Predicting locus of control for health in adults with sickle cell disease via mental health visits.

OBJECTIVE: This study examined the relationship between healthcare-seeking behavior and health locus of control orientations among individuals with sickle cell disease, specifically investigating whether mental health service utilization predicts internal versus external control beliefs while accounting for demographic factors. METHODS: Thirty-two African American adults with sickle cell disease were recruited from a Southeastern Comprehensive Sickle Cell center. Participants completed the Multidimensional Health Locus of Control scale (MHLC), which assesses internal control beliefs and exter

Infertility and family building perspectives among adults with sickle cell disease.

Sickle cell disease (SCD) and its treatments may lead to gonadal dysfunction. Limited research has examined how these effects translate into actual fertility outcomes and family-building perspectives. We aimed to determine the frequency of infertility and to examine family-building goals, knowledge and concerns among 91 adults with SCD from The Ohio State University Wexner Medical Center SCD clinic. Participants completed surveys capturing their demographic and medical information. Fertility status and family-building perspectives were measured using modified versions of surveys used in fertil

Genotype Differences and Hydroxyurea Utilization Among Adults With Moderate to Severe Sickle Cell Disease.

BACKGROUNDS: Hydroxyurea (HU) remains underutilized in adults with sickle cell disease (SCD) despite proven benefits. Current HU guidelines primarily target sickle cell anemia (SCA), overlooking other genotypes. OBJECTIVES: This study examined HU utilization patterns across genotypes among adults considered to have moderate to severe SCD manifestations by the 2014 National Heart, Lung, and Blood Institute (NHLBI) guideline criteria and identified factors associated with early HU use. METHODS: This retrospective cohort study analyzed electronic health records from the University of Pittsburgh

High concordance of physician-attestation with manual data abstraction for sickle cell type: an ASH RC Data Hub study.

Sickle cell disease (SCD) is the most common inherited blood disorder in the United States. The American Society of Hematology Research Collaborative (ASH RC) has built an SCD Data Hub (DH) to facilitate research and quality improvement using real-world data. To our knowledge, we report here the first analyses from the DH, focusing on cohort demographics and accurate identification of SCD diagnosis type. DH sites have data use agreements with ASH RC to extract and transfer electronic health record (EHR) data at least quarterly. Principal investigators (PI) provided additional attestation of SC

Evaluating alloimmunization risk in patients with sickle cell disease and in prenatal care patients with RHCE variants.

BACKGROUND AND OBJECTIVES: Clinically significant alloantibodies complicate transfusion and prenatal care, especially in individuals with genetic variants affecting high-frequency antigens. Many patients from African descent carry RHCE*ceVS alleles, which alter the expression of c (RH4), e (RH5) and hrB (RH31). However, the risk and clinical impact of alloimmunization remain uncertain. We evaluated the alloimmunization in a cohort of patients with a RHCE*ceVS genotype. MATERIALS AND METHODS: We conducted a retrospective study on 48 patients with a RHCE*ceVS genotype divided into three categor

Direct healthcare costs associated with sickle cell disease complications: a retrospective cohort study using routinely collected healthcare data in England.

OBJECTIVES: Due to the multisystemic nature of sickle cell disease (SCD), complications can occur together and thus discerning costs associated with individual complications requires a methodology that can estimate the costs of a given complication while accounting for the presence of other complications. In this study, we aimed to estimate period-based incremental costs associated with specific chronic complications in patients with SCD in England while accounting for multimorbidity. DESIGN/SETTING: All-cause primary and secondary care healthcare resource utilisation (HCRU) was obtained for

Epidemiological and comorbidity burden in transfusion-dependent patients with thalassemia and sickle cell disease in Greece.

TDT and SCD are hereditary hemoglobinopathies that require lifelong red blood cell transfusions. Modern treatment has increased survival, exposing patients to transfusion-related problems such iron excess. Despite this movement toward an elderly patient population, transfusion-dependent adult comorbidity data is scarce, especially in Greece. A retrospective, descriptive observational analysis of 105 adult transfusion-dependent patients (92 with TDT and 13 with SCD) followed at a Greek tertiary hemoglobinopathy center in 2024 was conducted. Organ system-specific chronic comorbidities were ident

Screening for sickle cell retinopathy: A comparison of systematic angiography and ultra-widefield imaging.

PURPOSE: Sickle cell disease is a prevalent genetic hemoglobinopathy affecting approximately 120 million people worldwide. Fluorescein angiography in the seven ETDRS fields is currently the gold standard for detecting proliferative forms of retinopathy, but ultra-widefield imaging (UWF) may represent a non-invasive alternative. We sought to compare the two screening strategies implemented in the Centre-Val de Loire region: systematic 7-field ETDRS fluorescein angiography at the University Hospital of Orléans versus non-angiographic ultra-widefield imaging (Optos©) at the University Hospital of

Priapism as an Initial Presentation of Sickle Cell Disease: A Case Report.

Priapism is a urological emergency defined as an erection lasting more than four hours unrelated to sexual stimulation. Most cases are ischemic and often related to hematologic disorders, especially sickle cell disease (SCD). About 30 to 40 percent of males with SCD experience priapism, usually starting in adolescence. The pathophysiology is linked to nitric oxide depletion, dysregulation of phosphodiesterase type 5, oxidative stress, and genetic factors. Although common, priapism is often overlooked and undertreated, which can lead to erectile dysfunction and psychological problems. A 21-year

A Digital Patient Engagement and Monitoring System to Improve Quality, Safety, and Continuity of Care for Pediatric Sickle Cell Anemia: A Cross-Sectional Study of Caregiver Perceptions.

