Challenges associated with dapsone for leprosy treatment in Indonesia - urgent need for access to alternative antimicrobial drugs.

Leprosy is effectively treated with multi-drug therapy (MDT), a regimen containing three antibiotic drugs, including dapsone - a sulfone drug associated with potentially life-threatening adverse drug reactions. Specifically, dapsone hypersensitivity syndrome (DHS), linked to HLA-B∗13:01 polymorphism, and hemolytic anemia associated with glucose-6-phosphate dehydrogenase deficiency (G6PDd). Both of these pharmacogenetic polymorphisms can be prevented through diagnostic screening before MDT initiation averting potential complications. However, in leprosy-endemic areas like Indonesia, access to t

Rasburicase-induced methemoglobinemia and haemolytic anaemia in a patient with G6PD deficiency.

Rasburicase is a recombinant urate oxidase enzyme indicated in patients at a high risk of tumour lysis syndrome. Rasburicase is contraindicated in patients with glucose-6-phosphate-dehydrogenase (G6PD) deficiency, given the risk of precipitating oxidative stress, haemolytic anaemia, as well as methemoglobinemia. We present a case of a man in his 40s without risk factors for G6PD deficiency, who after rasburicase administration developed severe haemolytic anaemia and hypoxia. Although studies validating the use of rasburicase were at a dosage of 0.2 mg/kg up to 5 days, there are new studies sug

Epidemiology of glucose-6-phosphate dehydrogenase deficiency in 10 malaria-endemic countries in sub-Saharan Africa: a systematic review.

INTRODUCTION: it is now clear that several antimalarials, such as primaquine, can cause severe hemolytic anemia in people with G6PD deficiency. Prescriptions for antimalarials should consider G6PD status, especially in malaria-endemic areas. The frequency of G6PD is quite high in some countries in sub-Saharan Africa. METHODS: a systematic review was conducted to summarize the findings on the G6PD deficiency epidemiology in the ten African nations most impacted by malaria. We conducted a systematic electronic literature search based on eligibility requirements using databases, PubMed, Google S

Heartmate 3 left ventricular assist device system in patients with glucose-6-phosphate dehydrogenase deficiency.

BackgroundGlucose-6-phosphate dehydrogenase (G6PD) deficiency is a genetic enzymatic disorder that affects millions of people worldwide and characterized by hemolysis under oxidative stress. Left ventricular assist devices (LVADs) have substantially enhanced survival and quality of life for individuals with advanced heart failure. However, their use is associated with the risk of hemolysis, thrombosis, and embolic events. These risks may be heightened in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency. Given the limited published research on this subject, the primary objectiv

Severe autoimmune hemolytic anemia associated with glucose-6-phosphate dehydrogenase deficiency as a complication of nivolumab treatment: A case report.

In recent years, immune checkpoint inhibitors, such as programmed death-1 (PD-1) inhibitors, have become key therapeutic options for specific cancers, used as first-, second-, or third-line treatments for various metastatic conditions. Nivolumab, a programmed death-1 (PD-1) inhibitor, is FDA-approved for metastatic renal cell carcinoma among other cancers. As the use of nivolumab becomes more widespread, understanding both its common and rare side effects is essential. Although nivolumab has been associated with autoimmune hemolytic anemia (AIHA), there are rare cases where this adverse effect

Rapid and reliable detection of G6PD mutations using recombinase polymerase amplification coupled with lateral flow strip.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common enzymopathy, affecting approximately 500 million people worldwide. It results from inherited mutations in the G6PD gene, causing increased susceptibility to drug-induced hemolytic anemia and severe neonatal jaundice. While phenotypic tests are commonly used, genetic testing is increasingly recognized for its value in the accurate diagnosis of G6PD deficiency, especially in heterozygous females and newborns. This study aimed to develop and evaluate a rapid, field-deployable genetic test for the detection of four common G6PD

Integrated Phenotypic and Genotypic Approaches for Accurate Diagnosis of G6PD Deficiency: Implications for Drug Safety in Thailand.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency holds critical health concerns, particularly due to its association with drug-induced hemolysis triggered by medications such as antimalarials. This condition poses significant risks in malaria-endemic regions where the prevalence and genetic diversity of G6PD deficiency further complicate management. Providing accurate and reliable G6PD status is vital to ensuring safe treatment, reducing complications, and improving healthcare outcomes in these populations. This study evaluated the integration of phenotypic and genotypic diagnostic methods

Severe neonatal hyperbilirubinaemia in preterm twins with glucose-6-phosphate dehydrogenase deficiency and UGT1A1 gene variants.

Neonatal unconjugated hyperbilirubinaemia may have a multifactorial aetiology, and different conditions may act together to increase the risk of severe hyperbilirubinaemia and bilirubin-induced neurological dysfunction. We report a novel case of severe neonatal unconjugated hyperbilirubinaemia requiring treatment with exchange transfusions in two premature twins with a similar clinical presentation. The aetiological investigation revealed glucose-6-phosphate dehydrogenase (G6PD) deficiency combined with heterozygosity for two variants in the UGT1A1 gene. These two conditions alone are common ge

Concurrent Case of Glucose-6-Phosphate Deficiency and Dehydrated Hereditary Stomatocytosis in a 4-Month-old Boy.

Glucose-6-phosphate (G6PD) deficiency is the most prevalent enzyme deficiency and is estimated to affect 400 million people. The patients are usually asymptomatic and diagnosed following hemolytic episodes triggered by oxidative stress. Another type of hemolytic anemia known as dehydrated hereditary stomatocytosis (DHSt) is estimated to affect less than 1 per 1,000,000 people. DHSt is caused by increased cation efflux and dehydration in red blood cells, which leads to decreased flexibility making them more vulnerable to lysis. Compared with G6PD, DHSt has a mild presentation, where most patien

A Case of Methemoglobinemia and Hemolysis from Sodium Chlorite Ingestion.

Sodium chlorite is a household and industrial cleaning agent that can cause oxidative stress leading to methemoglobinemia and severe hemolysis when ingested. There is a scarcity of published literature reporting this in humans. We report a case of intentional sodium chlorite ingestion for homeopathic purposes that resulted in methemoglobinemia and hemolysis, requiring admission to the intensive care unit. Following successful treatment with methylene blue, the patient developed worsening hemolytic activity. Outpatient testing later revealed she was non- glucose-6-phosphate dehydrogenase (G6PD)

Co-occurrence of acute hemolytic anemia and methemoglobinemia in a 74-year-old female with G6PD deficiency: A case report.

RATIONALE: Acute hemolysis and methemoglobinemia rarely coexist in patients with glucose-6-phosphate dehydrogenase (G6PD)-deficient disease. Methylene blue is widely recognized as the first-line treatment for methemoglobinemia; however, it can worsen hemolysis owing to its oxidative effects in patients with G6PD deficiency. PATIENT CONCERNS: A rare case of acute hemolytic anemia and methemoglobinemia induced by ingestion of fresh raw fava beans in a 74-year-old female with G6PD deficiency was reported in this paper. DIAGNOSES: Laboratory test results revealed anemia (low hemoglobin level of
