Armadillo meat consumption and Hansen's disease (leprosy): a case-control study in Brazil.

BACKGROUND: Hansen's disease (HD, leprosy) had a global incidence of approximately 200 000 new cases per annum over the last decade. Armadillos are recognized reservoirs of Mycobacterium leprae in the Americas and HD is considered a zoonosis in the USA. Our aim was to evaluate the association between armadillo meat consumption and HD in Brazil. METHODS: We conducted a case-control study (n=59 patients with HD, n=59 controls) in the state of Espírito Santo, Brazil, between February and December 2025. Data collected included clinical and sociodemographic, armadillo meat consumption and known (h

Ancient Mycobacterium leprae genomes reveal a potential origin and dispersal of the ancestral Branch 0.

Although previous studies have examined numerous ancient Mycobacterium leprae genomes, the available strains remain concentrated in western Eurasia. Ancient genomes from eastern Eurasia have remained virtually unexplored, limiting our ability to reconstruct the evolutionary history of M. leprae. In this study, we report the first two ancient M. leprae genomes recovered from this region. Phylogenetic analyses place both genomes within the ancestral Branch 0, and the observed phylogeographical patterns are consistent with the hypothesis that this lineage originated in southern East Asia. High-dose rifampicin and clarithromycin for 4 weeks versus standard-dose rifampicin and clarithromycin for 8 weeks to treat Buruli ulcer: An open-label, individually randomized, controlled trial in Ghana.

BACKGROUND: The current 8-week regimen of rifampicin and clarithromycin for Buruli ulcer (BU) is suboptimal. High-dose rifampicin could shorten treatment and improve outcomes. We evaluated whether a 4-week high-dose regimen could improve time to clearance of viable Mycobacterium ulcerans. METHODOLOGY: In this open-label, individually randomised trial conducted in Ghana, participants with PCR-confirmed BU were assigned (1:1) to either high-dose oral rifampicin (20mg/kg) with clarithromycin (15mg/kg) for 4 weeks (HR) or standard-dose rifampicin (10mg/kg) with clarithromycin (15mg/kg) for 8 week

System-wide investments generate health and enhance HIV, TB and malaria control in Malawi.

Global health initiatives have expanded access to HIV, tuberculosis, and malaria services in low-income countries, yet these programmes often operate within fragile health systems where shortages of healthcare workers and essential medicines limit their effectiveness and sustainability. Evaluating investments in health system capacity remains challenging because their benefits are difficult to attribute. Here, using a dynamic microsimulation model of Malawi's health system, we show that health system strengthening investments alone could avert 11.4% of disability-adjusted life years between 20

Mycobacterium tuberculosis infects human peripheral hematopoietic multipotent progenitors and promotes their myeloid differentiation in vitro.

Mycobacterium tuberculosis (Mtb) persists as a global health threat partly by establishing long-term reservoirs in myeloid cells. Although progenitors have been proposed as additional sites of persistence and immune reprogramming, it remains unclear whether circulating human hematopoietic stem/progenitor cells (HSPC) are permissive to Mtb infection. Here, we show that peripheral blood-derived CD34+ HSPC are permissive to Mtb, in vitro. Confocal microscopy showed Mtb cytoplasmic localization, while colony-forming unit (CFU) quantification demonstrated early intracellular Mtb growth. Flow cytome

A Comparison of Mechanisms Driving Lesion Outcomes During Lung Tumor and Tuberculosis Granuloma Formation.

Small cell lung cancer (SCLC) and tuberculosis (TB) are both deadly diseases that present with spatially complex lung lesions. These lesions share similarities, including spatial interactions between T cells and macrophages. Both SCLC and TB exhibit heterogeneous disease progression and responses to treatment; current experimental methods have few tools to investigate the spatiotemporal evolution of these lesions within human lungs. We have applied our computational agent-based model (ABM), GranSim, to extensively study heterogeneity of TB granuloma formation and treatment efficacy. We introdu

Synthesis and Evaluation of New Nitrofuranylamides With Collection of Mycobacterium tuberculosis Clinical Strains From Multidrug-Resistant Tuberculosis High-Burden Country.

