Use of diethylenetriamine penta-acetic acid (D.T.P.A.) in the clinical assessment of total body iron stores.

As a quantitative test of iron storage the six-hour urinary excretion of iron was measured after the intravenous infusion of the iron-chelating agent diethylenetriamine penta-acetic acid (D.T.P.A.). The value of this test in determining the presence and magnitude of the total body iron stores was assessed in patients with diseases of iron storage. In patients with idiopathic haemochromatosis and in their first-degree relatives with increased iron stores as measured by hepatic biopsy, the result of the D.T.P.A. test fell progressively as iron stores were reduced by venesection therapy. There wa

An account of 335 cases of megaloblastic anaemia of pregnancy and the puerperium.

The incidence of megaloblastic anaemia in pregnancy and the puerperium in north Staffordshire has steadily declined as a result of prophylaxis with folic acid. In the presence of advanced folic acid deficiency and with a florid megaloblastic marrow, the anaemia is usually severe, but in many patients the disease is relatively mild and the degree of anaemia is determined more by blood loss or associated iron deficiency than by the megaloblastosis. Microscopic examination of marrow films is still the most reliable method of diagnosis, although estimation of the labile serum folate has produced a

STUDIES ON THE SERUM IRON-BINDING CAPACITY.

The total iron-binding capacity of serum has been estimated using a colorimetric method, a radioisotopic technique, and a method employing immunodiffusion. The modified isotopic technique described was considered to be the method of choice. The total iron-binding capacity starts to increase at an early stage of an iron-deficiency anaemia. It is decreased in infectious and malignant disease. Close correlation exists between the level of saturation of the total iron-binding capacity and the haemoglobin level in patients with uncomplicated iron-deficiency anaemia. In patients with disease of the

WAARDENBURG'S SYNDROME AND HETEROCHROMIA IRIDUM IN A DEAF SCHOOL POPULATION.

Waardenburg's syndrome consists of lateral displacement of the inner canthi of the eyes (dystopia canthorum), a broad nasal root and confluent eyebrows, heterochromia iridum, a white forelock and congenital deafness. The syndrome is inherited as a dominant, but affected individuals do not necessarily have all of the characteristics cited.Five hundred and fourteen pupils at a school for the deaf were screened for features of this syndrome. Three cases were discovered. Eleven other deaf children were found to have heterochromia iridum and two more had white forelocks. The interocular dimensions

THE STUDY OF MENSTRUAL AND OTHER BLOOD LOSS, AND CONSEQUENT IRON DEFICIENCY, BY FE59 WHOLE-BODY COUNTING.

A recently established method of in vivo radioiron investigation in humans, employing a steel-room whole-body counter, has been applied to the study of Fe(59) absorption and loss in seven menstruating women, six with menorrhagia and hypochromic anemia. All six were found by this method to be iron-deficient, having radioiron absorptions of 53.7-97.5% (normal 5.7-24.7%). With almost 100% radioiron incorporation into the red-cell mass, subsequent drops in Fe(59) activity, when correlated with monthly menses, revealed estimated menstrual blood losses of 110-550 c.c. The single normal patient absor

Urinary excretion of histidine derivatives in megaloblastic anaemia and other conditions and comparison with the folic acid clearance test.

Urinary excretion of urocanic acid and formimino-glutamic acid in abnormal amounts following oral doses of histidine has been observed not only in most cases of megaloblastic anaemia, but in iron-deficiency anaemia, haemolytic anaemia, and neoplastic disease. The urinary excretion of these substances is increased in those disease states in which an abnormally rapid rate of removal from the plasma of intravenously injected folic acid has been demonstrated.

Treatment of nutritional anemia in infants.

While iron deficiency is a common cause of anemia in infants, requiring specific treatment by administration of iron, there are other causes which require treatment equally specific. Anemia due to a nutritional deficiency can be improved only by providing the needed nutriment in sufficient quantity. Two case reports are presented. One is of megaloblastic anemia in an infant, which was treated with a variety of vitamin preparations before administration of folic acid produced improvement. The other report is of anemia due to iron deficiency treated successfully with ferrous sulfate in a dosage

The newer hematinics, their use and abuse.

The newer hematinics are merely refinements of preexisting forms of treatment, but they have aided particularly in a better understanding of the deficiency states. The intrinsic factor of Castle has not been isolated from the gastric juice, and the interrelationships of this substance with the extrinsic factor (vitamin B(12)) and folic acid have not been defined at this time. Vitamin B(12) appears to be the active principle of refined liver extract and alone is probably adequate treatment for pernicious anemia. The other varieties of megaloblastic anemia may result from deficiency of vitamin B

IRON METABOLISM IN EXPERIMENTAL ANEMIA : "AVAILABILITY OF IRON".

In experimental anemia in dogs due to blood loss the term "available iron" as determined by the dipyridyl test has no physiological significance. Iron salts (100 per cent available by dipyridyl) given in optimum dose (560 mg. per 2 weeks) will cause a net production of 50 to 55 gm. hemoglobin above the control base line in anemic dogs. This means that an iron salt which is rated as 100 per cent available by the dipyridyl test is only 35 per cent physiologically available. The term "available iron (dipyridyl)" simmers down to iron not in the form of hematin compounds. The absorption of this "av
