Maternal First-Trimester Vitamin B12 Levels as a Determinant of Infant Deficiency: A Single-Center Retrospective Study.

Background Vitamin B12 deficiency in infancy may lead to growth retardation, anemia, and neurodevelopmental delay. Therefore, identifying maternal predictors is essential for early prevention. The significance of vitamin B12, particularly during pregnancy and early infancy, has been acknowledged. This study aims to identify the etiological factors in infants aged 9-12 months diagnosed with vitamin B12 deficiency. Methodology This study aimed to retrospectively evaluate the characteristics of infants aged 9-12 months with vitamin B12 levels ≤300 pg/mL and investigate the maternal vitamin B12 st

Newborn Screening Alone Cannot Prevent Most Cases of Severe Vitamin B12 Deficiency in the First Year of Life.

BACKGROUND/OBJECTIVES: Vitamin B12 (B12) is essential for the provision of methyl groups for numerous essential pathways. Infant B12 deficiency (B12D) can lead to severe, even irreversible neurological abnormalities. Maternal B12 status in pregnancy and during the breastfeeding period correlates significantly with the child's B12 status. B12D is a target disease in some newborn screening (NBS) programs. This study investigates whether infants that were clinically symptomatic and diagnosed with B12D in their first year of life could be retrospectively detected by the Austrian NBS algorithm. ME

Pancytopenia in the context of hypothyroidism and B12 deficiency: A case report and review literature.

BACKGROUND: This case of pancytopenia due to hypothyroidism and B12 deficiency is rare and severe as a reminder that, upon diagnosis of pancytopenia, endocrine disorders such as secondary thyroid hormone deficiency may need to be considered as a potential cause. CASE PRESENTATION: A 38-year-old male presented with progressive weakness, lethargy, and anorexia. Initial labs revealed severe pancytopenia, prompting a hematological workup. Bone marrow aspiration ruled out acute leukemia. Further investigations identified severe primary hypothyroidism (TSH: 200 mIU/L) and vitamin B12 deficiency as

Metformin Induced Vitamin B12 deficiency: A Cross sectional study. Untangling the connection.

BACKGROUND: Metformin is a widely popular drug to treat type 2 diabetes as it is efficacious, inexpensive and relatively well tolerated with minimal side effect profile. It is known that long term metformin use predisposes a person to develop Vitamin B12 deficiency, however guidelines to screen and treat are yet to be well established. Vitamin B12 deficiency is known to cause peripheral neuropathy and anemia. METHODS: Ours is a prospective, observational, cross sectional study based in a hospital setting in a tertiary care center. Study participants were subjected to serum Vitamin B12 analysi

Rural health challenges in Western Uganda: pernicious anemia masquerading as recurrent lower limb weakness-a case report.

BACKGROUND: Pernicious anemia, also called Biemer's disease, is an autoimmune disease and the most common cause of cobalamin deficiency globally. Various genetic, environmental, and immunological factors interplay to lead to its presentation. Pernicious anemia has a myriad of presentations, which can range from hematological and skin-related to neurological. Pernicious anemia has been reported among people of all ages worldwide, especially those over 60 years old. Its prevalence in the general population is about 0.1% and 1.9% for elderly patients over 60 years old. Like most other autoimmune

Evaluation of Etiological Causes and Incidence of Pancytopenia: A Case Series from a Tertiary Care Center.

Background and aim Pancytopenia is a clinical condition characterized by the simultaneous reduction of red blood cells, white blood cells, and platelets. It generally arises from conditions associated with bone marrow dysfunction or increased peripheral consumption of blood cells. Understanding the regional etiological models of pancytopenia is of paramount importance for optimizing diagnostic and therapeutic strategies specific to the local population. This study aimed to identify the etiological causes and examine the distribution of incidence rates in patients presenting with pancytopenia i

A long-standing undiagnosed case of vitamin B12 deficiency: a case report.

BACKGROUND: Pernicious anemia, an autoimmune disease, presents with gradual, nonspecific symptoms, often leading to delayed diagnosis owing to its overlap with other conditions and variability in laboratory findings, such as neurological symptoms without anemia or macrocytosis. CASE PRESENTATION: This case describes a 40-year-old Iranian woman with a decade-long history of nonspecific symptoms, including fatigue, widespread musculoskeletal pain, paresthesia, cognitive disturbances, and optic neuritis; misattributed to conditions such as fibromyalgia, hypothyroidism, and autoimmune diseases. Vitamin B12 Deficiency-Induced Massive Schistocytosis and Hemolytic Anemia Due to Pseudo-Thrombotic Microangiopathy: A Case Report.

A 37-year-old male presented with complaints of fatigue, weight loss, and decreased appetite. Laboratory examinations revealed hemolytic anemia, an elevated lactate dehydrogenase (LDH) level, and marked schistocytosis. His other laboratory hematological findings included nucleated red blood cells with abnormal morphologies in the peripheral blood, as well as a small peak that was indicative of schistocytes on a red blood cell histogram. A bone marrow examination demonstrated megaloblastic changes; therefore, a preliminary diagnosis of vitamin B12 deficiency was made. Intramuscular vitamin B12

Prevalence of Vitamin B12 Deficiency Among Diabetic Patients Taking Metformin in Jordan.

