The health of doctors and nurses is of paramount importance because they must be well to perform their jobs optimally under difficult conditions. However, the challenge of their working environment and the culture of their professions often lead to physical and mental illnesses. Despite this, there are several barriers to doctors and nurses seeking healthcare. In this study, the health status and health maintenance practices of doctors and nurses at two hospitals in Kingston, Jamaica, were assessed. This population was previously reported to have a 27% prevalence of probable mental distress ba

Mycobacterium bovis bacillus Calmette-Guerin (BCG) is a form of live attenuated M. bovis-based vaccine used to prevent tuberculosis and other mycobacterial infections. BCG immunization also provides cross-protection against a broad range of viral pathogens. Recent studies show that a coordinated induction of innate and adaptive immune responses is involved, though the mechanisms are less defined. To investigate the cross-protection of BCG against virus infection, we used a mouse model of respiratory syncytial virus (RSV) infection and demonstrated that systemic administration of live BCG alter

Mycobacteria infection is a serious public health problem and has been described as causing diseases in domestic animals. The objective of this study was to evaluate the immunopathogenesis of Mycobacterium avium subspecies hominissuis isolated from a domestic cat in mice. Mice were intravenously infected with 106 CFU, and the spleen, lung, liver, and submandibular glands were collected for a histopathology study and bacillary load evaluation. Lung cells, such as macrophages, NK1.1 cells, and CD4+ and CD8+ T lymphocytes were evaluated by flow cytometry. The infection spread to the liver, lungs,

BACKGROUND: Diagnosis of tuberculosis (TB) in resource-limited countries relies primarily on bacteriological confirmation using Ziehl-Neelsen (ZN) stain on sputum or other representative samples. However, this method has low sensitivity due to suboptimal sampling and techniques. While AFB culture remains a gold standard for diagnosing TB and other mycobacterial infections, its limitations include slow turnaround time, and the requirement for specialized resources and expertise. Neutrophils, monocytes, and lymphocytes are crucial in the pathogenesis of granulomatous inflammation and immune reac

Disseminated Mycobacterium bovis (M. bovis) infection is a rare but serious complication in immunocompromised individuals, particularly in those with advanced HIV/AIDS. Diagnosis is often delayed owing to overlapping clinical features with those of other mycobacterial infections. We report a case of disseminated M. bovis infection in a severely immunocompromised patient with AIDS. Rapid identification was achieved using molecular diagnostic techniques, specifically fluorescent PCR melting curve analysis, which enabled timely adjustments to antimicrobial therapy. The patient showed significant

In mycobacteria, the protein EtfD is thought to link β-oxidation of fatty acids with the electron transport chain, two processes that have attracted attention as targets for therapeutics to treat tuberculosis (TB) and other mycobacterial infections. It has been proposed that targeting β- oxidation could shorten treatment duration by killing non-replicating Mycobacterium tuberculosis within granulomas in the lungs. Here we show that Mycobacterium smegmatis , a fast growing and nonpathogenic model for energy metabolism in M. tuberculosis , relies on EtfD for extracting energy from β-oxidation. E

Cell death mechanisms play a fundamental role in mycobacterial pathogenesis. We critically reviewed 94 research manuscripts, 44 review articles, and 4 book chapters to analyze important discoveries, background literature, and potential shortcomings in the field. The focus of this review is the pathogen Mycobacterium tuberculosis (Mtb) and other Mtb and Mycobacterium avium complex microorganisms. Virulent strains hijack cell death processes by inhibiting autophagy, apoptosis, and pyroptosis while eliciting necrosis and ferroptosis to multiply intracellularly and spread within and between hosts.

The emergence of drug resistance remains a major challenge in the treatment of tuberculosis and other mycobacterial infections. To combat the rise in resistance, strategies that reduce the frequency of resistance mutations are urgently needed. Verapamil is a small-molecule compound that can enhance the potency of companion drugs in combination regimen. Here, we investigate if verapamil can decrease the resistance frequency of antimycobacterial drugs. The results show that verapamil significantly reduces the resistance frequency of multiple antimycobacterial agents, including the DNA gyrase inh

Goats represent a significant reservoir for tuberculosis (TB) in animals, contributing notably to public and animal health challenges, causing economic repercussions. Ante mortem diagnosis of TB is hindered by the limited sensitivity of available techniques and false-positive results from other mycobacterial infections, such as paratuberculosis (PTB). Nuclear Magnetic Resonance (NMR)-based metabolomics provides unique fingerprinting of the disease's metabolic status, making it a promising diagnostic tool. However, conventional high-resolution NMR has limitations in veterinary practice, where h

OBJECTIVES: We evaluated the utility of the M-Typer® assay, based on flow-injection tandem mass spectrometry mycolic acid profiling, in the diagnosis of TB and other mycobacterial infections.
METHODS: The ability of the M-Typer® assay to identify Mycobacterium species was first evaluated using 40 clinical strains of known species. In the second phase, clinical specimens from 451 patients with suspected TB or non-tuberculous mycobacteria (NTM) infections were tested. Results were compared with those obtained from Xpert MTB/RIF and MGIT culture.
RESULTS: The M-Typer® assay correctly classified

