Diabetic neuropathy (DN) is one of the most common and debilitating chronic complications of diabetes mellitus. It typically presents as a distal symmetric polyneuropathy (DSP), which is characterized by symmetric involvement of the distal extremities, manifesting as numbness, pain, paresthesia, and sensory loss. In addition to peripheral sensory involvement, patients may also experience autonomic neuropathy and focal nerve injuries. Persistent hyperglycemia can impair the structure and function of the nervous system through multiple interrelated mechanisms. Prolonged elevation of blood glucos

The lungs interface directly with the external environment, exposing them to airborne pathogens like endotoxins. We investigated whether the vagus nerve, which innervates the lungs-detects such pathogens. Using transcriptomics, tissue clearance imaging, electrophysiology, and cell-specific knockout models, we discovered that vagal sensory endings synapse with pulmonary neuroendocrine cells (PNECs). These nerve endings detect bacterial endotoxins primarily through the pain receptor TRPA1, not via Toll-like receptor 4 (TLR4). This detection triggers electrical excitation in vagal neurons and upr

Bone homeostasis essentially represents a balanced interplay among bone cells, dynamically maintaining bone mass and structural stability under the influence of both internal and external factors of the body. Bone tissue receives various types of neural innervation and, through the secretion of signaling factors, influences the function of bone cells, collectively forming the core framework of the "neuro-skeletal axis". This review systematically analyzes the roles of multiple neural regulatory mechanisms in the maintenance of bone homeostasis and the repair of bone injuries, as well as their

Facial nerve decompression is a surgical procedure performed for severe facial nerve paralysis associated with conditions such as Bell's palsy and Ramsay Hunt syndrome. Classical Western studies by Fisch first established the surgical principles of facial nerve decompression, providing the foundation for subsequent work on decompression extent and outcomes. However, the optimal extent of bony decompression of the facial nerve canal remains unclear, and a 180° removal of the surrounding bone has traditionally been performed based on empirical judgment. Nevertheless, more extensive bone removal

Effective host defense and immunosurveillance at barrier tissues require coordinated functions of multiple cell types. Here, we show that in the skin, sympathetic nerves engage with epidermal keratinocytes to regulate the local density of tissue-resident memory CD8+ T (TRM) cells, thereby influencing regional cancer immunosurveillance. Sympathetic nerves do not communicate directly with CD8+ TRM cells. Instead, they form synapse-like structures near basal keratinocytes and dynamically modulate epithelial-derived signals essential for skin CD8+ TRM formation via norepinephrine-ADRB2 signaling.

We examined the distribution of axons throughout the spinal cord of the rat that were either immunoreactive for the adrenaline-synthesizing enzyme, phenylethanolamine N-methyltransferase (PNMT), or derived from medullary C1 neurons, one of the three groups of neurons in the brain that synthesize PNMT. We observed that PMNT-immunoreactive axons, as well as C1 axons labelled with GFP from viral transduction, innervate most, but not all, sympathetic preganglionic neurons in the thoracolumbar spinal cord. GFP-positive C1 axons provided innervation to sympathetic preganglionic neurons that expresse

Chronic diabetic wounds, characterized by persistent inflammation and immune dysregulation, present a significant healing challenge. While traditional hydrogels aid tissue repair, their functionality is often compromised at low temperatures. Herein, we report a polyphenol-mediated conductive hydrogel that remains stable and functional at -80°C. Engineered from casein, dopamine-modified poly(3,4-ethylenedioxythiophene), and a synergistic antifreeze system (dopamine, gluten, glycerol), the hydrogel maintains its conductivity, bioactivity, and therapeutic efficacy after freezing. The hydrogel fun

The human body emits a bioelectromagnetic field primarily generated by the electrical activity of the heart, with additional contributions from the brain, muscles, and peripheral nerves. These endogenous fields are not isolated and can be modulated by external electromagnetic and magnetic influences. Current evidence suggests that the main mechanisms underlying such interactions include modulation of ion channels, radical pair dynamics, and ion cyclotron resonance. Several studies report sex-specific differences in responses to magnetic exposure. The main factors implicated in these difference

Given the rapidly expanding clinical use of glucagon-like peptide-1 receptor (GLP1R) agonists-well-known for their antidiabetic and antiobesity effects-it is increasingly important to understand the precise distribution of GLP1R expression in the human body, as this knowledge is crucial for elucidating both their therapeutic effects and side effects. In this study, we investigated Glp1r mRNA expression in the human nodose ganglion, a key sensory relay between the periphery and the brain. We analyzed postmortem paraffin-embedded nodose ganglia sections from 10 human donors, using RNAscope analy

