Clinical significance of a polymerase chain reaction assay for the detection of Mycobacterium tuberculosis.

Various investigators have reported rapid detection of Mycobacterium tuberculosis (MTB) in clinical samples by polymerase chain reaction (PCR). To improve the specificity and efficiency of PCR, the authors adopted a variety of conditions, then analyzed sputum specimens from 217 patients clinically suspected of having MTB. Sputum samples were sonicated to obtain MTB DNA. The DNA was subjected to PCR using primer sets from the region of 650-900 in MTB. The PCR product was detected by direct gel electrophoresis and Southern blot hybridization using digoxigenin-labeled MTB-specific probe. The resu

Development of a SOD ELISA to determine the immunological relatedness among mycobacteria.

This study reports on the standardization of an enzyme-linked immunosorbent assay (ELISA) system for the measurement of immunological distances (ImDs) of the superoxide dismutase (SOD) molecule among the cultivable mycobacteria, namely, Mycobacterium vaccae, M. phlei, M. smegmatis, M. avium, M. scrofulaceum, M. tuberculosis H37Ra, M. tuberculosis H37Rv, and M. bovis BCG, and M. leprae. SODs from cultivable mycobacteria were purified, antibodies were raised against these molecules, and ImDs between these anti-SOD antibodies and antigen (SODs) were determined by an immunoprecipitation technique

Potential role of rifabutin in prophylaxis for tuberculosis and infections due to multiple opportunistic pathogens.

Patients with advanced human immunodeficiency virus (HIV) disease are at risk for infections caused by protozoa, fungi, viruses, bacteria, and mycobacteria. Chemoprophylaxis is being used increasingly to prevent a growing number of opportunistic infections that occur in HIV-infected patients. Multiple opportunistic pathogen prophylaxis (MOPP) is based on the concept that use of antimicrobial agents with activity against a variety of opportunistic pathogens will result in better patient compliance, reduced toxicity, fewer drug interactions, and lower cost than does the use of numerous single ag

Clinical pharmacokinetics of rifabutin.

The clinical effectiveness of rifabutin for prophylaxis of disseminated Mycobacterium avium complex infection has recently been demonstrated in HIV-positive patients with low CD4 counts. Rifabutin is a newly marketed, semisynthetic antimycobacterial agent similar to rifampicin (rifampin) in structure and activity. However, rifabutin has important pharmacokinetic differences compared with rifampicin. Rifabutin has relatively low oral bioavailability; about 20% after single dose administration. With long term administration rifabutin induces its own metabolism and the metabolism of some other dr

Human CD4-CD8- alpha beta + T-cell receptor T cells recognize different mycobacteria strains in the context of CD1b.

Double-negative alpha beta+ T-cell receptor (TCR) human T cells have been reported to recognize antigen in the context of the HLA class I-like (Ib) CD1 complex. In particular, the CD1b molecule has been shown to act as the element of genetic restriction for antigens derived from Mycobacterium tuberculosis. The stenotopic nature of these major histocompatibility complex (MHC) class Ib molecules raised the question of whether the antigenic moiety recognized by CD4-CD8- alpha beta+ TCR T cells was shared by different mycobacteria. We demonstrate here that a CD4-CD8- alpha beta+ TCR T-cell line an

Laboratory reports of opportunistic and other mycobacterial infections and their relationship to notifications of tuberculosis in England and Wales.

SETTING: England and Wales, UK. OBJECTIVE: To investigate: (1) whether misclassification of opportunistic mycobacterial disease had contributed to the failure of tuberculosis notifications to continue declining; (2) whether laboratory reports of Mycobacterium tuberculosis complex infections could be used to validate trends in the tuberculosis notification system. DESIGN: Descriptive epidemiological study using laboratory reports of infections to the Public Health Laboratory Service Communicable Disease Surveillance Centre, Medical Research Council National Surveys of Tuberculosis and tubercu

Detection and identification of Mycobacterium tuberculosis directly from sputum sediments by Amplicor PCR.