Emergence of drug-resistant Mycobacterium tuberculosis necessitates the development of new drugs. Here, we studied the activity of new piperazine nitrofuranylamides against a collection of clinical strains from an multidrug-resistant tuberculosis (MDR-TB) high-burden country. A series of nitrofuranylamides 9-15 was synthesized and evaluated using a collection of diverse clinical strains representing globally spread genotypes (Beijing, LAM, and Haarlem; susceptible, MDR, and pre-extensively drug resistant). Minimal inhibitory concentrations (MIC) were determined using a resazurin assay. Whole-g

Factors perceived as relevant to starting antimicrobial treatment in patients with non-tuberculous mycobacterial lung disease in the Italian scenario: a repeated cross-sectional survey after three years of educational activities.

BACKGROUND: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasingly recognised, yet initiating antimycobacterial therapy remains challenging. Following a three-year period of national educational activity in Italy, we re-assessed which factors Italian clinicians perceive as relevant to the decision to start treatment. METHODS: We conducted a repeated cross-sectional web-based survey. The second wave was administered between November 2023 and February 2024 to members of the Italian SITA and IRENE networks, using the instrument administered in 2020. Participants rated the relev

Clinical characteristics and outcomes of localized versus disseminated nontuberculous mycobacterial disease in people living with HIV, 2004-2025.

PURPOSE: Advanced or untreated HIV remains an important risk factor for opportunistic infections, including nontuberculous mycobacteria (NTM). We assessed disease extent, recurrence, and mortality in adults with HIV and NTM disease over 21 years at University Hospital Düsseldorf. METHODS: We conducted a retrospective study at University Hospital Düsseldorf of adults with HIV and microbiologically confirmed or clinically diagnosed NTM disease between April 2004 and April 2025. Dissemination was defined as NTM detection at a normally sterile site, involvement of at least two non-contiguous site

Fatty acid-dependent cidality in Mycobacterium avium and Mycobacterium abscessus: intracellular acidification by itaconic acid.

Non-tuberculous mycobacterial (NTM) lung disease is now recognized as a chronic condition with considerable morbidity and mortality with incidence rising globally. This is concerning as these infections are often mis- or undiagnosed and currently available treatment regimens have lengthy (>1 year) treatment courses with variable to poor success rates. Identification of vulnerable targets and pathways in NTM pathogens is urgently needed to develop improved therapeutics. Itaconic acid (ITA), a macrophage-produced metabolite with immunomodulatory and antibacterial activity, inhibits growth of myc

Dopamine alters macrophage metabolism promoting intracellular Mycobacterium tuberculosis survival.

BACKGROUND: Dopamine (DA) is thought to aggravate progress of bacterial infections and enhance risk of intracellular bacterial infection like tuberculosis (TB). Though TB patients are known to present with high dopamine levels the causal link between them has remained unexplored. Mycobacterium tuberculosis (M. tb), the causative agent of TB, requires a copious supply of iron and carbon for survival and proliferation within host cells. METHODS: In vitro and in vivo administration of dopamine induced an increase of non-transferrin bound iron delivery into macrophages along with and lipid accumu

Application of an Open-Source Modular Framework for Pharmacokinetic, Pharmacodynamic, and Safety Simulations to Anti-Tuberculosis Drugs.

Tuberculosis (TB) treatment requires long-term multidrug regimens where dosing, efficacy, and safety optimization remain challenging. This tutorial establishes an integrated, open-source, modular framework connecting high-throughput PBPK (HT-PBPK) simulations, mechanistic PBPK models, pharmacodynamic endpoints, and safety predictions for anti-TB drugs. Using the Open Systems Pharmacology Suite and ESQhtpbpk (R package), physicochemical parameters were obtained from literature, with QSAR-predicted values supplementing missing experimental data to generate generic PBPK models. Mechanistic models

The Burden and Genetic Diversity of Mycobacterium bovis Infection in Humans in the Middle East and North Africa: A Systematic Review and Meta-Analysis.

Zoonotic tuberculosis (zTB) includes human tuberculosis caused by animal-associated Mycobacterium species, Mycobacterium bovis (M. bovis) being the major cause of zTB. This systematic review and meta-analysis was conducted to estimate the pooled burden of human M. bovis infection and describe the genetic diversity of M. bovis isolated from humans in the Middle East and North Africa (MENA) region. A literature search was conducted in PubMed/MEDLINE, Scopus, Web of Science, Embase, and Google Scholar, followed by manual screening of additional references. The literature used for this paper were

Clinical Recovery in Tuberculosis is Associated With the Functional Remodelling of Ag85A-Specific Antibody Avidity and Phagocytic Activity.