BACKGROUND: Auricular vagus nerve stimulation (aVNS) has emerged as a noninvasive neuromodulatory strategy with the potential to modulate central sensitization and inflammatory pathways. However, its role in fibromyalgia (FM) remains insufficiently explored.
OBJECTIVE: To investigate whether stimulation laterality (left vs right auricular branch of the vagus nerve) differentially influences clinical and biological outcomes in women with FM.
METHODS: In this randomized, double-blind, sham-controlled trial, 51 women with FM were allocated to sham stimulation, right-sided aVNS (aVNS-R), or left

BACKGROUND: Celiac plexus block (CPB) and celiac plexus neurolysis (CPN) are interventions used to treat chronic abdominal pain, particularly in cancer patients with pancreatic malignancy and patients who have chronic pancreatitis. Both CPB and CPN have been shown to significantly improve pain in patients with abdominal cancers while decreasing opioid consumption and side effects. Existing data on the technical variations and complications associated with both CPB and CPN are limited.
OBJECTIVES: We sought to examine the technical factors, patient demographic data, and intra- and post-operati

Intrinsic enteric neurons regulate gut motility, secretion, and absorption, whereas extrinsic dorsal root ganglia sensory neurons are key in colon interoception and visceral pain. A new study raises the question of whether some initial interoceptive signals could originate from enteric neurons.

Stimulation of sensory vagal pathways is typically delivered via invasive, cervical vagus nerve stimulation (cVNS) or noninvasive, trans-auricular nerve stimulation (taNS). While both methods are investigated therapeutically, their effects on brain physiology remain poorly understood, hindering mechanistic insights and stimulus optimization. In 6 awake nonhuman primates, we recorded cortical vagal-evoked potentials (VEPs) from subdural electrodes placed in prefrontal, sensorimotor and parietal cortical areas, in response to cVNS or taNS. Across 478 different taNS and cVNS protocols, we varied

BACKGROUND: Stroke induces complex pathophysiological responses that extend beyond the brain, yet the mechanisms through which peripheral signals influence stroke recovery remain largely unclear.
OBJECTIVE: Here, we identify a novel gut-brain neural circuit that promotes stroke recovery via kynurenic acid (KYNA) signalling.
DESIGN: In a training cohort (30 patients with acute ischaemic stroke (AIS) and 30 controls), untargeted metabolomics profiled intestinal metabolites and the key metabolite KYNA was validated in an independent cohort (100 patients with AIS and 100 controls) using targeted

Body-brain communication has emerged as a key regulator of tissue homeostasis1-5. Solid tumours are innervated by different branches of the peripheral nervous system and increased tumour innervation is associated with poor cancer outcomes6-8. However, it remains unclear how the brain senses and responds to tumours in peripheral organs, and how tumour-brain communication influences cancer immunity. Here we identify a tumour-brain axis that promotes oncogenesis by establishing an immune-suppressive tumour microenvironment. Combining genetically engineered mouse models with neural tracing, tissue

Chronic pelvic pain is a common and multifactorial clinical syndrome that markedly impairs quality of life in both men and women. Superior hypogastric plexus block (SHGPB) is an effective minimally invasive treatment. The sacral notch approach, a modification of the transdiscal technique, offers a more direct and technically simple puncture pathway. However, sex-related pelvic anatomical differences may affect puncture parameters, and imaging-based evidence remains limited. This study aims to evaluate sex-related differences in key anatomical and technical parameters of SHGPB via the sacral no

Exogenous ketone supplementation can elevate heart rate (HR) but the effects on cardiac autonomic modulation remain unclear. HR variability was analyzed from 18 healthy adults (23 ± 3 years; n = 10 females) ingesting 0.3 or 0.6 g/kg ketone monoester or placebo in a randomized crossover, double-blind design. Compared to placebo, both ketone doses increased plasma β-hydroxybutyrate and reduced the percentage of successive R-R intervals that differ by ≥50 ms (pRR50) and the root mean square of successive differences (RMSSD) at 45 min (both p < 0.02) and 120 min post-ingestion (both p < 0.01). There are numerous differences in arrhythmia propensity and cardiac electrophysiology between women and men. One proposed reason is differences in the autonomic nervous system (ANS) influence. We therefore compared the sympathetic and parasympathetic influence (sympathovagal balance) on the sinoatrial node (SAN) at rest in healthy young women (n = 15) and men (n = 15) with a mean age of 24 yr. Pharmacological blockade of the ANS influence on the SAN was induced by sequential bolus injections of atropine (0.04 mg/kg body wt) and propranolol (0.2 mg/kg body wt) during continuous electrocardiogra

PURPOSE: The rapid heart rate increase during the first seconds of exercise is primarily mediated by cardiac vagal withdrawal, which can be assessed by the 4-s exercise test (4sET), a pharmacologically validated and highly reliable procedure. However, the need for a cycle ergometer limits its applicability. Thus, we aimed to test the validity of a modified 4sET using wheelchair propulsion simulation (WPS) by comparing it with the traditional cycle ergometer protocol.
METHODS: Sixty healthy adults (30 men, 30 women; 22 ± 2 years) performed, in randomized order, three repetitions each of the tr
