This review summarizes recent advances in ligand trap therapies targeting activin type II receptors [ActRIIA/ACVR2A and ActRIIB/ACVR2B], which serve as shared receptors for members of the TGF-β family, including activins, GDF11, and myostatin [MSTN]. These receptors mediate Smad2/3 signaling and play critical roles in hematopoiesis, vascular homeostasis, and muscle regulation. Two peptide-based ligand traps have recently received clinical approval: luspatercept [ActRIIB-Fc], an erythroid maturation agent, and sotatercept [ActRIIA-Fc], a novel therapeutic agent for pulmonary arterial hypertensi

Aplastic anemia is a bone marrow failure disorder marked by cytopenias that impair oxygen delivery, immune defense, and hemostasis. Standard therapy traditionally combines immunosuppression with or without hematopoietic stem cell transplantation, and more recently incorporates thrombopoietin receptor agonists to stimulate residual hematopoiesis. Eltrombopag improved outcomes when added to immunosuppressive therapy, but its association with hepatotoxicity limits its suitability for patients with underlying liver disease or elevated baseline liver enzymes. Avatrombopag is a newer oral thrombopoi

Vacuoles in hematopoietic cell precursors have garnered significant attention in recent years due to their association with a newly characterized clonal hematopoiesis with acquired mutations of ubiquitin like modifier activating enzyme 1 (ubiquitin-activating enzyme E1, UBA1) gene associated with clinical systemic autoinflammatory manifestations, known as VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome. The cytomorphologic hallmark of this rare disorder is vacuolization involving the granulocytic and erythroid precursors. While affected vacuolated cells are seen in mo

Diamond-Blackfan anemia syndrome (DBAS) is marked by defective erythropoiesis caused by impaired ribosome biogenesis and aberrant signaling. Here, we investigate how ribosomal stress-induced activation of the NLRP1 inflammasome affects erythroid differentiation in DBAS. We demonstrate that FDA/EMA-approved tyrosine kinase inhibitors (TKIs) effectively mitigate defective erythropoiesis by inhibiting NLRP1 inflammasome activation. In K562 cells, nilotinib suppresses the ZAKα/P38/NLRP1/CASP1 axis, leading to increased GATA1 levels and upregulation of key erythroid genes. These effects were valida

PURPOSE: 225Ac-Targeted alpha therapy is a potent and promising option for patients with metastatic castration-resistant prostate cancer (mCRPC) and failure of guideline-based therapies and 177Lu-PSMA-radioligand therapy. Unfortunately, side effects associated with TAT can significantly affect quality of life. A combination treatment regimen adding 177Lu and reducing 225Ac activities may mitigate side effects while preserving sufficient anti-tumour efficacy. We therefore evaluated different combinations [225Ac]Ac-/[177Lu]Lu-PSMA-I&T (ALCT) with regard to response and adverse events.

BACKGROUND Anti-neutrophil cytoplasmic antibodies (ANCAs) are associated with a group of small-vessel vasculitides collectively known as ANCA-associated vasculitis (AAV), which can lead to rapidly progressive glomerulonephritis and multisystem involvement. In addition to other autoimmune diseases, AAV has been increasingly reported following SARS-CoV-2 infection. CASE REPORT We present the case of a 70-year-old woman with a past medical history of atrial fibrillation, sick sinus syndrome, breast cancer, gastroesophageal reflux disease, and anxiety, who developed a rare phenomenon of dual ANCA-

The clinical challenges and safety concerns associated with the use of erythropoiesis-stimulating agents (ESAs) have provided the rationale for developing novel therapeutic approaches that address the complex pathophysiology of anemia in chronic kidney disease (CKD). Hypoxia-inducible factor-prolyl hydroxylase inhibitors (HIF-PHIs) are a new class of oral agents that effectively increase and maintain hemoglobin levels in patients with CKD. These agents stimulate the endogenous production of erythropoietin and enhance iron metabolism by activating hypoxia-inducible factors. Despite their effica

INTRODUCTION: Biallelic variants in Fanconi Anemia-associated Nuclease 1 (FAN1) cause karyomegalic tubulointerstitial nephropathy (KIN), a condition poorly characterized in terms of kidney survival, patient survival, and clinical characteristics. Therefore, we undertook a cross-sectional collaborative study to better characterize KIN-FAN1.
METHODS: To gather data, we distributed a REDCap survey on clinical characteristics and genetic variants of KIN-FAN1 to colleagues and case report authors.
RESULTS: Based on the survey, we identified 86 families affected (122 individuals) from 22 countries

Ravulizumab is a second-generation C5i engineered from eculizumab to achieve immediate, complete, and sustained inhibition of terminal complement activity in PNH. The REACTION observational cohort study describes the effectiveness and tolerability of ravulizumab in Italian patients who were previously treated with eculizumab. Eighty-one PNH patients were enrolled in this study. The primary endpoint was the percentage change in lactate dehydrogenase (LDH) from baseline to the end of observation (52 weeks follow-up). Among secondary endpoints, transfusion avoidance, breakthrough hemolysis (BTH)

