Pathways and barriers to tuberculosis care for refugees in Taraba State, Nigeria.

INTRODUCTION: Refugee and displaced populations experience high tuberculosis (TB) burdens and face substantial barriers to accessing diagnosis and treatment. Taraba State in north-eastern Nigeria hosts approximately 80,000 refugees, primarily from Cameroon, many living in remote areas with limited TB services. Evidence on how refugees access TB care in this setting remains limited. This study examined the pathways refugees follow from symptom onset to TB diagnosis and identified barriers to accessing and sustaining care. METHODS: A mixed-methods study was conducted between May 2022 and Decemb

Healthcare workers' perceptions of the potential value of whole-genome sequencing for post-exposure management of rifampicin-resistant tuberculosis: a qualitative study.

BACKGROUND: Rifampicin-resistant tuberculosis (RR-TB) remains a public health challenge, with suboptimal treatment outcomes and challenging management of contacts. Whole genome sequencing (WGS) generates a comprehensive drug resistance profile and transmission inference with a single assay. Limited evidence exists on how healthcare workers (HCWs) perceive the value of WGS, especially for post-exposure management. METHODS: We conducted a qualitative study with 22 purposively sampled HCWs (9 doctors, 9 nurses, 4 community health workers) at 11 public health care facilities in the Western Cape,

Genome-wide identification and characterization of the PE/PPE gene family in Mycobacterium abscessus.

Mycobacterium abscessus (Mab) is a rapidly growing non-tuberculous mycobacterium that causes opportunistic infections and exhibits extensive intrinsic antibiotic resistance. The proline-glutamate (PE) and proline-proline-glutamate (PPE) protein families are mycobacteria-specific virulence factors implicated in immune modulation, antigenic variation, and persistence. While Mycobacterium tuberculosis (Mtb) harbors approximately 170 PE/PPE members, the repertoire of PE/PPE genes in Mab remains largely uncharacterized. Here, we performed a genome-wide survey of PE/PPE genes in Mab ATCC 19,977 and

Spatial spillover effects and regional differences of tuberculosis health education levels among China's floating population: a spatial lag model study (2014-2017).

BACKGROUND: The cross-regional floating population in China has brought new challenges for tuberculosis prevention and control. Tuberculosis health Education for the floating population can significantly enhance their health awareness and reduce the prevalence of the disease. However, the current status of health education for the floating population is not adequately assessed, highlighting the urgent need to identify the key factors that restrict effective health education and to safeguard the health rights and interests of this group. METHODS: This study utilized the data of China's migrant

Diverse infection models demonstrate robust resistance of Mycobacterium tuberculosis to innate immunity.

Mycobacterium tuberculosis (Mtb) is a robust activator of innate immunity. While innate immune activation of macrophages can impact Mtb replication in vitro, there is little in vivo evidence that the innate immune system controls Mtb before the onset of adaptive immunity. Prior work has generally used specific pathogen-free (SPF) mouse models and relatively large infectious doses, which may obscure the capacity of innate immunity to control Mtb. Here, we performed ultra-low dose Mtb infections and found that the initial innate immune response was unable to curb even minimal Mtb infectious dose

Formulation Design Dictates the Pulmonary Delivery and Antimicrobial Efficacy of Clofazimine-Amikacin Combinations against Mycobacterium abscessus.

Intracellular infections impose fundamental physicochemical barriers that can disrupt antibiotic synergy observed under well-mixed in vitro conditions. Mycobacterium abscessus, a macrophage-resident pathogen, exemplifies this challenge, as co-administered drugs must traverse multiple membrane barriers and achieve coordinated intracellular exposure. The combination of clofazimine (CFZ) and amikacin (AMK) exhibits robust, time-dependent synergy in broth, yet their divergent physicochemical properties (lipophilic versus hydrophilic) predict differential intracellular access. Here, we evaluated wh

Prevalence and determinants of metabolic abnormalities among adult tuberculosis patients at Gondar Town, Northwest Ethiopia: an institution-based comparative cross-sectional study.

BACKGROUND: Metabolic abnormalities, such as diabetes mellitus, hypertension, dyslipidemia, and central obesity, are significant contributors to the global burden of non-communicable diseases. Tuberculosis (TB) patients may be at increased risk of these conditions due to chronic inflammation, nutritional depletion, and anti-TB drug effects; however, locally grounded comparative evidence from resource-limited settings such as northwest Ethiopia remains scarce. This study aimed to assess the prevalence and determinants of metabolic abnormalities among TB patients in Gondar Town, Ethiopia. METHO

Alpha-galactosylceramide as mRNA vaccine adjuvant modulates cellular immunity against tuberculosis and contributes to antigen-specific lung-associated T cells with a TRM-associated phenotype following prime-pull strategy.

