Plummer-Vinson syndrome in a 10-year-old boy from Côte d'Ivoire: An exceptional paediatric case with African context.

Plummer-Vinson syndrome (PVS) is characterised by the triad of dysphagia, iron-deficiency anaemia, and proximal oesophageal webs. While well described in adults, paediatric cases remain exceptionally rare, particularly in sub-Saharan Africa. We report a 10-year-old boy from Côte d'Ivoire, presenting with progressive dysphagia of 3 years' duration and severe iron-deficiency anaemia (haemoglobin 6.0 g/dL) with neither haemoglobinopathy nor ova or parasites. Upper gastrointestinal endoscopy demonstrated a semi-circumferential post-cricoid oesophageal web; with normal surrounding mucosa and histol

Leading with dysphagia: an unusual view of a common symptom.

Plummer-Vinson syndrome (PVS) is characterized by the classic triad of post-cricoid dysphagia, iron-deficiency anemia and esophageal webs. PVS is commonly found in women of middle age especially in the fourth and fifth decade of life. The prevalence of PVS has decreased due to early detection of iron deficiency and repletion of iron stores. We report a case of a 81-year-old female patient who had a classic presentation of PVS, treated successfully with endoscopic procedure. To our knowledge, the current case is the fourth case of dysphagia related to Plummer-Vinson syndrome reported in an octo

Clinical Conundrum: Unveiling a Unique Presentation of Hypopharyngeal Carcinoma.

Dysphagia is a common symptom encountered in clinical practice, typically associated with a wide range of etiologies, including structural abnormalities, inflammatory conditions, neoplasms, and neurological disorders. However, the combination of subcutaneous emphysema, vocal cord palsy, enlarged arytenoids, and pooling of saliva in a dysphagic patient represents a rare and intriguing presentation. A 33-year-old female presented at a tertiary care hospital in Western India with hoarseness of voice, difficulty in swallowing, productive cough, and neck pain for two months with an abrupt increase

Tongue carcinoma in Plummer-Vinson syndrome: a modern rarity.

Plummer-Vinson syndrome (PVS), formerly known as Paterson-Brown-Kelly syndrome or sideropenic dysphagia, is a sporadic condition defined by the typical triad of symptoms, which are iron deficiency anaemia, oesophageal strictures and dysphagia. PVS predominantly affects women between the ages of 40 and 70. PVS, if left untreated or undiagnosed, may be associated with an increased risk of developing squamous cell carcinoma of the upper gastrointestinal tract or oral cavity, even in the absence of other risk factors. This case study describes a woman who is in her 50s who developed carcinoma of t

Prevalence of Oral Submucous Fibrosis With Other Oral Potentially Malignant Disorders: A Clinical Retrospective Study.

Background  Oral submucous fibrosis (OSMF) is a chronic, progressive, and potentially malignant oral disorder that causes scarring of the oral cavity, pharynx, and upper oesophagus. It is most common in Southeast Asia, but it is also found in other parts of the world. Oral potentially malignant disorders (OPMDs) are a group of oral lesions that have an increased risk of developing into oral cancer. The study aimed to evaluate the prevalence of OSMF associated with other OPMDs. The presence of multiple OPMDs existing in one patient is a significant finding, as it is associated with an elevated

Plummer-Vinson Syndrome with Simultaneous Mid-esophageal Growth.

Patterson Brown Kelly or Plummer-Vinson syndrome is characterized by dysphagia, iron deficiency anemia and post-cricoidal esophageal web. Waldenstorm introduced the term 'sideropenic dysphagia' because of absence of stainable iron in the bone marrow. There is increased incidence of upper aero-digestive tract carcinoma in patients with Plummer-Vinson syndrome has been well established. The reported rates range from 4% to 16%, with almost all cases occurring at the postcricoid location. We have reported here a case of a 48-year-old woman with dysphagia , upper esophageal web and iron deficiency a

Estimation of hemoglobin, serum iron, total iron-binding capacity and serum ferritin levels in oral submucous fibrosis: A clinicopathological study.

