Revisiting the anatomy of the retroperitoneum and renal fascia. Aspects applied to radiology, laparoscopic and robotic surgery - a narrative review.

This review aims to provide a comprehensive analysis of the anatomy of the retroperitoneal and abdominal fascias. We analyzed papers published in the past 70 years in the databases of Pubmed, Embase and Scielo. We excluded case reports, editorials and opinions of specialists. Studies were excluded if they lacked clear anatomical descriptions of the fascias, if full-text access was unavailable, or if they were published in languages other than Portuguese, English, or Spanish. This narrative review addresses the anatomical integration, surgical techniques, and functional importance of the fascia

Ganglioneuroma of the sacral canal: an unusual location with MR imaging findings.

Ganglioneuroma is a rare, benign, slow-growing tumor arising from embryonic neural crest cells that differentiate into the sympathetic ganglia. Accordingly, it can occur anywhere along the sympathetic chain, from the clivus to the sacrum, although it is seen most commonly in the mediastinum, retroperitoneum, adrenal glands, and neck. Although involvement at the sacral level is uncommon, ganglioneuroma typically arises from the anterior surface of the sacrum, with presacral lesions occasionally extending through the sacral foramina into the sacral canal. Only two cases of ganglioneuroma confine

Case Report: Solitary fibrous tumor of the pelvis associated with Doege-Potter syndrome.

INTRODUCTION: Solitary fibrous tumor (SFT) is an uncommon mesenchymal spindle cell neoplasm (incidence ~1.4 per million). Although most SFT occur in the thoracic cavity, they can also arise in the retroperitoneum, pelvis, head, and neck. While the majority exhibit benign behavior, approximately 5% are associated with paraneoplastic syndromes, most notably Doege-Potter syndrome (DPS). DPS is characterized by non-islet cell tumor hypoglycemia (NICTH) resulting from tumor hypersecretion of high-molecular-weight insulin-like growth factor 2 (IGF2), which saturates insulin and IGF receptors causing

[IgG4-related lesion of the retroperitoneum, ureter, and kidney, mimicking infiltrating endometriosis].

Urinary tract endometriosis is rare, and ureteral involvement is particularly insidious due to its asymptomatic nature. The absence of symptoms can lead to ureteral stricture, hydronephrosis, chronic kidney disease up to end-stage renal failure, and irreversible loss of kidney function. Diagnosis requires imaging methods to assess the degree of obstruction, while treatment involves surgical release of the ureter from endometriotic infiltrates to preserve organ function. Our observation illustrates the difficulty of establishing the etiology of a retroperitoneal tumor-like infiltrate leading to

Primary borderline retroperitoneal mucinous cystic neoplasm and situs inversus totalis: a case report.

Primary retroperitoneal mucinous cystic neoplasms (PRMCNs) are rare tumors classified into cystadenoma, PRMCN with borderline malignancy (PRMCN-BM), and cystadenocarcinoma. We report a 60-year-old woman with left flank abdominal pain whose MRI revealed situs inversus totalis alongside a complex cystic lesion in the left retroperitoneum measuring 4.8 × 5.2 × 4.5 cm. She underwent laparotomy with complete resection of the lesion, and histopathology confirmed a primary retroperitoneal neoplasm with borderline malignancy, with an uneventful postoperative course and no recurrence after 8 months. To

Phenotype, treatment, and determinants of prognosis in Erdheim-Chester disease: an international, retrospective, cohort study.

BACKGROUND: Erdheim-Chester disease is a rare histiocytosis driven by MAPK pathway gene mutations. Its clinical presentation is heterogeneous, and there is a shortage of data on natural history and prognosis. We aimed to characterise clinical presentation, associated conditions, treatments, outcomes, and determinants of prognosis in a large international Erdheim-Chester disease cohort. METHODS: We conducted an international, retrospective, cohort study of patients with biopsy-proven Erdheim-Chester disease diagnosed between Jan 1, 1985, and Dec 31, 2024. Data were collected in ten countries b

CT and MRI imaging features of atypical spindle cell lipomatous tumor in the retroperitoneum: a case report.

Atypical spindle cell lipomatous tumor (ASCLT) is a rare benign adipocytic neoplasm, and primary involvement of the retroperitoneum is exceedingly uncommon. Because its imaging features overlap with those of other fat-containing tumors, particularly atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WDL), preoperative diagnosis remains challenging. We report a case of primary retroperitoneal ASCLT in a 58-year-old woman whose lesion was incidentally detected during a routine physical examination. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a small, well-d

Computed tomography findings of a retroperitoneal chronic expanding hematoma with multiple hemangiomas: A case report and literature review.

