Newly Diagnosed VEXAS (Vacuoles, E1 Enzyme, X-linked, Autoinflammatory, Somatic) Syndrome in a Patient Presenting With Legionella Pneumonia.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a newly described inflammatory disease characterized by recurrent autoimmune manifestations, such as chondritis and uveitis, in the setting of classic laboratory abnormalities, including elevated inflammatory markers and macrocytic anemia. It is caused by an acquired mutation in the UBA1 gene within the myeloid lineage, ultimately resulting in cell death and systemic inflammation. Timely diagnosis can help expedite treatment and encourage monitoring for complications of the disease, most notably a predilection to infe

Fever of unknown origin as the primary manifestation of extrapulmonary sarcoidosis with bone marrow involvement: a rare clinical scenario.

Isolated bone marrow involvement in sarcoidosis is a rare condition, mostly manifested by cytopenia, hypercalcemia and/or hypercalciuria. We present the case of a 48-year-old man with a prolonged fever meeting the criteria for fever of unknown origin, in whom a suspicion of sarcoidosis only emerged after non-necrotizing granulomas were identified in the bone marrow, one year after the onset of the disease. The patient exhibited only mild anemia, moderate leukocytosis, thrombocytosis, and increased erythrocyte sedimentation rate, C-reactive protein levels. Other potential causes of fever of unk

Vexas Syndrome Without Macrocytosis: A Case Highlighting an Expanding Clinical Phenotype and Diagnostic Delay.

VEXAS (Vacuoles, E1 enzyme, X-linked, Autoinflammatory, Somatic) syndrome is a recently established adult-onset autoinflammatory disorder caused by somatic mutations in the ubiquitin-activating enzyme 1 (UBA1) gene. The condition predominantly affects men in later stages of life and is characterised by recurrent systemic inflammation, fever, pulmonary involvement, dermatological manifestations, haematological abnormalities, and frequent corticosteroid dependence. Due to its heterogeneous clinical presentation, diagnosis can often be delayed, effectively resulting in significant morbidity and m

Excess folate elevates formaldehyde genotoxicity in cell lines but not in mice or humans.

Folate metabolites are chemically unstable: spontaneous decomposition releases formaldehyde, a genotoxin in blood stem cells and a human carcinogen. Despite this, folic acid consumption frequently exceeds the Recommended Dietary Allowance and is prescribed at high doses for patients with blood disorders. However, the impact of excess folate on endogenous formaldehyde genotoxicity in vivo has not been studied. We find that excess tetrahydrofolate (THF) treatment of cell lines elevates formaldehyde-DNA adducts and genotoxicity. To test this in vivo, we fed a high-folic acid diet (10-fold above s

Non-genotoxic transplantation and in vivo selection through epitope editing.

The short-term and long-term effects of genotoxic pre-transplant conditioning remain barriers to the broader application of haematopoietic stem/progenitor cell (HSPC) transplantation and gene therapies1-4. Although monoclonal antibodies targeting KIT have been proposed as alternatives to chemotherapy or radiotherapy5-7, their pharmacokinetics hinder clinical applications owing to the risk of depleting transplanted HSPCs. Here, to address this issue, we identified amino acid changes in the extracellular domain of KIT that disrupt the binding of two therapeutic monoclonal antibodies8,9, which im

Red lines and green lights: Gene therapy for inherited erythroid disorders beyond the haemoglobinopathies.

Gene therapy is revolutionizing treatment paradigms for inherited haematological and immunological conditions. Recent successes, including the United States Food and Drug Administration (FDA) approval of gene therapy products for sickle cell disease and beta-thalassaemia, highlight the translational path of gene therapies for erythroid-specific disorders. In contrast, gene therapy development for other inherited erythroid disorders remains largely preclinical. Here, we examine the emerging landscape of gene therapies for inherited non-haemoglobinopathy erythroid disorders, focusing on the stat

Diagnostic Challenge of Pediatric Gaucher Disease in a Low-Resource South Asian Setting: A Case Report.

