Complete Resolution of Unprovoked Left Renal Vein Thrombosis and Pulmonary Embolism Associated With Severe Iron Deficiency Anemia: A Five-Year Follow-Up Case Report and Dual-Energy CT Appraisal.

Combined renal vein thrombosis (RVT) and pulmonary embolism (PE) in young adults without nephrotic syndrome, malignancy, or an identified major thrombophilia is uncommon. Severe iron deficiency anemia (IDA) has been associated with increased thrombotic risk, although a causal relationship cannot be established from an individual case. A 21-year-old male patient presented with flank and chest pain. Diagnostic workup confirmed acute left RVT and an acute embolism of the posterior basal segmental artery of the right lower lobe with pulmonary infarction, concurrent with profound microcytic IDA (he

A Case of Isolated Abnormal Behavior Associated With Vitamin B12 Deficiency.

Vitamin B12 is a water-soluble nutrient found mainly in animal-derived foods. Vitamin B12 deficiency is a common condition, presenting with hematologic and neurologic abnormalities, but rarely presents as isolated psychosis in a young adult. A 28-year-old male patient was brought in for severe agitation and confusion. Laboratory evaluation revealed severe vitamin B12 deficiency (serum vitamin B12 of <73.8 pmol/L) with elevated homocysteine (25.4 umol/L) and methylmalonic acid (0.95 umol/L) levels. The patient received vitamin B12 replacement, after which his confusion and behavioral disturbanc

[Severe pancytopenia in an infant secondary to vitamin B12 deficiency].

Introduction: severe vitamin B12 deficiency presents a diagnostic challenge due to its various clinical manifestations. Case report: the case we present here, a 17-month-old infant who presented with asthenia, hyporexia, paleness and regression of maturational patterns of approximately 2 months of evolution, illustrates the unique characteristics of severe vitamin B12 deficiency, highlighting the key differentiators from other conditions, including decreased reticulocyte counts and elevated lactate dehydrogenase levels, indicative of suppressed erythropoiesis. Discussion: advanced cobalamin de

A Rare Autoimmune Hemolytic Overlap: Coombs-Positive Pernicious Anemia.

Pernicious anemia is a type of autoimmune disease caused by the presence of anti-intrinsic factor and antiparietal cell antibodies, leading to vitamin B12 deficiency and ineffective erythropoiesis. Autoimmune hemolytic anemia is a variant of anemia in which the direct Coombs test is positive. It is uncommon to find a mixed anemia that has both a pernicious anemia component and an autoimmune anemia component with a positive direct Coombs test, as this association is rarely described. Two cases in which this association was found are presented. The first case involved a 65-year-old woman present

Pernicious Anemia Resulting in Intramedullary Hemolysis, Masking Underlying Polycythemia Vera and Mild Alpha-Thalassemia-A Case Report.

BACKGROUND: Vitamin B12 deficiency can cause severe intramedullary hemolysis and cytopenias. Myeloproliferative neoplasms (MPNs), particularly polycythemia vera (PV), are typically characterized by hyperproliferation but may remain undetected when masked by concomitant deficiencies. CASE PRESENTATION: A 48-year-old woman presented with fatigue, weight loss, and pancytopenia. Laboratory tests showed severe macrocytic anemia, hemolysis, and markedly reduced vitamin B12 levels. Gastroscopy and antibody testing confirmed autoimmune atrophic gastritis with pernicious anemia. Bone marrow biopsy rev

Vitamin B12 deficiency in long-term metformin use and clinician awareness: a scoping review.

BACKGROUND: Metformin is the first-line treatment for type 2 diabetes mellitus (T2DM). Long-term use of metformin has been associated with vitamin B12 deficiency, which may lead to serious complications such as anaemia and neuropathy. Although international bodies have recommended screening for vitamin B12 deficiency in patients on long-term metformin, it is unclear how aware clinicians are of this adverse effect and to what extent such guidance is being followed in practice. METHODS: A scoping review was conducted using Joanna Briggs Institute (JBI) methodology. Databases searched included M

Helicobacter pylori Infection and Anemia: The Potential Role of Vitamin C and Vitamin B12.

