Obesity and MASLD (NAFLD) are chronic, low-grade systemic inflammatory diseases resulting from complex interactions among diet, adipose tissue, gut microbiota, liver, muscle, pancreas, and the brain. Central to this integrative framework are AA and other polyunsaturated fatty acids (PUFAs), which regulate inflammation, insulin sensitivity, neuroendocrine signaling, and energy homeostasis.Excess consumption of energy-dense foods rich in saturated and trans fats leads to adipocyte hypertrophy, altered membrane fluidity, increased antigenicity, and enhanced recruitment of macrophages and T cells

Polyunsaturated fatty acids (PUFAs), especially gamma-linolenic acid (GLA), arachidonic acid (AA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA), can block HMG-CoA reductase activity and thus decrease cholesterol synthesis/formation. Cholesterol has antioxidant actions and thus reduces the formation of ROS (reactive oxygen species). In contrast, PUFAs augment the generation of ROS. PUFAs undergo peroxidation to form lipid peroxides that are toxic to tumor cells but not normal cells. GPX4 (glutathione peroxidase) reduces the formation of lipid peroxides and thus enhances tumor cel

Polychromatic flow cytometry enables the detection and characterization of markers that are helpful in defining the phenotype of various cell subsets. Here, we described a flow cytometry-based method to characterize the phenotype of naïve, memory, and effector T cells. Being able to differentiate these cells is crucial in understanding immune response and immune profiling. Naïve T cells enable the body to fight off new, unrecognized infections and diseases, and Memory T cells are enriched for response to recall antigens. Furthermore, the antigen-experienced T-cell populations can be broadly di

Dendritic cells (DCs) are powerful antigen-presenting cells involved in regulating immune response. Plasmacytoid dendritic cells (pDCs) are a subtype of DCs present in small quantities in the bone marrow, peripheral blood, and lymph nodes. They are an important component of our immune system's regulation and response to diseases. They activate T cells and play a critical role in immune tolerance. In this chapter, we review the immunophenotypic features of pDCs and provide a practical protocol for pDCs enumeration in the peripheral blood and in bone marrow samples.

Immunophenotyping by multiparameter flow cytometry is a rapid and efficient technique to simultaneously assess and correlate multiple individual cell properties like size and internal complexity along with antigen expression in a population of cells. This method is utilized for rapid characterization of the blasts and classification of acute myeloid leukemia (AML) in both the peripheral blood (PB) and bone marrow (BM). This technique is not only useful in the initial diagnosis but also in monitoring and determining the prognosis of the disease through minimal residual disease (MRD) testing. Th

Immunophenotyping by flow cytometry is an important component in the diagnostic evaluation of patients with acute lymphoblastic leukemia. This technique further permits the detection of minimal residual disease after therapy, a robust prognostic factor that may guide individualized treatment. We describe here laboratory methods for both the initial characterization of lymphoblasts at diagnosis and the detection of rare leukemic lymphoblasts after treatment. In addition to antibody combinations suitable for diagnosis and detection of minimal residual disease, we describe procedures for peripher

Flow cytometry is the cornerstone for establishing the diagnosis of chronic lymphocytic leukemia (CLL), owing to its characteristic and well-defined immunophenotype that enables accurate distinction from other leukemias and lymphomas. Beyond diagnosis, flow cytometry provides essential prognostic information and allows sensitive detection of minimal residual disease (MRD), a strong predictor of clinical outcome. CLL MRD assessment is increasingly used to guide risk stratification, therapeutic decision-making, and treatment duration in the era of targeted therapies and immunotherapies. This cha

Regulatory T cells, or Tregs, are a unique type of CD4⁺ T cell that act as the immune system's brakes-they stop it from turning against the body's own tissues. For about twenty years now, researchers have been digging into what sets Tregs apart from other T cells, both in their composition and function (1). Markers like CD25 (IL-2Rα), Foxp3, and CTLA-4 usually show up when Tregs are active, but these markers aren't always reliable-they can shift depending on where the cells are and what they are doing. Foxp3 is a transcription factor that Tregs need to develop and function. Mutations on Foxp3