Sputum specimens received for the diagnosis of tuberculosis or other mycobacterial infections were tested by a PCR-based assay and culture techniques. Results of the PCR assay (Amplicor Mycobacterium tuberculosis Test) were compared with results of standard culture techniques with cultures held for 6 weeks. One thousand nine specimens were included: 301 retrospective specimens (frozen at -70 degrees C and later tested by PCR) and 708 prospective specimens (tested within 1 day of processing). One hundred sixty-two (16%) of the specimens were culture positive for M. tuberculosis; 83 (51%) of the

Comparison of the resistance of C57BL/6 and C3H/He mice to infection with Mycobacterium paratuberculosis.

Susceptibility of C57BL/6 (Bcgs) and C3H/HeN (Bcgr) mice to an intraperitoneal infection with Mycobacterium paratuberculosis strain 19698 was compared (by histopathology and the number of mycobacteria isolated from the spleen). Mycobacterial counts from the spleen of Bcgr mice progressively decreased over the course of infection but remained unchanged in Bcgs mice. Granulomatous lesions and acid-fast bacteria were consistently present in the liver and lymph nodes of Bcgs mice, whereas lesions were transient or absent in Bcgr mice. These results indicate that Bcgr mice are inherently resistant

Tuberculosis in Africa--any news?

The tuberculosis situation in Africa in the AIDS era has become bleak. The tuberculosis incidence has increased in most sub-Saharan African countries, diagnosis has become more difficult, response to treatment, though initially good, is eventually less effective, and patient compliance, which has been a major problem in tuberculosis control before the HIV epidemic, has now become even more difficult. BCG vaccination, already ineffective before the AIDS era in preventing tuberculosis transmission, is now even less an appropriate tool in tuberculosis control. During a four year stay in Agogo Hos

An outbreak of tuberculosis in a captive herd of Arabian oryx (Oryx leucoryx): diagnosis and monitoring.

An outbreak of tuberculosis induced a mortality of 25 per cent in a captive herd of Arabian oryx (Oryx leucoryx). The diagnostic screening tests used on live animals included the comparative skin test, indirect and comparative ELISA tests and lymphocyte transformation tests. Difficulties in the interpretation of these tests stemmed principally from the facts that false negatives and false positives were encountered and that the threshold of positivity was difficult to establish with the ELISA test. The presence of other mycobacterial infections in the environment was almost certainly a complic

An Id reaction to Mycobacterium leprae: first documented case.

We present a case of generalized sarcoidal dermatitis in a patient with tuberculoid leprosy. After careful consideration of the reactional states in leprosy and recognizing that id reactions occur in other mycobacterial infections, we concluded that the patient's eruption was likely an id reaction to his Mycobacterium leprae infection. It was most reminiscent of the tuberculids: lichenoid tuberculid and lichen scrofulosorum. However, its histomorphologic appearance was distinctive, showing elongated granulomata along with foci of incipient perineural granuloma formation, similar to the histomo

Serologic reactivity against Mycobacterium paratuberculosis antigens in patients with sarcoidosis.

Although sarcoidosis has clinical and histopathologic similarities to some forms of tuberculosis and other mycobacterial infections, attempts to establish a mycobacterial etiology have not been successful. Using cytoplasmic antigens derived from a wild strain of Mycobacterium paratuberculosis in an enzyme-linked immunosorbent assay, patients with sarcoidosis were found to have immunoglobulin levels significantly higher than those found in a control population in the IgG, but not in IgA or IgM antibody classes. Results were comparable to those reported from patients with Crohn's disease, where

Mycobacterial infection in an inner city children's hospital.

Childhood tuberculosis is perceived by many as a disease of the past. Experience in a children's hospital serving a deprived population suggested that tuberculosis and other mycobacterial infections were not declining in clinical practice. Fifty three tuberculous and 11 atypical mycobacterial infections were identified between 1978 and 1992. There was no decline in tuberculosis and nine of the 11 atypical infections occurred in the last five years. Altogether 40% of cases of tuberculosis were in non-Asian children; 32% had arrived in the UK or visited family overseas in the previous year; and

Detection and identification of Mycobacterium tuberculosis directly from sputum sediments by amplification of rRNA.

