Ironing Out the Deficiency: Tracking Iron in Celiac Disease Before and After the Gluten-Free Diet.

Celiac disease (CeD) is a gluten-induced immune-mediated enteropathy that preferentially involves the proximal duodenum. Consequently, iron deficiency is common in CeD, impacting at least 10% of newly diagnosed individuals. In this narrative review, we aim to investigate the mechanisms, prevalence, treatment, and monitoring of iron deficiency and the impacts of a gluten-free diet (GFD) on iron deficiency in individuals with CeD. We will also review the role of and approach to iron supplementation in this population. Specifically, we will explore whether mucosal healing on a GFD is sufficient f

Haemophilus influenzae Bacteremia With Polyarthritis and Uterine Fibroids in a 31-Year-Old Woman: A Case Report.

Haemophilus influenzae (H. influenzae) can, in rare cases, infect the female reproductive tract, typically presenting with fever, lower abdominal pain, and vaginal discharge. Rarely, infection manifests with diffuse polyarticular pain and systemic symptoms. We report the case of a woman in her early 30s who presented to the emergency department (ED) with a two-day history of polyarticular joint pain, malaise, chills, and weakness. Examination revealed tachycardia and tenderness of multiple joints on passive and active range of motion. Laboratory evaluation showed leukocytosis, elevated C-react

Complexities of Occult and Obscure Gastrointestinal Bleeding: A Case Report.

Small-bowel vascular malformations represent an uncommon but important source of obscure bleeding that can lead to iron deficiency anemia. Diagnosis and management can be challenging due to the intermittent nature and inaccessibility of small-bowel lesions. We present a 36-year-old man with recurrent iron deficiency anemia (IDA) requiring monthly iron infusions, in whom investigations using multiple modalities revealed the unusual finding of multiple small-bowel vascular malformations. Extensive investigations have failed to definitively localize a culprit bleeding point. This case highlights

The Vital Role of Family Physicians in the Screening and Early Detection of Lynch Syndrome: A Case Report.

Lynch syndrome (LS) is an autosomal dominant disorder associated with an increased risk of colorectal, endometrial, and other malignancies. This report describes the case of a 53-year-old female patient with a significant family history of colorectal cancer. She presented with metrorrhagia and iron deficiency anemia. Given her strong familial cancer history, besides the transvaginal ultrasound, a colonoscopy was performed despite a recent normal result. The new colonoscopy revealed a vegetative and ulcerated neoplasm at the rectosigmoid junction, confirmed as adenocarcinoma by biopsy. Genetic

Reduced gastrointestinal iron uptake in pulmonary arterial hypertension: a prospective cross-sectional study.

BACKGROUND: Iron deficiency (ID) is common in patients with pulmonary arterial hypertension (PAH) and is associated with worse clinical outcomes. The etiology of ID in PAH is poorly understood. The aim of this study was to systematically determine whether differences in oral iron absorption exist between PAH patients with and without chronic ID compared with healthy controls. METHODS: This single-center prospective, cross-sectional cohort study enrolled 45 subjects: 15 PAH patients with chronic ID, 15 PAH patients without ID and 15 healthy age and sex matched controls. Chronic ID was defined b

Combined Oral Contraceptive Pills Versus Progestin-Only Pills for Heavy Menstrual Bleeding: A Systematic Review.

Heavy menstrual bleeding (HMB) is a common gynecological condition among women of reproductive age that significantly affects quality of life and often leads to iron deficiency anemia. Hormonal treatments are usually prescribed in combination to include combined oral contraceptives (COCs) and progestin-only pills (POPs) in treating HMB. This systematic review aims to compare the effectiveness and safety of COCs and POPs in the management of HMB in terms of menstrual blood loss, hemoglobin levels, side effects, and patient satisfaction. The literature search was conducted on PubMed, Scopus, Web

Specific Features of Cerebral Venous Thrombosis Associated With Iron Deficiency Anemia.

BACKGROUND: Anemia has emerged as a risk factor for cerebral venous thrombosis (CVT) and has been associated with poorer outcomes. A specific association has been observed with microcytic anemia, most commonly caused by iron deficiency anemia (IDA). However, detailed data on the clinical, radiological, and therapeutic profile of CVT in patients with IDA remain limited. METHODS: We analyzed consecutive patients with CVT and moderate or severe IDA from the prospective Lariboisière Hospital cohort with CVT (1998-2023). Patients with other types of anemia were excluded. Clinical, neuroimaging, tr

Optimizing Treatment in Refractory Copper Deficiency: A Case Report.

Copper deficiency, though rare, is increasingly observed in patients who have undergone Roux-en-Y gastric bypass due to impaired absorption in the stomach, proximal duodenum, and small intestine. Deficiency may manifest as anemia, cytopenias, and myeloneuropathy. Standard treatment includes oral or intravenous (IV) copper supplementation, with IV replacement preferred in cases of malabsorption. We present a case of treatment-resistant copper deficiency in a woman in her late forties with a history of Roux-en-Y gastric bypass and iron-deficiency anemia, who presented with lower extremity parest

Molecularly imprinted polymer nanoparticle-based sequential competitive fluorescence assay for serum ferritin assessment.

BACKGROUND: The protein ferritin indicates the amount of iron reserves stored in the body and is crucial for diagnosing iron-deficiency anemia and assessing iron overload. Immunoassays, which depend on the selective interaction between anti-ferritin antibodies and ferritin antigens, were employed through various methodologies. The limitations of immuno-based approaches include the short effective storage duration of reagents (i.e., antibodies, enzymes) and high cost. A molecularly imprinted polymer nanoparticles (nanoMIPs)-based sequential competitive fluorescence assay was developed to evalua

Gold Nanoparticles Regulate Gut Microbiota and Immune Responses for Overcoming Iron Supplementation Side Effects in Anemia Therapy.

Iron deficiency anemia (IDA) is a common nutritional deficiency disease caused by iron deficiency. Oral iron supplementation, the simplest and most commonly used iron repletion strategy in the clinic, primarily induces gastrointestinal inflammation, which subsequently elicits complications, including nausea, gastrointestinal bleeding, and constipation. Oral iron supplementation, the most simple and widespread method of iron replenishment in the clinic, primarily causes gastrointestinal inflammation, which, in turn, leads to complications such as nausea, gastrointestinal bleeding, and constipat

The Small Bowel's Big Secret: A Case of Hidden Adenocarcinoma Unmasked by Capsule Endoscopy.

Small bowel adenocarcinoma (SBA) is an uncommon gastrointestinal malignancy that frequently presents with vague or nonspecific symptoms, leading to delayed diagnosis and poor outcomes. Because the small bowel is largely inaccessible to standard endoscopy, many patients endure prolonged abdominal pain, iron deficiency anemia, and weight loss before the underlying pathology is identified. Capsule endoscopy can reveal lesions that are often missed on upper or lower endoscopy and cross-sectional imaging, but its use remains limited in many centers due to issues of availability, cost, or delayed cl

Time-dependent trade-offs among intravenous iron formulations for iron-deficiency anemia: a longitudinal systematic review and network meta-analysis.

Intravenous iron formulations differ in benefits and risks that evolve across clinically relevant timepoints. A comprehensive network meta-analysis of randomized trials compared different intravenous iron formulations with oral iron and with each other. In addition to overall estimates, results are presented by predefined timepoints (4, 8, 12 and 24 weeks) for hemoglobin, ferritin, transferrin saturation, serious adverse events, and hypophosphatemia. Over 4-8 weeks, ferric carboxymaltose more consistently increased iron stores (ferritin/transferrin saturation) compared with the alternatives at