Anemia is a major global public health problem and is most commonly associated with iron deficiency; however, deficiencies in other micronutrients, including vitamin B12, may also contribute to its development. Increasing evidence suggests that Helicobacter pylori infection may influence the occurrence of anemia through several mechanisms related to alterations in the gastric environment. Chronic gastric inflammation and increased gastric pH associated with H. pylori infection may impair the absorption of non-heme iron and reduce the concentration of vitamin C in gastric juice. Since vitamin C

Unveiling the clinical connections between vitamin B12 deficiency anemia, Hashimoto thyroiditis, and hypothyroidism: New insights from Mendelian randomization studies.

Clinical evidence indicates a strong association between nutritional anemia and hypothyroidism. However, whether nutritional factors contribute directly to the onset of hypothyroidism remains unclear. In this study, we aimed to address this knowledge gap by examining the causal relationship between nutritional anemia and hypothyroidism. We conducted rigorous univariate and multivariate Mendelian randomization (MVMR) analyses using data from the Integrative Epidemiology Unit Open Genome-wide Association Studies database. We found that both vitamin B12 deficiency anemia (B12DA) and iron deficien

Hemoglobin Trajectories After Metformin Initiation Compared With Dipeptidyl Peptidase-4 Inhibitors: A Real-World Cohort Study.

Background Metformin is associated with vitamin B12 deficiency, and we recently reported latent iron and copper deficiency among metformin users, suggesting a potential contribution to anemia risk. However, real-world evidence on hemoglobin trajectories after metformin initiation remains limited. We evaluated short- and long-term hemoglobin changes after metformin initiation compared with dipeptidyl peptidase-4 (DPP-4) inhibitors in Japanese patients with type 2 diabetes. Methods This single-center retrospective cohort study used electronic medical record data from Kobe University Hospital fro

Sensory Ataxia Mimicking Diabetic Neuropathy in a Long-Term Metformin User: A Case Report From Korea.

A 66-year-old man with long-standing diabetes and prolonged metformin exposure presented with a 10-year history of distal paresthesia and progressive imbalance, previously attributed to diabetic neuropathy. Bedside examination demonstrated a positive Romberg sign, impaired distal joint position sense, and reduced vibration sense, suggesting sensory ataxia with posterior column involvement. Mild macrocytosis without anemia was identified. In a resource-limited primary care setting where confirmatory testing was not immediately available, empirical methylcobalamin was initiated. The patient subs

A Study of Anemia in the Geriatric Population at a Tertiary Care Center.

BACKGROUND: Anemia is among the most prevalent hematological disorders in the elderly population and is still a significant public health concern across the world. In the older population, this disorder usually has a multi-factorial origin, related to nutritional deficiency, chronic inflammation, renal insufficiency, or occult blood loss. Even mild anemia in the elderly has been associated with reduced physical ability, cognitive deficit, increased rates of admission to hospital, and increased mortality. Even though it is clinically important, anemia in the elderly is frequently underdiagnosed

Vitamin B12 Deficiency Presenting as Anemia and Functional Decline in an Older Adult: A Case Report.

Anemia is a common condition in older adults and is associated with increased morbidity, geriatric syndromes, and functional decline. We report the case of an 86-year-old woman who presented with severe anemia accompanied by neurologic, hematologic, and oral manifestations, in whom vitamin B12 deficiency was identified as the underlying etiology. Following targeted parenteral cyanocobalamin therapy, the patient demonstrated marked hematologic recovery and substantial improvement in gait, strength, and independence in basic activities of daily living, as documented through a comprehensive geria

Severe Macrocytic Anemia Uncovering Pernicious Anemia and Early-Stage Gastric Adenocarcinoma: A Case Report.

Pernicious anemia (PA) is an autoimmune cause of vitamin B12 deficiency, often secondary to chronic atrophic gastritis, which increases the risk of gastric malignancies. Early signs, like mild macrocytosis, are frequently overlooked, especially in patients with other risk factors such as long-term metformin use. We present a case illustrating the full clinical trajectory from long-standing, unexplored macrocytosis to severe anemia, ultimately leading to the diagnosis of PA and detection of early-stage gastric adenocarcinoma on surveillance esophagogastroduodenoscopy (EGD). Informed consent was

Diagnostic Overshadowing in Lennox-Gastaut Syndrome: Immerslund-Gräsbeck Syndrome Unmasked by Radiosurgical Stress.

