BACKGROUND: Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms composed of epithelioid cells that co-express melanocytic and smooth muscle markers. They can arise in various anatomic sites, most commonly the uterus, retroperitoneum, gastrointestinal tract, and liver. While most PEComas are sporadic and associated with TSC1/TSC2 mutations leading to mTOR pathway activation, a distinct molecular subset harbors TFE3 gene rearrangements, often exhibiting more aggressive clinical behavior. Given their rarity, the diagnosis and management of PEComas remain challenging and

Small bowel reconstruction of the ureters is the main and universal method of restoring the patency of the upper urinary tract in patients with long strictures, especially after radiation therapy. Such surgical intervention cannot always be performed using a laparoscopic approach. In the most challenging cases, the choice lies between open reconstructive procedure and nephrectomy. We present a clinical case of a 45-year-old patient with post-radiation changes in the abdominal cavity and retroperitoneum who underwent 14 laparotomies, half of them due to peritonitis, repeated ureteral stenting,

Liposarcomas (LPS) are a group of tumors with different subtypes, some of which are more aggressive. They most often arise in the retroperitoneum, while occurrence within the peritoneal cavity is uncommon. Symptoms are usually nonspecific. We report the case of a combined intraperitoneal and retroperitoneal dedifferentiated LPS (DDLPS), which presented significant diagnostic and therapeutic challenges. We report the case of a 63-year-old woman with a history of recurrent retroperitoneal LPS, who presented to our department with right hypochondriac discomfort and progressive abdominal distentio

Purpose: Diffuse malignant peritoneal mesothelioma is a rare disease requiring management in specialized referral centers. We report our institutional experience with epithelioid diffuse malignant peritoneal mesothelioma, focusing on a standardized treatment algorithm and on pre-operative cisplatin-pemetrexed as a bridge to cytoreductive surgery and HIPEC. Methods: Consecutive patients treated between 2016 and 2024 were retrospectively reviewed. Treatment allocation was based on diagnostic laparoscopy, PCI assessment, and multidisciplinary evaluation. Patients with technically resectable disea

A primigravida in her 20s presented to the emergency department at 30 weeks gestation with acute onset, sharp right lower abdominal pain with associated nausea and emesis. Symptoms were refractory to intravenous antiemetics and multimodal pain regimen. Laboratory work-up was unremarkable. Abdominal ultrasound showed mild right hydronephrosis with perinephric oedema and complex fluid collection. Follow-up MRI revealed a large fluid collection in the right retroperitoneum, consistent with a right renal forniceal rupture. A right double-J ureteral stent was placed with resolution of symptoms. Undifferentiated embryonal sarcoma of the liver (UESL) is a rare, highly aggressive malignant mesenchymal tumor that predominantly affects children and typically carries a poor prognosis. We report a unique UESL case with long-term survival harboring a novel MAD1L1::ERG gene fusion. A 15-year-old female underwent segmentectomy for a 15-cm hepatic mass, without a precise diagnosis initially. From 2018 to 2025, she experienced multiple recurrences involving the liver, right kidney, and retroperitoneum. Histopathological examination across all recurrences consistently showed classic UESL features

Low-grade fibromyxoid sarcoma (LGFMS) is a rare, slow-growing deceptively bland-looking fibroblastic sarcoma that usually develops in the deep soft tissues of the extremities. Development of the tumor in the abdomen, pelvis, and retroperitoneum is extremely rare. We analyzed 26 cases of LGFMS originating at these sites. The patients age ranged from 5 to 75 years (median, 39) with a slight female predominance (54%). Most tumors were located in the abdominal wall/abdomen or peritoneal tissues (n = 11; 42%), followed by the intestinal wall/mesentery (n = 7; 27%), pelvis (n = 4; 15%), retroperiton

Bronchogenic cysts are congenital foregut malformations most often found in the mediastinum or lung parenchyma. Abdominal locations are exceedingly rare and most commonly found in the left retroperitoneum. They may mimic other intra-abdominal masses and pose a preoperative diagnostic challenge. A 17-year-old female presented with 4 days of diffuse, mild abdominal pain. Ultrasound and MRI identified a 5.5 × 3.8 × 3.1 cm unilocular, hypoechoic cyst adjacent to the terminal ileum at the ileocecal valve. Preoperative differential diagnoses included an ileal duplication cyst. Single-incision laparo

