Case Report: Subacute combined degeneration of the spinal cord mimic accompanying adaptor protein-3B2-IgG.

We here report the first case of subacute combined degeneration (SCD)-mimic accompanying adaptor protein-3B2 (AP3B2) antibody, expanding the clinical spectrum of AP3B2 antibody-associated disorders. A 55-year-old woman presented with progressive limb numbness, gait instability, and sensory ataxia over six years, unresponsive to prolonged vitamin B12 therapy. Neurological examination revealed combined posterior column, lateral column, and peripheral nerve involvement. Cervical spinal MRI demonstrated posterior column hyperintensity, while electrophysiology confirmed sensory-predominant peripher

Necrotizing Fasciitis Caused by Pseudomonas aeruginosa in an Immunocompromised Patient: A Case With Fulminant Evolution.

Necrotizing fasciitis is a rare, life-threatening infection involving the subcutaneous tissue and fascia, with potential for rapid progression and high mortality. Here, we report the case of a 56-year-old woman with a history of chronic lymphocytic leukemia (under watchful waiting), hypogammaglobulinemia, and megaloblastic anemia who was admitted to the emergency department with progressive swelling of her right forearm following an insect bite. The patient rapidly developed septic shock with multiorgan failure. Broad-spectrum antibiotics were started, and she underwent urgent surgical debride

Patient-reported characteristics of pernicious anaemia: a first step to initiate James Lind Alliance Priority Setting Partnership driven research.

BACKGROUND: Pernicious anaemia (PA) is characterised by vitamin B12 deficiency due to autoimmune-mediated loss of gastric parietal cells and intrinsic factor - a specific transporter for B12's intestinal uptake. The Pernicious Anaemia Society (PAS) is a patient-driven charity that recently identified 10 research priorities for improved diagnosis and management of PA through a James Lind Alliance Priority Setting Partnership. To facilitate research addressing these priorities, the aim of this study was to survey PAS members to identify and characterise a cohort of patients to form a PA research

Rogers Syndrome and Callosal Disconnection in the Setting of Moyamoya Disease.

Rogers syndrome, also known as thiamine-responsive megaloblastic anemia (TRMA), is a rare autosomal recessive disorder characterized by diabetes mellitus (DM), sensorineural hearing loss, and megaloblastic anemia responsive to thiamine supplementation. Moyamoya disease is a progressive steno-occlusive cerebrovascular disorder involving stenosis of the terminal internal carotid arteries with subsequent formation of fragile collateral vessels, resulting in ischemic or hemorrhagic strokes. We present the first documented case of the co-occurrence of Rogers syndrome and moyamoya disease, involving

Unravelling the Puzzle: Highlighting a Case of VEXAS (Vacuoles, E1 Enzyme, X-linked, Autoinflammatory, Somatic) Mimicker Presented With Inflammatory Symptoms and Pancytopenia.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is an autoinflammatory illness that arises due to inactivating mutations in the X-linked UBA1 gene and progresses to impaired ubiquitination, accumulation of misfolded proteins, and activation of numerous inflammatory pathways. Several reactive conditions may mimic the clinical and morphological findings of VEXAS syndrome; therefore, careful differentiation from more common diseases is essential. We present an 82-year-old female who had clinical and morphological features suggestive of VEXAS syndrome; however, a diagnosi

Anemia in Heart Failure: A Systematic Review of Prevalence and Prognostic Impact.

This systematic review explores the potential association between anemia and heart failure (HF) by synthesizing findings from existing scholarly literature. A thorough search strategy was employed across four major databases (PubMed, Scopus, Web of Science, and ScienceDirect) to identify relevant studies. A total of eight studies, encompassing 2,452 participants, were included in the review, of whom 1,230 (50.2%) were men. Iron deficiency anemia (IDA) emerged as the most frequently reported type, although two studies also identified cases of megaloblastic anemia. The prevalence of anemia among

Non-malignant hematologic conditions and subsequent cancer risk: a prospective cohort study and Mendelian randomization analysis.

