Pernicious anemia predominates among Mexican adults with vitamin B12 deficiency: clinical and laboratory findings from a tertiary referral center.

BACKGROUND: Vitamin B12 deficiency is a reversible cause of hematologic and neurologic morbidity, but its etiologic spectrum in Latin American referral centers remains poorly defined. METHODS: We retrospectively studied consecutive adults with vitamin B12 deficiency evaluated in hematology and oncology clinics at a Mexican tertiary referral center from January 2013 to July 2023. Etiologies were classified as pernicious anemia, non-surgical malabsorption, post-surgical malabsorption, reduced intake, drug-related causes, or unknown, and pernicious anemia was compared with all other etiologies.

Autoimmune gastritis: a hidden gateway to cardia and oesophageal cancers beyond non-cardia gastric adenocarcinoma.

Autoimmune gastritis (AIG) has been traditionally recognised as a precursor to type I gastric neuroendocrine tumours and non-cardia gastric adenocarcinoma. However, emerging but scattered evidence suggests that its oncogenic footprint may extend beyond the gastric body to the cardia, gastro-oesophageal junction (GOJ) and oesophagus. In this review, we synthesise the available epidemiological and clinical studies linking AIG and its late manifestation, pernicious anaemia, to cardia/GOJ adenocarcinoma and oesophageal cancers. We discuss putative histopathological, immunological and other mechani

Pernicious Anemia Presenting as Pseudo-Thrombotic Microangiopathy: A Case Report.

Thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening thrombotic microangiopathy (TMA) characterized by microangiopathic hemolytic anemia, thrombocytopenia, and end-organ dysfunction, with high untreated mortality that can be effectively reduced with timely plasma exchange (PLEX) and immunosuppressive therapy. However, several conditions can produce a clinically similar pseudo-TMA picture, among which severe vitamin B12 deficiency is an underrecognized and potentially treatable mimic that occurs due to ineffective intramedullary erythropoiesis, megaloblastic fragmentation of st

Serum Concentrations of Gastric Intrinsic Factor-A Pilot Study on Patients Undergoing Bariatric Surgery, Presenting with Portal Hypertension, or Suspected Pernicious Anemia.

Background/Objectives: Small quantities of gastric parietal cell-derived Intrinsic Factor (IF) are present in circulation. Here, we explore circulating concentrations of IF in different medical conditions affecting the stomach, anticipating finding both low and high concentrations depending on the state of the parietal cells. Methods: We measured serum IF in (I) patients before and after bariatric surgery; (II) patients diagnosed with portal hypertension; and (III) patients with suspected pernicious anemia referred for measurement of IF autoantibodies. Results: Patients undergoing bariatric su

Systemic Sclerosis Presenting With Chronic Atrophic Gastritis and Retinal Artery Occlusion: A Case Report.

Systemic sclerosis (SSc), also known as scleroderma, is a rare autoimmune disease characterized by fibroproliferative changes, vascular dysfunction, and small-vessel proliferation. It can affect multiple organ systems, including the gastrointestinal tract, skin, heart, eyes, kidneys, and lungs. We report a case of a 67-year-old woman with systemic sclerosis who tested negative for specific antibodies and simultaneously developed two rare complications: autoimmune atrophic gastritis resulting in megaloblastic anemia and acute retinal artery occlusion. Notably, she presented with megaloblastic a

Nitrous Oxide-Induced Pancytopenia and Peripheral Neuropathy From Chronic Whip-It™ Use: A Case Report.

Nitrous oxide (N2O) is a colorless, odorless gas gaining popularity among recreational drug users for its euphoric effects. Whip-it!™ is a low-cost, freely available form of N2O commonly abused in parties and nightclubs. Chronic N2O use can inactivate vitamin B12, causing severe deficiency, anemia, neuropathy, and myelopathy. Here, we report a 19-year-old female with no significant past medical history who was found unresponsive in her apartment. She admitted to daily Whip-it™ use over the preceding three months. In the emergency department, she reported generalized weakness and tingling in al

A Rare Case of Possible Iron Overload and B12 Deficiency in Autoimmune Polyglandular Syndrome Type 3B.

