Case Report: Myelodysplastic changes and 7q deletion in a case of vitamin B12 deficiency: a diagnostic pitfall.

Vitamin B12 plays a crucial role in DNA synthesis, and its deficiency can occasionally present with atypical hematological manifestations mimicking myelodysplastic syndrome (MDS). There is limited literature describing reversible chromosomal abnormalities associated with vitamin B12 deficiency. We report an 11-year-old girl who presented with generalized body aches for 1 month and fever, easy fatigability, and respiratory distress for 2 days. Examination revealed pallor and hepatomegaly, with no splenomegaly. Investigations showed severe anemia (hemoglobin 3.1 g/dL), thrombocytopenia, a low co

Hematologic Malignancies With RUNX1::ZFPM2 Translocation: A Case Report and Literature Review.

UNLABELLED: RUNX1 rearrangements are recurrent in myeloid neoplasms, but rare fusion partners remain poorly characterized. We report the first acute myeloid leukemia (AML) with a RUNX1::ZFPM2 fusion generated by t(8;21)(q23;q22). A 72-year-old woman presented with acute myelomonocytic leukemia carrying concomitant IDH2 and NRAS mutations. Azacitidine-venetoclax induced transient remission with a marked decrease in the fusion transcript. However, persistent cytopenias, molecular relapse, and treatment resistance led to death 1 year after diagnosis. This case expands the spectrum of RUNX1-rearra

Murine Typhus Mimicking Preeclampsia with Severe Features.

Background Murine typhus, a flea-borne rickettsiosis endemic to southern Texas, can cause headache, thrombocytopenia, and transaminitis that overlap with the criteria for preeclampsia with severe features. Case A 22-year-old gravida 5 with gestational hypertension presented at 32 5/7 weeks of gestation with 5 days of fever and headache. Worsening thrombocytopenia, transaminitis, and refractory headache were initially attributed to preeclampsia with severe features, and magnesium sulfate was administered. Persistent fever prompted empiric doxycycline for suspected murine typhus; defervescence o

Renal-Limited Thrombotic Microangiopathy Presenting as Acute on Chronic Kidney Failure in the Absence of Schistocytosis.

We report a rare presentation of renal-limited thrombotic microangiopathy (TMA). TMA is a clinicopathologic syndrome resulting from endothelial injury and activation of the coagulation and platelet pathways, leading to the formation of platelet-rich microthrombi within small vessels. This loss of blood flow due to the thrombi can result in tissue ischemia and end-organ injury. Systemic TMA classically manifests with the triad of microangiopathic hemolytic anemia (MAHA), thrombocytopenia, and organ dysfunction. However, TMA may be limited predominantly to the kidneys, resulting in acute or prog

Platelet Indices as Biomarkers of Disease Severity and Morbidity in Hospitalized Dengue Patients.

BACKGROUND: Dengue fever is a rapidly expanding mosquito-borne illness with significant morbidity in children. While thrombocytopenia is a key feature, platelet count alone poorly predicts disease severity. Platelet indices such as mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT) may provide better insight into disease progression and complications. MATERIALS AND METHODS: This hospital-based cross-sectional observational study was conducted in the Department of Pediatrics, at a tertiary care hospital over 18 months. A total of 120 children aged 1-16 years

CD19+ CD 38+ regulatory B lymphocytes as exploratory prognostic markers of steroid-induced remission in treatment-naïve adult immune thrombocytopenia.

Corticosteroids are commonly used as first-line therapy for immune thrombocytopenia (ITP) to suppress immune-mediated platelet destruction. However, some patients fail to respond, and prolonged use is associated with significant adverse effects, necessitating alternative therapeutic modalities. The aim of this study was to evaluate the role of Cluster of differentiation (CD)19+CD38+ B cells with presumed regulatory potential as exploratory prognostic markers of steroid-induced response in treatment-naïve adult ITP patients. This prospective cohort study included 37 adults with newly diagnosed

Efficacy and safety of ifupinostat in relapsed or refractory diffuse large B-cell lymphoma: a multicenter, single-arm phase 2 study.