INTRODUCTION: Oral submucous fibrosis (OSMF) is also considered as an Asian version of sideropenic dysphagia, wherein chronic iron deficiency leads to mucosal susceptibility to irritants, such as chillies and areca nut products. Hemoglobin (Hb) levels, in particular, serum iron levels, are considered as biochemical indicators for nutritional assessment. Serum iron content can thus be a predictor for the progression of the condition, and there appears to be an association between serum iron content and oral carcinogenesis. Therefore, biochemical assessment of oral precancerous conditions may he

Gastric cancer occurring in a patient with Plummer-Vinson syndrome: a case report.

Plummer-Vinson syndrome (sideropenic dysphagia) is characterized by dysphagia due to an upper esophageal or hypopharyngeal web in patients with chronic iron deficiency anemia. The main cause of dysphagia is the presence of the web in the cervical esophagus, and abnormal motility of the pharynx or esophagus is also found to play a significant role in this condition. This syndrome is thought to be precancerous because squamous cell carcinoma of hypopharynx, oral cavity or esophagus takes place in 10% of those patients suffering from this malady, but it is even more unusual that Plummer-Vinson sy

Treatment of iron deficiency anemia with Ferro-Folgamma.

Iron deficiency anemia is a hypochromic anemia in which hemoglobin poor synthesis is due to a decrease in the amount of iron in the body. The decrease of iron quantity has many causes: insufficient intake of aliments rich in iron (meat, viscera, green vegetables), increased necessities during growth period, pregnancy, erythrocytes hyperregeneration, high-performance sportsmen, increased loss by digestive way, genito-urinary way, respiratory, hemorrhagic syndromes. Clinically, symptoms and signs specific to all types of anemia and those specific to lack of iron occur besides the symptoms and si

[Oral precancerous conditions--an overview].

Precancerous conditions are defined as a generalized state associated with a significantly increased risk of cancer. For the oral and perioral region, sideropenic dysphagia, oral lichen planus (OLP), oral submucous fibrosis (OSF), syphilis, discoid lupus erythematosus, xeroderma pigmentosum as well as epidermolysis bullosa play a role as precancerous conditions. The number of published cases of oral or perioral malignant transformation for some of these diseases is small. In addition, carcinomas of the tongue and the oral cavity associated with syphilis are of historical interest, although pub

Oral cancer. The importance of early diagnosis and treatment.

Oral cancer is an important health issue. The WHO predicts a continuing worldwide increase in the number of patients with oral cancer, extending this trend well into the next several decades. In the US the projected number of new cases of oral and oropharyngeal cancer will exceed 31,000 per year. Mortality due to cancers in this region exceeds the annual death rate is the US caused by either cutaneous melanoma or cervical cancer. Significant agents involved in the etiology of oral cancer in Western countries include sunlight exposure, smoking and alcohol consumption. Use of the areca or betel

Celiac disease presenting as the Paterson-Brown Kelly (Plummer-Vinson) syndrome.

We describe two patients with Paterson-Brown Kelly (Plummer-Vinson) syndrome whose iron deficiency anemia was due to celiac disease. They presented with dysphagia 13 and 9 yr, respectively, before celiac disease was diagnosed. Neither had gastrointestinal symptoms suggestive of malabsorption. Celiac disease is a recognized cause of chronic iron deficiency and should be considered as an etiological factor for sideropenic dysphagia.

The causation of oral precancer and cancer.

Most cases of oral cancer result from the action of exogenic carcinogenic agents, some of which act synergistically in producing their effects. The evidence implicating tobacco and alcohol abuse is overwhelming and cannot be refuted. Other clearly identifiable aetiological agents include betel nut chewing and excessive exposure to sunlight. While there is increasing evidence of a viral causation, this is not yet clearly established. Similarly, the role of Candida albicans remains uncertain. Lichen planus and discoid lupus erythematosus may constitute important predisposing conditions, but the

An alternative pathogenetic pathway for oral submucous fibrosis (OSMF).

Oral submucous fibrosis is a potent pre-cancerous condition which occurs almost exclusively in Indians but cases have been reported from several countries. The definite aetiology of OSMF is still unknown and there is no known treatment for it. Chilles, tobacco use, vitamin deficiencies and betel quid chewing have been implicated. Ramanathan is of the view that OSMF seems to be the Asian version of sideropenic dysphagia. He suggests that OSMF appears to be an altered oral mucosa following prolonged period of chronic deficiency of iron and/or vitamin "B" complex especially folic acid. This chang
