Hepatitis A-associated oxidative haemolysis in a patient with glucose-6-phosphate dehydrogenase deficiency with co-inherited sickle cell trait.

An adult male patient in his late teens presented with pruritus, nausea and progressive jaundice after travel to West Africa. He did not get a pre-travel hepatitis A vaccination or malaria chemoprophylaxis. Laboratory testing demonstrated hepatocellular injury with alanine aminotransferase (ALT) 4002 U/L (reference <60), marked hyperbilirubinaemia (total bilirubin 632 µmol/L, reference <20) and a mildly prolonged international normalised ratio (INR). Concurrently, he developed non-immune haemolysis with undetectable haptoglobin, elevated lactate dehydrogenase and reticulocytosis, as well as bi

Unraveling the Cause of Hemolytic Anemia in the Emergency Department: A Destructive Path.

Hemolytic anemia is a rare but potentially life-threatening condition in the emergency department (ED) that requires timely recognition and early intervention. Hemolytic anemia results from either the intravascular or extravascular breakdown of red blood cells. The most common causes of secondary hemolytic anemia include lymphoproliferative disorders, autoimmune disorders, and infections. This case will demonstrate how an urgent diagnosis of hemolytic anemia can lead to the underlying cause: diffuse large B-cell lymphoma. This case illustrates how laboratory evaluation, early subspecialty invo

Integrated enzymatic and molecular diagnosis of G6PD deficiency: a one-year experience identifying five novel variants.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency, caused by pathogenic variants in the G6PD gene, is the most common X-linked enzymopathy in the world. The clinical manifestations range from drug-induced hemolytic anemia to neonatal hyperbilirubinemia and chronic hemolysis. The G6PD status needs to be accurately assessed in several clinical settings, including drug prescription, assisted reproductive technologies, and the diagnostic workflow of hemolytic anemia. In this study, the contribution of molecular analysis in the diagnostic workflow of patients with suspected G6PD deficiency was re

Review of the relationship between deficiency of Glucose-6-phosphate dehydrogenase deficiency and diabetes and its complications.

Diabetes is a chronic condition characterised by hyperglycaemia due to impaired insulin secretion, insulin action, or both. Oxidative stress reduces insulin sensitivity and contributes to complications. Glucose-6-phosphate dehydrogenase (G6PD) deficiency, the most common enzymatic disorder of erythrocytes, impairs NADPH production and reduces the ability to counter oxidative stress. Given the involvement of redox imbalance in diabetes pathogenesis, the role of G6PD deficiency in diabetes development and progression remains unclear. Two reviewers independently searched PubMed, Cochrane Library,

This article critically discusses whether accumulation of lactic acid, or in reality lactate and/or hydrogen (H+) ions, is a major cause of skeletal muscle fatigue, i.e. decline of muscle force or power output leading to impaired exercise performance. There exists a long history of studies on the effects of increased lactate/H+ concentrations in muscle or plasma on contractile performance of skeletal muscle. Evidence suggesting that lactate/H+ is a culprit has been based on correlation-type studies, which reveal close temporal relationships between intramuscular lactate or H+ accumulation and

Temporary Glucose - 6 phosphate dehydrogenase enzyme deficiency, an additional risk factor for invasive fungal infection in immune-compromised AML patients.

Invasive fungal infections (IFI) represent a major threat to acute myeloid leukemia (AML) patients, especially after induction chemotherapy. Intense chemotherapeutic drugs further weaken the immune function of AML patients which increases their susceptibility to life-threatening fungal infection, including invasive aspergillosis (IA). At present, there is no single reliable serological marker or method to predict accurately the risk of IA after induction chemotherapy. We hypothesized that some anti-cancer medicines and antibiotics may trigger temporary Glucose-6-Phosphate Dehydrogenase (G6PD)

Severe rhabdomyolysis as a likely isolated presentation of glucose-6-phosphate dehydrogenase (G6PD) deficiency.

