Severe aplastic anemia developed in a young man with an extensive family history of leukemia, pancytopenia, and neutropenia. Deoxyuridine suppression of labelled thymidine uptake tests were performed in the bone marrows of 58 patients with megaloblastic anaemia (haemoglobin less than 10.0 g/dl) and invariably gave values (range 10.3-58.8%) above the range in 16 control marrows (range 1.0-9.0%). Most, but not all, megaloblastic anemia is produced by "ineffective erythropoiesis" in the bone marrow due to either folic acid or vitamin B12 deficiency. Three patients who were taking co-trimoxazole developed acute pancytopenia due to megaloblastic arrest, and two of the three died while pancytopenic. One patient had a pre-existing megaloblastic anaemia. In the other two patients, there was no macrocytosis or neutrophil hypersegmentation despite the severe megaloblastosis. Because specific treatment is needed urgently, it is important to distinguish megaloblastic arrest from drug-induced hypoplasia.

Iron absorption has been studied in patients with dermatitis herpetiformis (DH). Four patients out of 20 had iron deficiency, defined as absence of or only traces of haemosiderin in bone marrow smears. These four had adequate absorption of ferrous iron. In 26 patients with severe iron deficiency and microcytic anemia (MCV less than 70 fl), serial red cell size distribution histograms (erythrograms) were taken before and during iron therapy. Patients treated when phenytoin for periods of several months or years have slightly, but significantly lower whole blood and red cell folate values than comparable controls. This holds true irrespective of whether folate is determined microbiologically with Lactobacillus casei or by a radioligand method using the bovine milk binder. Serum folic acid levels (Lactobacillus casei assay) were determined in 467 subjects representing various socioeconomic classes from rural and urban central Iran. Of the subjects 170 were pregnant females in different trimesters. The mean serum folate levels of the various groups studied ranged from 7.5 to 11.1 ng/ml and are within the normal reported range of 5.9 to 21.0 ng/ml.
