Routine biochemical screening for hypothyroidism in 2000 geriatric inpatients proved valuable and practicable and yielded 46 cases (2.3%). A non-specific clinical picture was particularly common, with less than a third of the cases showing "typical" signs and symptoms. Psychiatric manifestations, especially depression, were important and frequent and responded well to thyroxine. There was a preponderance of female cases of hypothyroidism and a strong association with other autoimmune diseases, notably pernicious anaemia and rheumatoid arthritis.

The common and rarer causes of vitamin B12 and folic acid deficiency are discussed with special reference to pathophysiological aspects and conditions in Switzerland. The overall incidence of Addisonian pernicious anemia was found to be 80 per 100,000 patients, whereas the incidence of other B12 deficiency states was 18 per 100,000 patients. Folic acid deficiency was found in 23 of 100,000 patients, though this figure appears to be an underestimate. Nutritional (latent) folate deficiency seems to be the most frequent vitamin deficiency in Switzerland.

The DNA bentonite flocculation test was used to investigate the presence of antibody to DNA. In most sera a prozone-like phenomenon was found, caused by a non-specific inhibition of serum. Anti-DNA was found with extreme frequency in patients with SLE, chronic liver disease, acute hepatitis, infectious mononucleosis, and blood donors over the age of 60, but was found rarely or not at all in patients with some other disease or blood donors under 60 years of age.

The first commercial kit for the radioisotopic assay of vitamin B(12) in serum-the Phadebas B(12) Test produced higher values than the radioisotopic method of Raven, Robson, Walker, and Barkham (1969) and the Lactobacillus leichmannii microbiological assay. Its normal range was 300-1100 pg/ml and its reproducibility was similar to that of the other radioisotopic method. It should be possible to lower the results obtained by the Phadebas method by modifying its standard curve and to reduce the time taken for the assay by shortening its incubation period.

Two siblings with megaloblastic anemia responsive to parenteral vitamin B(12) were studied to elucidate the cause of the B(12) deficiency. Gastric juice from both contained acid and functional intrinsic factor. Serum contained transcobalamin II and lacked antibodies to intrinsic factor.