Ifupinostat (BEBT-908) is a novel dual-targeted inhibitor of histone deacetylase (HDACs) and phosphatidylinositol 3-kinase alpha (PI3Kα). We evaluated the efficacy and safety of ifupinostat monotherapy in patients with relapsed or refractory (r/r) diffuse large B-cell lymphoma (DLBCL) after at least 2 prior lines of systemic therapy. Eligible participants received intravenous ifupinostat at 22.5 mg/m² on days 1, 3, 5, 8, 10, and 12 of a 21-day cycle. The primary endpoint was objective response rate (ORR) assessed by blinded independent central review (BICR). Secondary endpoints included diseas

Comparative Efficacy and Safety of Bruton Tyrosine Kinase and Spleen Tyrosine Kinase Inhibitors in Immune Thrombocytopenia: A Network Meta-Analysis of Clinical Trials.

INTRODUCTION: Targeted kinase inhibitors have shown statistically and clinically significant improvements in platelet response among patients with primary ITP, supporting their utility as effective targeted therapeutic strategies. We evaluated the efficacy and safety of Bruton tyrosine kinase (BTK) and spleen tyrosine kinase (SYK) inhibitors in primary immune thrombocytopenia (ITP) using network meta-analysis. METHODS: Following PRISMA-NMA guidelines, five trials, including 660 patients, were analyzed. Comparing rilzabrutinib, HMPL-523 (sovleplenib), and fostamatinib against placebo. The prim

Autologous hematopoietic stem cell transplantation in a patient with relapsed diffuse large B-cell lymphoma and type 3 von Willebrand disease: a case report.

Autologous hematopoietic stem cell transplantation (auto-HSCT) is standard therapy for eligible patients with relapsed diffuse large B-cell lymphoma (DLBCL). Type 3 von Willebrand disease (VWD) poses additional haemostatic challenges during postconditioning aplasia. We report a 65-year-old man with DLBCL in second complete remission and type 3 VWD who underwent auto-HSCT. During bone marrow aplasia, complicated by mucositis and melena, intensified VWF/FVIII replacement and transfusion support were required. Following a switch to a concentrate with a higher VWF-to-FVIII ratio, improved haemosta

A review of the monitoring of argatroban by anti-IIa methods, ecarin assays, point-of-care testing methods and specialized drug quantification methods for patients with heparin-induced thrombocytopenia.

Heparin-induced thrombocytopenia (HIT) is caused by anti-PF4/heparin IgG antibodies stimulating platelet activation, causing thrombocytopenia and thrombosis. Classical HIT occurs due to exposure to heparin, whereas autoimmune HIT occurs via heparin-independent platelet activation (caused by anti-PF4/polyanion IgG) and relates to a platelet count fall that begins or persists without heparin. Treatment for HIT requires transitioning patients onto alternative anticoagulation. The direct thrombin inhibitor argatroban has been licensed for use in patients with HIT and was recommended to be monitore

A new reporter strategy reveals a subset of nonmigratory hematopoietic stem cells located in a dynamic bone marrow niche.

Interactions of hematopoietic stem cells (HSCs) with the bone marrow microenvironment are critical to regulate stem cell function in homeostasis, emergency hematopoiesis and ageing. The dynamic behavior of endogenous HSCs in their niches has been challenging to study due to the complexity of markers required to define pure HSCs. Multiple recently developed reporter strategies advanced our capacity to identify HSCs in situ. Yet, they provide different levels of HSC enrichment and are frequently not stable in situations of perturbed hematopoiesis, leading to contradictory observations of HSC loc

Primary adrenal small cell neuroendocrine carcinoma in a patient with sarcoidosis and synchronous colon adenocarcinoma.