Mycobacterium avium ssp. paratuberculosis (MAP) has been implicated in the development of Crohn's disease (CD) for over a century. Similarities have been noted between the (histo)pathological presentation of MAP in ruminants, termed Johne's disease (JD), and appearances in humans with CD. Analyses of disease presentation and pathology suggest a multi-step process occurs that consists of MAP infection, dysbiosis of the gut microbiome, and dietary influences. Each step has a role in the disease development and requires a better understanding to implementing combination therapies, such as antibio

The diagnosis of drug-resistant tuberculosis (TB) by molecular testing of Mycobacterium tuberculosis drug resistance genes is becoming increasingly common clinically. However, M. bovis, as an uncommon pathogen of human TB, may interfere with the test results. A comprehensive understanding of phylogenetically informative mutations in the drug resistance genes of M. bovis is required to distinguish true resistance-conferring mutations. We analyzed 53 drug resistance genes in 165 M. bovis isolated from humans using whole-genome sequencing data and found that 98.2% (162/165) of isolates have pyraz

Treatment of Mycobacterium abscessus infections are problematic due to inherent multidrug resistance and lack of response to antibacterials commonly used as therapy for other mycobacterial infections. We report the clinical success of five patients who received definitive-treatment with an omadacycline-containing combination regimen for M. abscessus infection.

The cytoplasmatic biosynthesis of the stem peptide from the peptidoglycan in bacteria involves six steps, which have the role of three ATP-dependent Mur ligases that incorporate three consecutive amino acids to a substrate precursor. MurE is the last Mur ligase to incorporate a free amino acid. Although the structure of MurE from Mycobacterium tuberculosis (MtbMurE) was determined at 3.0 Å, the binding mode of meso-Diaminopimelate (m-DAP) and the effect of substrate absence is unknown. Herein, we show the structure of MurE from M. thermoresistibile (MthMurE) in complex with ADP and m-DAP at 1.

Pathogen-induced epigenetic modifications can reshape anti-infection immune processes and control the magnitude of host responses. DNA methylation profiling has identified crucial aberrant methylation changes associated with diseases, thus providing biological insights into the roles of epigenetic factors in mycobacterial infection. In this study, we performed a genome-wide methylation analysis of skin biopsies from patients with leprosy and healthy controls. T helper 17 differentiation pathway was found to be significantly associated with leprosy through functional enrichment analysis. As a k

BACKGROUND: Buruli ulcer disease (BUD) caused by Mycobacterium (M.) ulcerans is characterized by necrotic skin lesions. As for other mycobacterial infections, e.g., tuberculosis, the immune response is important for host protection. B-cells may play a role in antimycobacterial immunity but studies characterizing the B-cell repertoire and memory generation in BUD and during the course of treatment are scarce.
METHODS: We investigated the adaptive immune cell repertoire in children with BUD and healthy matched controls by flow cytometry. Analyses prior to treatment, also in a study group of pat

Nontuberculous mycobacteria (NTM) are an increasingly common cause of respiratory infection in people with cystic fibrosis (PwCF). Relative to those with no history of NTM infection (CF-NTMNEG), PwCF and a history of NTM infection (CF-NTMPOS) are more likely to develop severe lung disease and experience complications over the course of treatment. In other mycobacterial infections (e.g., tuberculosis), an overexuberant immune response causes pathology and compromises organ function; however, since the immune profiles of CF-NTMPOS and CF-NTMNEG airways are largely unexplored, it is unknown which

Although chemotherapeutic treatment regimens are currently available, and considerable effort has been lavished on the development of new drugs for the treatment of tuberculosis (TB), the disease remains deeply intractable and widespread. This is due not only to the nature of the life cycle and extraordinarily disseminated habitat of the causative pathogen, principally Mycobacterium tuberculosis (Mtb), in humans and the multi-drug resistance of Mtb to current drugs, but especially also to the difficulty of enabling universal treatment of individuals, immunocompromised or otherwise, in widely d

BACKGROUND: Buruli ulcer (BU) is a neglected tropical disease caused by Mycobacterium ulcerans that affects skin, soft tissues, and bones, causing long-term morbidity, stigma, and disability. The recommended treatment for BU requires 8 weeks of daily rifampicin and clarithromycin together with wound care, physiotherapy, and sometimes tissue grafting and surgery. Recovery can take up to 1 year, and it may pose an unbearable financial burden to the household. Recent in vitro studies demonstrated that beta-lactams combined with rifampicin and clarithromycin are synergistic against M. ulcerans. Co

Mycobacterium abscessus is the most common rapidly growing non-tuberculous mycobacteria to cause pulmonary disease in patients with impaired lung function such as cystic fibrosis. M. abscessus displays high intrinsic resistance to common antibiotics and inducible resistance to macrolides like clarithromycin. As such, M. abscessus is clinically resistant to the entire regimen of front-line M. tuberculosis drugs, and treatment with antibiotics that do inhibit M. abscessus in the lab results in cure rates of 50% or less. Here, we identified epetraborole (EPT) from the MMV pandemic response box as