Gaucher disease is a rare autosomal recessive lysosomal storage disorder that is caused by a deficiency of the enzyme "β-glucocerebrosidase", leading to the accumulation of glucocerebroside within macrophages. It commonly presents with hepatosplenomegaly, cytopenias, and bone marrow infiltration. Gaucher disease is a pan-ethnic lysosomal storage disorder reported worldwide. Although few cases have been described in Pakistan and other South Asian countries, limited awareness and restricted access to diagnostic testing may contribute to underdiagnosis and delayed recognition in this region. We r

A Case of Refractory Thrombocytopenia in Pregnancy.

Immune thrombocytopenia is an acquired autoimmune disorder characterized by isolated thrombocytopenia due to antibody-mediated platelet destruction and impaired platelet production. Thrombocytopenia in pregnancy is common, but severe thrombocytopenia requires prompt evaluation to distinguish benign gestational thrombocytopenia from immune thrombocytopenia, hypertensive disorders of pregnancy, hemolysis, elevated liver enzymes, low platelet count syndrome, and thrombotic microangiopathies. We present the case of a 19-year-old gravida 1 para 0 patient with sickle cell trait and iron deficiency a

Losartan Is Not Associated With Decreased Postoperative Arthrofibrosis After Hip Arthroscopy.

PURPOSE: To determine whether losartan is associated with lower prevalence of hip adhesions, instability, and repeat procedures after hip arthroscopy and to assess this association among men, women, and patients under 30 years of age. METHODS: The TriNetX database was queried to identify all patients who underwent hip arthroscopy. Two cohorts were generated: one consisting of patients prescribed losartan after surgery and a comparison group excluding patients with any documented prescription for commonly used angiotensin-converting enzyme inhibitors or angiotensin-receptor II blockers. Intermittent organophosphate pesticide exposure induces hepatic injury in goats with subclinical fatty liver via immune activation and energy metabolism reprogramming.

Organophosphate pesticides are widely used in livestock production, yet their impact on the liver, particularly with repeated low-dose exposure in animals with underlying metabolic stress, remains poorly understood. We examined this using a goat model where treated animals received intermittent trichlorfon poisoning followed by antidote rescue, mimicking field conditions. Unexpectedly, our control goats turned out to have fatty liver, forcing us to reframe the study around how OP exposure worsens pre-existing liver damage. Poisoned goats showed aggravated fat accumulation, focal cell death, an

Risk of Heart Failure Hospitalization Associated With Hypoxia-Inducible Factor Prolyl Hydroxylase Inhibitors Versus Erythropoiesis-Stimulating Agents: A Nationwide Claims-Based Cohort Study.

PURPOSE: Hypoxia-inducible factor prolyl hydroxylase inhibitors (HIF-PHIs) are increasingly used to treat renal anemia in patients with chronic kidney disease (CKD). We evaluated the risk of heart failure associated with HIF-PHIs compared with erythropoiesis-stimulating agents (ESAs) in routine clinical practice. METHODS: We conducted a nationwide, retrospective, active-comparator cohort study using a Japanese claims database. We included adults with non-dialysis CKD and heart failure initiating HIF-PHIs or ESAs alongside diuretic therapy between August 2020 and March 2025. The primary outcom

A rare TERT p.Ala880Thr variant associated with familial pulmonary fibrosis and multisystem involvement: Intrafamilial heterogeneity in a Vietnamese kindred - A case series.

Short telomere syndromes (STS) are rare telomere biology disorders caused by germline variants affecting telomere maintenance and may present with organ-limited disease (e.g., pulmonary fibrosis) or multisystem involvement. We report a Vietnamese family carrying a heterozygous TERT p. Ala880Thr variant. The proband was a 42-year-old man with cryptogenic cirrhosis who was incidentally found to have a probable usual interstitial pneumonia (UIP) pattern on high-resolution computed tomography (HRCT) and later developed progressive interstitial lung disease (ILD). He also had marked premature grayi

Integrative Long-Read Multi-Omics of a Patient With GPI Deficiency: A Molecular Case Study of a Candidate Dual-Effect GPI Variant.