Primary adrenal neuroendocrine carcinoma (NEC), particularly the small cell subtype, is exceptionally rare. In patients with sarcoidosis, inflammatory lymphadenopathy may mimic a metastatic disease, making disease detection difficult. A 76-year-old man with sarcoidosis was referred for an enlarging left adrenal incidentaloma that had increased in size from 7 mm to 19 × 14 mm over 3 years. Hormonal evaluation revealed unremarkable findings; laparoscopic adrenalectomy was performed because malignancy could not be excluded. Histopathological examination revealed small cell NEC with positive synap

Complex hybrid repair of multifocal visceral and aortoiliac pseudoaneurysms associated with neurosarcoidosis.

We report the case of a 36-year-old man with neurosarcoidosis who developed multiple pseudoaneurysms of the superior mesenteric artery (SMA), infrarenal aorta, and bilateral iliac arteries. Fluorodeoxyglucose-positron emission tomography demonstrated no significant uptake within the aneurysmal segments. Given the complex vascular anatomy, a staged hybrid approach was undertaken. Initial treatment consisted of a hepatic artery-to-distal SMA bypass followed by plug embolization of the SMA aneurysms. Subsequently, aortoiliac reconstruction was performed with a left aortouni-iliac endograft, femor

Diagnostic challenges of masked arginine vasopressin deficiency in neurosarcoidosis complicated by sodium-glucose cotransporter 2 inhibitor use.

A wide variety of endocrine disorders caused by sarcoidosis have been reported to date. We report the case of an 82-year-old male patient who presented with hypercalcemia and anterior pituitary hormone deficiency at the time of his sarcoidosis diagnosis and was subsequently diagnosed with arginine vasopressin deficiency (AVP-D) 6 months after the initiation of glucocorticoid therapy. Magnetic resonance imaging revealed an enlarged pituitary gland, thickened pituitary stalk, and loss of the posterior pituitary bright spot, raising the suspicion of hypothalamic-pituitary sarcoidosis. Despite imp

Heerfordt-Waldenström Syndrome With Cardiac Sarcoidosis.

Heerfordt-Waldenström syndrome is a rare presentation of sarcoidosis with only a handful of cases reported worldwide. It consists of a tetrad of uveitis, parotitis, and VII nerve palsy and chronic fever. Symptomatic cardiac involvement in sarcoidosis is an uncommon but potentially lethal manifestation. A timely diagnosis and treatment can result in the prevention of morbidity and mortality. We report a case of a young woman with multisystem sarcoidosis who presented with constitutional features, uveitis, multiple cranial nerve palsies, leptomeningitis, and ankle oligoarthritis. An exploratory simulation of the optimization of leukocyte transmigration, indicating that barrier routing is governed by context-dependent biochemical cues rather than hardwired programs.

INTRODUCTION: Leukocyte infiltration across the blood-brain barrier contributes to many neuroinflammatory disorders, yet the physical mechanisms that determine how immune cells cross the barrier remain incompletely understood. In particular, it is unclear how leukocytes choose between paracellular migration through endothelial junctions and transcellular migration through the endothelial cell body. This question is especially relevant during central nervous system inflammation, where different T-lymphocyte subsets, including Th1 and Th17 cells, appear to preferentially use distinct migration r

A natural neuronal receptor-derived MRI probe targets β-amyloid oligomers for the diagnosis of Alzheimer's disease.

Early-to-mid detection of Alzheimer's disease (AD) remains challenging because clinically used amyloid positron emission tomography (PET) probes primarily visualize fibrillar β-amyloid (Aβ) deposits and require radioactive isotopes. Magnetic resonance imaging (MRI) offers a radiation-free alternative, but its application in molecular AD diagnosis is limited by insufficient blood-brain barrier (BBB) penetration and poor specificity toward early-to-mid Aβ species. Here, we developed a BBB-crossing liposomal MRI probe, Lipo@D1D2-Gd, by integrating a natural Aβ oligomer-binding receptor domain wit
