Targeting homologous recombination deficiency with intensified chemotherapy versus standard chemotherapy followed by olaparib in stage III breast cancer (SUBITO): an open-label, randomised, controlled, phase 3 trial.

BACKGROUND: Patients with stage III, human epidermal-growth-factor-receptor 2 (HER2; also known as ERBB2)-negative breast cancer with homologous recombination deficiency (HRD) had a 4-year overall survival of 35% after anthracycline-based chemotherapy versus 78% after intensified alkylating chemotherapy with autologous stem cell rescue (IACT) in a post-hoc analysis of an earlier randomised controlled trial. In this study, we aimed to prospectively assess 4-year overall survival with IACT and establish whether this approach remains superior to a contemporary HRD-targeting regimen in patients wi

Phase I/II Study of Sonrotoclax (BGB-11417) Monotherapy in Patients With Mantle Cell Lymphoma Previously Treated With Anti-CD20 Therapy and a Bruton Tyrosine Kinase Inhibitor.

PURPOSE: Mantle cell lymphoma (MCL) is a rare and typically aggressive B-cell non-Hodgkin lymphoma characterized by recurrent relapse after short remissions. Sonrotoclax (BGB-11417) is a next-generation B-cell lymphoma 2 inhibitor with greater selectivity and potency than venetoclax, a shorter half-life, and no drug accumulation. Sonrotoclax monotherapy was evaluated in Bruton tyrosine kinase inhibitor-pretreated patients with relapsed/refractory (R/R) MCL. METHODS: BGB-11417-201 (ClinicalTrials.gov identifier: NCT05471843) is an ongoing global, open-label, phase I/II trial. Sonrotoclax was o

Pearson syndrome: expanding the clinical spectrum of a mitochondrial cytopathy-a case report.

BACKGROUND: Pearson syndrome (PS) is a rare multisystem mitochondrial disorder characterized by single large-scale mitochondrial DNA deletions (SLSMDs). It typically presents in infancy with refractory sideroblastic anemia, exocrine pancreatic insufficiency, and failure to thrive. Due to its heterogeneous manifestations and resemblance to other hematological conditions, early diagnosis remains a clinical challenge. CASE PRESENTATION: A south asian male infant presented with persistent pancytopenia, severe anemia unresponsive to intravenous and oral iron and multivitamin supplements, exocrine

Phase 2 dose-expansion trial of OBI-3424, a DNA-alkylating prodrug, in patients with advanced solid tumors expressing AKR1C3.

BACKGROUND: OBI-3424 is an investigational small-molecule prodrug. In a dose-escalation trial, the OBI-3424 recommended phase 2 dose (RP2D) was 12 mg/m2 administered every 21 days. In this phase 2 dose-expansion trial, we evaluated the safety and efficacy of OBI-3424 in pancreatic adenocarcinoma and other solid tumor types ("basket" cohort). METHODS: Patients with advanced solid tumors and an AKR1C3 IHC H-score of ≥ 100 were treated at the RP2D of OBI-3424. Tumor response, progression-free survival (PFS), overall survival (OS), and treatment-emergent adverse events (TEAEs) were assessed (www.

Vacuoles, E1 Enzyme, X-linked, Autoinflammatory, Somatic (VEXAS) Syndrome Mimicking Antineutrophil Cytoplasmic Antibody (ANCA)-Associated Vasculitis: A Delayed Diagnosis in a Patient With Multisystem Inflammation and Neutrophilic Dermatosis.

Vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic (VEXAS) syndrome is a recently described adult-onset autoinflammatory disorder caused by somatic mutations in the UBA1 gene, characterized by systemic inflammation and hematologic abnormalities. Due to its broad and overlapping clinical manifestations, VEXAS is frequently misdiagnosed as other rheumatologic or hematologic conditions, leading to delays in diagnosis. We present the case of a 67-year-old man initially diagnosed with antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis who developed progressive multisystem inflam

VEXAS syndrome unmasked from relapsing polychondritis and infection mimicry: a case-based review.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a UBA1-driven hemato-inflammatory disorder that mimics relapsing polychondritis, vasculitis, and myelodysplastic syndromes, often causing diagnostic delay. We describe a case presenting with relapsing polychondritis-like features and summarize, through a focused literature review, its diagnostic pathway and therapeutic implications. A 74-year-old man presented with three weeks of fever, auricular and nasal chondritis, iritis, and pulmonary infiltrates. Inflammatory markers were elevated with macrocytic anaemia. Antimi

Systemic therapy, gastrectomy, cytoreductive surgery, and hyperthermic intraperitoneal chemotherapy versus systemic therapy alone for gastric cancer with limited peritoneal metastases (PERISCOPE II): final results of a multicentre, randomised, controlled, phase 3 trial after an unplanned commissioned interim analysis.

BACKGROUND: Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (HIPEC) is widely used for patients with gastric cancer and peritoneal metastases. However, the efficacy of this treatment has never been compared with that of systemic therapy. We aimed to investigate the efficacy of gastrectomy with cytoreductive surgery and HIPEC for patients with gastric cancer and limited peritoneal metastases, compared with systemic therapy alone. METHODS: In this European, multicentre, randomised, controlled, phase 3 trial (PERISCOPE II) in eight European tertiary referral hospitals, we re

Progressive macrocytic anaemia over 14 years leading to the diagnosis of VEXAS syndrome in a patient initially diagnosed with adult-onset Still's disease.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a recently recognised autoinflammatory disease caused by somatic mutations in UBA1. Its clinical features include recurrent fever, systemic inflammation affecting multiple organs, and progressive haematologic abnormalities such as macrocytic anaemia. Because these manifestations overlap with other autoinflammatory diseases, including adult-onset Still's disease, the diagnosis can be challenging. In this case report, we describe a patient initially diagnosed with adult-onset Still's disease who was ultimately found to

When Lupus Strikes Twice: Pancreatitis and Nephritis in an Overlap Syndrome - A Rare Multisystem Flare.

