BACKGROUND: Aplastic anaemia (AA) and paroxysmal nocturnal haemoglobinuria (PNH) are closely related acquired bone marrow failure syndromes sharing an immune-mediated pathogenesis. Cytogenetic abnormalities are uncommon in AA-PNH overlap syndrome, and their significance remains uncertain. We report a case of non-severe AA-PNH overlap syndrome harbouring a low-level chromosome 17p deletion in the absence of morphologic or molecular evidence of myeloid neoplasm.
CASE PRESENTATION: A 56-year-old female presented with recurrent aphthous ulcers and pancytopenia. Complete blood count revealed haemo

Nitrous oxide (N2O) is a colorless, odorless gas gaining popularity among recreational drug users for its euphoric effects. Whip-it!™ is a low-cost, freely available form of N2O commonly abused in parties and nightclubs. Chronic N2O use can inactivate vitamin B12, causing severe deficiency, anemia, neuropathy, and myelopathy. Here, we report a 19-year-old female with no significant past medical history who was found unresponsive in her apartment. She admitted to daily Whip-it™ use over the preceding three months. In the emergency department, she reported generalized weakness and tingling in al

A lack of vegetables or other folate-rich foods in the diet leads to folate insufficiency. The most significant cause of this malformation may be a lack of folic acid. Folic acid deficiency, a critical maternal health component, can result in megaloblastic anemia, neural tube anomalies, cardiovascular complications, cancer, and Deoxyribonucleic acid (DNA) imbalance. Beetroot (Beta vulgaris) is one of the most widely consumed vegetables worldwide. Red beets have attracted more attention because of their rich source of bioactive substances, including betalain, indolent nitrates, polyphenols, and

BACKGROUND: Folate is essential for normal growth and in human health throughout the lifecycle. Clinical deficiency of folate impairs DNA synthesis and results in megaloblastic anaemia, while suboptimal folate status before and in early pregnancy results in an elevated risk of neural tube defects (NTD). The evidence on the association of folate status with other health outcomes is largely fragmented and understudied. We conducted a series of umbrella reviews examining the association between folate and multiple health outcomes in various populations and settings.
METHODS: We searched MEDLINE,

We report a seven-week-old male infant, born to consanguineous (double cousin) parents, incidentally through routine newborn screening, following an abnormal acylcarnitine profile caused by elevated formiminoglutamate (FIGLU), with subsequent biochemical confirmation of glutamate formiminotransferase cyclodeaminase (FTCD) deficiency. The infant was born at 37 weeks' gestation via spontaneous vaginal delivery, was vigorous at birth, and required no resuscitation. He remained clinically asymptomatic at the time of metabolic and genetic evaluation. Plasma acylcarnitine analysis demonstrated eleva

INTRODUCTION: Methotrexate (MTX) is a cornerstone therapy for rheumatoid arthritis (RA), yet its hematological toxicity remains a significant clinical concern. This systematic review synthesizes evidence on MTX-induced hematological side effects to inform safer management.
METHODS: A comprehensive search of Medline/PubMed (1991 to 2024), SCOPUS (1995 to 2024), and the Cochrane Library identified 108 studies reporting hematological adverse events associated with MTX in RA patients. Search terms included "RA," "MTX," and "hematologic side effects". Exclusion criteria included the use of biologi

Megaloblastic anemia may present with biochemical features suggestive of hemolysis, creating diagnostic uncertainty. We describe two middle-aged women who presented with severe symptomatic macrocytic anemia, markedly elevated lactate dehydrogenase (LDH), undetectable haptoglobin, and low reticulocyte counts. Further investigation confirmed a combined vitamin B12 and folate deficiency. No definitive underlying cause was identified, although long-term proton pump inhibitor (PPI) use was considered a potential contributing factor to vitamin B12 deficiency. Both patients were treated with vitamin

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Pancytopenia is an important hematological problem encountered in routine clinical practice associated with a multitude of disease states. The possible causes of pancytopenia can be influenced by geography, socioeconomic conditions, and endemic illnesses. Information regarding the underlying clinical conditions and morphologic features of blood cells of pancytopenia is limited and varied across different regions. Thus, this study was designed to assess the peripheral morphologic features of blood cells and the underlying clinical causes of pancytopenia.
METHODS: A facility-based c

BACKGROUND: Pernicious anemia, also called Biemer's disease, is an autoimmune disease and the most common cause of cobalamin deficiency globally. Various genetic, environmental, and immunological factors interplay to lead to its presentation. Pernicious anemia has a myriad of presentations, which can range from hematological and skin-related to neurological. Pernicious anemia has been reported among people of all ages worldwide, especially those over 60 years old. Its prevalence in the general population is about 0.1% and 1.9% for elderly patients over 60 years old. Like most other autoimmune

