Normal and abnormal regulation of beta-msh in man.

The regulation of plasma beta-melanocyte-stimulating hormone (beta-MSH) in man has been studied utilizing a radioimmunoassay previously described (1). In normal subjects plasma beta-MSH values ranged from 20 to 110 pg/ml. Metyrapone increased and dexamethasone decreased plasma beta-MSH levels. Surgical stress stimulated beta-MSH secretion. Plasma beta-MSH levels were elevated in patients with untreated Addison's disease and untreated congenital adrenal hyperplasia, and these levels fell to normal during glucocorticoid therapy. In patients with Cushing's syndrome due to pituitary adrenocorticot

The gastrointestinal stimulus to insulin release. I. Secretin.

The gastrointestinal stimulus to the release of insulin has been investigated in man by the use of a radioimmunoassay for secretin. Serum secretin levels rose rapidly after the oral ingestion of glucose or protein and preceded the elevation of serum insulin. An intravenous infusion of highly purified secretin caused a release of insulin when the serum secretin levels were within the physiological range. Infusion of hydrochloric acid into the duodenum caused an elevation of serum secretin and serum insulin levels in normal subjects. A similar response of secretin and insulin was seen after intr

Tropical sprue in expatriates from the tropics living in the continental United States.

The results of clinical, laboratory and therapeutic observations conducted over a seven year period in 40 expatriates from the tropics who presented in New York City with overt tropical sprue have been described. The majority of subjects presented with symptoms referable to the gastrointestinal tract, weight loss and weakness. Only nine were symptomatic at the time of arrival; the remainder developed symptoms within several months to 14 years after arrival. Thirty-five subjects had a megaloblastic anemia; this was a secondary to a combined deficiency of folate and vitamin B12 in 25 and to defi

Effect of vitamin B12 and folic acid deficiency on small intestinal absorption.

Three patients are described, and they provide further evidence that deficiency of folic acid and vitamin B(12) may sometimes affect small intestinal function. Malabsorption of both xylose and vitamin B(12) returned to normal in one patient after treatment of a megaloblastic anaemia due to dietary deficiency of folic acid. Impaired absorption of vitamin B(12) was corrected by vitamin B(12) therapy in the other two patients. The initial cause of the vitamin B(12) deficiency in one patient was not apparent, but she was taking Gynovlar 21, which may have been an aetiological factor. In the third

Value of estimating methylamalonic acid excretion in anaemia.

A rapid technique suitable for routine pathology laboratories has been used to estimate methylmalonic acid excretion in a 24-hour urine collection following a 10g. valine load. Levels above 40 mg./24 hours were found only in patients with vitamin B(12) deficiency. Patients with pernicious anaemia treated more than 24 hours before urine collection and patients with other types of anaemia had methylmalonic acid levels below 25 mg./24 hours. This method of demonstrating vitamin B(12) deficiency can be applied rapidly in debilitated patients so that specific treatment can be instituted within 36 ho

The incidence of adrenal and other antibodies in the sera of patients with idiopathic adrenal insufficiency (Addison's disease).

The histories of 118 (sixty-one males, fifty-seven females) with idiopathic Addison's disease were reviewed and their sera tested for adrenal, thyroid, gastric and parathyroid antibodies. Adrenal antibodies were found in approximately 50% of patients with idiopathic Addison's disease. The majority of these patients, particularly the females, had an associated diagnosed disease. Studies of antibodies to various tissues is of assistance in detecting the frequent sub-clinical occurrence of other diseases in idiopathic Addison's disease. The onset of idiopathic Addison's disease in sixty-four of t

In vitro studies on antibodies to intrinsic factor.

In vitro experiments on anti-intrinsic factor antibodies in the sera from patients with pernicious anaemia are reported. These studies show that antibody activity which blocks the binding of B12 by intrinsic factor (antibody I) is on a different molecule from that which unites with intrinsic factor-B12 (antibody II), and that antibody II unites with either intrinsic factor (IF) or intrinsic factor-B12 (IFB12). Antibody I exchanges readily with the B12 of IFB12. The complex of IF and antibody I will absorb antibody II. The data are interpreted as supporting the conclusion that IF has at least t

Estimation of intrinsic factor and detection of intrinsic factor antibodies using non-radioactive cyanocobalamin and microbiological assay.