Background Type 2 diabetes mellitus prevalence in Jordan is high and increasing, leading to an increase in the use of metformin, which is considered the first-line treatment. Metformin, which belongs to the biguanides family of anti-diabetic medication, is known for its low cost, bodyweight neutrality, effectiveness, and good safety profile. In addition, it showed that it improves body lipid profile, inflammatory markers, and decreases cardiovascular events. In spite of that, a reduction in vitamin B12 levels was reported as a possible long-term side effect of metformin. Vitamin B12 deficiency

Vitamin B12 Deficiency Exhibiting as Pancytopenia: A Diagnostic Conundrum.

Pancytopenia is caused by impaired production and peripheral destruction of blood cells, leading to decreased levels of red blood cells (RBCs), white blood cells (WBCs), and platelets. The etiologies can be classified as reversible and irreversible causes. One of the reversible causes is Vitamin B12 deficiency. Vitamin B12 deficiency can cause various types of manifestations, including hematologic as well as neurological symptoms, and sometimes mimicking serious hematologic conditions. A 46-year-old male presented with generalized body weakness, easy fatigability, and occasional nausea with vo

Patient-reported characteristics of pernicious anaemia: a first step to initiate James Lind Alliance Priority Setting Partnership driven research.

BACKGROUND: Pernicious anaemia (PA) is characterised by vitamin B12 deficiency due to autoimmune-mediated loss of gastric parietal cells and intrinsic factor - a specific transporter for B12's intestinal uptake. The Pernicious Anaemia Society (PAS) is a patient-driven charity that recently identified 10 research priorities for improved diagnosis and management of PA through a James Lind Alliance Priority Setting Partnership. To facilitate research addressing these priorities, the aim of this study was to survey PAS members to identify and characterise a cohort of patients to form a PA research

Anemia in inflammatory bowel disease-A comprehensive review.

Anemia is a frequent complication in inflammatory bowel disease (IBD) patients. The etiology is multifactorial, with iron deficiency and anemia of chronic disease being the main reasons. Other causes include vitamin B12 and folate deficiency, hemolytic anemia and medications such as azathioprine and sulfasalazine. Apart from physical symptoms, it is associated with several negative outcomes, including poor quality of life, increased risk of hospital admission, increased risk of surgery and higher treatment costs. Diagnostic evaluation aims to identify the underlying cause and severity to deter

Physiology of vitamin B12: a study on its molecular mechanisms using a Caenorhabditis elegans model.

Vitamin B12 (B12) is a water-soluble substance that is a member of the B-vitamin family. Its recommended daily dose in adult men and women is 2.4 µg, which is the lowest among the 13 vitamins. B12 deficiency causes megaloblastic anemia and neurological disorders. It is also associated with cognitive decline, growth retardation, infertility, and other symptoms. Nevertheless, the detailed mechanisms of which remain unclear. Caenorhabditis elegans is a small organism, with a length of approximately 1 mm and a lifespan of approximately 3 weeks. It has similar fundamental biological structures, suc

Dietary supplementation with conjugated linoleic acid induces anemia with combined features of megaloblastic and hemolytic anemia.

This study investigates whether dietary supplementation with conjugated linoleic acids (CLA) induces anemia in mice. Male Kunming White mice were randomly assigned to two groups: one fed a CLA-supplemented diet and the other a linoleic acid (LA)-supplemented diet as control. Compared with those in LA group, periodic manual erythrocyte counts revealed red blood cells (RBCs) in CLA group significantly reduced on days 47 (p < 0.05), 61 (p < 0.01), and 86 (p < 0.001). Complete blood count (CBC) analysis demonstrated that CLA group exhibited 48.96 % lower RBCs (p < 0.01) and 46.49 % reduced hematoc

New Insights into Folate-Vitamin B12 Interactions.

Folate and vitamin B12 (B12) are essential cofactors in folate-mediated one-carbon metabolism (FOCM). FOCM includes a series of methyl transfer reactions for methionine regeneration and de novo synthesis of nucleotides, including thymidylate. Deficiency in either folate or B12 can result in negative health outcomes including megaloblastic anemia, with additional neurocognitive impairments observed as a result of B12 deficiency. While folate deficiency is not common in the United States due to mandatory folic acid fortification, B12 deficiency is observed more frequently, particularly in certai

Mean Corpuscular Volume and Risk of Autism Spectrum Disorder.

BACKGROUND: Autism spectrum disorder (ASD) can be diagnosed as early as 18 months old, but more reliably after two years. Notably, no laboratory test exists to identify mothers at higher risk of having a child who will later be diagnosed with ASD or to identify at-risk infants before the manifestation of symptoms. One frequently described risk factor for neurodevelopmental disorders is vitamin B12 and folate deficiency, which results in macrocytic anemias. METHODS: We evaluated whether increased mean corpuscular volume (MCV), an indicator of macrocytic anemias in the mother or child, is assoc