In complex neurodevelopmental disorders such as Lennox-Gastaut syndrome (LGS), treatable metabolic comorbidities may be masked by the severity of the primary phenotype. We report an 8-year-old boy with LGS and bilateral polymicrogyria whose underlying Imerslund-Gräsbeck syndrome was unmasked following a palliative Gamma Knife corpus callosotomy. Although the patient underwent Gamma Knife corpus callosotomy, his seizures remained uncontrolled. Consequently, vagus nerve stimulator implantation was planned; however, severe anemia (hemoglobin: 3.3 g/dL) was detected on admission for the procedure.

Subacute Combined Degeneration as a Red Herring for Acute Traumatic Spinal Cord Injury.

Subacute combined degeneration (SCD) is a potentially reversible neurologic disorder caused by vitamin B12 deficiency that classically affects the dorsal columns of the spinal cord. Clinical manifestations include sensory loss, gait instability, weakness, and, less commonly, autonomic dysfunction. With these neurologic manifestations, patients may present with recurrent falls, and SCD can mimic the presentation of acute traumatic spinal cord injury (SCI). In this case, a 66-year-old man with a recent history of nitrous oxide use presented with bilateral lower extremity weakness, sensory change

Comparison of Sublingual and Intramuscular Vitamin B12 in Children With Nutritional Vitamin B12 Deficiency Megaloblastic Anemia.

Background and aim Vitamin B12 deficiency remains a major cause of nutritional anemia in children, particularly in low- and middle-income countries. While intramuscular (IM) vitamin B12 is the traditional treatment, the sublingual route offers a simpler, painless alternative with potentially comparable efficacy. Thus, this study was planned to compare the efficacy of sublingual versus IM vitamin B12 supplementation in children with confirmed vitamin B12 deficiency anemia. Materials and methods A prospective comparative study was conducted among children aged 1-15 years at a tertiary care cente

Unveiling the Pernicious Truth: A Case Report on the Rare Presentation of Severe Vitamin B12 Deficiency.

Vitamin B₁₂ deficiency typically causes megaloblastic anemia but can rarely present with hemolytic features, leading to diagnostic confusion with autoimmune or microangiopathic processes. Pernicious anemia, an autoimmune cause of severe B₁₂ deficiency, is frequently underrecognized and may coexist with other autoimmune disorders. We report a 45-year-old woman with Hashimoto's thyroiditis who presented with dizziness, profound fatigue, and symptomatic anemia. Laboratory evaluation revealed severe macrocytic anemia (hemoglobin 3.5 g/dL, mean corpuscular volume (MCV) 136.9 fL) with thrombocytopen

Vitamin B12 Deficiency Presenting as Pseudothrombotic Microangiopathy and Intestinal Pseudo-Obstruction.

Vitamin B12 deficiency is a common and reversible cause of megaloblastic anemia, but in severe cases, it may mimic thrombotic microangiopathy (TMA), a presentation known as pseudothrombotic microangiopathy (pseudo-TMA). This rare entity is frequently underrecognized and may lead to inappropriate therapies if not promptly identified. We report the case of a 62-year-old man who presented with abdominal pain, distension, and constipation, initially raising concern for intestinal obstruction or malignancy. Laboratory evaluation revealed macrocytic anemia, thrombocytopenia, elevated lactate dehydro

Vitamin B12 Deficiency-Associated Pseudo-Thrombotic Microangiopathy in a Patient on Longstanding Oral B12 Supplementation.

Vitamin B12 deficiency is a common cause of normocytic or megaloblastic anemia. In 2.5% of cases, it can manifest as pseudo-thrombotic microangiopathy (pseudo-TMA), which mimics thrombotic thrombocytopenic purpura (TTP), an emergent hemolytic microangiopathy. This case report discusses a stable-appearing 45-year-old female with progressive fatigue, heavy menstrual bleeding, bleeding gums, and easy bruising. Laboratory testing demonstrated low vitamin B12 (121 pg/mL), pancytopenia, exaggeratedly elevated LDH, undetectable haptoglobin, low reticulocyte production index (RPI), and positive intrin