Chronic expanding hematoma (CEH) is an uncommon lesion that gradually increases in size over time. A hemangioma is a benign vascular tumor that may involve the skin, mucosa, or visceral organs; however, involvement of retroperitoneal organs is rare. Herein, we describe the case of a 56-year-old woman with a CEH adjacent to the right ovary and multiple congenital hemangiomas involving the skin, subcutaneous tissue, oral cavity, and liver. CEH was completely resected because it was suspected to be the cause of anemia and because an ovarian tumor or soft tissue sarcoma could not be excluded. In t

Inflammatory myofibroblastic tumor of the greater omentum in a 7-year-old girl.

Inflammatory myofibroblastic tumor is a rare neoplasm. Its preferred location is intrathoracic; there are few reports of intra-abdominal involvement, where it may affect the mesentery and retroperitoneum. A 7-year-old girl was brought in for evaluation due to fever, nonspecific lower abdominal pain, fatigue, and progressive weight loss. Laboratory tests revealed anemia, thrombocytosis, and elevated CRP. An abdominal ultrasound showed a hypoechoic mass with well-defined borders and heterogeneous content, measuring approximately 67 × 52 × 79 mm. Complete excision was performed via laparotomy. It

Solitary splenosis mimicking retroperitoneal tumor: a case report.

INTRODUCTION: Splenosis is a rare benign disorder characterized by autotransplantation of splenic tissue after splenic trauma or splenectomy. Isolated retroperitoneal splenosis is extremely uncommon and can be easily misdiagnosed as a malignant neoplasm, which may lead to unnecessary surgical procedures. CASE PRESENTATION: A 38-year-old woman with a history of traumatic total splenectomy 6 years earlier was admitted with acute right upper abdominal pain. Imaging examinations showed cholecystolithiasis, choledocholithiasis, and a 2.2 cm solid mass in the right retroperitoneum at the upper pole

Peritoneal disseminations from appendiceal vs. colorectal mucinous adenocarcinoma. An Italian multicentric comparative study.

BACKGROUND: We compared baseline characteristics, outcomes and patterns of failure between patients undergoing cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) for peritoneal disseminations from appendiceal (MACA) or colorectal mucinous adenocarcinoma (MCRC), in order to assess clinical and prognostical differences. METHODS: Prospectively collected data for 402 patients treated for MACA (n = 218) or MCRC (n = 184) in 17 Italian centers were retrospectively analyzed. RESULTS: Colorectal origin was associated with lower peritoneal cancer index (PCI) (P < 0.001),

Laparoscopic resection of a low-grade appendiceal mucinous neoplasm in a patient with autosomal dominant polycystic kidney disease: a case report.

INTRODUCTION AND IMPORTANCE: Appendiceal mucinous neoplasms (AMNs) are rare, and preoperative diagnosis can be challenging, particularly when other abdominal cystic diseases coexist. Autosomal dominant polycystic kidney disease (ADPKD), which is characterized by multiple cystic formations, can complicate both differential diagnosis and surgical planning. CASE PRESENTATION: A 77-year-old man with ADPKD and chronic renal failure was referred to our hospital following a positive fecal occult blood test during an evaluation for anemia. Colonoscopy revealed a large submucosal tumor-like elevation

From Leiomyoma to Smooth Muscle Tumor of Uncertain Malignant Potential: A Rare Recurrent Right Retroperitoneal Mass in a Young Male.

Smooth muscle tumors of uncertain malignant potential (STUMP) are rare mesenchymal neoplasms that exhibit histopathological features intermediate between benign leiomyoma and leiomyosarcoma. STUMP is overwhelmingly a disease of the uterus in perimenopausal and postmenopausal women; extra-uterine STUMPs are exceedingly uncommon, and involvement of the retroperitoneum in a male patient is exceptionally rare, with only a handful of cases described in the literature to date. We report a unique case of a young male who initially underwent excision of a retroperitoneal leiomyoma and subsequently dev

Pyelolymphatic backflow demonstrated on computed tomography urography in acute ureteric obstruction.

A man in his early 40s presented with acute right flank pain, vomiting and dysuria. Ultrasonography demonstrated an obstructing right upper ureteric calculus with a contralateral renal calculus. CT urography revealed fine, serpiginous contrast-opacified tubular channels extending from the right renal pelvis into the retroperitoneum without evidence of urinoma. Delayed excretory phase imaging demonstrated convergence towards the para-aortic region, consistent with pyelolymphatic backflow rather than collecting system rupture. The imaging pattern reflected markedly elevated intrarenal pressure a
