Dissemination and Implementation of a Telehealth-Enabled Program for Providing Infectious Disease Expertise in Rural Settings.

BACKGROUND: Telehealth can facilitate improving antibiotic use and related antimicrobial stewardship activities in facilities underserved by infectious diseases (IDs) expertise. We describe implementation of the Department of Veterans Affairs (VA) videoconference antimicrobial stewardship team (VAST) to connect multidisciplinary teams from rural VA medical centers (VAMCs) with geographically distant ID experts. METHODS: We implemented VASTs at 10 rural VAMCs from September 2021 to February 2024. VASTs consisted of regular meetings between rural VAMCs and an off-site ID expert who met by video

The histology of rhynchosaur (Diapsida, Archosauromorpha) ankylothecodonty.

The study of the connection between the teeth and the jaw is important for understanding the palaeobiology of vertebrates, but inconsistent terminology and incomplete sampling have made it difficult to assess the evolutionary significance of some of the related characters. Among archosauromorphs, tooth attachment in dinosaurs and crocodylians is nearly identical to that of mammals in featuring a ligamentous connection (gomphosis), whereas closely related forms appear to have teeth fused to the jaws (ankylosis), as in most other amniotes. Hence, studying tooth attachment of stem-archosaurs is p

Biophysical investigations of the membrane interactions of collagen VI-derived host defense peptides.

Collagen VI is an extracellular matrix protein forming complex microfibrillar networks in connective tissues. Specifically, we focused on its role in innate immunity, in particular on cationic sequence motifs from the α3(VI)-chain, which exhibit strong antibacterial properties against both Gram-positive and Gram-negative bacteria in vitro and in vivo. Cytotoxicity assays revealed minimal to no adverse effects, even at concentrations effective against bacterial pathogens. This favorable safety profile suggests that these antimicrobial peptides selectively target bacterial membranes while sparin

Reconceptualizing gingival keratinization: from epithelial autonomy to artificial intelligence-driven therapeutic planning.

Gingival keratinization is critical for homeostasis of periodontal health, which has been previously thought to be connective tissue (CT) driven. Recent data refute that model instead of supporting it, implicating epithelial-intrinsic factors and local environmental signals as the dominant regulators of keratinization. This review critically appraises the role of CT in gingival keratinization and other possible mechanisms. A comprehensive review was conducted on literature from animal models, human case studies, and genetic research comparing the classical CT-directed model with recent hypothe

Objective measurement of Cranial Rhythmic Impulse (CRI): visual analysis of an observational case series.

UNLABELLED: One of the traditional principles of osteopathy is the presence of primary cranial movement, also defined as Cranial Rhythmic Impulse (CRI) in relevant literature. Though the presence of such movement is recognized by some and disputed by others, there is little evidence to support either stance. One of the main issues concerns the possibility of getting objective, simultaneous tracks of the physiological rhythms responsible for possible variations in size involving the skull and the whole body. The only data available in literature date back to old works which, according to the au

Novel Hip MRI Sequence Provides Consistent Osseous Morphology Dimensions for FAI Evaluation Compared With CT.

BACKGROUND: Prior studies have reported that imaging evaluation of osseous morphology in femoroacetabular impingement (FAI) is best performed with CT, which exposes patients to ionizing radiation. In recent years, a number of studies have evaluated whether various novel MRI protocols, which do not expose patients to ionizing radiation, can effectively assess osseous morphology in patients with FAI. Our institution incorporated in- and out-of-phase sequences into a routine MRI protocol to better assess acetabular version; however, it is unknown how in- and out-of-phase MRI compares with CT imag

A review and empirical findings of fasciae and muscle interactions in low back pain.

BACKGROUND: Low Back Pain (LBP) is a global musculoskeletal disorder affecting quality of life, with 90% of cases categorized as nonspecific, indicating that the underlying cause is unknown. One of the current treatment modalities that physiotherapists use are fascia tissue manipulations (FTMs), such as soft tissue mobilization, myofascial release, and elastic tape, to enhance joint mobility and muscle flexibility in LBP individuals. PURPOSE: This review and experimental research explore the hypothetical mechanisms of FTMs using Skin Displacement (SKD), either by hand or with elastic tape. M

The Compartmental and Fibrillar Polyhedral Architecture of Fascia: An Assessment of Connective Tissue Anatomy Without Its Abstract Classifications.

The process of dissection is essential to the study of anatomy, with the variety of colours, shapes, patterns and textures revealing the distinctive features of each anatomical system, but it can also be misleading, because while the body's constituent 'parts' have traditionally been classified according to their appearance, assumed functions and perceived importance, this basic information can be interpreted in different ways. Living organisms are intrinsically indeterminate, which implies that the conclusions arrived at through the study of anatomy are not necessarily congruent with the anat

The wide gape of snakes: A comparison of the developing mandibular symphysis in sauropsids.

The origin and evolution of snakes has been marked by the acquisition of many morphological and functional novelties, one of which is the possession of a highly kinetic skull allowing for the consumption of prey that are often larger than their head diameter. One feature of the iconic wide gape of macrostomate (large-mouthed) snakes is due to changes in the rostral midline where the left and right hemi-mandible come together. Across vertebrates, the two sides of the lower jaw are held together by the mandibular symphysis. In snakes, the two halves of the lower jaw do not fuse and the symphysis

Functional anatomy of entheses and enthesis organs: A celebration of Professor Mike Benjamin's contribution to enthesis biology.