Flow cytometric analysis can provide valuable information on the viability, phenotype, and therapeutic potential of cell therapy products using a minimal amount of cell numbers. In this chapter, we describe methods for the analysis of clinical products that incorporate extra considerations within the protocols to help maintain consistency and confidence of staining. This can include methods for evaluating the transduction efficiency for T cells expressing chimeric antigen receptors (CAR-T), characterizing the frequency of diverse lymphocyte populations within polyclonal cell cultures and posti

Measurable residual disease (MRD) serves as a critical biomarker of prognosis, treatment efficacy, and clinical outcome. It captures the presence of residual tumor cells below the detection threshold of conventional microscopy. Multiparametric flow cytometry (MFC) offers a rapid, cost-efficient, and widely applicable platform for MRD detection through leukemia-associated immunophenotypes (LAIPs) and deviation-from-normal (DfN) antigen maturation patterns. This technique underpins a wide range of clinical applications, including risk stratification, therapeutic decision-making, and post-transpl

Electroporation-based delivery of CRISPR/Cas systems has emerged as a powerful and versatile approach for gene editing in primary human T cells, enabling efficient, transient, and nonviral modification while minimizing genomic integration risks. This chapter focuses on the principles and practical implementation of electroporation (nucleofection) for the delivery of Cas9 ribonucleoprotein (RNP) complexes into human T cells, highlighting critical parameters that influence editing efficiency, cell viability, and scalability for research and clinical applications. We provide a comprehensive proto

Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

High-resolution characterization of antigen-specific T cell responses is essential for understanding cellular immunity to Chlamydia trachomatis infection in humans. This chapter describes two complementary flow cytometry-based approaches for measuring human antigen-specific T cell responses: intracellular cytokine staining (ICS) and activation-induced marker (AIM) staining. Both assays enable single-cell-level analysis following antigen stimulation but differ in their readouts. ICS quantifies functional cytokine production in response to short- or long-term stimulation, whereas the AIM assay i

Reticular dysgenesis (RD) is a rare autosomal recessive severe combined immunodeficiency characterized by profound neutropenia, T-cell lymphopenia and sensorineural hearing loss, and is caused by biallelic loss-of-function variants in AK2. We identified a homozygous AK2 splice-site variant (c.498 + 1G > A) in a boy with RD and concurrent B-cell acute lymphoblastic leukemia (B-ALL), a clinical combination not previously reported in association with an AK2 variant. An in vitro minigene assay showed that c.498 + 1G > A disrupts the canonical 5' splice donor site and generates an aberrant transcri

BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID.
CASE PRESENTATIONS: All three patients had de novo variants i

BACKGROUND: Steatotic liver disease (SLD) is highly prevalent among individuals with cardiovascular disease, but often remains undiagnosed until advanced sequelae such as fibrotic liver disease and hepatocellular carcinoma develop. Transthoracic echocardiography routinely captures incidental subcostal views of the liver that are not used clinically. The EchoNet-Liver algorithm uses artificial intelligence (AI) to predict the presence of liver disease based on routine clinical echocardiograms. Prospective evaluation in real-world practice is needed to understand the clinical utility and impac

Mycophenolate mofetil (MMF) is an immunosuppressive agent used extensively in post-transplant management and the treatment of autoimmune diseases. Its hematological adverse effects manifest primarily as anemia, leukopenia, and thrombocytopenia induced by bone marrow suppression. Adverse reactions associated with coagulation dysfunction are rarely reported. This report describes a case of hypofibrinogenemia attributed to MMF in a renal transplant recipient. The patient developed fibrinogen levels below 0.6 g/L, accompanied by cutaneous ecchymosis, after initiating MMF therapy. Fibrinogen levels