Seven hundred fifty-eight processed sputum sediments received for the diagnosis of tuberculosis or other mycobacterial infections were tested by utilizing a rRNA target amplification assay and traditional culture techniques. The results from the rRNA target amplification assay (Gen-Probe Amplified Mycobacterium Tuberculosis Direct Test), available in 5 h, were compared with the results from standard culture techniques held for 6 weeks. A total of 119 specimens (16%) were culture positive for Mycobacterium tuberculosis. Overall sensitivity, specificity, positive predictive value, and negative p

Isolation of mycobacteria from patients seropositive for the human immunodeficiency virus (HIV) in south east England: 1984-92.

BACKGROUND: Tuberculosis and other mycobacterial infections are well recognised complications of HIV infection and surveillance is thus required. METHODS: All mycobacteria isolated from HIV positive subjects and referred to the Public Health Laboratory Service South East Regional Tuberculosis Centre (SERTC) from the first such case in 1984 until the end of 1992 were reviewed. RESULTS: A total of 803 mycobacteria isolated from 727 HIV positive subjects were referred to the SERTC during the study period. A single species was isolated from 660 patients: 150 members of the tuberculosis complex (

Ultrastructure of mycobacterial surfaces by freeze-substitution.

The global status of tuberculosis has recently received much public attention, particularly in some developed countries that are now reporting an increase in cases after several years of decline. A number of factors have contributed to the resurgence in tuberculosis and other mycobacterial infections, including homelessness, increased urban overcrowding among the poor, increased drug abuse and the AIDS epidemic. In addition, the intrinsic nature of mycobacterial impermeability to some antibiotics, and their ability to survive in host environments has been attributed to the unique chemistry and

Purification of MPB70 and production of specific monoclonal antibodies.

MPB70 is a protein secreted into the culture filtrate of Mycobacterium bovis BCG (substrain Tokyo 172), which is able to induce a delayed-type hypersensitivity (DTH) skin reaction in guinea pigs immunized with BCG-Tokyo. By high-pressure chromatofocusing and size-exclusion high performance liquid chromatography, a further purified MPB70 protein was obtained, which was visualized as a single band with a molecular mass of 22 kDa by sodium dodecylsulfate-polyacrylamide gel electrophoresis. A series of hybridoma cell lines that produced monoclonal antibodies (MAbs) against the purified MPB70 prote

Mycobacterial infections following heart valve replacement.

Mycobacterial infection is an uncommon but very serious complication of heart valve replacement and other forms of cardiac surgery. Tuberculosis has been a rare complication of valve replacement in the industrially developed countries owing to the low incidence of that disease in such countries. Most reported cases are associated with the insertion of human allograft valves. Valvular tuberculosis could become a more serious problem if heart valve replacement surgery is used to any extent in countries where tuberculosis is common. The majority of other mycobacterial infections occurring after h

Critical comments on the treatment of leprosy and other mycobacterial infections with clofazimine.

The usefulness of clofazimine (CLO, CAS 2030-63-9) in the treatment of mycobacterial infections with special emphasis on treatment of leprosy is critically discussed. Skin discolouration which decreases compliance, placenta passage, excretion in mother's milk which endanger the embryo or baby respectively, saturation kinetics in absorption and difficulties to determine free drug concentration are severe problems. The observed antagonism in the combination of CLO with other drugs, especially with dapsone, is another argument against its application in the therapy of mycobacterial infections. In

[Pulmonary tuberculosis and other mycobacterial infections in patients with AIDS].

Nowadays, the acquired immune deficiency syndrome (AIDS) is most certainly the major cause of the increased incidence of tuberculosis (TBC). A total of 138 patients (pts) with AIDS were referred to the Department of Infectious Diseases of the Hospital of Pisa, 1990-1991; 14 of them (10.1%) presented pulmonary TBC. In our study, we analyzed the chest films of the latter group of patients. Based on the X-ray patterns, the findings were classified as follows: hilar lymph nodes were observed in 4 pts (28.6%), isolated lymph nodes in 2 cases; associated with parenchymal involvement in 2 pts; acute