The molecular determinants of phenotypic severity in red cell enzymopathies are often obscured by the disconnect between coding sequence variants and their regulatory landscapes. Here we present a single-patient molecular case study that uses an integrative multi-omic approach-combining short-read WGS, PacBio HiFi long-read sequencing, native CpG methylation profiling, and Iso-Seq full-length transcriptomics-to characterize a severe, transfusion-dependent hemolytic anaemia. We identified a compound heterozygous state in the glucose-6-phosphate isomerase (GPI) gene, with no wild-type allele pre

An Endemic Region of Thiamine-Responsive Megaloblastic Anemia Caused by an SLC19A2 c.1223+1G>A Founder Mutation.

Thiamine-responsive megaloblastic anemia (TRMA) is a rare autosomal recessive disorder caused by biallelic loss of function variants in the SLC19A2 gene. It typically presents with a triad of megaloblastic anemia, diabetes mellitus, and sensorineural deafness. In this work, we analyzed ten children with suspected TRMA: nine exhibited the full triad and one, a younger sibling of a patient with full triad, did not develop hearing loss by the age of 18 months. All patients became transfusion-independent on high-dose thiamine therapy. Whole-genome sequencing identified homozygosity for the canonic

CD39 polymorphism enables lung thrombosis in sickle cell disease.

Sickle cell disease (SCD) is the most common monogenic-hemolytic disorder affecting people of African ancestry. Adenosine diphosphate (ADP) released following intravascular hemolysis activates platelets by stimulating purinergic receptors to promote thrombosis. Despite brisk intravascular hemolysis, which releases high levels of ADP into plasma, and evidence of platelet and hemostatic activation, it remains elusive why only a subset of SCD patients develop lung thrombosis. Using real-time in vivo lung microscopy, we report a surprising finding that humanized SCD mice are protected from ADP-ind

Glucose-6-phosphate dehydrogenase deficiency is associated with improved survival in patients with acute myeloid leukemia treated with venetoclax and azacitidine.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency impairs cellular redox balance through reduced NADPH production and is the most common enzymatic disorder-causing anemia. Venetoclax combined with azacitidine (Ven-Aza) targets leukemic stem cells by disrupting oxidative phosphorylation and inducing mitochondrial stress. This study hypothesized that G6PD deficiency may enhance the efficacy of Ven-Aza in acute myeloid leukemia (AML) by reducing leukemic cell metabolic resilience. METHODS: The authors studied 73 consecutive patients with newly diagnosed (ND) AML treated with Ven-Az

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is associated with elevated mean corpuscular volume (MCV) and bilirubin.

BACKGROUND: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a common hereditary hemolytic disorder. To provide evidence for the clinical evaluation and management of this disorder, we investigated the differences in erythrocyte and biochemical parameters between G6PD-deficient and G6PD-normal individuals. After excluding samples with positive preliminary thalassemia screening, anemia, or liver dysfunction, a total of 2,582 health checkup individuals, 236 male military conscripts, and 3,125 males undergoing preconception check-ups were enrolled. All subjects were divided into a G6PD-norm

Persistent Anemia Revealing Underlying B-cell Malignancies: A Report of Two Cases Highlighting a Diagnostic Gap in Primary Care.

Persistent anemia in older adults is commonly attributed to nutritional deficiency, chronic disease, or age-related change, particularly when routine laboratory investigations are unrevealing. However, plasma cell and lymphoplasmacytic disorders may present subtly, with anemia as the only consistent abnormality. We report two cases in which extended laboratory evaluation led to early identification of clinically significant B-cell malignancies. The first case involved a man in his early 70s with stable, asymptomatic normocytic anemia despite normal iron studies, vitamin B12 and folate levels,

Beyond iron deficiency: A comprehensive national survey of anaemia etiology in Sri Lankan young adults.

Previous studies in Sri Lanka have explored the prevalence and causes of anaemia, mainly emphasizing iron deficiency while overlooking other important factors such as enzymopathies, membranopathies, and haemoglobinopathies. Moreover, many studies reported unexplained cases of anaemia even after detailed evaluation. This study aimed to determine the prevalence and underlying causes of anaemia among community-based young adults in Sri Lanka. A descriptive cross-sectional study was conducted from January 2023 to December 2024 among young adults aged 18-30 years. Data and blood samples were collec
