Intra-abdominal aggressive fibromatosis: a single-center case series of 12 patients focusing on surgical management and clinicopathological features.

BACKGROUND: Intra-abdominal aggressive fibromatosis (AF) is a rare disease with invasive growth and local recurrence. However, few studies have reported the clinical and pathological features of intra-abdominal AF. The aim of this study is to systematically elucidate the clinical characteristics, imaging manifestations, treatment and pathological features of intra-abdominal AF. METHODS: A total of 12 patients with intra-abdominal AF diagnosed in Xijing Hospital, First Affiliated Hospital of Air Force Medical University from April 2021 to September 2024 were included. All the specimens underwen

Isolated Lymphatic Injury Mimicking Bladder Rupture in a Young Male Following Blunt Trauma: A Case Report.

Blunt abdominal trauma can be difficult to evaluate, especially when an unusual injury mimics a far more common one. We present a case of a 17-year-old boy who was struck in the abdomen by bicycle handlebars. His initial imaging suggested a possible bladder rupture because of the presence of both intraperitoneal and extraperitoneal fluid. A retrograde CT cystogram, however, ruled out any bladder injury. Because of his worsening condition, he underwent an urgent laparoscopy followed by exploratory laparotomy, which revealed a large volume of milky-white free fluid, suggestive of chyle, within t

A case report of transcatheter arterial embolization for extra-renal retroperitoneal angiomyolipoma associated with tuberous sclerosis complex.

An 83-year-old male who underwent left nephrectomy about 40 years ago presented with a fatty mass in the retroperitoneum on CT images. The patient was diagnosed with extra-renal retroperitoneal angiomyolipoma (AML) through CT-guided retroperitoneal biopsy. Based on additional tests, the patient was also diagnosed with tuberous sclerosis complex (TSC). Given the substantial size (over 4 cm) of the tumor and the presence of an aneurysm, transcatheter arterial embolization (TAE) was performed to prevent rupture. Transcatheter embolization of the left testicular artery, left inferior phrenic arter

Reduced beam hardening in urogenital imaging with photon-counting CT: a retrospective direct comparison with conventional CT.

BackgroundBeam-hardening artefacts and image noise are very common causes of reduced image quality with computed tomography (CT) imaging of the lower pelvis and perirenal area affecting precision of diagnoses in urogenital imaging.PurposeTo investigate whether photon-counting CT (PCCT) improves image quality and/or reduces beam-hardening artefacts in the lower pelvis and in the perirenal area compared to energy-integrating CT (EIDCT).Material and MethodsWe retrospectively identified 35 patients scanned using both EIDCT and PCCT. Four radiologists read both PCCT and EIDCT images. Readers evalua

Case Report: A giant dedifferentiated liposarcoma in the retroperitoneum.

This article presents a 60-year-old woman presented with a one-year history of abdominal distension, which acutely worsened over four days prior to admission. Imaging revealed a large retroperitoneal liposarcoma. Exploratory laparotomy identified a massive tumor (approximately 45×40×13 cm, weighing 8 kg) invading the left kidney, adrenal gland, adnexa, left hemicolon, and mesentery. En bloc resection was performed. Postoperative pathology combined with fluorescence in situ hybridization (FISH) testing demonstrated MDM2/CDK4 amplification, confirming dedifferentiated liposarcoma (DDLPS). The pa

Exploring the Limits of Perigastric Surgery in Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy for Pseudomyxoma Peritonei: A Functional Burden with Preserved Oncologic Outcomes.

INTRODUCTION: Cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) is the standard treatment for pseudomyxoma peritonei (PMP), and complete cytoreduction remains essential for prognosis. While gastrectomy has traditionally represented a major threshold of surgical aggressiveness, the functional impact of increasingly extensive perigastric dissection is not well defined. METHODS: We retrospectively analyzed 110 consecutive patients who underwent CRS and HIPEC for appendiceal PMP between 2016 and 2024. Perigastric procedures were categorized into five Gastric Cytor

Incidence, Predictors, and a Bedside Risk Score for Chylous Ascites after Open Abdominal Aortic Surgery: A 20-Year Population-Based Cohort Study.

