplantation (HSCT), yet the clinical impact of non-ABO red blood cell (RBC) alloimmunisation remains less defined. We describe the clinical course of a man in his 50s with chronic eosinophilic leukaemia who underwent a mismatched unrelated donor (MMUD) HSCT. Despite a major ABO mismatch and high-titre pre-existing anti-E antibodies against an E-antigen positive donor, the patient achieved successful trilineage engraftment without haemolysis. Six months after the transplant, he developed severe pancytopenia and a positive direct an

Loeffler's Endomyocarditis With Embolic Stroke Secondary to Chronic Eosinophilic Leukemia With Therapy-Related Molecular Features.

BACKGROUND: Loeffler's endomyocarditis is a severe manifestation of hypereosinophilia characterized by eosinophil-mediated myocardial injury, mural thrombus formation, and endomyocardial fibrosis. While often idiopathic, it may arise from clonal eosinophilic disorders carrying higher risk of end-organ damage. CASE SUMMARY: A 77-year-old man with diffuse large B-cell lymphoma in remission presented 5 years after R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) chemotherapy with subacute confusion and dyspnea. Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Efficacy of anti-interleukin 5 therapy in hypereosinophilic syndrome: An updated systematic review and meta-analysis.

Background: Hypereosinophilic syndromes (HES) are marked by persistent eosinophilia, absence of a primary cause, and evidence of eosinophil-mediated organ damage. HES presents a spectrum of clinical manifestations, with prognosis and treatment varying based on the subtype, including myeloid/lymphoid neoplasms and chronic eosinophilic leukemia, not otherwise specified. The primary treatment goal is to reduce eosinophil levels to prevent organ damage, typically by using glucocorticoids and immunosuppressive agents. However, these treatments often have limited efficacy and considerable adverse ef

Chronic Eosinophilic Leukemia Positive for FIP1L1-PDGFRa.

FIP1L1-PDGFRα-positive chronic eosinophilic leukemia (CEL) is a rare subtype of myeloproliferative neoplasm characterized by organ damage caused by eosinophilic granules containing cytokines and humoral factors. These include interleukin-5 (IL-5), interleukin-4 (IL-4), and interleukin-13 (IL-13). We present a case study of a 71-year-old male who initially presented with cough, fever, night sweats, and weight loss. Fluorescence in situ hybridization (FISH) and cytogenetic testing confirmed that the patient had FIP1L1-PDGFRα-positive chronic eosinophilic leukemia. Treatment with imatinib and pre
