Does folic acid harm people with vitamin B12 deficiency?

Oral folic acid given before and during pregnancy can prevent about 75% of fetal neural tube defects. Even in large dose (20 mg daily) folic acid has never been shown to harm normal people, but it has acquired a bad reputation in pernicious anaemia. Before 1930, if untreated patients survived the anaemia, they succumbed to peripheral neuritis, subacute combined degeneration of the spinal cord, and death. The speed of this progression was extremely variable. From 1947 onwards, there were many reports of rapid neurological deterioration during administration of folic acid as sole therapy to peop

Deficiency testing for iron, vitamin B12 and folate.

Dietary cells and the vitamins B12 and folate are necessary for the production of the red blood cells (erythrocytes), which carry oxygen from the lungs to the tissues and carbon dioxide from tissues to lungs. Deficiency of either one results in anaemia, which is characterised by low haemoglobin concentration. Symptoms result from reduced tissue oxygenation and include weakness, lethargy, palpitation, headache and shortness of breath. The first-time laboratory test of all patients suspected of being anaemic is the full blood count. Results of a full blood count may suggest the anaemia is caused

Vegetarian diets and children.

Although the general health and development of vegan and vegetarian children seem to be normal, there may be subtle differences compared with omnivores. They are at increased risk of iron deficiency, and impaired psychomotor development associated with iron deficiency has been reported in macrobiotic infants. Fortunately, this impairment is not permanent, and follow-up studies have reported higher-than-average intelligence quotients among older macrobiotic children. Several other hazards of vegetarian diets have been identified, including vitamin B12 deficiency, rickets, and a bulky diet that

Inherited errors of cobalamin metabolism and their management.

Cobalamins are essential biological compounds structurally related to haemoglobin and the cytochromes. Although the basic cobalamin molecule is only synthesized by micro-organisms, all mammalian cells can convert this into the coenzymes adenosylcobalamin (AdoCbl) and methylcobalamin (MeCbl). AdoCbl is the major form in cellular tissues, where it is retained in the mitochondria. MeCbl predominates in blood plasma and certain other body fluids such as breast milk; in cells MeCbl is found in the cytosol. Inherited disorders of cobalamin metabolism are single gene defects, transmitted as recessive

Vegetarian diets and children.

The diets and growth of children reared on vegetarian diets are reviewed. Excessive bulk combined with low energy density can be a problem for children aged < or = 5 y and can lead to imparied growth. Diets that have a high content of phytate and other modifiers of mineral absorption are associated with an increased prevalence of rickets and iron-deficiency anemia. Vitamin B-12 deficiency is a real hazard in unsupplemented or unfortified vegan and vegetarian diets. It is suggested that vegans and vegetarians should use oils with a low ratio of linoleic to linolenic acid in view of the recently

Nutritional implications of recombinant human erythropoietin therapy in renal disease.

The treatment of anemia in patients with renal failure has been dramatically changed with the development of recombinant human erythropoietin (r-HuEPO). This review discusses the pathogenesis of the anemia renal failure and the biology of erythropoietin. Causes of poor response to r-HuEPO therapy are outlined, and the importance of adequate available iron is highlighted. Parameters used to measure iron adequacy include serum iron levels, transferrin saturation, and ferritin levels. Other nutritional deficiencies, such as folic acid and vitamin B-12, can also impair r-HuEPO response. Clearly, t

Agriculture-related anaemias.

Man evolved as a hunter-gatherer, and the invention and spread of agriculture was followed by changes in diet, the environment and population densities which have resulted in globally high prevalences of anaemias due to nutritional deficiencies of iron, folate and (locally) vitamin B12, to infestations by hookworm and schistosomes, to malaria, and to the natural selection for the genes for sickle-cell diseases, beta-thalassaemias, alpha-thalassaemias, glucose-6-phosphate dehydrogenase deficiency, ovalocytosis and possibly (locally) elliptocytosis. The present explosion of population is driving

[Hematologic significance of erythrocytic macrocytosis: prospective analysis of 109 successively studied cases].

The aim of the present work was to perform a prospective analysis of the significance of macrocytic red cells through the study of all patients with MCV higher than 105 fl (those treated with cytotoxic or immunosuppressing drugs were excluded). Conventional clinical, haematologic and biochemical studies were carried out on every patient, along with B12 and folate levels, bone marrow examination and bone marrow karyotype and, whenever B12 deficiency was present, complete Schilling's test. Special attention was paid to the aetiological inquiry and post-therapeutical course. A series of 109 patie

[Requirements of nutrients which participate in erythropoiesis].

