BACKGROUND: Vitamin B12 (cobalamin) deficiency is a well-known cause of hematologic and neurological disorders; however, its presentation can be highly variable, leading to diagnostic challenges. The etiology is diverse: while the most common cause is dietary insufficiency, other potential causes include malabsorption syndromes, autoimmune gastritis, gastrointestinal disorders, chronic infections, and genetic defects. Clinical presentation varies significantly, ranging from clinically silent macrocytosis to life-threatening anemia or pancytopenia. Neurological and psychiatric manifestations ma

BACKGROUND: Thrombotic microangiopathy (TMA) during pregnancy is most commonly due to hypertensive disorders such as the hemolysis, elevated liver, low platelet (HELLP) syndrome and preeclampsia (PE). In these cases, delivery is the primary therapeutic intervention. However, distinguishing these pregnancy-related TMAs from other etiologies is critical, as alternative or coexisting causes may necessitate additional diagnostic and therapeutic strategies.
CASE REPORT: We report a complex and unique case of a pregnant patient with prior bariatric surgery who developed a severe early-onset HELLP.

BACKGROUND: Vitamin B12 (cobalamin) deficiency can cause anemia, peripheral neuropathy, visual impairment and irreversible neurological damage. Little is known about public awareness of these neurological effects in Saudi Arabia. This study assessed awareness of the neurological impact of vitamin B12 deficiency among adults in Riyadh, Saudi Arabia.
METHODS: We conducted a population-based cross-sectional survey in Riyadh between March and April 2024. Adults aged ≥18 years were recruited via social media. Individuals affiliated with the health sector were excluded to reduce bias. A validated q

BACKGROUND & AIMS: Nutritional anemia is a widespread public health issue, impacting about one-quarter of the global population (24.3 % in 2021; ∼1.92 billion people). Iron-deficiency anemia (IDA) is the most prevalent form, primarily caused by inadequate dietary intake, poor iron absorption, or increased physiological needs. However, the complexity of nutritional anemia extends beyond iron deficiency, involving other micronutrients such as vitamin B12, folate, zinc, copper, and vitamin A. Additionally, anti-absorptive dietary inhibitors (e.g., phytates, polyphenols, oxalates), chronic infecti

Vitamin B12 and folate deficiency can lead to severe macrocytic anemia, apart from other consequences such as central and peripheral neurological symptoms. We report a case of a 23-year-old male presenting with severe pancytopenia, who was treated initially with red blood cell transfusions, intramuscular cyanocobalamin, and oral folate. The patient's laboratory parameters were normal after 50 days of therapy. During the investigation, alcohol use disorder (AUD) was identified and considered to be the cause of the vitamin deficiencies once other etiologies were excluded. While these vitamin def

Vitamin B12 (cobalamin) is a critical micronutrient involved in hematopoiesis and neurological function. Its deficiency, commonly presenting with anemia and neurological symptoms, is particularly relevant in oncology. While anemia affects up to 60% of cancer patients, the contribution of vitamin B12 deficiency to cancer-related anemia remains underexplored. Additionally, cobalamin-related neuropathy manifests or exacerbates existing chemotherapy-induced peripheral neuropathy (CIPN), a serious side effect of chemotherapy. Prevalence estimates in cancer populations range widely (6-48%), with hig

Vitamin B12, also known as cyanocobalamin, methylcobalamin, and adenosylcobalamin, is an essential vitamin that can be primarily obtained through the human diet. Although animal-based foods supply adequate B12 for most people, deficiency remains common in older adults, vegetarians and vegans, and in those with gastrointestinal disease or malabsorption. Deficiency can lead to megaloblastic anemia, neurologic symptoms, and pregnancy complications. In healthy individuals, supplementation raises serum B12 levels to a similar extent as dietary intake; however, in disease states such as megaloblasti

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a systemic inflammatory disorder that may present with diverse clinical manifestations, including anemia of inflammation. We report the case of a 102-year-old woman who developed rapidly progressive anemia following a pubic fracture complicated by vascular injury. She presented with impaired mobility after a fall and was admitted for orthopedic management. During hospitalization, progressive anemia and fever were observed, prompting further evaluation. Imaging demonstrated a hematoma adjacent to the fracture site and ascites,