Autoimmune polyglandular syndromes (APS) are a rare group of disorders characterized by the dysfunction of multiple endocrine and non-endocrine organs due to autoimmune destruction. APS type 3B (APS3B) is a subtype characterized by the coexistence of autoimmune thyroiditis and pernicious anemia, more commonly observed among middle-aged females. Here, we describe a unique case, workup, and diagnosis of APS3B in a 58-year-old Hispanic male who presented with an atypical presentation of APS3B complicated by metformin use and possible transfusional iron overload. Workup revealed positive intrinsic

Hippophae rhamnoides: Way to Ameliorate Depressive Augury by Cyanocobalamin.

INTRODUCTION: Vitamins are essential for maintaining wellness since they are involved in several distinct physiological functions. In addition to being necessary for the methylation cycle and the creation of monoamine oxidase, vitamin B complex is also required for deoxyribonucleic acid synthesis, phospholipid preservation, and repair. Vitamin B deficiency or excess may affect dementia, cognitive decline, and memory. Deficits in these vitamins, particularly B12 (cobalamin, or vitamin B12), are therefore strongly linked to depression. Cyanocobalamin is used to assist in managing deficiencies, a

Transcobalamin II deficiency mimicking myelodysplastic syndrome in a child: a case report.

Transcobalamin II (TCN2) deficiency is a rare autosomal recessive metabolic disorder that impairs vitamin B12 transport and can present with megaloblastic anemia and neutropenia, often mimicking hematologic diseases such as myelodysplastic syndrome (MDS). We report a 5-year-old male presenting with pallor, fatigue, and fever. On admission, he had acute anemia crisis and neutropenia, with chest CT revealing a lung infection, while serum folate and vitamin B12 levels were normal. Bone marrow smears and flow cytometry suggested possible MDS; however, genetic testing identified compound heterozygo

Persistent Anemia Revealing Underlying B-cell Malignancies: A Report of Two Cases Highlighting a Diagnostic Gap in Primary Care.

Persistent anemia in older adults is commonly attributed to nutritional deficiency, chronic disease, or age-related change, particularly when routine laboratory investigations are unrevealing. However, plasma cell and lymphoplasmacytic disorders may present subtly, with anemia as the only consistent abnormality. We report two cases in which extended laboratory evaluation led to early identification of clinically significant B-cell malignancies. The first case involved a man in his early 70s with stable, asymptomatic normocytic anemia despite normal iron studies, vitamin B12 and folate levels,

Optimizing diagnostic thresholds of total vitamin B12 (B12) for identifying cobalamin deficiency in adults with macrocytic anemia.

OBJECTIVES: Identifying individuals with vitamin B12 (B12) deficiency is challenging due to poor harmonization across total B12 assays. To establish clinically meaningful thresholds for the Roche assay, we characterized B12 concentrations associated with deficiency by comparing individuals with macrocytic anemia and other anemia subtypes or no anemia. METHODS: We retrospectively analysed 10 years of laboratory data from adults tested for total B12 and folate (Roche Cobas), homocysteine (Abbott Architect), and haematological parameters (Sysmex XE/XN). Individuals receiving vitamin supplementat

Case Report: Imerslund Grasbeck syndrome: a rare cause of megaloblastic anemia in a well-nourished child.

Megaloblastic anemia, characterized by macrocytic anemia, is most commonly caused by nutritional vitamin B12 deficiency; however, inherited disorders of cobalamin absorption should be considered in children with adequate dietary intake. One such disorder is Imerslund-Gräsbeck syndrome (IGS), a rare autosomal recessive condition characterized by selective intestinal malabsorption of vitamin B12 due to mutations in the CUBN or AMN genes. We report a 7-year-old boy, born of a third-degree consanguineous marriage, who presented with generalized rash, easy fatigability, and recurrent oral ulceratio

Unusual Micronutrient Deficiencies as Causes of Anemia.

Anemia due to micronutrient deficiencies has received little attention in public health and modern-day clinical practice. Increasing numbers of persons, mainly in low- and middle-income nations, are faced with malnutrition and malabsorption syndromes, giving rise to various micronutrient deficiencies that can lead to anemias refractory to treatment with iron, folate or vitamin B12. Though relatively uncommon, such underlying nutrient deficiencies may be difficult to recognize as they can coexist with more common causes and there may be few or no specific clinical characteristics pinpointing a

Beyond iron deficiency: A comprehensive national survey of anaemia etiology in Sri Lankan young adults.