In patients with systemic lupus erythematosus (SLE), an unusual yet potentially serious complication is acute pancreatitis. When patients have other underlying disorders such as Sjögren's syndrome and dermatomyositis (collectively referred to as "overlap syndromes"), the signs and symptoms of acute pancreatitis can vary from patient to patient. Therefore, when evaluating a patient for acute pancreatitis in the context of one or more overlap syndromes, it may be difficult to get a definitive diagnosis due to similar clinical presentations and hence, determining when to initiate treatment may no

Serum meprin α levels for the detection of systemic inflammatory response syndrome.

BACKGROUND: Systemic inflammatory response syndrome (SIRS) is a frequent critical condition in clinical patients marked by dysregulated immune activation and high mortality. Early initiation of appropriate interventions are important for patient outcome, but molecular markers for diagnosis are not SIRS-specific. METHODS: We performed hematological analyses and health-status assessments on a transgenic disease mouse model that recapitulates elevated epidermal levels of the metalloprotease meprin α (K5Mα) reported in inflammatory skin diseases. In a cohort of intensive care patients that either

Fever of unknown origin as the primary manifestation of extrapulmonary sarcoidosis with bone marrow involvement: a rare clinical scenario.

Isolated bone marrow involvement in sarcoidosis is a rare condition, mostly manifested by cytopenia, hypercalcemia and/or hypercalciuria. We present the case of a 48-year-old man with a prolonged fever meeting the criteria for fever of unknown origin, in whom a suspicion of sarcoidosis only emerged after non-necrotizing granulomas were identified in the bone marrow, one year after the onset of the disease. The patient exhibited only mild anemia, moderate leukocytosis, thrombocytosis, and increased erythrocyte sedimentation rate, C-reactive protein levels. Other potential causes of fever of unk

Excess folate elevates formaldehyde genotoxicity in cell lines but not in mice or humans.

Folate metabolites are chemically unstable: spontaneous decomposition releases formaldehyde, a genotoxin in blood stem cells and a human carcinogen. Despite this, folic acid consumption frequently exceeds the Recommended Dietary Allowance and is prescribed at high doses for patients with blood disorders. However, the impact of excess folate on endogenous formaldehyde genotoxicity in vivo has not been studied. We find that excess tetrahydrofolate (THF) treatment of cell lines elevates formaldehyde-DNA adducts and genotoxicity. To test this in vivo, we fed a high-folic acid diet (10-fold above s

Non-genotoxic transplantation and in vivo selection through epitope editing.

The short-term and long-term effects of genotoxic pre-transplant conditioning remain barriers to the broader application of haematopoietic stem/progenitor cell (HSPC) transplantation and gene therapies1-4. Although monoclonal antibodies targeting KIT have been proposed as alternatives to chemotherapy or radiotherapy5-7, their pharmacokinetics hinder clinical applications owing to the risk of depleting transplanted HSPCs. Here, to address this issue, we identified amino acid changes in the extracellular domain of KIT that disrupt the binding of two therapeutic monoclonal antibodies8,9, which im

Red lines and green lights: Gene therapy for inherited erythroid disorders beyond the haemoglobinopathies.

Gene therapy is revolutionizing treatment paradigms for inherited haematological and immunological conditions. Recent successes, including the United States Food and Drug Administration (FDA) approval of gene therapy products for sickle cell disease and beta-thalassaemia, highlight the translational path of gene therapies for erythroid-specific disorders. In contrast, gene therapy development for other inherited erythroid disorders remains largely preclinical. Here, we examine the emerging landscape of gene therapies for inherited non-haemoglobinopathy erythroid disorders, focusing on the stat

Diagnostic Challenge of Pediatric Gaucher Disease in a Low-Resource South Asian Setting: A Case Report.

Gaucher disease is a rare autosomal recessive lysosomal storage disorder that is caused by a deficiency of the enzyme "β-glucocerebrosidase", leading to the accumulation of glucocerebroside within macrophages. It commonly presents with hepatosplenomegaly, cytopenias, and bone marrow infiltration. Gaucher disease is a pan-ethnic lysosomal storage disorder reported worldwide. Although few cases have been described in Pakistan and other South Asian countries, limited awareness and restricted access to diagnostic testing may contribute to underdiagnosis and delayed recognition in this region. We r

A Case of Refractory Thrombocytopenia in Pregnancy.

Immune thrombocytopenia is an acquired autoimmune disorder characterized by isolated thrombocytopenia due to antibody-mediated platelet destruction and impaired platelet production. Thrombocytopenia in pregnancy is common, but severe thrombocytopenia requires prompt evaluation to distinguish benign gestational thrombocytopenia from immune thrombocytopenia, hypertensive disorders of pregnancy, hemolysis, elevated liver enzymes, low platelet count syndrome, and thrombotic microangiopathies. We present the case of a 19-year-old gravida 1 para 0 patient with sickle cell trait and iron deficiency a