No studies have examined the association between non-malignant hematologic diseases and the risk of various cancers. To examine the associations between non-malignant hematologic diseases and cancer risk, we performed a cohort study and Mendelian randomization analysis. The association between hematologic diseases and incident cancer risk was assessed using time-varying Cox proportional hazards regression with multivariable hazard ratios (HRs) and 95% confidence intervals (CIs). Over a total of 5 031 372 person-years of follow-up in the UK Biobank, 32 589 (6.8%) patients were diagnosed with he

Enhanced folate content in bioengineered lettuce stably expressed under field conditions.

Folate (vitamin B9) is an essential nutrient that plays a crucial role in various bodily functions. Its deficiency can lead to health issues, such as megaloblastic anemia, neural tube defects in the developing fetus and increased risk of cardiovascular diseases. Consequently, there is interest in increasing folate content in food crops by classical and molecular breeding. Since folate is a water-soluble vitamin that is sensitive to heat, we chose to manipulate its synthesis pathway in lettuce, which is often used for salads and sandwiches, without heat treatment, which makes it a good vehicle

Physiology of vitamin B12: a study on its molecular mechanisms using a Caenorhabditis elegans model.

Vitamin B12 (B12) is a water-soluble substance that is a member of the B-vitamin family. Its recommended daily dose in adult men and women is 2.4 µg, which is the lowest among the 13 vitamins. B12 deficiency causes megaloblastic anemia and neurological disorders. It is also associated with cognitive decline, growth retardation, infertility, and other symptoms. Nevertheless, the detailed mechanisms of which remain unclear. Caenorhabditis elegans is a small organism, with a length of approximately 1 mm and a lifespan of approximately 3 weeks. It has similar fundamental biological structures, suc

Dietary supplementation with conjugated linoleic acid induces anemia with combined features of megaloblastic and hemolytic anemia.

This study investigates whether dietary supplementation with conjugated linoleic acids (CLA) induces anemia in mice. Male Kunming White mice were randomly assigned to two groups: one fed a CLA-supplemented diet and the other a linoleic acid (LA)-supplemented diet as control. Compared with those in LA group, periodic manual erythrocyte counts revealed red blood cells (RBCs) in CLA group significantly reduced on days 47 (p < 0.05), 61 (p < 0.01), and 86 (p < 0.001). Complete blood count (CBC) analysis demonstrated that CLA group exhibited 48.96 % lower RBCs (p < 0.01) and 46.49 % reduced hematoc

New Insights into Folate-Vitamin B12 Interactions.

Folate and vitamin B12 (B12) are essential cofactors in folate-mediated one-carbon metabolism (FOCM). FOCM includes a series of methyl transfer reactions for methionine regeneration and de novo synthesis of nucleotides, including thymidylate. Deficiency in either folate or B12 can result in negative health outcomes including megaloblastic anemia, with additional neurocognitive impairments observed as a result of B12 deficiency. While folate deficiency is not common in the United States due to mandatory folic acid fortification, B12 deficiency is observed more frequently, particularly in certai

Severe Macrocytic Anemia in a Young Alcoholic Male: A Case Report.

Vitamin B12 and folate deficiency can lead to severe macrocytic anemia, apart from other consequences such as central and peripheral neurological symptoms. We report a case of a 23-year-old male presenting with severe pancytopenia, who was treated initially with red blood cell transfusions, intramuscular cyanocobalamin, and oral folate. The patient's laboratory parameters were normal after 50 days of therapy. During the investigation, alcohol use disorder (AUD) was identified and considered to be the cause of the vitamin deficiencies once other etiologies were excluded. While these vitamin def

Vitamin B12: A Comprehensive Review of Natural vs Synthetic Forms of Consumption and Supplementation.

Vitamin B12, also known as cyanocobalamin, methylcobalamin, and adenosylcobalamin, is an essential vitamin that can be primarily obtained through the human diet. Although animal-based foods supply adequate B12 for most people, deficiency remains common in older adults, vegetarians and vegans, and in those with gastrointestinal disease or malabsorption. Deficiency can lead to megaloblastic anemia, neurologic symptoms, and pregnancy complications. In healthy individuals, supplementation raises serum B12 levels to a similar extent as dietary intake; however, in disease states such as megaloblasti

Hematological Correlations as Predictors of Disease Manifestations in Psychiatric Inpatients.