An otherwise healthy young adult male with known glucose-6-phosphate dehydrogenase (G6PD) deficiency presented with severe rhabdomyolysis following routine exercise in the absence of haemolytic anaemia. While G6PD deficiency classically presents with oxidative stress-induced haemolysis, usually reported in the context of infection, medications or toxins, rhabdomyolysis is a rare and atypical manifestation. In this case, no clear precipitating factor for rhabdomyolysis was identified, and the patient did not develop haemolysis. Work up ruled out typical causes of acquired rhabdomyolysis. This p

Glucose-6-phosphate dehydrogenase deficiency is associated with improved survival in patients with acute myeloid leukemia treated with venetoclax and azacitidine.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency impairs cellular redox balance through reduced NADPH production and is the most common enzymatic disorder-causing anemia. Venetoclax combined with azacitidine (Ven-Aza) targets leukemic stem cells by disrupting oxidative phosphorylation and inducing mitochondrial stress. This study hypothesized that G6PD deficiency may enhance the efficacy of Ven-Aza in acute myeloid leukemia (AML) by reducing leukemic cell metabolic resilience. METHODS: The authors studied 73 consecutive patients with newly diagnosed (ND) AML treated with Ven-Az

HbA1c underperforms in identifying abnormal glucose tolerance in the presence of G6PD deficiency: Insight from the Africans in America study.

G6PD deficiency (G6PD-D) variants are associated with lower hemoglobin A1c (HbA1c) concentrations, raising concerns about the diagnostic efficacy of HbA1c for abnormal glucose tolerance (Abnl-GT) in Africans, in whom risk of G6PD-D and Abnl-GT is high. G6PD-D is assessed using genotyping or an enzymatic assay, but because G6PD-D is X-linked, the enzymatic assay is necessary for determining status for women heterozygous for deficiency variants. We assessed: 1) ability of HbA1c to detect Abnl-GT by G6PD-D; 2) concordance of genotyping and enzymatic assay for G6PD-D in sub-Saharan Africans living

Ethnic Variation in G6PD Deficiency: Epidemiology and Mutation Spectrum in Southern China's Multiethnic Hub, Nanning.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency exhibits high prevalence in malaria-endemic regions and areas along the historic Maritime Silk Road. This study investigated the burden and genetic architecture of G6PD deficiency in Nanning, the ethnically diverse capital of Guangxi in southern China, where Southeast Asian gene flow has shaped population genetics. We screened 14 403 individuals for G6PD activity at The First Affiliated Hospital of Guangxi Medical University, defining deficiency as activity < 60% of adjusted male median (AMM = 2045 U/L). Among 2513 deficient cases (prevalence

Newborn screening in an Eastern Indian province: prevalence and trends from a burgeoning newborn screening unit.

Neonatal screening has not been demanded by pediatricians nor parents in India because of limited resources, inadequate health information, early hospital discharges, a large rate of deliveries at home, and challenges with follow-up of results. This study was conducted based on the laboratory e-records from the Newborn Screening Unit, where Dried Blood Spot-based analysis was performed for G6PD deficiency, congenital adrenal hyperplasia, neonatal hypothyroidism, biotinidase deficiency, and galactosemia. The total study population comprised 3210 neonates. Overall, 329 children (10.2%) had some

Haemolysis and methaemoglobinaemia following naphthalene ball ingestion.

Naphthalene poisoning is a rare but potentially life-threatening cause of haemolysis and methaemoglobinaemia due to severe oxidative stress. We report a case of a previously healthy man in his 30s who presented at the emergency department with scleral icterus, peripheral oxygen saturation of 85% on pulse oximetry, haemoglobinuria and chocolate-coloured blood following the accidental ingestion of a 'white air freshener ball'. Arterial blood gas analysis revealed elevated methaemoglobin (MetHb) levels. Laboratory results showed severe haemolysis. Bedside solubility and flotation tests on the whi

Neonatal genetic screening of Glucose-6-phosphate dehydrogenase deficiency through next-generation sequencing.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common inherited enzymopathy worldwide. Current neonatal screening, based on enzymatic assays, often fails to identify heterozygous females due to X-chromosome inactivation. This study aimed to characterize by genotype, enzymatic activity and haematological parameters infants carrying pathogenic G6PD variants and to explore genotype-phenotype correlations. Within the NeoGen project, 4067 newborns underwent whole-exome sequencing (WES). Infants with pathogenic or likely pathogenic variants of the G6PD gene were tested for quantitat

Redox biology from the perspective of the red blood cell.

Decades of research have generated extensive knowledge on oxidant production and antioxidant systems in red blood cells. This work has given us not only a detailed understanding of redox control in these cells but also fundamental information that has helped shape the broader field of redox biology. This review covers aspects of this history and also highlights unresolved issues regarding erythrocyte antioxidant defenses. Hemoglobin is both the principal source of reactive oxidants, produced by autoxidation or interaction with redox-active xenobiotics, and a major target of oxidants that escap
