The Clinical Potential for Therapeutic Drug Monitoring in Alternative Matrices: Considerations for Systemic and Compartmentalized Pharmacology.

Therapeutic drug monitoring (TDM) using alternative matrices beyond traditional venous blood-based approaches, such as capillary blood-based microsampling, cerebrospinal fluid, breast milk, and peripheral blood mononuclear cells, has the potential to enhance precision medicine. Microsampling techniques provide a less invasive and cost-effective alternative for implementing TDM in populations from remote and access-limited areas. Evaluation of drug distribution across physiologic compartments based on pharmacokinetic principles can improve drug efficacy and minimize adverse effects in diverse c

Evaluation of Olaparib Efficacy Following CDK4/6 Inhibitor Therapy in BRCA-Mutated, Hormone Receptor-Positive, HER2-Negative Metastatic Breast Cancer: A Turkish Oncology Group (TOG) Study.

BACKGROUND: Poly(ADP-ribose) polymerase (PARP) inhibitors are established treatment options for patients with germline BRCA1/2-mutated hormone receptor-positive/human epidermal growth factor receptor 2-negative (HR+/HER2-) metastatic breast cancer. However, evidence regarding their optimal sequencing after cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitor-based therapy remains limited. OBJECTIVE: To evaluate the real-world efficacy and safety of olaparib after prior CDK4/6 inhibitor plus endocrine therapy, with particular emphasis on treatment sequence and line of olaparib use. PATIENTS AND

Identification of an hCG Isoform Which Is Similar in Sequence to a Bile Salt-Stimulated Lipase.

During the protein identification and quantitation of human breast milk samples, an hCG isoform was identified using an NCBI human database, but a further look into the protein sequence by running an NCBI BLAST revealed the similarity to a human bile salt-stimulated lipase protein. Here we used known protein databases to determine whether or not the identified protein is truly an hCG isoform or is actually a bile salt-stimulated lipase protein instead. Using NCBI BLAST, NCBI Align, UniProt Align, and ExPasy Align, we determined that the hCG isoform identified is more similar to the bile salt-s

Sequence-activity mapping and model-guided engineering of the phenol-responsive DmpR-Po σ54 promoter.

Promoters are core cis-regulatory elements that determine gene expression levels and dynamic responses, and their quantitative engineering directly dictates the performance of synthetic biological systems such as whole-cell biosensors. However, the transcriptional output of complex inducible systems, exemplified by σ54-dependent promoters, relies on long-range coordination among distal bacterial enhancer-binding proteins, DNA architectural proteins, and RNA polymerase. The combined effects of multiple regulatory regions on promoter output remain poorly understood, which limits the cross-backgr

Association between radiotherapy laterality and cardiovascular disease mortality in male breast cancer patients receiving postoperative radiotherapy: a competing risks analysis and standardized mortality ratio assessment using SEER data.

BACKGROUND: The association between radiotherapy (RT) laterality and cardiovascular disease (CVD) mortality in male breast cancer (MBC) remains unknown. We evaluated whether left-sided RT confers higher CVD mortality than right-sided RT in MBC patients, and quantified excess CVD mortality relative to the general U.S. male population. METHODS: This retrospective cohort study included 2,152 MBC patients receiving postoperative RT from the SEER database (2000-2020). Fine-Gray competing risks regression estimated subdistribution hazard ratios (sHRs) for CVD death comparing left- vs. right-sided R

Calendar-defined missed-workday gaps in radiotherapy: a site- and phase-stratified audit with linkage sensitivity analyses.

BACKGROUND: Missed treatment workdays can prolong overall treatment time and may reduce tumor control, yet routine audits often conflate weekends, statutory holidays, and departmentally accepted rest schedules with untreated institutional workdays. METHODS: We quantified calendar-defined missed-workday gaps-inter-fraction intervals containing ≥1 untreated institutional workday after excluding routine rest days-among patients with first treatment between June 2021 and December 2025 who were linkable between the MOSAIQ radiotherapy information system and the institutional oncology database (1,3

Background parenchymal enhancement in ultrafast MRI: Inter-reader agreement before and after training, and correlations with prognostic factors of breast cancer.

PURPOSE: To assess the inter-reader agreement in background parenchymal enhancement (BPE) categorization on ultrafast dynamic contrast-enhanced (DCE) MRI before and after training and its relationship with breast cancer prognostic factors. MATERIALS AND METHODS: We retrospectively analyzed the cases of 220 patients (median, age 60 years; 220 breast cancers) who underwent ultrafast DCE MRI. BPE was qualitatively classified as minimal, mild, moderate, or marked by three readers: Reader1 (R1), a radiology resident in the second year of training; Reader2 (R2), an experienced general radiologist w

Structure-guided computational design and mechanistic understanding of the p95HER2-targeting NAZ-mAb antibody and its variants.

