The occurrence of dietary deficiency of Vitamin B12 is reported in two young caucasian adults. In one case megaloblastic anaemia occurred and in the other optic atrophy. The importance of the dietary history, the significance of folic acid intake and the inter-relationship of Vitamin B12 and cyanide detoxication are discussed.

The possible role of cobalamins in the utilization of serum methyltetrahydrofolate has been investigated by means of radiolabeled methyltetrahydrofolate in subjects suffering from pernicious anemia. Folate deficiency, detected by measuring serum and red cell levels, was seen within a group of low birthweight infants who had been followed for a period of 9 months. A similar group treated with 14 × 100 μg folic acid by intramuscular injection over a period of 28 days during the first 5 weeks of life had significantly higher folate levels. Vitamin B(12) levels in erythrocytes were low in untreated hypochromic anaemia, rose to abnormally high levels during therapy with iron alone, and finally slowly fell to normal. These changes were similar to those previously found in pernicious anaemia in response to vitamin B(12) therapy and in folate-deficiency anaemia in response to folic acid, thus changes in erythrocyte B(12) levels are not always due directly to changes in B(12) metabolism but may be secondary to changes in the levels of other haematinic factors.

A technique has been developed for accurately assaying serum and urine (57)Co cyanocobalamin during the conventional Schilling test. Serum and urine radioactivity in a large number of patients has been correlated and the precise normal limits have been determined by statistical analysis. The implications of this for investigating conditions other than pernicious anaemia and the malabsorption syndromes are stressed.

A rapid technique suitable for routine pathology laboratories has been used to estimate methylmalonic acid excretion in a 24-hour urine collection following a 10g. valine load. A simplified microbiological assay for determining the folate content of red cells is described. As in previously reported methods Lactobacillus casei is used as test organism but two modifications are introduced. Non-anemic women attending a public antenatal clinic were given, daily, a multivitamin tablet containing 78 mg. of elemental iron. The follow-up studies included an analysis of their diets. There is a significant fall in the serum folic acid level during pregnancy, reaching its lowest level at term. This is most pronounced in twin pregnancies. A similar but less spectacular fall occurs in the vitamin B(12) concentration. In megaloblastic anaemia both folic acid and vitamin B(12) levels are lower than in other pregnant women.