Tuberculosis (TB) remains the leading cause of death from a single infectious agent worldwide, highlighting the need for improved vaccines beyond the limited protection currently provided by the Bacillus Calmette-Guérin (BCG) vaccine. Here we investigated the immunological impact of incorporating alpha-Galactosylceramide (αGC) into mRNA lipid nanoparticles (mRNA-LNPs) as a vaccination strategy against Mycobacterium tuberculosis (Mtb), the pathogen causing TB. Using Mtb antigens ESAT-6 and Ag85B, we demonstrate that αGC-adjuvanted mRNA-LNPs induce controlled activation of invariant natural kill

In vitro and in vivo antibacterial activity of pravibismane against Mycobacteroides abscessus.

The increasing prevalence of non-tuberculous mycobacterial lung infection represents a global challenge. Infections caused by Mycobacteroides abscessus commonly occur in individuals with underlying predisposing lung diseases such as cystic fibrosis (CF), non-CF-bronchiectasis, chronic obstructive pulmonary disease (COPD), and tuberculosis (TB). The unmet need in the treatment of M. abscessus pulmonary infection is driven by multidrug-resistance, ability to form biofilms, lengthy treatment, and drug-related side effects. Pravibismane was evaluated for its potential to reduce M. abscessus burden

Comparative histopathological and immunohistochemical features of central nervous system and pulmonary tuberculosis: an exploratory retrospective study.

Central nervous system tuberculosis (CNS-TB) is a rare and particularly severe form of extrapulmonary tuberculosis (EPTB). Early diagnosis and precise treatment are crucial in managing this condition. Pathological and immunohistochemical features may differ between CNS-TB and pulmonary tuberculosis (PTB). This study included 9 patients of CNS-TB, and analyzed their clinical manifestations, pathological features, and the immunohistochemical(IHC) features. The findings were then compared with PTB to explore the similarities and differences in how tuberculosis manifests in the brain versus the lu

Comparing a straight-to-biopsy versus pleural aspiration-first approach in malignant and tuberculous pleural effusions.

BackgroundPleural aspiration has limited diagnostic yield in malignant (MPE) and tuberculous pleural effusions (TPE), more so when requiring molecular profiling in MPE and due to the paucibacillary nature of TPE. In our institution, patients with suspected MPE or TPE are offered a straight-to-biopsy approach with pleural biopsy as the initial diagnostic procedure, or a pleural aspiration-first approach, through shared decision-making.ObjectivesTo compare clinical outcomes between a straight-to-biopsy strategy and a pleural aspiration-first approach in patients diagnosed with MPE or TPE.DesignW

Analytical and clinical performance evaluation of the STANDARD™ M10 MTB/NTM assay for the simultaneous detection of Mycobacterium tuberculosis and nontuberculous mycobacteria.

The global surge in nontuberculous mycobacteria (NTM) infections, which clinically mimic Mycobacterium tuberculosis (MTB) infections, underscores the need for rapid differential diagnosis. This study evaluated the analytical and clinical performance of the STANDARD™ M10 MTB/NTM assay, a fully automated, cartridge-based, sample-to-result real-time PCR system. Analytical performance was assessed using contrived artificial sputum samples to evaluate inclusivity (3 MTB-complex, 12 NTM species), cross-reactivity (65 respiratory pathogens), and interference (33 substances). The limit of detection (L

A case of recurrent pleural effusion: tuberculous pleurisy or other disease? A case report.

BACKGROUND: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M-protein, skin changes) is a paraneoplastic syndrome caused by a plasma cell proliferative disorder. Although POEMS syndrome can cause polyserous effusions, the report of pleural effusion as the main manifestation is not common. Here, we describe a case of POEMS syndrome misdiagnosed as tuberculous pleurisy due to massive pleural effusion. CASE PRESENTATION: A 70-year-old man was admitted with chest tightness and was diagnosed with tuberculous pleurisy. Despite receiving anti-tuberculosis treatment, patient exhibited i

Mycobacterium tuberculosis complex Lineage 8 causes drug resistant pulmonary tuberculosis also in South Kivu, Eastern Democratic Republic of Congo.

BACKGROUND: Lineage 8 (L8) occupies a unique phylogenetic position as a sister clade to all other known Mycobacterium tuberculosis complex (MTBC) lineages. Although the initial identification of L8 strains was limited to Rwanda and Uganda, the complete geographical distribution of this lineage remains undetermined due to limited genomic surveillance across eastern and central Africa. METHODS: We performed whole genome sequencing (WGS) on MTBC isolates from rifampicin-resistant tuberculosis (TB) patients' sputum in Bukavu, South Kivu province, eastern Democratic Republic of Congo (DRC).