Introduction The management of chronic conditions like sickle cell anemia (SCA) requires high levels of patient and caregiver engagement to ensure adherence to treatment and prevent complications. Digital health interventions offer a promising avenue to support this engagement. This study evaluates the awareness, perceived usefulness, and readiness of caregivers to adopt a digital patient engagement and monitoring system for pediatric SCA management. Methods A cross-sectional survey was conducted using a structured Arabic-language questionnaire administered to 165 primary caregivers of childre

Diagnostic Limitations of Hemoglobin A1c in the Setting of Compound Hemoglobinopathy: A Case Report of Sickle Cell Disease, Alpha Thalassemia, and Occult Diabetes.

This case report illustrates the diagnostic limitations of hemoglobin A1c (HbA1c) in patients with coexisting hemoglobinopathies. Sickle cell disease (SCD) and alpha thalassemia are inherited disorders characterized by chronic anemia and altered red blood cell turnover, both of which can significantly interfere with the accuracy of HbA1c measurements. In such cases, HbA1c may appear falsely low, potentially delaying the diagnosis and treatment of diabetes mellitus. We describe a 63-year-old female with previously undiagnosed SCD and alpha thalassemia whose longstanding hyperglycemia and diabet

The emerging role of complement inhibitory approaches in the treatment of acute sickle cell disease complications.

INTRODUCTION: Sickle cell disease (SCD) is a common and life-threatening hematological disorder that affects almost 100,000 people in the United States and millions worldwide. The wide phenotypic variability of SCD results from a complex pathophysiology involving coagulation, inflammation, leukocyte and endothelial cell activation, and other cellular and humoral mechanisms. To improve the prognosis of SCD patients, we must gain a better understanding of the underlying molecular pathogenesis and offer new therapies. AREAS COVERED: In SCD and other hemolytic disorders, continual red blood cell

Exploring the temporal relationship between psychosocial risk factors and recurrent ischemic priapism.

BACKGROUND: The development of recurrent ischemic priapism (RIP) is associated with mental health disorders, psychiatric medications, and substance use, but the temporal relationship between the onset of RIP and the development of these risk factors is poorly understood. AIM: This study aimed to investigate the chronology of psychosocial and substance use risk factors relative to the initial clinical presentation of RIP. METHODS: We conducted a retrospective cohort study of 73 adult patients diagnosed with RIP at a single academic center, identified via manual chart review of the electronic

Newborn Screening for Hemoglobinopathies and Thalassemias: Brief History, Recent Activities, and Global Status-2026.

Newborn bloodspot screening (NBS) began in Guthrie's laboratory in 1961 for phenylketonuria. A federal study the following year formed the basis for expanding NBS as a public health function. Diseases detectable through NBS gradually expanded, eventually including sickle cell anemia, which was included in the screening panel in New York in 1975. Universal inclusion of full population screening for sickle cell anemia was included in all US NBS programs by 2006. Through the years, NBS for sickle cell anemia has expanded to include other clinically significant hemoglobin disorders (both hemoglobi

Newborn screening for sickle cell disease in Angola: Implementation challenges and emerging data on hemoglobinopathy prevalence.

Sickle Cell Disease (SCD) is an autosomal recessive disorder with a substantial global burden. Despite its particularly high incidence in sub-Saharan Africa, early diagnosis remains limited in many countries. The objective of this study was to implement a newborn screening programme for SCD in one of the largest maternity hospitals in Angola and to support the subsequent pediatric follow-up of the affected children. Between June 2023 and December 2024, all children born in or attending the main hospital for vaccination were screened after parental or guardian consent. Blood was collected by he

Non-aneurysmal Subarachnoid Hemorrhage in an Adult With Sickle Cell Anemia: A Case Report and Review of the Literature.

Moyamoya disease (MMD) is a rare, idiopathic cerebrovascular disorder characterized by bilateral stenosis of the internal carotid arteries (ICAs) and the formation of collateral vessels at the base of the brain. In patients with sickle cell disease (SCD), chronic vaso-occlusive changes can produce moyamoya-like vasculopathy, resulting in moyamoya syndrome (MMS) and increasing the risk of aneurysm formation and rupture. Nonetheless, presentation of MMS as non-aneurysmal subarachnoid hemorrhage (NASAH) is extremely rare, with few reported cases. Our case is about a 28-year-old male, a known case

Genetic Contribution to Asthma Informs Acute Chest Syndrome Pathophysiology and Risk Stratification.

Acute chest syndrome (ACS) is a severe complication of sickle cell disease (SCD) occurring in ~50% of patients, some presenting frequent episodes. We lack tools to identify patients at high risk of ACS occurrence or frequent episodes. Epidemiological studies have found an association between asthma and ACS, but whether this link is causal is unclear. We used polygenic scores (PGS) to analyze whether the genetic propensity for asthma was associated with ACS and could be used to stratify the risk of ACS. We identified that PGS for asthma (PGSasthma) was associated with ACS rate (number of episod