Unicentric Castleman disease (UCD) is a rare benign lymphoproliferative disorder commonly found in the mediastinum and retroperitoneum, with mesenteric involvement being exceptionally uncommon. We present a case of a 21-year-old male with a history of constipation, fatigue, and recurrent right iliac fossa pain for six months. Initial imaging suggested a gastrointestinal stromal tumor (GIST); however, the final diagnosis of UCD hyaline vascular (HV) variant was made based on immunohistochemical (IHC) analysis and histopathological reports. Surgical excision led to complete resolution of symptom

Retroperitoneal fibrosis is a fibro-inflammatory condition characterised by the development of dense tissue within the retroperitoneum, which may result in the compression of adjacent structures. We report the case of a 69-year-old woman with a history of breast cancer who initially presented with right-sided hydronephrosis. Cross-sectional imaging demonstrated retroperitoneal soft tissue encasing the ureters, consistent with retroperitoneal fibrosis. She underwent unilateral ureteric stenting followed by bilateral nephrostomy insertion and was commenced on corticosteroid therapy. Following re

Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory condition producing inflammatory masses in the retroperitoneum that can obstruct retroperitoneal structures, including vessels and ureters. RPF typically presents in middle-aged men and may be associated with IgG-4-related disease (IgG-4 RD). This report describes a 34-year-old woman who presented 6 months postpartum with left leg swelling and lower abdominal pain. Contrast-enhanced CT revealed a homogenous retroperitoneal soft-tissue mass encasing the aorta and iliac arteries, with inferior vena cava (IVC) obstruction and collateral v

Subhepatic appendicitis is a particularly rare anatomical variant presenting with atypical right upper quadrant (RUQ) pain mimicking hepatobiliary pathology. This mimicry frequently leads to diagnostic delay and subsequent increased risk of complications. A 41-year-old man presented to the Emergency Department with acute RUQ pain, radiating to the back and shoulder, accompanied by constitutional and gastrointestinal symptoms. RUQ ultrasound visualized no gallbladder abnormalities; subsequent CT scan revealed a dilated appendix with a rounded tip and possible presence of an abscess. Initial sur

In testicular germ cell tumors (TGCTs), staging is a major determinant of disease-specific outcomes and dictates clinical management. The 8th edition of the American Joint Committee on Cancer (AJCC) staging manual recommends that tumor deposits in the spermatic cord that are discontinuous with the primary tumor be considered pathologic stage M1 (clinical stage [CS] III). This recommendation is controversial because, if patients do not show other metastases, it results in upstaging from CS I to III. This is relevant because CSI patients are candidates for active surveillance, whereas chemothera

BACKGROUND: Plexiform neurofibromas are benign peripheral nerve sheath tumors that typically present in childhood and are most often associated with neurofibromatosis type 1. They usually manifest as diffuse, infiltrative masses involving a single nerve distribution. Adult-onset, multifocal, discrete retroperitoneal plexiform neurofibromas are rare and sparsely reported. This case is presented to expand the known phenotypic spectrum and to emphasize the diagnostic challenges posed by atypical presentations.

Inflammatory myofibroblastic tumour (IMT) is an uncommon mesenchymal neoplasm with intermediate malignant potential, typically arising in the lungs, abdomen, or retroperitoneum. Breast involvement is extremely rare, with limited reported cases. To the best of our knowledge, less than 50 cases of this lesion in the breast have been reported to date and cytological descriptions are very rare. A 47-year-old female presented with a painless mass in the right breast. Imaging revealed a well-defined lobulated soft tissue mass categorised as BIRADS 4A. Background: Papillary thyroid carcinoma (PTC) is typically associated with an excellent prognosis, and distant metastases are uncommon. When present, metastases most frequently involve the lungs and bones, while intra-abdominal dissemination is exceedingly rare and poorly characterized. The optimal management of such atypical metastatic patterns, particularly the role of surgery, remains undefined. Case Presentation: A 69-year-old female presented eight years after total thyroidectomy, cervical lymph node dissection, and radioactive iodine therapy for PTC with progressive abdominal pain, nause
