Treatment monitoring by biomarker analysis in a Phase I dose-expansion study of AZD2811 for relapsed/refractory small-cell lung cancer.

BACKGROUND: Aurora kinase B (AURKB) is overexpressed in lung cancer and is associated with poor prognosis. AZD2811 is an AURKB inhibitor that demonstrated tolerability during a Phase I dose-escalation study in patients with advanced solid tumours, including small-cell lung cancer (SCLC). Here we report the dose-expansion results. METHODS: Eligible patients received nanoparticle-formulated AZD2811 500 mg IV (Day 1; 21-day cycles) with granulocyte colony-stimulating factor (Day 8). Dose-expansion endpoints included: preliminary antitumour activity, safety/tolerability, pharmacokinetics, and bio

Acute Pancreatitis as a Therapeutic Complication in Case of Paroxysmal Nocturnal Hemoglobinuria.

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare acquired hematopoietic disorder requiring careful therapeutic management. In resource-limited settings, traditional treatments may carry significant risks. A 32-year-old female with PNH developed acute pancreatitis 10 months after initiation of danazol and deflazacort therapy. Initial presentation included typical hemoglobinuria, pancytopenia, and indirect hyperbilirubinemia. Flow cytometry confirmed PNH clones despite a negative Ham's test. Treatment with danazol 200 mg twice daily and deflazacort 30 mg daily initially provided clinical impr

Diagnostic Accuracy of Exchangeable Copper for Grading the Severity of Wilson Disease.

Wilson disease (WD) diagnosis remains challenging due to variable biomarker performance across clinical presentations. Exchangeable copper (CuEXC) directly quantifies non-ceruloplasmin-bound copper (NCC), the pathogenic copper fraction. We assessed the diagnostic accuracy of CuEXC across WD phenotypes and its utility for evaluating disease severity. In this prospective study (NCT05231876) at the French National Reference Center for WD, we enrolled 158 treatment-naïve patients (48% female; mean age 20 ± 14 years; 44% ≤ 18 years) referred between 2009 and 2025. Clinical presentations included as

A phase 1 study of ASTX727 plus talazoparib in patients with triple-negative or hormone resistant/HER2-negative metastatic breast cancer.

BACKGROUND: Poly(adenosine diphosphate ribose) polymerase (PARP) is recruited to DNA damage sites along with epigenetic factors such as DNA methyltransferase 1 (DNMT1). Inhibitors of DNMT modulate reactive oxygen species (ROS)-cyclic adenosine monophosphate (cAMP)/Protein Kinase A signaling and induce a "BRCAness phenotype" that further sensitizes cells to PARPi. In preclinical studies, combined DNMTi + PARPi therapy was effective in both triple-negative (TNBC) and hormone resistant (HRBC) models with intact BRCA. METHODS: The authors conducted a phase 1 study combining the oral DNMTi ASTX727

Concurrent Hemolytic Anemia and Methemoglobinemia Revealing Glucose-6-Phosphate Dehydrogenase (G6PD) Deficiency in an Infant Following Peanut Ingestion.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is among the most common enzyme disorders affecting red blood cells. It often remains unnoticed until the individual is exposed to oxidative stress, which can trigger rapid hemolysis. Methemoglobinemia develops when hemoglobin becomes oxidized into a form that cannot efficiently transport oxygen. This leads to impaired oxygen delivery to tissues and cyanosis that does not improve despite supplemental oxygen. Although uncommon, methemoglobinemia can develop as a serious complication during episodes of severe oxidative stress. We describe the c

Coombs-Negative Hemolytic Anemia and Thrombocytopenia Associated With Sarcoidosis Confined to the Spleen and Bone Marrow: A Case Report.

Sarcoidosis is a systemic granulomatous disease that typically involves the lungs and lymphatic system. Isolated splenic sarcoidosis with bone marrow involvement is exceedingly rare, and hemolytic anemia associated with sarcoidosis is usually Coombs-positive. We report a case of a 61-year-old man who presented with massive splenomegaly, Coombs-negative hemolytic anemia, and thrombocytopenia. Serum angiotensin-converting enzyme (ACE) and soluble interleukin-2 receptor (sIL-2R) levels were markedly elevated, strongly suggesting malignant lymphoma. Bone marrow biopsy revealed non-caseating granul

The clinical landscape of HIF2α inhibitors in oncology.

