Activation of Platelet Caspase-1 and P-Selectin Over-Expression in Sickle Cell Disease Patients: An Involvement of NLRP3 Inflammasome?

Sickle cell disease (SCD) is characterized by hemolytic anaemia, painful vaso-occlusive crises, and chronic inflammation resulting partly from platelet activation. It has been shown that platelets express the nucleotide-binding domain leucine-rich repeat containing protein 3 (NLRP3) and the apoptosis-associated speck-like protein containing a CARD (ASC), forming the NLRP3 inflammasome, which controls activation of caspase-1 and cleavage of interleukin-1β (IL-1β). Bruton tyrosine kinase (BTK), known to activate the NLRP3 inflammasome, is also expressed in platelets. In SCD, platelets are activa

Hepatitis A-associated oxidative haemolysis in a patient with glucose-6-phosphate dehydrogenase deficiency with co-inherited sickle cell trait.

An adult male patient in his late teens presented with pruritus, nausea and progressive jaundice after travel to West Africa. He did not get a pre-travel hepatitis A vaccination or malaria chemoprophylaxis. Laboratory testing demonstrated hepatocellular injury with alanine aminotransferase (ALT) 4002 U/L (reference <60), marked hyperbilirubinaemia (total bilirubin 632 µmol/L, reference <20) and a mildly prolonged international normalised ratio (INR). Concurrently, he developed non-immune haemolysis with undetectable haptoglobin, elevated lactate dehydrogenase and reticulocytosis, as well as bi

Local ancestry-aware genome-wide meta-analysis uncovers novel genetic loci for sickle cell disease nephropathy.

In the United States, sickle cell disease (SCD) is a rare inherited hemoglobinopathy affecting about 100,000 individuals, mostly with African ancestry. SCD causes damage to multiple organ systems and SCD nephropathy (SCDN) is a common complication associated with early mortality. We previously performed a genome-wide association study (GWAS) for SCDN and identified a modest number of genome-wide significant loci. Here, we leveraged the ancestral composition of participants from two well-characterized adult SCD cohorts to boost statistical power and perform a local ancestry-aware GWAS for estim

Unraveling the Cause of Hemolytic Anemia in the Emergency Department: A Destructive Path.

Hemolytic anemia is a rare but potentially life-threatening condition in the emergency department (ED) that requires timely recognition and early intervention. Hemolytic anemia results from either the intravascular or extravascular breakdown of red blood cells. The most common causes of secondary hemolytic anemia include lymphoproliferative disorders, autoimmune disorders, and infections. This case will demonstrate how an urgent diagnosis of hemolytic anemia can lead to the underlying cause: diffuse large B-cell lymphoma. This case illustrates how laboratory evaluation, early subspecialty invo

Integrated enzymatic and molecular diagnosis of G6PD deficiency: a one-year experience identifying five novel variants.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency, caused by pathogenic variants in the G6PD gene, is the most common X-linked enzymopathy in the world. The clinical manifestations range from drug-induced hemolytic anemia to neonatal hyperbilirubinemia and chronic hemolysis. The G6PD status needs to be accurately assessed in several clinical settings, including drug prescription, assisted reproductive technologies, and the diagnostic workflow of hemolytic anemia. In this study, the contribution of molecular analysis in the diagnostic workflow of patients with suspected G6PD deficiency was re

Biallelic loss-of-function PTPN6 variants lead to inflammatory lung disease and hemolytic anemia.

Severe inflammatory lung disease, including acute respiratory distress syndrome (ARDS), is life-threatening with diverse causes and few targeted therapies. Inborn errors of immunity (IEIs) underlie a subset of cases. We describe a novel IEI caused by biallelic loss-of-function variants in PTPN6, encoding the immunoregulatory phosphatase SHP1. Seven children from five families developed early-onset anemia and severe inflammatory lung disease, at times presenting as ARDS. The causative variants were shown to destabilize SHP1 and abolish its phosphatase activity. These findings expand the genetic

Leucine, autoimmune diseases, and circulating mediators shape the causal landscape of lymphoid malignancies.

INTRODUCTION: While genetic abnormalities underlying lymphoid malignancies (LM) are well-characterized, the environmental and metabolic determinants driving LM remain largely unexplored. Autoimmune diseases are known risks, yet the specific circulating mediators and potential protective dietary factors linking these conditions to LM require systematic elucidation. OBJECTIVES: This study aimed to comprehensively screen for causal risk factors of LM, identify the metabolic mediators bridging autoimmune diseases and LM, and validate the therapeutic potential of leucine as a protective factor. Review of the relationship between deficiency of Glucose-6-phosphate dehydrogenase deficiency and diabetes and its complications.

Diabetes is a chronic condition characterised by hyperglycaemia due to impaired insulin secretion, insulin action, or both. Oxidative stress reduces insulin sensitivity and contributes to complications. Glucose-6-phosphate dehydrogenase (G6PD) deficiency, the most common enzymatic disorder of erythrocytes, impairs NADPH production and reduces the ability to counter oxidative stress. Given the involvement of redox imbalance in diabetes pathogenesis, the role of G6PD deficiency in diabetes development and progression remains unclear. Two reviewers independently searched PubMed, Cochrane Library,

Clinical outcomes among patients treated with Lu-PSMA-617 and metastasis-directed EBRT for metastatic prostate cancer.