Proteins, some minerals and vitamins, play important roles in erythropoiesis and the survival of the red blood cell. This article deals specifically with the physiological requirements and recommended intakes of iron, folate and vitamin B12. A comparison of the physiologic iron requirements according to age and sex, and the amount of iron which is actually absorbed from the diets consumed by the lower socioeconomic strata of the Venezuelan population; indicates that these diets do not satisfy the requirements at all ages. Such disparity is most marked in children below three years of age, in a

[Macrocytosis, megaloblastosis and folate status in chronic alcoholics].

The figures obtained in simultaneous radioimmunological assays of serum folate and vitamin B12 concentrations and erythrocyte folate concentration in 74 patients are presented. All these patients had a regular daily intake of over 80 g ethyl alcohol and presented an increase in mean blood cell volume. Most of the patients were males hospitalised with liver disease and/or anaemia. All were given bone narrow needle aspirations and liver biopsies. 46% of the patients presented a reduction in erythrocyte folates but vitamin B12 deficiency was rarely encountered. Drugs and folate metabolism.

Folates are a group of compounds which are required in the diet and are important in DNA, amino acids and possibly also amine metabolism. The biologically active folates are in the tetrahydro form. Tetrahydrofolates are produced from unreduced dietary folates by the enzyme dihydrofolate reductase. A number of drugs such as aminopterin, methotrexate (amethopterin), pyrimethamine, trimethoprim and triamterene act as folate antagonists and produce folate deficiency by inhibiting this enzyme. With other drugs which produce low serum and tissue concentrations of folate such as anticonvulsants, anti

An evaluation of the importance of gastric acid secretion in the absorption of dietary calcium.

Since calcium solubility is a prerequisite to calcium absorption, and since solubility of calcium is highly pH-dependent, it has been generally assumed that gastric acid secretion and gastric acidity play an important role in the intestinal absorption of calcium from ingested food or calcium salts such as CaCO3. To evaluate this hypothesis, we developed a method wherein net gastrointestinal absorption of calcium can be measured after ingestion of a single meal. A large dose of cimetidine, which markedly reduced gastric acid secretion, had no effect on calcium absorption in normal subjects, and

Nutritional status of vegetarian children.

Thirty-nine preschool children consuming different types of vegetarian diets were studied. Type and amount of carbohydrate, fat, protein, and amount of sodium and cholesterol provided by their diets were more like intakes suggested in the proposed Dietary Goals for the United States than to levels in usual diets of nonvegetarian children. Macrobiotic vegetarian children consumed less animal food than did other vegetarian children. The mean intake of vitamin D of macrobiotics was an eighth of the Recommended Dietary Allowance and mean serum alkaline phosphatase values were elevated. The mean in

Effect of experimental folate deficiency on lipid metabolism in liver and brain.

1. Rats were given a purified folate-deficient diet containing 5 g succinylsulphathiazole/kg for 4-5 months in two experiments. Control rats were supplemented with folic acid in the drinking-water. 2. Weight gain was much below normal in the folate-deprived rats after the first month. Very low folate levels were recorded in blood, liver and peripheral nerve (12-33% of control). In the central nervous system, including the cerebrospinal fluid, the folate depletion was less conspicuous (50-80% of control). Only marginal signs of anaemia were found and no signs of neurological dysfunction were de

Evidence regarding the minimal daily requirement of dietary vitamin B12.

The minimal daily requirement of dietary vitamin B12 is defined as that amount which must be ingested in the diet to prevent the occurrence of megaloblastosis, anemia, or other manifestations of vitamin B12 deficiency. An attempt has been made to define this requirement by studying hematological responses to varying amounts of the vitamin in five south Indian subjects with vitamin B12 deficiency anemia (one due to gastric atrophy and four due to dietary deficiency). Amounts ranging from 0.07 to 0.25 micrograms of dietary vitamin B12 were shown to be inadequate but amounts ranging from 0.3 to 0

Serum and plasma zinc, copper and iron concentrations in Aboriginal communities of North Western Australia.

Two aboriginal communities situated in the tropical north-west of the Australian continent have been investigated in regard to trace metal status (zinc, copper, and iron) and other laboratory and epidemiological information. A total of 364 persons, ranging in age from 5 to 77 years were studied. The incidence of hypozincemia (serum or plasma zinc concentration less than 0.71 micrograms/ml) of the two communities when combined was 24.4%, while hypercupremia (defined as serum or plasma copper levels greater than 1.38 micrograms/ml) was 47.9%. Depressed serum iron levels were demonstrated in more