Tuberculosis (TB) remains a critical global health threat. While research has historically prioritized T-cell immunity, antibodies against highly immunogenic candidates like the Mtb antigen Ag85A are increasingly recognized for their protective roles via Fcγ receptor-mediated effector functions. This study characterized the longitudinal dynamics of B-cells and phagocyte populations alongside Mtb-specific antibody features during TB treatment. A cohort of 20 patients with active TB was followed prospectively for 6 months, with peripheral blood samples collected at baseline (T0), at two (T2) and

Evaluation of a multiplex qPCR assay for rapid diagnosis of fungal pneumonia and tuberculosis in people with advanced HIV disease.

Fungal pneumonias and tuberculosis (TB) remain major causes of mortality among people living with advanced HIV disease (PLWHA). Their overlapping clinical manifestations delay diagnosis in developed and resource-limited settings, with greater impact where access to rapid diagnostic tools is limited. This study evaluated the performance of a multiplex quantitative PCR (qPCR) assay for simultaneous detection of Histoplasma capsulatum, Cryptococcus neoformans, Pneumocystis jirovecii, and Mycobacterium tuberculosis complex in respiratory samples from PLWHA. A cross-sectional study was conducted at

Inflammatory burden and oedema provide complementary information on subsequent pericardial tethering in tuberculous pericarditis: a longitudinal segment-level CMR study.

BACKGROUND: The relationship between pericardial inflammation and subsequent constriction in tuberculous (TB) pericarditis remains poorly understood. Late gadolinium enhancement (LGE) has shown conflicting prognostic associations, suggesting that inflammation alone may not adequately reflect disease stage or reversibility. We hypothesised that inflammatory burden and oedema provide complementary information regarding subsequent pericardial tethering, and that segments without oedema would be more likely to demonstrate tethering at follow-up than oedematous segments. METHODS: Participants with

Temporal proximity to counseling is associated with longitudinal bedaquiline and antiretroviral therapy adherence in multidrug-resistant tuberculosis and HIV treatment.

BACKGROUND: Medication adherence is a persistent challenge in the treatment of multidrug-resistant tuberculosis (MDR-TB) and HIV. Routine counseling notes are an underutilized source of information on patient adherence context. METHODS: We applied natural language processing (NLP) to routine counseling notes from a randomized controlled trial of MDR-TB/HIV treatment in South Africa. Topics were derived using topic modeling; notes were semantically matched to topics and clustered using K-means. We fitted linear mixed-effects (LME) regression models with participant-level random intercepts and

Human CD1c-autoreactive T-cells recognise Mycobacterium tuberculosis-infected antigen-presenting cells and display cytotoxic effector programmes.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains the leading cause of death from infection globally yet the contribution of non-classical T-cell pathways to human immunity remains poorly defined. CD1c-autoreactive T-cells, which recognise self-lipids presented by the antigen-presenting molecule CD1c, are frequent in human blood but their role during infection remains unclear. Here, we investigate how CD1c-expressing antigen-presenting cells (APCs) and Mtb infection shape CD1c-autoreactive T-cell responses using engineered human APC systems, complemented by single-cell tra

Exosomal programmed death ligand 1 in patients with Mycobacterium avium complex lung disease.

BACKGROUND: Mycobacterium avium complex lung disease (MAC-LD) is increasing worldwide. Exosomal programmed death-ligand 1 (PD-L1) has been scarcely investigated in MAC-LD. METHODS: Plasma exosomes and their PD-L1 from MAC-LD patients and healthy controls were measured and used for co-culturing Jurkat cells with or without PD-L1/PD-1 blocking antibodies. MAC-infected mice were treated with exosome inhibitors to assess exosomal PD-L1 and bacterial burden. RESULTS: Plasma exosomal PD-L1 levels were significantly higher in MAC-LD patients (1,477±1,243 pg/mL) vs. controls (183.8±113.9 pg/mL; P <

When "Leprosy" Is Not Leprosy by Polymerase Chain Reaction: Detection of Nontuberculous Mycobacterial Deoxyribonucleic Acid in Nasal Swabs from Patients with Chronic Skin Lesions in a Leprosy-endemic Area.

BACKGROUND: Leprosy remains a major cause of chronic dermatoneurological morbidity in endemic settings, and early etiologic confirmation is often challenging in primary care, where diagnosis may remain syndromic. Molecular testing can strengthen confirmation of Mycobacterium leprae , yet performance depends on specimen type and target selection, and leprosy-like phenotypes may overlap with cutaneous disease caused by nontuberculous mycobacteria (NTM). This study evaluated paired nasal versus buccal mucosal swabs from patients with chronic skin lesions and clinical suspicion of leprosy to chara