This review celebrates the work of Professor Mike Benjamin, whose anatomical research transformed our understanding of entheses - the sites where tendons, ligaments and other connective tissues attach to bone. This review aims to provide an overview of Professor Benjamin's foundational concepts, including the enthesis organ, functional entheses and the synovio-entheseal complex and their relevance to musculoskeletal health and disease. Entheses are biomechanically complex regions that accommodate the transition between compliant soft connective tissues and rigid bone by natural macroscopic and

An experimental-computational approach for measuring charged solute diffusivity through human synovium.

Intra-articular drug delivery has shown promise in targeting arthritic joints, but its therapeutic efficacy is hindered by the synovium, a multilayered connective tissue that rapidly clears locally delivered drugs from the joint space. To better understand the mechanisms behind synovial drug clearance, we previously developed a finite element model of synovium as a multiphasic tissue and used an inverse method to determine the effective diffusivity (Deff) of neutral solutes through synovium, which was found to decrease with increasing molecular weight. Here, we adapted this experimental-comput

A high frequency ultrasound-based platform to non-destructively quantify geometric, acoustic and mechanical properties of thin, engineered soft connective tissues in vitro.

Assessment of temporal changes in the functional properties of in vitro tissue engineered (TE) constructs is typically done by destructive endpoint analysis, which increases experimental resources, analysis time, and financial burden. To overcome the limitations of existing practices, a novel Modular High Frequency Ultrasound Bulge Testing System (mHFUS-BTS) was designed to perform repeated, non-destructive and non-invasive characterization of cell-seeded biomaterial sheets throughout culture. Quantitative HFUS was validated to accurately measure the acoustic properties (i.e., speed of sound,

Cancer-associated fibroblasts promote tumor progression in fusion-positive rhabdomyosarcoma.

In children, rhabdomyosarcoma is the most common tumor originating in soft connective tissue. The PAX3/PAX7-FOXO1 fusion-positive alveolar subtype has poor clinical outcomes, with frequent recurrence, metastasis, and low survival. Current therapies may be limited by the tumor microenvironment containing cancer, immune, and fibroblast cells. In cancer, cells that transform into cancer-associated fibroblasts support growth and other malignant properties of the tumor. To study the role of cancer-associated fibroblasts, we isolated cells from the lung metastases of a patient with rhabdomyosarcoma

Orthopedic Surgical Management of Soft Tissue Sarcomas in a Referral Center: 5-Year Oncological Outcomes.

INTRODUCTION: Soft tissue sarcomas (STS) are malignant tumors of connective tissue, characterized by a wide heterogeneity in their presentations, resulting in variable therapeutic strategies between reference centers. However, clinical outcomes from an orthopedic and trauma surgery department of university hospitals are poorly documented. We evaluated the 5-year local recurrence rate of patients managed according to our local strategy and operated in our department, which is certified for the management of both soft tissues and bone sarcomas. We also assessed overall survival, metastasis and r

Defining the Stem Cell Niche in Developing Pericoronal Dental Follicles.

OBJECTIVE: The pericoronal dental follicle is a connective tissue structure encased in epithelium present around the developing tooth germ before erupting and emerging in the oral cavity. Stem cells from pericoronal follicles can be obtained and cultured under suitable conditions in vitro. This study builds upon our previous research, with the primary objective of identifying stem cell subpopulations in the oral cavity based on marker expression and differentiation potential. MATERIALS AND METHODS: Dental follicle stem cells (DFSCs) were obtained and cultured under standard cell culturing con

Negative pressure wound therapy for combat-related extremity vascular injuries: clinical experience from the war in Ukraine.

BACKGROUND: Extremity vascular injuries are among the most challenging problems in military surgery. They are frequently accompanied by extensive soft tissue loss and heavy contamination, which increases the risk of infection and limb loss. Although negative pressure wound therapy (NPWT) is widely used in civilian practice, its role in combat vascular injuries remains unclear. The war in Ukraine provided an opportunity to evaluate NPWT as part of staged surgical care under modern battlefield conditions. METHODS: We retrospectively reviewed 85 service members with severe combat-related extremi

Three-Dimensional Printing Parameter Assessment of Elastomers for Tendon Graft Applications.

Additive manufacturing has significantly advanced patient-specific medical devices, particularly for hard tissue repair, yet applications in soft tissue remain limited. Existing approaches for 3D-printed soft tissue devices employ mainly biogels and bioinks for regenerative purposes, while synthetic grafts for tendons and ligaments remain non-customizable in shape and mechanics. This study investigates the mechanical performance of 3D-printed thermoplastic polyurethane (TPU) elastomers as a function of printing parameters, informing customizable connective tissue graft designs. A TBC1D9-Rab29 axis controls homeostatic NF-κB signaling and selective IL-6 production in epithelial cells.

BACKGROUND: Interleukin-6 (IL-6) plays a crucial role in inflammation and immune defense; however, its intracellular trafficking and the mechanisms regulating its expression remain poorly understood. METHODS: We investigated epithelial cell responses to poly(dA:dT) stimulation and group A Streptococcus infection, using depletion and overexpression assays, NF-κB readouts, proteomics, co-immunoprecipitation, immunofluorescence imaging, and analysis of necrotizing soft tissue infection transcriptomes. RESULTS: TBC1D9, a Rab GTPase-activating protein, selectively regulates IL-6: its depletion re