Human epidermal growth factor receptor 2 (HER2) is an oncogenic receptor tyrosine kinase in breast cancer and other malignancies. A subset of HER2-positive tumours expresses 611-CTF-p95HER2, a tumour-specific, hyperactive truncated isoform associated with metastasis and treatment resistance that lacks most of the extracellular domain targeted by conventional HER2-directed antibodies. We previously developed NAZ-mAb (formerly known as Oslo-2), a monoclonal antibody against 611-CTF-p95HER2. Here, we describe a computational antibody-engineering workflow for designing variants of NAZ-mAb. Startin

Breast cancer burden risk attribution and productivity loss in Chinese women aged 25 to 49.

Breast cancer among Chinese women aged 25-49 years has substantial clinical and societal consequences. We integrated Global Burden of Disease (GBD) 2023 estimates for 1990-2023 with Global Cancer Observatory (GLOBOCAN) 2022, United Nations population projections, International Labour Organization labour-force participation, and World Bank indicators. Incident cases rose from 50,064 to 87,221, while the weighted death rate declined from 9.25 to 4.72 per 100,000. Demographic changes explained 63.0% of additional cases. In exploratory extreme gradient boosting (XGBoost) analyses interpreted with

Effect of Pain Science Education and Behavioral Graded Activity on Pain in Survivors of Breast Cancer: A Study Protocol for a Multicenter Randomized Controlled Trial.

IMPORTANCE: Pain affects up to half of survivors of breast cancer and is one of the most common long-term consequences of cancer treatment, yet it remains under-recognized and undertreated. Pharmacological options often provide limited relief and may cause adverse effects. Evidence from other pain populations supports a biopsychosocial approach combining pain education and physical activity. OBJECTIVE: The objective is to evaluate whether pain science education (PSE) with behavioral graded activity (BGA) versus usual care (UC), reduces pain intensity and interference in survivors of breast ca

Health state values in cancer patients and the general population: evidence from SF-6Dv2.

BACKGROUND: Patient and general population health state values (HSVs) often diverge, yet evidence on the updated SF-6D is scarce. This study compared HSVs for identical SF-6Dv2 health states elicited from cancer patients and the general population using the composite time trade-off (cTTO) method. METHODS: Data were drawn from two valuation studies in Quebec, Canada: a general population sample completing an online panel (n = 1,179) and patients with breast or colorectal cancer first assessed via face-to-face interviews and subsequently online (n = 202). HSVs for 156 SF-6Dv2 health states were

Change in alcohol consumption and all-cause mortality among postmenopausal women diagnosed with breast cancer.

PURPOSE: Evidence is inconsistent regarding an association between alcohol consumption and survival following a breast cancer diagnosis; therefore, we examined how change in alcohol consumption affects all-cause mortality (ACM) in a population of breast cancer survivors. METHODS: We prospectively assessed pre-diagnosis, pre- to post-diagnosis, and post-diagnosis change in alcohol consumption among 1,558 postmenopausal women (mean age-at-diagnosis: 71 years) diagnosed with non-metastatic invasive breast cancer from the American Cancer Society Cancer Prevention Study-II Nutrition Cohort (baseli

Isolated cerebellopontine angle metastasis from breast cancer: The added value of MR spectroscopy.

Cerebellopontine angle (CPA) tumors are predominantly vestibular schwannomas and meningiomas; metastatic lesions in this location are rare, particularly in the absence of multiple brain metastases. We report the case of a 53-year-old woman with a history of invasive ductal carcinoma of the breast in HER2-positive metastatic relapse, in whom brain MRI revealed a single 28 × 24 mm mass in the right cerebellopontine angle. The lesion was isointense on T1- and T2-weighted images, contained fluid components, and showed restricted diffusion (ADC 0.64 × 10⁻³ mm²/s) with intense peripheral enhancement

Impact of Childbearing and Breastfeeding on Breast Density Change, and Subsequent Breast Cancer Risk in Postmenopausal Women: A Mediation Analysis.

We examined the associations of parity and breastfeeding with postmenopausal breast density, longitudinal changes in breast density, and breast cancer risk, and assessed the extent to which breast density mediates these relationships. We analyzed a cohort of women who underwent mammography in 2009-2010 and had at least one follow-up mammogram through 2016. Parity and breastfeeding history were self-reported, breast density was classified using Breast Imaging Reporting and Data System (BI-RADS), and relative risks (RRs) were estimated using Poisson regression. Mediation analysis evaluated the c