Macrocytosis is relatively common and can be due to various etiologies. More common causes include vitamin B12/folate deficiency, liver disease, alcohol use, hypothyroidism, and myelodysplastic syndrome. A work-up often yields the etiology, but in some patients, it remains elusive. We present a case of a 75-year-old female with persistent and unexplained macrocytosis. A work-up for the common causes was unremarkable. However, she had a markedly elevated triglyceride level. This report implies a possible association between hypertriglyceridemia and macrocytosis. Further study is warranted to ex

BACKGROUND: Iron deficiency (ID) and micronutrient deficiencies (MNDs) remain significant public health concerns among children. ID is a common diagnosis in pediatric practice; however, other accompanied MNDs may be overlooked. Here, we aimed to determine MNDs accompanying ID and iron deficiency anemia (IDA) in children.
METHODS: This prospective single-center study included 157 children (6 months-18 years of age) who were referred to the outpatient pediatrics clinic with a possible diagnosis of ID and IDA.
RESULTS: Of 157 children, 76 (48%) were found to have ID and 27 (17%) had IDA. Vitami

Introduction Vitamin B12 deficiency is a common nutritional deficiency, particularly among individuals with poor dietary intake or malabsorption disorders. Given the potential for serious health consequences-including anemia, neuropathy, and cognitive decline-routine vitamin B12 monitoring is critical, particularly in patients with chronic conditions. This study aimed to determine the prevalence of vitamin B12 deficiency and borderline deficiency in male outpatients attending a hospital clinic. Methods A retrospective cross-sectional study was conducted, including male patients attending outpa

BACKGROUND: Preoperative anaemia is a major risk factor for perioperative morbidity. Because iron deficiency is widely assumed to be the main cause of anaemia in surgical patients, treatment efforts have focused mostly on iron supplementation. However, the aetiology of anaemia is multifactorial. To further understand the underlying causes and consider a comprehensive approach to anaemia management, we studied the prevalence and aetiology of preoperative anaemia in patients undergoing major surgery.
METHODS: This prospective, multicentre, observational cohort study was done in 79 hospitals in

The true burden of micronutrient deficiencies among women of reproductive age (WRA) in many resource-limited settings is likely underestimated, as data on several "neglected micronutrients" remain sparse. We used data from a cross-sectional survey of 100 nonpregnant WRA in the region of Oromia, Ethiopia to describe the prevalence of various micronutrient deficiencies and other indicators of nutritional status. The prevalences of anemia (hemoglobin < 13.4 g/dL, adjusted for altitude) and iron deficiency (inflammation-adjusted serum ferritin < 15 ug/L) were 7.1% and 4.0%, respectively. However,

Nutritional deficiencies are prevalent in populations across the world. Fortification of staple foods has been used as an alternative to supplementation to address many deficiencies. One such staple is salt, which has long been fortified with iodine, but more recently with iron, folate, and other micronutrients. Our objective was to determine the effects of fortified salt on nutritional and health outcomes among children, adolescents, and adults. We conducted a systematic review of published and unpublished literature using a pre-defined search strategy. Abstracts and full texts were screened

BACKGROUND: Significant physiological changes occur in adolescence, including the onset of menarche in girls. However, the extent to which menstrual bleeding contributes to anaemia and micronutrient deficiencies remains unclear. This study assesses the prevalence of anaemia and micronutrient deficiencies and examines their association with menarcheal status, menstrual bleeding severity and other factors among adolescent girls in Lebanon.
METHODS: Data were collected as part of a nationally representative cross-sectional study involving 2254 adolescent girls, 10-19 years of age. Household inte

AIMS: To investigate whether localization of beta-catenin is helpful in differentiating primary ovarian mucinous carcinoma and colorectal adenocarcinoma metastatic to the ovary. Extra-ovarian cancers which metastasize to the ovaries, especially from colorectal adenocarcinoma, frequently mimic primary ovarian carcinomas, particularly endometrioid and mucinous types. Distinguishing primary ovarian carcinoma from metastatic colorectal carcinoma is important for both therapeutic and prognostic reasons. Even after thorough histological examination, metastatic colorectal adenocarcinomas are still of

Background The prevalence and severity of anemia among older people are significant health concerns. Hence, the initial medical service provider for older people with any symptoms or signs admitted under any department should be aware of the blood hemoglobin status and if anemia is present and its severity at the very outset, rather than later. This can help treat the presenting problem, along with the workup and management of anemia at the earliest. Hence, we thought of assessing the severity of anemia in the older age group in the eastern coastal belt of India. Methods A prospective observat