Previous studies in Sri Lanka have explored the prevalence and causes of anaemia, mainly emphasizing iron deficiency while overlooking other important factors such as enzymopathies, membranopathies, and haemoglobinopathies. Moreover, many studies reported unexplained cases of anaemia even after detailed evaluation. This study aimed to determine the prevalence and underlying causes of anaemia among community-based young adults in Sri Lanka. A descriptive cross-sectional study was conducted from January 2023 to December 2024 among young adults aged 18-30 years. Data and blood samples were collec

Incidental Detection of Glutamate Formiminotransferase Deficiency Through Newborn Screening in a Clinically Asymptomatic Infant: Molecular Findings and Counseling Considerations.

We report a seven-week-old male infant, born to consanguineous (double cousin) parents, incidentally through routine newborn screening, following an abnormal acylcarnitine profile caused by elevated formiminoglutamate (FIGLU), with subsequent biochemical confirmation of glutamate formiminotransferase cyclodeaminase (FTCD) deficiency. The infant was born at 37 weeks' gestation via spontaneous vaginal delivery, was vigorous at birth, and required no resuscitation. He remained clinically asymptomatic at the time of metabolic and genetic evaluation. Plasma acylcarnitine analysis demonstrated eleva

Megaloblastic anemia due to hereditary intrinsic factor deficiency presenting as recurrent anemia in a young girl-a case report.

BackgroundChildhood anemia is a very common problem in India, out of which more than one-third is attributed to vitamin B12 deficiency.AimTo follow up on a girl with recurrent anemia who was later diagnosed with hereditary intrinsic factor deficiency.MethodsAn eight-year-old girl presented with recurrent anemia, easy fatigability, poor appetite, and abdominal pain for one month. The hemoglobin was 4.3 g/dL, and the peripheral smear showed a dimorphic picture. Serum vitamin B12 was severely low at <50 pg/mL. After a packed red blood cell transfusion, she was advised to take oral vitamin B12 and

Comparative efficacy of biweekly preventive supplementation, with multiple micronutrients and iron folic acid or iron folic acid alone, on hemoglobin concentrations and anemia prevalence in children aged 6-59 months: a randomized controlled trial in rural India.

BACKGROUND: Anemia affects more than two-thirds of children aged <5 y in India, despite biweekly iron folic acid (IFA) supplementation under the Anemia Mukt Bharat national program. Multiple micronutrient (MMN) deficiencies may also contribute to the presence of anemia, but the incremental benefit of MMN supplementation in addition to IFA remains unclear. OBJECTIVES: We aimed to compare the efficacy of biweekly preventive supplementation with MMNs plus IFA compared with IFA alone on hemoglobin concentrations and prevalence of anemia in children aged 6-59 mo. METHODS: In this individually ran

ISPAD, bridging clinical expertise and AI for autoimmune-related pernicious anemia diagnosis.

BACKGROUND: Pernicious anemia (PA) is a severe clinical consequence of autoimmune gastritis. It results from immune-mediated damage to gastric parietal cells in the oxyntic mucosa. This process leads to intrinsic factor deficiency and subsequent vitamin B12 malabsorption. This complex autoimmune response, combined with non-specific clinical manifestations, generates substantial diagnostic uncertainty. This uncertainty frequently results in misdiagnosis or delayed diagnosis and, consequently, multiple adverse outcomes, including irreversible neurological complications. METHODS: To address the

Anemia Management in a Jehovah's Witness Patient With Hip Fracture.

Jehovah's Witnesses are a religious group whose beliefs regarding blood transfusion may pose important clinical and ethical challenges. As such, more patients refuse whole-blood transfusions. Patients with hip fractures often present with anemia, which increases the likelihood of requiring perioperative blood transfusion and increases morbidity and mortality after surgery. We present here the case report of an 80-year-old female patient, a Jehovah's Witness, with no previous history of thromboembolic events. She was admitted due to a right hip fracture after a fall from a standing height. She

The effect of colchicine on micronutrients in children with newly diagnosed familial Mediterranean fever.

OBJECTIVES: Colchicine is an essential component of familial Mediterranean fever (FMF) treatment. It is suggested that it may affect vitamin B12 levels by affecting intestinal absorption. This research focused on examining the impact of colchicine therapy on anemia parameters, as well as levels of vitamin B12 and folate in patients who have recently been diagnosed with FMF. METHOD: This prospective cohort investigation assessed children diagnosed with FMF between October 2016 and February 2017, evaluating anemia parameters as well as vitamin B12 and folate levels at baseline and at the third