Metastasis-directed external beam radiation therapy (EBRT) delivered for limited-volume disease, integrated with systemic radioligand therapy, may improve long-term disease control across the spectrum of metastatic prostate cancer. We examined 88 patients with metastatic prostate cancer who received both lutetium-177 PSMA-617 (LuPSMA) radiopharmaceutical therapy and metastasis-directed external beam radiation therapy (EBRT) between June 2022 and August 2025. EBRT was delivered to a median of three metastatic sites, most commonly bone (69.3 %) and lymph nodes (64.8 %). The PSA50 response rate t

This article critically discusses whether accumulation of lactic acid, or in reality lactate and/or hydrogen (H+) ions, is a major cause of skeletal muscle fatigue, i.e. decline of muscle force or power output leading to impaired exercise performance. There exists a long history of studies on the effects of increased lactate/H+ concentrations in muscle or plasma on contractile performance of skeletal muscle. Evidence suggesting that lactate/H+ is a culprit has been based on correlation-type studies, which reveal close temporal relationships between intramuscular lactate or H+ accumulation and

A Preliminary Investigation on the Utility of Embedding Fluency-Building Strategies within the TAGteach Procedure.

UNLABELLED: TAGteach is a trademarked intervention package designed to promote skill acquisition by combining the use of an audible stimulus as immediate feedback with several other behavior analytic procedures. Although TAGteach effectively improves the accuracy of sports performance, previous researchers have noted that TAGteach does not reliably promote synchronized, coordinated (i.e., fluent) sport movements. To date, no study has evaluated the efficacy of embedding fluency-building strategies within the standard TAGteach procedure. We used a concurrent multiple baseline across skills desi

Screen-Detected Coeliac Disease in a Nationally Representative Cohort: Health Indicators, Long-Term Outcomes and Mortality.

BACKGROUND: Population-based screening for coeliac disease could reduce underdiagnosis, but benefits and harms remain debated. AIM: We studied health outcomes in screen-detected coeliac disease patients at diagnosis and following a long-term gluten-free diet. METHODS: Data were collected from Health 2000 and 2011 studies representing the Finnish population. Coeliac disease was screened in 6339 individuals by transglutaminase and endomysial antibodies, with dual seropositivity as the diagnostic criterion, histologically validated in this cohort. Survey-weighted analyses enabled population-lev

MR Optimum: A web-based open-source tool for standardized signal-to-noise ratio evaluation in MRI.

Signal-to-noise ratio (SNR) is a key performance metric in magnetic resonance imaging (MRI) to evaluate pulse sequences, receive coils, and image reconstruction algorithms. A variety of methods have been proposed to estimate SNR. However, the lack of consistent and broadly available open-source implementations has been a challenge for reliable SNR comparisons in clinical and research settings. To address this gap, this work introduces MR Optimum, a cloud-native, open-source platform for standardized SNR analysis. MR Optimum integrates established SNR estimation techniques within a flexible, mo

Temporary Glucose - 6 phosphate dehydrogenase enzyme deficiency, an additional risk factor for invasive fungal infection in immune-compromised AML patients.

Invasive fungal infections (IFI) represent a major threat to acute myeloid leukemia (AML) patients, especially after induction chemotherapy. Intense chemotherapeutic drugs further weaken the immune function of AML patients which increases their susceptibility to life-threatening fungal infection, including invasive aspergillosis (IA). At present, there is no single reliable serological marker or method to predict accurately the risk of IA after induction chemotherapy. We hypothesized that some anti-cancer medicines and antibiotics may trigger temporary Glucose-6-Phosphate Dehydrogenase (G6PD)

Seroprevalence of Celiac Disease in Mexican Mestizo Patients With Autoimmune Thyroid Disease.

GOALS: This study aimed to assess the seroprevalence of celiac disease in Mexican patients with autoimmune thyroid diseases, hypothesizing that prevalence would align with rates observed in other populations. BACKGROUND: The association between celiac disease and autoimmune thyroid diseases has been documented globally, with varying seroprevalence rates. Historically, Mexican Mestizos were considered at low risk for celiac disease, yet recent findings suggest similar prevalence to other regions. However, data regarding Hispanic patients with autoimmune thyroid diseases are limited. STUDY: Th

Elraglusib, a Glycogen Synthase Kinase 3β Inhibitor, plus Chemotherapy with or without Immunotherapy in Patients with Recurrent, Metastatic Salivary Gland Carcinoma.

PURPOSE: Glycogen synthase kinase 3β (GSK-3β) is a known therapeutic target in cancer. Aberrant nuclear GSK-3β (nGSK-3β) expression has been shown in some salivary gland cancers. Elraglusib is a small-molecule GSK-3β inhibitor with immunomodulatory potential. We hypothesized that elraglusib plus platinum-based chemotherapy and immunotherapy priming would be a novel treatment approach for salivary gland cancer. A two-factor structure for cannabis use disorder identification test and its associations with demographic factors and cannabis use motives.

Glioblastoma (GBM) is associated with poor prognosis due to its resistance to surgery, irradiation, and conventional chemotherapy. Thus, efficient therapeutic approaches for the treatment of GBM are urgently needed. HSP70 is an antiapoptotic protein that participates in the inhibition of both mitochondrial and membrane receptor apoptosis pathways and is highly expressed in glioma tissues. Here, we investigated a derivative of apoptin; specifically, a chicken anemia viral protein with selective toxicity toward cancer cells that can inhibit hyperactive molecules, including HSP70. p27Kip1 regulates γ-globin production.

Sickle cell disease (SCD) and β-thalassemia are devastating genetic disorders resulting from defects in the β-globin subunit of adult hemoglobin. Both disorders are ameliorated by the induction of γ-globin, a component of fetal hemoglobin (HbF). Therefore, the development of safe, effective, and widely available inducers of HbF is needed. Here, we discovered that slow cycling erythroid cells exhibit increased γ-globin expression. To understand the molecular basis of this, we screened all cyclin-dependent kinase inhibitors (CDKIs) for their ability to induce HbF using CRISPR activation. We foun