BACKGROUND: Chylous ascites is a rare but morbid complication of retroperitoneal vascular dissection, associated with prolonged hospitalization and reported mortality of 11-18%. No population-based incidence data or risk stratification tool exists. We aimed to estimate procedure-specific incidence, identify independent predictors, and derive an internally validated bedside risk score. METHODS: Retrospective cohort study of 6,382 consecutive open abdominal aortic procedures (2003-2022) across 3 centers in Vojvodina, Serbia. The mean age was 69.1 ± 8.6 years; 79.8% were male. Radiological concern for liposarcoma unmasked as extra-adrenal myelolipoma: A case report.

Extra-adrenal myelolipomas (EAMLs) are rare, benign tumors composed of mature adipose tissue and trilineage hematopoiesis, most commonly arising in the retroperitoneum. When large or radiographically atypical, they can mimic malignant entities, such as well-differentiated liposarcomas, often necessitating tissue diagnosis. The present case report describes the case of a 79-year-old male patient with a history of pituitary adenoma treated with resection and radiation, resulting in secondary adrenal insufficiency and central hypothyroidism, who presented with progressive weakness, recurrent fall

A case report of retroperitoneal schwannoma with malignant transformation and unusual size.

INTRODUCTION: Schwannomas are typically benign tumors of peripheral nerves, rarely found in the retroperitoneum and even more rarely undergoing malignant transformation into malignant peripheral nerve sheath tumors (MPNSTs). Their nonspecific clinical presentation and overlapping radiologic features with benign variants pose significant diagnostic challenges. OBSERVATION: We report the case of a 27-year-old male presenting with a progressively enlarging right-sided retroperitoneal mass associated with lower limb pain and radiculopathy. Imaging revealed a giant retroperitoneal tumor involving

Laparoscopic Anterior-Approach Right Posterior Sectionectomy for Huge Hepatocellular Carcinoma: Technique and Short-Term Outcomes.

BACKGROUND: Laparoscopic liver resection is technically demanding for hepatocellular carcinoma (HCC) ≥10 cm in the right liver. In open liver resection, the anterior approach is established for large right-lobe tumors because it avoids parenchymal compression during mobilization.1, 2 However, its laparoscopic use remains limited,3 particularly in right posterior sectionectomy (RPS). This study describes the technique and perioperative outcomes of laparoscopic RPS using the anterior approach for huge HCCs. METHODS: Among 799 minimally invasive liver resections performed between 2011 and 2025,

Jejunal Mesenteric Well-Differentiated Liposarcoma With Isolated Omental Metastases: Evidence of Focal Dedifferentiation.

A malignant tumor of adipocytic origin, liposarcoma typically affects the deep soft tissues and retroperitoneum. Although primary mesenteric liposarcoma is uncommon, its association with isolated omental metastasis is a rare entity. A 65-year-old male presented with a painless, progressively enlarging abdominal mass. Imaging revealed multiple fat-attenuated intra-abdominal lesions. Surgical exploration confirmed a large lobulated mesenteric mass and an omental mass. Wide local excision with omentectomy and jejuno-jejunal anastomosis was performed. Histopathology confirmed well-differentiated l

Retroperitoneum and Pelvic Extraperitoneum: Anatomic Landmarks, Imaging Features, and Patterns of Disease Spread.

The retroperitoneum and pelvic extraperitoneum are complex anatomic spaces that contain numerous vital structures such as major vessels, lymphatics, nerves, and abdominopelvic organs. These regions are divided by fascial planes into distinct compartments with defined boundaries and contents, which guide the spread of disease within and beyond the abdominopelvic cavity. The retroperitoneum represents the abdominal part of the extraperitoneum located posterior to the parietal peritoneum and extends from the diaphragm superiorly to the pelvic brim inferiorly. The pelvic extraperitoneum, also refe

Combined Laparoscopic and Choledochoscopic Necrosectomy for Infected Pancreatic Necrosis: A Case Report.