Hypoxia-inducible factor 2α (HIF2α; also known as endothelial PAS domain-containing protein 1) had long been considered an undruggable transcription factor until the discovery of an allosteric pocket within its PAS-B domain enabled the development of selective small-molecule antagonists. Belzutifan, the first-in-class HIF2α inhibitor, has since demonstrated substantial efficacy in patients with von Hippel-Lindau (VHL) disease-associated tumours, sporadic clear-cell renal cell carcinoma (ccRCC), and pheochromocytoma or paraganglioma, thereby validating HIF2α as a therapeutic target in patients

Family studies in Gaucher Disease: a key resource for early diagnosis and personalized treatment strategies.

Gaucher Disease (GD) is an inherited metabolic disorder caused by mutations in the GBA1 gene, which is responsible for the synthesis of the enzyme glucocerebrosidase (GCase). The clinical manifestations, which are extremely heterogeneous, include splenomegaly, hepatomegaly, anaemia and bone complications. GD is an autosomal recessive condition, meaning that the clinical phenotype manifests itself only in the presence of two mutated alleles, which are inherited from both parents, who are generally healthy and asymptomatic. However, other family members (brothers, sisters, grandparents, uncles,

A Case of Hemolytic Uremic Syndrome Due to a Pathogenic Variant in the DGKE Gene.

Complement dysregulation is frequently implicated in the thrombotic microangiopathy (TMA) known as atypical hemolytic uremic syndrome (aHUS). Diacylglycerol kinase epsilon (DGKE) mutations encode a non-complement regulatory protein, and pathogenic variants in DGKE define a distinct form of aHUS. Indeed, the DGKEgene encodes a key enzyme involved in intracellular signaling. While eculizumab and other anti-C5 monoclonal antibodies are widely used in complement-related aHUS, their relevance in DGKE-associated forms remains controversial. We report a case of a patient followed from the age of eigh

Gender, Anaemia, and Azotaemia as Correlates of Thyroid Dysfunction in Chronic Kidney Disease: A Cross-Sectional Study From North India.

Background and objective Thyroid dysfunction is a well-recognised but under-investigated complication of chronic kidney disease (CKD). While the overall prevalence of hypothyroidism in CKD is documented, its association with specific clinical variables, particularly including gender, haemoglobin (Hb), and azotaemia, has not been systematically characterised in populations from the Indian subcontinent. Hence, this study was designed to identify the correlates of thyroid dysfunction in CKD patients on conservative management. Methods A cross-sectional observational study was conducted involving

Phase II study of ramucirumab plus erlotinib for treatment-naïve patients with EGFR-mutant non-squamous non-small cell lung cancer and pleural effusion (RELAY-Effusion).

INTRODUCTION: Malignant pleural effusion (MPE) develops in approximately 30-40% of patients with non-small cell lung cancer (NSCLC), including those harboring epidermal growth factor receptor (EGFR) mutations, and is generally associated with reduced responsiveness to EGFR tyrosine kinase inhibitors. This study aimed to investigate the efficacy of ramucirumab in the context of MPE, which remains unexplored. METHODS: In this single-arm, multicenter, phase II study, we enrolled treatment-naïve patients with EGFR-mutant NSCLC complicated by MPE. They received ramucirumab (10 mg/kg) every 2 weeks

Case Report: ALK-positive histiocytosis with a novel PTRH2::ALK fusion masquerading as a liver abscess in an infant.

BACKGROUND: ALK-positive histiocytosis is a rare form of cutaneous histiocytosis and has been explicitly designated as a novel histiocytic neoplasm in recent classification systems. This disease may involve multiple organs throughout the body. Its diagnosis primarily relies on pathological confirmation, and molecular evidence of ALK rearrangement is required to establish a definitive diagnosis. For patients with unresectable or disseminated ALK-positive histiocytosis, therapy with ALK inhibitors has demonstrated considerable efficacy. CASE PRESENTATION: An 8-month-old Chinese male infant was

Discovery of the Orally Bioavailable Isoform Selective Janus Kinase 1 (JAK1) Compound Povorcitinib (INCB054707) for the Treatment of Inflammatory and Autoimmune Diseases.

Aberrant JAK2 signaling is a key driver in cancers such as myeloproliferative neoplasms (MPNs), whereas JAK1 acts as a central mediator of autoimmune and inflammatory diseases. Since JAK2 supports blood cell production, its inhibition can lead to anemia and other cytopenias, making selective JAK1 inhibition a potentially safer therapeutic strategy opposed to dual JAK1/JAK2 inhibition. We initially identified novel scaffold 9, which selectively inhibited JAK1 over JAK2. Iterative breakthroughs led to the discovery of povorcitinib (INCB054707, 22), which showed improved pharmacokinetics due to i