Background/Objective. Psychiatric disorders exhibit significant symptomatic and etiopathological heterogeneity, complicating diagnosis and treatment. Hematological parameters may serve as indicators of overall health and predictors of psychiatric symptom manifestation and remission, particularly in long-term hospitalized patients. This study evaluated hematological and biochemical markers, including vitamin B12, vitamin D, and glucose levels, to explore their potential role in psychiatric disorders and disease progression. Methods. This prospective observational study was conducted from 1 Janu

Unmasking Pernicious Anemia: A Reversible Cause of Pancytopenia Due to Severe Vitamin B12 Deficiency.

Vitamin B12 (cobalamin) is a vital cofactor in DNA synthesis and hematopoiesis, and its deficiency can lead to a wide spectrum of clinical manifestations, ranging from asymptomatic macrocytosis to severe hematologic and neurologic complications. While megaloblastic anemia is the most common presentation, prolonged or profound deficiency can result in pancytopenia due to ineffective hematopoiesis and intramedullary cell death. This can mimic more serious bone marrow disorders, including leukemia or aplastic anemia, potentially leading to invasive diagnostic procedures if the deficiency is not p

School feeding programs for improving the physical and psychological health of school children experiencing socioeconomic disadvantage.

RATIONALE: School feeding aims to alleviate hunger and enhance child outcomes. Since the first Cochrane systematic review of school meals, there has been a marked increase in studies and reviews of school feeding programs. However, most systematic reviews are geographically limited and use qualitative analysis. We reviewed worldwide papers and performed several meta-analyses, providing a more comprehensive picture of the effectiveness of school feeding. OBJECTIVES: 1. To assess effectiveness of school feeding programs for improving the physical and psychological health of children experiencin

Out-of-hospital birth training, experience and confidence of Australian rural volunteer ambulance officers.

INTRODUCTION: Unplanned out-of-hospital births represent less than 1% of ambulance requests for assistance. However, these call-outs have a high risk of life-threatening complications, which are particularly complex in rural or remote settings with limited accessibility to specialist care support. Many community hospitals no longer provide obstetrics care, so birth parents must travel to larger regional or metropolitan hospitals for assistance. Increased travel time may increase the risk of unplanned out-of-hospital birth and/or complications such as postpartum haemorrhage and neonatal mortali

Tropical Sprue: A Rare Cause of Malabsorption Syndrome.

Chronic diarrhea is a common disorder in tropical regions, affecting residents, visitors, and even expatriates. It may stem from a myriad of infectious, inflammatory, and even malignant causes. In patients in whom no etiology has been found, tropical sprue (TS) is an important diagnosis to consider. We report the case of a 60-year-old man originally from Guatemala, presenting with chronic diarrhea and megaloblastic anemia due to severe vitamin B12 deficiency. Biopsies of the small bowel revealed partial villous atrophy and inflammatory infiltrate with the participation of eosinophils. The diag

Identification of novel pathogenic variants of CUBN in patients with isolated proteinuria.

BACKGROUND: Although proteinuria is long recognized as an independent risk factor for progressive chronic kidney diseases, not all forms of proteinuria are detrimental to kidney function, one of which is isolated proteinuria caused by cubilin (CUBN)-specific mutations. CUBN encodes an endocytic receptor, initially found to be responsible for the Imerslund-Gräsbeck syndrome (IGS; OMIM #261100) characterized by a combined phenotype of megaloblastic anemia and proteinuria. METHODS: After analyzing their clinical and pathological characterizations, next-generation sequencing for renal disease gen

Measurement of the major ignored burden of multiple myeloma, pernicious anaemia and of other haematological conditions on partners and family members: A cross-sectional study.

BACKGROUND: Having a haematological condition can adversely affect the quality of life (QoL) of family members/partners of patients. It is important to measure this often ignored burden in order to implement appropriate supportive interventions. OBJECTIVE: To measure current impact of haematological conditions on the QoL of family members/partners of patients, using the Family Reported Outcome Measure-16 (FROM-16). METHODS: A cross-sectional study, recruited online through patient support groups, involved UK family members/partners of people with haematological conditions completing the FROM