Infected pancreatic necrosis often necessitates intervention when initial step-up management fails. We present the case of a 63-year-old female with severe acute necrotizing pancreatitis complicated by extensive, multiloculated infected collections that persisted despite prolonged percutaneous catheter drainage and antibiotic therapy. Imaging demonstrated necrosis involving both the pancreatic head and tail regions, a distribution that limited the effectiveness of unilateral retroperitoneal or purely endoscopic approaches. Given the failure of drainage and the complex anatomical extent of dise

Prognostic ceiling effect of the Peritoneal Cancer Index in super-extended pseudomyxoma peritonei of appendiceal origin: impact of anatomical disease distribution and surgical complexity.

BACKGROUND: Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy is widely adopted as the principal treatment for selected patients with pseudomyxoma peritonei from low-grade appendiceal mucinous neoplasms. Prognostic uncertainty persists in patients with a very high peritoneal tumor burden. This study assessed predictors of survival in the overall cohort and explored whether established prognostic factors remain applicable in patients with super-extended disease and a very high Peritoneal Cancer Index (PCI). METHODS: Consecutive patients with pseudomyxoma peritonei from low-g

Retroperitoneal anatomy and avascular spaces for pelvic surgery: Cadaveric dissection atlas.

This paper presents a comprehensive cadaveric dissection atlas detailing the retroperitoneal anatomy essential to advanced pelvic surgery. Developed by a multidisciplinary team of anatomists, gynecologists, and gynecologic oncologists, the study provides a systematic, layer-by-layer guide for navigating the pelvic avascular spaces. It details five critical surgical corridors: the presacral (retrorectal) space, the pararectal space (divided into the Latzko and Okabayashi compartments), the paravesical space (medial and lateral compartments), the prevesical (retropubic/Retzius) space, and the me

A diagnostic dilemma between epithelioid leiomyoma and PEComa in the right retroperitoneum: A case report of risk-adapted management with long-term recurrence-free follow-up.

RATIONALE: The accurate classification of retroperitoneal neoplasms remains challenging, particularly when distinguishing between epithelioid leiomyoma and perivascular epitheloid cell tumor (PEComa) due to significant histological and immunohistochemical overlap. This distinction carries profound therapeutic implications, as leiomyomas are benign while PEComas possess malignant potential. The diagnostic dilemma is compounded by the documented phenomenon of aberrant HMB45 expression in otherwise conventional leiomyomas. PATIENT CONCERNS: A 47-year-old woman presented with an incidentally disc

Retroperitoneal Robot-Assisted Nephroureterectomy: Technical Description and Single Center Experience.

INTRODUCTION: Robot-assisted radical nephroureterectomy (RANU) is increasingly used for upper tract urothelial carcinoma (UTUC). The retroperitoneal approach avoids bowel mobilization, but its adoption in robotic surgery remains limited because of the confined working space and technical complexity. We described our step by step retroperitoneal technique and evaluated perioperative outcomes in an initial single center series. METHODS: We retrospectively reviewed consecutive patients who underwent intended fully robot assisted retroperitoneal RANU at our institution between July 2022 and Septe

Laparoscopic Minimally Invasive Approach for Left Paracaval Retroperitoneal Schwannoma.

BACKGROUND: Schwannomas are rare mesenchymal tumors and represent the most frequent benign nerve tumor in the retroperitoneum.1,2 Their close relationship with major vascular structures can make surgical management technically demanding. Laparoscopic resection in the aortocaval region is rarely reported3-6 and requires meticulous dissection to preserve adjacent vessels.7,8 METHODS: We report the case of a 55-year-old man with an incidentally detected retroperitoneal mass.