Background and aim Pancytopenia is a clinical condition characterized by the simultaneous reduction of red blood cells, white blood cells, and platelets. It generally arises from conditions associated with bone marrow dysfunction or increased peripheral consumption of blood cells. Understanding the regional etiological models of pancytopenia is of paramount importance for optimizing diagnostic and therapeutic strategies specific to the local population. This study aimed to identify the etiological causes and examine the distribution of incidence rates in patients presenting with pancytopenia i

BACKGROUND: Pernicious anemia, an autoimmune disease, presents with gradual, nonspecific symptoms, often leading to delayed diagnosis owing to its overlap with other conditions and variability in laboratory findings, such as neurological symptoms without anemia or macrocytosis.
CASE PRESENTATION: This case describes a 40-year-old Iranian woman with a decade-long history of nonspecific symptoms, including fatigue, widespread musculoskeletal pain, paresthesia, cognitive disturbances, and optic neuritis; misattributed to conditions such as fibromyalgia, hypothyroidism, and autoimmune diseases. A 37-year-old male presented with complaints of fatigue, weight loss, and decreased appetite. Laboratory examinations revealed hemolytic anemia, an elevated lactate dehydrogenase (LDH) level, and marked schistocytosis. His other laboratory hematological findings included nucleated red blood cells with abnormal morphologies in the peripheral blood, as well as a small peak that was indicative of schistocytes on a red blood cell histogram. A bone marrow examination demonstrated megaloblastic changes; therefore, a preliminary diagnosis of vitamin B12 deficiency was made. Intramuscular vitamin B12

Background Type 2 diabetes mellitus prevalence in Jordan is high and increasing, leading to an increase in the use of metformin, which is considered the first-line treatment. Metformin, which belongs to the biguanides family of anti-diabetic medication, is known for its low cost, bodyweight neutrality, effectiveness, and good safety profile. In addition, it showed that it improves body lipid profile, inflammatory markers, and decreases cardiovascular events. In spite of that, a reduction in vitamin B12 levels was reported as a possible long-term side effect of metformin. Vitamin B12 deficiency

Nitrous oxide (N2O), an anesthetic gas used for dental procedures, has recently become increasingly recognized as a potential drug of misuse and abuse, with an increasing number of case studies reporting serious adverse effects, including megaloblastic anemia, myelopathy, neuropathy, subacute combined degeneration, psychosis, and even death. There is also growing research support for the use of N2O in the treatment of several psychiatric disorders, including major depressive disorder. In this issue, we present the case of a 37-year-old man with no significant psychiatric history who presented

Pancytopenia is caused by impaired production and peripheral destruction of blood cells, leading to decreased levels of red blood cells (RBCs), white blood cells (WBCs), and platelets. The etiologies can be classified as reversible and irreversible causes. One of the reversible causes is Vitamin B12 deficiency. Vitamin B12 deficiency can cause various types of manifestations, including hematologic as well as neurological symptoms, and sometimes mimicking serious hematologic conditions. A 46-year-old male presented with generalized body weakness, easy fatigability, and occasional nausea with vo

Pellagra is a disease of niacin (vitamin B3) deficiency and is classically characterized by the triad of diarrhea, dermatitis, and dementia, and has the potential to cause death. Pellagra can be caused by either insufficient dietary intake or dysfunctional utilization of niacin. Because niacin can be found in nearly every food group (meat, dairy, vegetables, grains, etc.), deficiency is rarely seen in resource-rich countries where food scarcity is less prevalent. In this case, a 50-year-old woman presented to the emergency department with abdominal pain, watery diarrhea, altered mental status,

This report describes the rare case of a patient with autoimmune hemolytic anemia due to cryoagglutinins secondary to Mycoplasma pneumoniae infection, coexisting with pernicious anemia. A 56-year-old man presented with a ten-day history of cough and mucocutaneous pallor. Laboratory studies revealed megaloblastic anemia with low vitamin B12 levels, positive antibodies against intrinsic factor and parietal cells, as well as hemolysis parameters and a positive direct Coombs test for complement (C3d) with cryoagglutinins active at low temperatures. M. pneumoniae infection was confirmed by indirect

We here report the first case of subacute combined degeneration (SCD)-mimic accompanying adaptor protein-3B2 (AP3B2) antibody, expanding the clinical spectrum of AP3B2 antibody-associated disorders. A 55-year-old woman presented with progressive limb numbness, gait instability, and sensory ataxia over six years, unresponsive to prolonged vitamin B12 therapy. Neurological examination revealed combined posterior column, lateral column, and peripheral nerve involvement. Cervical spinal MRI demonstrated posterior column hyperintensity, while electrophysiology confirmed sensory-predominant peripher