A method employing non-radioactive vitamin B(12) and microbiological assay is described for estimating intrinsic factor in gastric juice and for detecting antibody to intrinsic factor in serum. Satisfactory agreement was obtained between the results by this method and by a modification of the method of Ardeman and Chanarin (1963). During the first hour after gastric stimulation 11 patients with pernicious anaemia secreted between 0 and 240 units of intrinsic factor compared with between 1,600 and 39,000 units in 21 patients with other conditions. The results in three out of four patients with

A clinical and immunological study of adrenocortical insufficiency (Addison's disease).

Fifty-one patients with adrenocortical insufficiency were subdivided into three groups according to the nature of their adrenal disease; twelve patients with idiopathic, twenty-three patients with probable idiopathic and sixteen patients with tuberculous adrenal insufficiency. The importance of objective confirmation of a clinical diagnosis of adrenal insufficiency is stressed and the difficulties of classification of many patients with adult onset adrenal insufficiency are discussed. Idiopathic and probable idiopathic adrenal insufficiency had a sex ratio that was predominantly female (2·5:1)

Method of assay of red cell folate activity and the value of the assay as a test for folate deficiency.

A simplified microbiological assay for determining the folate content of red cells is described. As in previously reported methods Lactobacillus casei is used as test organism but two modifications are introduced. First, haemolysis is carried out in water containing 1 g.% of ascorbic acid; secondly, haemolysates are not incubated before the assay. Using this assay, recovery of pteroylglutamic acid added in two different concentrations to five different whole blood samples was 97.0 +/- 1.9 S.E. % and 106.1 +/- 4.7 S.E. % respectively. The coefficient of variation of the assay was between 11.2 a

An account of 335 cases of megaloblastic anaemia of pregnancy and the puerperium.

The incidence of megaloblastic anaemia in pregnancy and the puerperium in north Staffordshire has steadily declined as a result of prophylaxis with folic acid. In the presence of advanced folic acid deficiency and with a florid megaloblastic marrow, the anaemia is usually severe, but in many patients the disease is relatively mild and the degree of anaemia is determined more by blood loss or associated iron deficiency than by the megaloblastosis. Microscopic examination of marrow films is still the most reliable method of diagnosis, although estimation of the labile serum folate has produced a

Immunoassay of gastric intrinsic factor and the titration of antibody to intrinsic factor.

A modification of the charcoal method for the detection and titration of antibody to intrinsic factor in serum and for the assay of intrinsic factor in human gastric juice is described. The antibody was detected in a titre greater than 5 ng units/ml in 52% of the sera of 162 patients with Addisonian pernicious anaemia. False positive results may occur if the serum is withdrawn within 24 hr of the parenteral administration of normal therapeutic doses of vitamin B12. In the analysis of human gastric juice, the charcoal method of immunoassay clearly distinguishes between intrinsic factor and othe

Measurement of intestinal absorption of 57-Co vitamin B-12 by serum counting.

The results of the measurement of vitamin B(12) absorption by counting the radioactivity of 5 ml. serum obtained eight to 10 hours after the ingestion of an oral dose of 0.5 mug. vitamin B(12) labelled with 0.5 muc. (57)Co are compared with those obtained with the urinary excretion (Schilling) test. Inadequate urine collection and impaired renal function were responsible for low results in the Schilling test in four of the 12 control subjects, and an incomplete urine collection in four patients with pernicious anaemia could have led to doubt about the validity of the low result. The measuremen

The incidence and prevention of folate deficiency in a pregnant clinic population.

Non-anemic women attending a public antenatal clinic were given, daily, a multivitamin tablet containing 78 mg. of elemental iron. The follow-up studies included an analysis of their diets. A total of 311 patients were included, of which one group received a supplement of 0.5 mg. folic acid and 0.005 mg. vitamin B(12). The incidence of megaloblastic bone marrow change in the unsupplemented group was 26% and of low blood folates approximately 50%. The incidence of megaloblastic changes was sharply reduced in the supplemented group and the blood folates were elevated to supranormal levels, indic

THE DIRECT ANTIGLOBULIN (COOMBS) TEST IN MEGALOBLASTIC ANAEMIA.

Folic acid deficiency with the picture of a megaloblastic bone marrow may develop in haemolytic anaemia, and, on the other hand, both vitamin B(12) and folic acid deficiency may produce signs of haemolysis. As the correct interpretation of a positive antiglobulin reaction associated with megaloblastic erythropoiesis is particularly important, the effect of deficiency of vitamin B(12) and folic acid on the results of the test was investigated in 32 patients with vitamin B(12) or folic acid deficiency and a positive antiglobulin reaction was obtained in ten. There was no correlation between the