BACKGROUND: Iron-refractory iron deficiency anemia (IRIDA) is a rare hereditary disorder caused by pathogenic variants in TMPRSS6, characterized by microcytic anemia, low circulating iron levels, and inappropriately high hepcidin levels. Although IRIDA is typically an autosomal recessive disorder, some individuals with a monoallelic pathogenic exonic TMPRSS6 variant exhibit the phenotype, suggesting additional contributing factors. The mechanisms underlying monoallelic IRIDA remain unclear, complicating diagnosis. This study aimed to investigate the potential role of non-coding TMPRSS6 variant

Five rare variants in BRIP1/FANCJ, initially identified in ovarian cancer (OC) or breast cancer (BC) cases by the adult hereditary cancer clinics, were investigated for their candidacy as clinically relevant variants. These variants were investigated genetically in a population exhibiting genetic drift and molecularly assayed for biological impact. Using in silico tools, population-based genetic databases and other resources, three of the five reported BRIP1 variants were likely to be damaging: c.797C>T; p.Thr266Met, c.2087C>T; p.Pro696Leu and c.2990_2993delCAAA; p.Thr997ArgfsTer61. Human parvovirus B19 (B19V) infection is a significant but underrecognized complication, commonly linked not only to aplastic anemia but also pancytopenia, especially in hematopoietic stem cell transplantation (HSCT) recipients. While B19V's tropism for erythroid progenitor cells (EPCs) is well-documented, its restriction to EPCs fails to fully explain the pathogenesis of pancytopenia. In this study, we used PrimeFlow RNA assay, and single-cell full-length transcriptome sequencing (scFAST-seq) to show that B19V could infect hematopoietic stem cells (HSCs) and initiate viral transcription, resu

OBJECTIVE: Testosterone replacement therapy (TRT) in prostate cancer survivors with hypogonadism remains controversial due to concerns that restoring testosterone may increase the risk of disease recurrence. We performed a cohort study of men with localized prostate cancer treated with radiotherapy with or without androgen deprivation therapy (ADT), who received TRT, and a narrative review of published studies evaluating TRT after radiotherapy.
METHODS: Biochemical and clinical recurrence, patient-reported symptoms, prostate-specific antigen, testosterone, and hemoglobin were analyzed in this

Glucose-6-phosphate dehydrogenase (G6PD) deficiency, the most common erythrocyte enzymatic disorder, predisposes patients to oxidative stress-induced acute hemolysis. Affected patients are at risk of developing hemolysis in the setting of triggers such as certain drugs, infections, or foods (fava beans). Although some drugs like antimalarials (primaquine, tafenoquine), sulfonamides, and dapsone are well-known triggers of hemolysis, reports of this adverse effect are rare with standard bismuth-based quadruple therapy for Helicobacter pylori. In this report, we present a 65-year-old male who dev

Sickle cell disease (SCD) is a hereditary disorder characterized by HBB variants, leading to chronic hemolytic anemia and vaso-occlusion. Hepatobiliary complications, including cholelithiasis, are common but underreported. This study investigated the rates and risk factors for cholelithiasis, cholecystitis, and cholecystectomy in a large Brazilian SCD cohort. Data from 2,778 individuals across six referral centers in the REDS-III Brazilian SCD cohort were analyzed. Clinical, laboratory, and genetic data were obtained retrospectively at enrollment and prospectively during follow-up. Gallbladder

BACKGROUND: Neoadjuvant treatment comprising six to nine cycles of anti-HER2 based chemotherapy yields high pathological complete response rates and excellent survival outcomes in patients with stage II-III HER2-positive breast cancer but comes with noteworthy side-effects. The TRAIN-3 study investigated if patients treated with this regimen who have a rapid complete radiological response on MRI are candidates for early surgery. The study showed that one-third of patients with hormone receptor-negative tumours and one-sixth of patients with hormone receptor-positive tumours had a pathological

Fibroblast activation protein (FAP) is highly expressed in many cancers, especially sarcomas, and represents a promising theranostic target. We present an updated retrospective analysis of 90Y-FAP inhibitor (FAPI)-46 treatment in patients with sarcoma or other solid tumors. Methods: We performed monocentric analysis of patients with progressive sarcoma or metastatic cancer who were eligible for 90Y-FAPI-46 therapy after approved treatments had been exhausted and who showed high FAP expression (SUVmax, ≥10 in over 50% of lesions on 68Ga-FAPI-46 PET). After therapy, 90Y-FAPI-46 scintigraphy conf

Polyprenal reductase is an enzyme encoded by the SRD5A3 gene, which is involved in the synthesis of dolichol from polyprenol. Dolichol serves as a carrier for glycan precursors or monosaccharides in N-linked glycosylation. Pathogenic variants in SRD5A3 can result in a congenital disorder of glycosylation (CDG), SRD5A3-CDG, which is inherited in an autosomal recessive manner. Most plasma proteins are glycosylated and changes in the glycosylation of several glycoproteins are associated with pathological consequences. Despite the critical role of SRD5A3 in glycosylation, the impact of its deficie

OBJECTIVE: To compare the efficacy and safety of extended interval (Q3-4W) enzyme replacement therapy (ERT) versus standard biweekly (Q2W) ERT in clinically stable type 1 Gaucher disease (GD) patients.
METHODS: We emulated a target trial with a sequential trial design, using data from the French Gaucher Disease Registry. Eligible patients were treated for ≥2 years biweekly without clinical events. Every 3 months, switchers to Q3-4W were matched to Q2W patients by age, sex, referral center follow-up, disease history (bone events, anemia, thrombocytopenia, splenectomy, and hepatosplenomegaly),
