Hematologic deficiencies have previously been described in some cases of recurrent aphthous ulceration (RAU). Ninety patients with RAU and twenty-three healthy control subjects were evaluated by complete blood count, serum iron, and total iron-binding capacity, serum vitamin B12, and serum or red blood cell folate. Only three of ninety RAU patients proved to have abnormalities. There were no statistically significant differences between the patient and control populations, and no one in either group had abnormal serum vitamin B12 or serum or RBC folate assays. Red cell volume distribution curves may be readily obtained using electronic cell counters, and the proportion of cells that deviate from the normal population may be determined. In this study, we report the results of red cell volume distribution analyses in pregnant and non-pregnant females. Several vitamins have been demonstrated to interfere with the pathogenesis of some metabolic diseases, mainly by three different mechanisms: 1) vitamin malabsorption, 2) errors in vitamin metabolism, 3) vitamin dependent syndromes. The latter is due to a deficiency of the apoenzyme whose coenzyme is the vitamin itself. In this case pharmacological, instead of nutritional doses of the vitamin may be needed. Clinical and pathological findings in a case of subacute combined degeneration of the cord are described. The patient was a severe alcoholic but there was no vitamin B12 deficiency. Lesions were similar to those observed in Lichtheim's disease. Biological tests demonstrated frank folic acid deficiency, with very low serum and red cells levels. Clinical features included only mild macrocytic anemia, and flexion quadriplegia after a period of disorders of equilibrium. Higher functions were unaffected. Apart from alcoholism, no other toxic or digestive causative factors were found.

To permit quantitative study of red cell volume heterogeneity, the coefficient of variation (CV) of erythrocyte volume was determined in blood samples from 868 subjects, including normal individuals and those with various hematologic disorders. Treatment of severe iron deficiency with iron-poly(sorbitol-gluconic acid) complex (Ferastral) intramuscular 10 ml (iron 500 mg) on alternate days has been shown highly effective and well tolerated. In order to see whether the time of treatment could be shortened, 20 Nigerians with severe iron deficiency (mostly from hookworm infection) were treated with daily intramuscular Ferastral 10 ml until their calculated total requirement of iron was met. A total of 4939 apparently healthy Fijian and Indian subjects living in Fiji were tested for anaemia by determination of the microhaematocrit of a sample of capillary blood. The prevalence of anaemia during childhood varied with age but was similar for Fijians and Indians of either sex. The overall prevalences were: 0-4 years, 20.3%; 5-9 years, 3.7%; 10-14 years, 23.5%. Utilizing the ability of molybdenum dioxide dichloride to form a black insoluble precipitate with tetrahydrofolate, a cytochemical test for rapid detection of tetrahydrofolate in human leukocytes was developed. The test may be useful in distinguishing deficiency of vitamin B12 or folate or both from other disorders of erythropoiesis characterized by megaloblastic erythropoiesis.

Charts of 122 patients with megaloblastic anemia due to either B12 or folic acid deficiency were reviewed for the presence of fever. Fever, defined as a temperature of 100 F (37.8 C) or more, was present in approximately 40% of the patients with both. The elevation was usually minimal but was sometimes over 104 F (40 C), usually in those with more severe anemia and thrombocytopenia, and greater elevations of SGOT and bilirubin concentration. A simple, rapid and inexpensive cytochemical method for the detection of vitamin B 12 deficiency was applied in several types of anemias and matched with the levels of vitamin B12 and folic acid in the serum of the patients. It was found that in patients with low vitamin B12 levels the stained erythrocytes and the erythroid precursors showed a yellowish brown discoloration, which was not detected in folic acid deficiency and all other types of anemias. This test therefore may be used for differentiation between B12 and folate deficiency whenever megaloblastic anemia is diagnosed.

Human populations native to areas of intense sunlight tend to be heavily melanized. Previous explanations for this relationship have invoked only weak selective pressures. The utilization of [14C]formate for serine synthesis by lymphocytes was impaired in all the patients with pernicious anaemia and in 70% of other patients with megaloblastic anaemia.
